Background: Noise is the most common agent of occupational exposure. It may induce both auditory and extraauditory dysfunction and increase the risk of work accidents. The purpose of this study was to estimate the fraction of accidents attributable to noise occupational exposure in a mid-size city located in southeastern Brazil. Materials and Methods: In this population case-control study, which included 108 cases and 324 controls, the incidence rate ratio of work accidents controlled for several covariables was obtained by classifying occupational noise exposure into three levels, as well as determining the prevalence in each level. Results: Based on these data, the attributable fraction was estimated as 0.6391 (95 CI = 0.2341-0.3676), i.e., 63 of the work accidents that took place in the study site were statistically associated with occupational noise exposure. Discussion: The causes of this association as well as its implications in the prevention of work accidents are discussed.

Noise is the most frequent type of occupational exposure and can lead to both auditory and extra-auditory dysfunction as well as increasing the risk of work accidents. The purpose of this study was to estimate the attributable fraction of work accidents related to occupational noise exposure in a medium-sized city in Southeast Brazil. In this hospital-based case-control study, including 600 cases and 822 controls, the odds ratio of work accidents (controlled for several covariables) was obtained classifying occupational noise exposure into four levels and determining the prevalence at each level. Based on these data, the calculated attributable fraction was 0.3041 (95%CI: 0.2341-0.3676), i.e., 30% of work accidents in the study area were statistically associated with occupational noise exposure. The authors discuss the causes of this association and the implications for the prevention of work accidents.

BACKGROUND: Highway maintenance workers are constantly and simultaneously exposed to traffic-related particle and noise emissions, and both have been linked to increased cardiovascular morbidity and mortality in population-based epidemiology studies. OBJECTIVES: We aimed to investigate short-term health effects related to particle and noise exposure. METHODS: We monitored 18 maintenance workers, during as many as five 24-hour periods from a total of 50 observation days. We measured their exposure to fine particulate matter (PM2.5), ultrafine particles, noise, and the cardiopulmonary health endpoints: blood pressure, pro-inflammatory and pro-thrombotic markers in the blood, lung function and fractional exhaled nitric oxide (FeNO) measured approximately 15 hours post-work. Heart rate variability was assessed during a sleep period approximately 10 hours post-work. RESULTS: PM2.5 exposure was significantly associated with C-reactive protein and serum amyloid A, and negatively associated with tumor necrosis factor α. None of the particle metrics were significantly associated with von Willebrand factor or tissue factor expression. PM2.5 and work noise were associated with markers of increased heart rate variability, and with increased HF and LF power. Systolic and diastolic blood pressure on the following morning were significantly associated with noise exposure after work, and non-significantly associated with PM2.5. We observed no significant associations between any of the exposures and lung function or FeNO. CONCLUSIONS: Our findings suggest that exposure to particles and noise during highway maintenance work might pose a cardiovascular health risk. Actions to reduce these exposures could lead to better health for this population of workers.

Police officers are exposed to impact noise coming from firearms, which may cause irreversible injuries to the hearing system.Aim: To evaluate the noise exposure in shooting stands during gunfire exercises, to analyze the acoustic impact of the noise produced by the firearms and to associate it with tonal audiometry results.Study design: Cross-sectional.Materials and methods: To measure noise intensity we used a digital sound level meter, and the acoustic analysis was carried out by means of the oscillations and cochlear response curves provided by the Praat software. 30 police officers were selected (27 males and 3 females).Results: The peak level measured was 113.1 dB(C) from a .40 pistol and 116.8 dB(C) for a .38 revolver. The values obtained for oscillation and Praat was 17.9 +/- 0.3 Barks, corresponding to the rate of 4,120 and 4,580 Hz. Audiometry indicated greater hearing loss at 4,000Hz in 86.7% of the cases.Conclusion: With the acoustic analysis it was possible to show cause and effect between the main areas of energy excitation of the cochlea (Praat cochlear response curve) and the frequencies of low hearing acuity.

