A cianose persistente constitui complicação freqüente no pós-operatório da cirurgia de Norwood modificada por Sano. Sua ocorrência pode ser explicada pela obstrução dinâmica da anastomose proximal do conduto sintético, que liga o ventrículo direito ao tronco pulmonar, evidenciada à ecocardiografia. A fim de melhorar a saturação arterial de oxigênio, foi recentemente descrita a utilização de betabloqueador, no período pós-operatório. Neste relato, descrevemos a utilização de propranolol em dois casos de cirurgia de Norwood modificada por Sano, demonstrando diminuição do gradiente no tubo sintético, aumento dos níveis de saturação arterial de oxigênio, diminuição da freqüência cardíaca e aumento da pressão arterial, traduzido em melhora clínica. Concluímos que autilização de betabloqueador nesses casos mostrou-se benéfica.

The Norwood I operation continues to be a procedure with significant operative mortality. One well-accepted risk factor for death after the first step of the Norwood operation is critical preoperative status. We describe herein a new concept for the treatment of patients with hypoplastic left heart syndrome (HLHS) in very poor preoperative condition. This is a case report of a child who was born in a rural hospital. On the second day of life he was referred to our center in multiorgan failure. There were signs of liver dysfunction and the child was anuric. Therapy was started immediately with prostaglandin and vasodilators as well as diuretics, milrinone, and dobutamine. However, systemic perfusion continued to be insufficient. Finally, the child was placed on a ventilator. On the fourth day of life, bilateral pulmonary artery (PA) banding was performed and circulation stabilized immediately. Two hours after the operation urine output started. Liver function stabilized over the next couple of days. Two days after PA banding the child was weaned from the ventilator. On the 12th day of life a Norwood operation with PA debanding and a right ventricle-PA conduit was performed, and 2 days postoperatively the child was weaned from the ventilator. Twenty days after the operation he was discharged home. When the boy was 4 months old a bidirectional cavopulmonary anastomosis was performed. In selected cases of patients with HLHS with very poor hemodynamic conditions, a rapid two-stage approach with bilateral banding followed by a Norwood operation after cardiac stabilization can be recommended.

It was published in 1851. Scale [ca. 1:20,000]. Covers the towns of Dedham, Westwood, and Norwood. The image inside the map neatline is georeferenced to the surface of the earth and fit to the Massachusetts State Plane Coordinate System, Mainland Zone (in Feet) (Fipszone 2001). All map collar and inset information is also available as part of the raster image, including any inset maps, profiles, statistical tables, directories, text, illustrations, or other information associated with the principal map. This map shows features such as roads, railroads, drainage, public buildings, schools, churches, cemeteries, industry locations (e.g. mills, factories, mines, etc.), private buildings with names of property owners, town boundaries and more. Relief is shown by hachures. Includes inset: Dedham village. Scale [ca. 1:5,900]. This layer is part of a selection of digitally scanned and georeferenced historic maps of Massachusetts from the Harvard Map Collection. These maps typically portray both natural and manmade features. The selection represents a range of regions, originators, ground condition dates (1755-1922), scales, and purposes. INTRODUCTION: Hypoplastic left heart syndrome (HLHS) is a major cause of cardiac death during the first week of life. The hybrid approach is a reliable, reproducible treatment option for patients with HLHS. Herein we report our results using this approach, focusing on its efficacy, safety and late outcome. METHODS: We reviewed prospectively collected data on patients treated for HLHS using a hybrid approach between July 2007 and September 2014. RESULTS: Nine patients had a stage 1 hybrid procedure, with seven undergoing a comprehensive stage 2 procedure. One patient completed the Fontan procedure. Five patients underwent balloon atrial septostomy after the hybrid procedure; in three patients, a stent was placed across the atrial septum. There were three deaths: two early after the hybrid procedure and one early after stage two palliation. Overall survival was 66%. CONCLUSIONS: In our single-center series, the hybrid approach for HLHS yields intermediate results comparable to those of the Norwood strategy. The existence of dedicated teams for the diagnosis and management of these patients, preferably in high-volume centers, is of major importance in this condition.

An infrequent but devastating late complication of Fontan circulation is protein-losing enteropathy (PLE), which results from unbalanced lymphatic homeostasis. Surgical decompression of the thoracic duct by redirecting its drainage to the pulmonary venous atrium has been introduced recently as a possible treatment. This report describes a single-institution experience with this innovative procedure in 2 patients with failing Fontan circulation with PLE refractory to optimized medical therapy.

In this report we describe the twelveth case in the literature of absence of the aortic valve cusps, associated with hypoplastic left-sided heart syndrome in a neonate. Clinical and hemodynamic conditions in our patient resemble the classical features of this syndrome except for a greater development of the ascending aorta and the left ventricular cavity, due to aortic insufficiency. A patch was unsuccessfully inserted at the aortic annulus to exclude the left ventricle from the circulation. In addition the Norwood operation was performed.

