Erythema induratum of Bazin is a disease that usually affects women, in whom erythematous subcutaneous nodules and plaques appear on the posterior part of the lower extremities, some of which ulcerate. In many countries, tuberculosis is still the main etiologic factor. We report a case of a 40-year-old woman who presented a course of protracted and recurrent episodes over five years of cutaneous lesions on her legs. These tend to involute, but new crops appear at irregular intervals. It was painful, erythematous-violaceous nodules, some of which drained a reddish secretion. The histopathologic features of the lesions demonstrated inflammatory infiltration, with predominance of neutrophils in dermis and hypodermis, necrotizing vasculitis in the arterioles and septal fibrosis. There was no granuloma. The Ziehl-Neelsen stain did not revealed acid-fast bacilli, and the culture of biopsy specimen was negative. The tuberculin skin test was strongly positive (17 mm). The chest X-ray was normal. Few months later she presented adynamia and urinary complaints, such as polacyuria and dysuria. It has been done an urynalysis, which demonstrated acid pH urine, sterile pyuria and microscopic hematuria. It was then raised the diagnostic hypothesis of renal tuberculosis. The urine culture for M. tuberculosis was positive in two out of ten samples. The treatment was instituted with rifampin, isoniazid and pyrazinamide, with complete regression. This case illustrates a clear association between erythema induratum and renal tuberculosis, demonstrated by the remission of the cutaneous lesions after the treatment of the renal tuberculosis.

Abnormalities of renal function have been demonstrated inpatients with visceral leishmaniasis; although there was a trend toward normalization following antiparasitic therapy, some abnormalities persisted. With thepurpose of studying the long- term clinical course of renal involvement in visceral leishmaniasis, 32 patients with a diagnosis of this parasitic disease were evaluated in the endemic area and at least 6 months after the clinical cure of the disease and compared with a control group of 28 individuals. No patient had a history or clinical findings suggestive of renal disease and all were normotensive. Laboratory evaluation was normal in all except 3 patients with abnormal urinalysis. Mild proteinuria and microscopic hematuria were seen in a single urinalysis in one patient (although three other urinalysis were normal), and leucocyturia in two female patients. It was concluded that the renal involvement in visceral leishmaniasis is mild and transient, with normal renal function observed on long-term follow-up after cure of the parasitic infection.

Introducción: El síndrome nefrótico idiopático es una entidad con una tasa de prevalencia de 16/100.000 niños, en la cual ocurre pérdida de proteínas a través del filtro glomerular; la proteinuria > 40mg/sc/hora, se acompaña de edema, hipoproteinemia, albumina < 2,5g/dL. La ausencia de datos de prevalencia de nefropatía de cambios mínimos en nuestro medio limita la perspectiva real para lograr un manejo integral de nuestros niños y el enfoque a seguir por parte del grupo de pediatría. Materiales y métodos: Estudio de corte transversal descriptivo, se revisan historias clínicas de los niños con síndrome nefrótico idiopático con biopsia renal, que asistieron a la consulta de nefrología pediátrica en la Fundación Cardio Infantil durante un período de 14 años. Resultados: La prevalencia de nefropatía de cambios mínimos en nuestro subgrupo de pacientes con biopsia renal es de 24,2%. En esta, se presentaron 50% con hematuria macroscópica y 43,7% con hematuria microscópica. La insuficiencia renal crónica se presentó en un sólo paciente con 6,25% y la corticoresistencia en 3 pacientes con 18,7%. Discusión: La prevalencia de nefropatía de cambios mínimos en nuestra población es la tercera parte de lo reportado en la literatura mundial en población general con síndrome nefrótico idiopático. Esta prevalencia menor en nuestro estudio se puede deber posiblemente por tratarse la población de nuestro estudio un subgrupo de pacientes con indicación de biopsia renal además de ser la Fundación Cardio Infantil, central de referencia que llegan remitidos patologías más complejas.