OBJECTIVE: To analyze the association between noise levels present in preschool institutions and vocal disorders among educators. METHODS: Cross-sectional study conducted in 2009 with 28 teachers from three preschool institutions located in the city of Sao Paulo (Southeastern Brazil). Sound pressure levels were measured according to Brazilian Technical Standards Association, with the use of a sound level meter. The averages were classified according to the levels of comfort, discomfort, and auditory damage proposed by the Pan American Health Organization. The educators underwent voice evaluation: self-assessment with visual analogue scale, auditory perceptual evaluation using the GRBAS scale, and acoustic analysis utilizing the Praat program. To analyze the association between noise and voice evaluation, descriptive statistics and the chi-square test were employed, with significance of 10% due to sample size. RESULTS: The teachers' age ranged between 21 and 56 years. The noise average was 72.7 dB, considered as damage 2. The professionals' vocal self-assessment ranked an average of 5.1 on the scale, being considered as moderate alteration. In the auditory-perceptual assessment, 74% presented vocal alteration, especially hoarseness; of these, 52% were considered mild alterations. In the acoustic assessment the majority presented fundamental frequency below the expected level. Averages for jitter, shimmer and harmonic-noise ratio showed alterations. An association between the presence of noise between the harmonics and vocal disorders was observed. CONCLUSIONS: There is an association between presence of noise between the harmonics and vocal alteration, with high noise levels. Although most teachers presented mild voice alteration, the self-evaluation showed moderate alteration, probably due to the difficulty in projection.

The rationalization of work in the Dentistry has been taking the professional to work for ways and systems based in the ergonomics, turning their work efficient and less tiring. Since their academic formation, the dentists surgeons are concerned with the high productivity in clinic and with the final result of the work, neglecting the way as it is executed, which reduce their work capacity and exhibits them to occupational diseases that could be minimized and/or forewarned. This research had as the main objective to investigate the knowledge of the Dentistry academics of Rio Grande do Norte Federal University concerning the Noise-induced Hearing Loss (NIHL), relating them at the noise levels that they are exposed, as well as to the preventive measures taken during the clinical activities. Was observed that 95% of the individuals know that the dentist surgeon is a professional in risk for NIHL. Among the causes of NIHL, the one that obtained the largest frequency citation was the high-speed handpieces, reminded by 92,4% of the academics. Among the students which enumerated protective measures for NIHL, 92% mentioned the use of the ear plugs, although 97% of the researched have told do not use any kind of preventive measure related to the noise. Was also observed that 96% of the academics notice the noise during the clinical attendance, what inconvenience 28,1% of them. Related the noise levels, the high-speed handpieces of the academics presented a medium value of 80,5 dB varying from 72,3 to 88,3 dB. The average of the ambient noise observed at the Integrated Clinic was about 74,8 dB. In spite of the noise levels in this research were observed below the established limits of tolerance by the legislation, they can provoke damages to the Dentistry professionals' health, or that suggests the need of an intervention and use of immediate preventive measures able to generate a healthy atmosphere of work and less risky

OBJETIVO: Analisar o comportamento de pressão arterial (PA) e a freqüência cardíaca (Fc) de indivíduos ao longo da jornada de trabalho em dois ambientes com estresses ambientais distintos. MÉTODOS: Foram avaliados 46 funcionários, trabalhadores de uma indústria processadora de madeira, de Botucatu, SP, sendo 27 funcionários da linha de produção (esforço físico moderado-intenso, altas temperaturas e elevados níveis de ruído) (G1), e 19 da administração (sem esforço físico, salas aclimatadas, baixos níveis de ruído) (G2). Todos foram submetidos a avaliação antropométrica da composição corporal (obesidade e adiposidade) e bioquímica do sangue (lipidemia) e, adicionalmente, o registro da PA e da Fc em três momentos do turno de serviço: início, meio e fim. RESULTADOS: Houve semelhança na variação da PA entre G1 e G2, mas com maiores elevações de PA e Fc em G1. Os resultados mostraram grande variabilidade na resposta da PA, levando à subdivisão dos grupos G1 e G2 em respondedores (GR, aumento maior de 10% na PA média) e não respondedores (GN). Os subgrupos GR e GN apresentaram semelhanças nos padrões antropométrico e bioquímico diferindo apenas na resposta pressórica e no caso do GR1 na história familiar de hipertensão. Comparando os subgrupos GR1 e GR2, foi constatado que os primeiros apresentaram maiores variações de PA e Fc que os segundos. CONCLUSÕES: A variação individual da resposta pressórica e da Fc conforme o tipo de estresse ambiental indica ser este um fator adicional a ser considerado na avaliação da pressão arterial e, talvez, na gênese da hipertensão arterial de operários.