OBJETIVOS: Relatar uma estratégia cirúrgica para a operação de Norwood na síndrome de hipoplasia do coração esquerdo (SHCE), que possibilite tempo curto de parada circulatória hipotérmica e reconstrução do arco aórtico com pericárdio autólogo. Comparar os resultados das técnicas anastomose de Blalock-Taussig modificado (B-Tm) e tubo ventrículo direito para artéria pulmonar (VD-AP) no restabelecimento da circulação pulmonar. MÉTODOS: Estudo retrospectivo compreendendo 71 neonatos portadores de SHCE, consecutivamente operados entre março de 1999 e fevereiro de 2006. Foi usada a mesma técnica de reconstrução da neo-aorta e duas técnicas diferentes de restabelecimento da circulação pulmonar: anastomose de B-Tm nos primeiros 37 neonatos e tubo VD-AP nos últimos 34. A canulação do canal arterial para a perfusão arterial foi a parte principal da estratégia cirúrgica para diminuir o tempo de parada circulatória hipotérmica. RESULTADOS: A sobrevida geral foi de 74,64%, sendo de 67,57% no grupo B-Tm e de 82,35% no grupo tubo VD-AP (p = 0,1808). Os índices de mortalidade entre o primeiro e o segundo estágios foram de 40% e de 4,4%, respectivamente, nos grupo B-Tm e tubo VD-AP (p = 0,0054). Os tempos de parada circulatória hipotérmica foram, respectivamente, de 45,79 + 1,99 minutos e de 36,62 + 1,62 minutos (p = 0,0012). Coarctação tardia da aorta ocorreu em cinco pacientes (7,2%). CONCLUSÃO: Essa estratégia cirúrgica resultou em tempo curto de parada circulatória, baixa mortalidade e boa morfologia da neo-aorta, com baixa incidência tardia de coarctação aórtica. A maior sobrevida ao primeiro estágio com o tubo VD-AP não foi significante, mas a mortalidade interestágios foi estatisticamente menor na comparação com o procedimento B-Tm.

Ryanodine receptor 1 (RYR1) mutations are a common cause of congenital myopathies associated with both dominant and recessive inheritance. Histopathological findings frequently feature central cores or multi-minicores, more rarely, type 1 predominance/uniformity, fiber-type disproportion, increased internal nucleation, and fatty and connective tissue. We describe 71 families, 35 associated with dominant RYR1 mutations and 36 with recessive inheritance. Five of the dominant mutations and 35 of the 55 recessive mutations have not been previously reported. Dominant mutations, typically missense, were frequently located in recognized mutational hotspot regions, while recessive mutations were distributed throughout the entire coding sequence. Recessive mutations included nonsense and splice mutations expected to result in reduced RyR1 protein. There was wide clinical variability. As a group, dominant mutations were associated with milder phenotypes; patients with recessive inheritance had earlier onset, more weakness, and functional limitations. Extraocular and bulbar muscle involvement was almost exclusively observed in the recessive group. In conclusion, our study reports a large number of novel RYR1 mutations and indicates that recessive variants are at least as frequent as the dominant ones. Assigning pathogenicity to novel mutations is often difficult, and interpretation of genetic results in the context of clinical, histological, and muscle magnetic resonance imaging findings is essential.

Arterial hypertension has been reported as a complication of surgical closure of an abdominal wall defect. No report studying the incidence, the characteristics and the clinical significance of hypertension after surgical correction of an omphalocele or gastroschisis has been published so far. The medical records of all newborns with surgically corrected gastroschisis or omphalocele identified in two centers were retrospectively evaluated. Arterial hypertension was defined as a mean daily systolic and/or diastolic blood pressure value higher than the 95 percentile for age and/or weight, according to literature data. The timing of surgery, weight gain, plasma creatinine and the use of diuretics or vasoactive drugs were compared between the groups with and without hypertension. Seventy-two patients were identified and included in the study, 29 with omphalocele and 43 with gastroschisis. Those with omphalocele were born at a mean age of 37.3+/-2.6 weeks with a mean birth weight of 2,971+/-715 g, and those with gastroschisis were born at 36.1+/-2.0 weeks with a mean birth weight of 2,527+/-498 g. Blood pressure values of 66 patients were available for analysis. Of the omphalocele patients, 46.2% (12/26) developed systolic hypertension, compared to 17.5% (7/40) of the patients with gastroschisis (P =0.024). Hypertension was always transient, lasting an average of 4 and 1 day in the omphalocele and gastroschisis groups, respectively. Two patients with omphalocele were given anti-hypertensive therapy. There was no difference between patients with or without hypertension regarding weight gain, use of vasoactive drugs or diuretics, mean weekly creatinine values or the timing of surgery. Newborns with an abdominal wall defect frequently present with transient arterial hypertension. Hypertension occurs significantly more often, is more severe and lasts longer in patients with omphalocele than in patients with gastroschisis. In both groups, hypertension is transient and rarely requires therapy. The cause of hypertension remains unclear.