OBJETIVO: Verificar se a exposição ocupacional ao ruído é fator de risco relevante para acidentes do trabalho. MÉTODOS: Estudo de caso-controle de base populacional. Os dados foram coletados entre 16/5/2002 e 15/10/2002, na cidade de Botucatu, Estado de São Paulo. Os casos foram definidos como trabalhadores que sofreram acidentes ocupacionais típicos nos últimos 90 dias, identificados por intermédio de amostragem aleatória sistemática de domicílios residenciais. Os controles foram trabalhadores não acidentados, aleatoriamente alocados a partir da mesma população que originou os casos, emparelhados na razão 3:1 segundo sexo, faixa etária e setor censitário de moradia. Ajustou-se um modelo de regressão logística múltipla, tendo como variável independente a exposição ocupacional ao ruído, controlada por covariáveis de interesse. RESULTADOS: Foram analisados 94 casos e 282 controles. Ajustando-se um modelo de regressão logística condicional múltipla observou-se que trabalhar sempre e às vezes exposto a ruído intenso associou-se a um risco relativo de acidentar-se de 5,0 (IC 95%: 2,8-8,7; p<0,001) e 3,7 (IC 95%: 1,8-7,4; p=0,0003), respectivamente, tendo como referência trabalhar não exposto a ruído, controlado para diversas covariáveis. CONCLUSÕES: Com base nos resultados encontrados, justifica-se o investimento em programas de conservação auditiva particularmente voltados para o controle da emissão de ruídos na fonte. Essas medidas objetivam não apenas a manutenção da saúde auditiva, mas também a diminuição da acidentabilidade dos trabalhadores.

The purpose of this study was to verify whether occupational noise exposure is a significant risk factor for work accidents in the city of Piracicaba, São Paulo State, Brazil. This hospital-based case-control study included 600 workers aged 15-60 who suffered typical occupational accidents between May and October 2004 and were seen at the Piracicaba Orthopedics and Trauma Center. The control group comprised 822 workers, aged 15-60, who were also seen at the Center, and either had a non-occupational accident or were accompanying someone who had suffered an accident. A multiple logistic regression model was adjusted with work accident as an independent variable, controlled by covariables of interest such as noise exposure. The risk of having a work accident was about twice as high among workers exposed to noise, after controlling for several covariables. Occupational noise exposure not only affected auditory health status but was also a risk factor for work accidents.

Hearing loss and tinnitus impact the lives of workers in every instance of their lives. Aim: this paper aims to investigate the existence of a dose-response relationship between hearing loss and tinnitus by determining whether higher levels of hearing loss can be associated with increased tinnitus-related discomfort. Materials and method: this cross-sectional case study assessed 284 workers exposed to occupational noise through pure tone audiometry. Test results were categorized as defined by Merluzzi. Individuals complaining of tinnitus answered the adapted and validated Brazilian Portuguese version of the Tinnitus Handicap Inventory. A generalized linear model was adjusted for binomial data to test the interaction between these factors. Results: over 60% of the ears analyzed had hearing loss, while more than 46% of them had tinnitus. Tinnitus prevalence and risk rates increased as pure tone audiometry results got worse. The association between both, considering all hearing loss degrees, was statistically significant. Conclusion: the results point to a statistical association between hearing loss and tinnitus; the greater the hearing loss, the greater the discomfort introduced by tinnitus. 2009 © Revista Brasileira de Otorrinolaringologia.

INTRODUÇÃO: A audição é um dos sentidos mais importantes para o ser humano, e seu funcionamento está interligado à sua produtividade, o que não é diferente aos músicos, já que ela é de suma importância para a qualidade de seu trabalho e permanência na carreira. O desenvolvimento de um programa de prevenção de perdas auditivas tem por objetivo modificar o comportamento dos músicos em relação à sua audição, uma vez que, constantemente, estão expostos a níveis de pressão sonora elevados e ao surgimento de lesões irreversíveis. Contudo, se medidas preventivas não forem realizadas corretamente, as exposições dos músicos frente à intensidade sonora elevada podem trazer prejuízos à saúde e alguns destes, irreversíveis como, a Perda Auditiva Induzida por Níveis de Pressão Sonora Elevados (PAINPSE) ou Perda Auditiva Induzida por Música (PAIM). OBJETIVO GERAL: submeter os músicos ao programa de prevenção de perdas auditivas (PPPA) e verificar sua eficácia. MATERIAL E MÉTODOS: Participaram componentes de quatro bandas musicais, correspondendo a um total de 16 participantes. Esses membros foram submetidos ao Programa de Prevenção de Perdas Auditivas (PPPA) que engloba as seguintes etapas: (1) medição do nível de pressão sonora no ensaio e show; (2) entrevista específica, Audiometria Tonal Liminar e de Altas Frequências, Logoaudiometria, Imitanciometria e Emissões Otoacústicas por estímulo Transientes e Produto de Distorção; (3) orientação sobre a utilização do Equipamento de Proteção Individual (EPI); e (4) a realização de medidas educativas por meio de workshops. RESULTADOS: O Nível de Pressão Sonora (NPS) durante os ensaios e apresentações/shows, encontram-se elevados, sintomas não auditivos estão presentes em 68, 75% da população total da amostra, presença de zumbido após o show em 100% da amostra; maiores dificuldades de compreensão de fala no ruído nos músicos que tocam baixo (75%). Ao traçar o perfil audiológico do músico foram encontrados: maiores médias dos limiares audiológicos por frequência das bandas estudadas em 500Hz e 3KHz (B1), 3KHz e 4KHz (B2), 3KHz, 4KHz e 6KHz (B3) e em 3KHz (B4); as maiores médias dos limiares audiológicos por frequência dos instrumentos estudados foram em 3KHz, 4KHz e 6KHz (voz), 3KHz e 4KHz (guitarra), 3KHz, 4KHz e 6KHz (baixo) e 3KHz, 4KHz (bateria); presença de entalhe nas frequências de 2KHz, 4KHz, 6KHz e 8KHz na audiometria tonal liminar; já na audiometria de altas frequências em todas as frequências apareceram ao menos um caso, reflexos ausentes em 4KHz (ipsilateral e contralateral); ausência de resposta em 4KHz para todos os baixistas bilateralmente (100%) quando pesquisado EOE por estímulo transiente e na produto de distorção foram encontradas ausência de respostas em 50% da amostra na frequência de 6KHz, sendo assim pesquisada a curva de crescimento (dp growth rate) aparecendo resposta em 75dB em quase 100% dos casos em que houve necessidade de sua realização. Quanto aos achados obtidos da avaliação realizada pelos participantes (músicos) referente ao website, os resultados mostraram que o mesmo atende às necessidades propostas, ou seja, a promoção da saúde auditiva em músicos. CONCLUSÃO: Existe a necessidade de serem tomadas medidas preventivas e a inserção dos músicos em um Programa de Prevenção de Perdas Auditivas (PPPA) a fim de proporcioná-los maiores condições de qualidade de vida e em seu trabalho, já que necessitam da sua audição para desempenhar com eficácia suas atividades e se manter no mercado de trabalho atuando como músico.