O prognóstico das crianças com tumores sólidos malignos recidivados ou refratários permanece desfavorável apesar dos avanços significativos alcançados nos últimos anos em Oncologia Pediátrica. Estudos visando identificar opções terapêuticas mais eficazes torna-se, portanto, de fundamental importância para estes pacientes. Existem evidências na literatura que demonstram que as drogas utilizadas neste estudo tem, quando usadas isoladamente ou em combinação, atividade em neoplasias malignas recidivadas ou refratárias. A Carboplatina (C) apresenta respostas objetivas em um grande número de pacientes pediátricos com câncer, assim como a Ifosfamida (I) e o Etoposide (E). A combinação destas 3 drogas, em um regime que passaremos a designar como ICE, tem potencial de aumentar os índices de resposta, embora aumente também os riscos de toxicidade. O objetivo principal deste estudo foi avaliar a resposta e a toxicidade deste regime em pacientes com tumores sólidos malignos recidivados ou refratários diagnosticados antes dos 18 anos de idade. O ICE consistiu de Ifosfamida na dose 3g/m2/dia IV por 3 dias consecutivos associada a mesna como uroprotetor, em doses equivalentes, Etoposide 160 mg/m2/dia IV por 3 dias consecutivos e Carboplatina 400 mg/m2/dia IV durante 2 dias. Os ciclos foram repetidos com intervalos de 21 a 28 dias. Foram revisados 21 prontuários de pacientes tratados com este regime, entre julho de 1996 a novembro de 2000. Todos os pacientes foram avaliados para toxicidade e 19 pacientes foram avaliados para resposta. Um total de 93 ciclos de ICE foram administrados. A média dos ciclos de ICE recebidos foi de 4,4 (1-8). Os pacientes receberam um máximo de 8 ciclos. Todos os pacientes incluídos no estudo, receberam no mínimo 1 esquema quimioterápico prévio. A taxa de resposta completa + parcial foi de 52,6%. Os efeitos tóxicos incluíram mielossupressão, febre, naúseas ou vômitos, nefrotoxicidade, leve disfunção hepática e neurotoxicidade. Em 78% dos ciclos houve neutropenia grau 4 (contagem de neutrófilos menor de 500/microlitro). Trombocitopenia graus 3 e 4 foi observado em 73,1% dos ciclos administrados e em 82% destes foram necessárias transfusões de plaquetas. Anemia grau 3 a 4 ocorreu em 61,2% dos ciclos e em 75 (80,6%) dos 93 ciclos administrados foi necessário transfusão de glóbulos vermelhos. Nenhum dos pacientes apresentou hematúria macroscópica e em 19,3 % dos ciclos houve hematúria microscópica. Duas crianças apresentaram nefrotoxicidade tubular renal. Em conclusão, este estudo mostra que o ICE é uma combinação ativa em crianças com tumores sólidos refratários/recidivados. Embora esteja associado a mielossupressão severa, a incidência de infecção encontra-se dentro de índices considerados aceitáveis para este grupo de pacientes. O dano tubular renal é a toxicidade não hematológica mais significativa e, portanto, recomenda-se cuidados e monitorização da função renal durante o período de tratamento. Embora o uso do ICE seja factível mesmo sem o uso de fatores de crescimento hematopoiéticos em pacientes previamente submetidos a quimioterapia, a maioria deles necessita terapia de suporte, principalmente de transfusões de hemoderivados e antimicrobianos. A identificação de pacientes e patologias com maior índice de respostas requer a realização de estudos no futuro com maior número de pacientes a nível multi-institucional.

OBJETIVOS: Avaliar a associação entre os parâmetros clinicolaboratoriais e alteração morfológica de biópsias renais em crianças com síndrome nefrótica. MÉTODOS: Os dados foram obtidos dos prontuários médicos de 43 crianças com síndrome nefrótica submetidas a biópsia renal. RESULTADOS: Vinte e oito pacientes eram do sexo masculino (65,1%), idades entre 1,4 a 12 anos (média de 4,7±3,2). Quarenta e dois pacientes (97,7%) apresentaram edema; 83,7%, oligúria e 32,5%, hipertensão arterial. A média de proteinúria foi 15,3g/1,73m²SC/dia e 55,8% apresentaram hematúria microscópica. As biópsias renais mostraram: glomerulonefrite proliferativa mesangial (GNPM) em 37,2%, glomeruloesclerose segmentar e focal (GESF) em 27,9%, alterações glomerulares mínimas (LM) em 25,6%, glomerulonefrite membranoproliferativa (GNMP) em 7% e glomerulonefrite membranosa (GNM) em 2,3%. Vinte e seis pacientes (60,5%) apresentaram resistência ao corticosteróide. Idade, sexo, hipertensão arterial, oligúria, uréia e creatinina séricas não mostraram diferenças estatísticas significativas entre os pacientes com GNPM, GESF e LM. Os pacientes com GNPM e GESF apresentaram maior freqüência de hematúria microscópica (p < 0,003 e p < 0,03; respectivamente). Os pacientes com GESF apresentaram maiores níveis de proteinúria (p < 0,01 versus LM e p < 0,05 versus GNPM). Os pacientes com LM apresentaram sensibilidade ao corticosteróide (p < 0,001 versus GESF e p = 0,047 versus GNPM). CONCLUSÕES: Sexo, idade, presença de hipertensão arterial ou oligúria e níveis séricos de uréia e creatinina não auxiliaram na diferenciação entre pacientes com GNPM, GESF e LM. Os pacientes com LM apresentaram sensibilidade a corticosteróides e menos probabilidade de apresentar hematúria microscópica. Pacientes com GESF apresentaram maiores níveis de proteinúria.

OBJECTIVE: This study sought to outline the clinical and laboratory characteristics of minimal change disease in adolescents and adults and establish the clinical and laboratory characteristics of relapsing and non-relapsing patients.METHODS: We retrospectively evaluated patients with confirmed diagnoses of minimal change disease by renal biopsy from 1979 to 2009; the patients were aged >13 years and had minimum 1-year follow-ups.RESULTS: Sixty-three patients with a median age (at diagnosis) of 34 (23-49) years were studied, including 23 males and 40 females. At diagnosis, eight (12.7%) patients presented with microscopic hematuria, 17 (27%) with hypertension and 17 (27%) with acute kidney injury. After the initial treatment, 55 (87.3%) patients showed complete remission, six (9.5%) showed partial remission and two (3.1%) were nonresponders. Disease relapse was observed in 34 (54%) patients who were initial responders (n = 61). In a comparison between the relapsing patients (n = 34) and the non-relapsing patients (n = 27), only proteinuria at diagnosis showed any significant difference (8.8 (7.1-12.0) vs. 6.0 (3.6-7.3) g/day, respectively, p = 0.001). Proteinuria greater than 7 g/day at the initial screening was associated with relapsing disease.CONCLUSIONS: In conclusion, minimal change disease in adults may sometimes present concurrently with hematuria, hypertension, and acute kidney injury. The relapsing pattern in our patients was associated with basal proteinuria over 7 g/day.

Post-streptococcal vasculitis is not awell-recognized non-suppurative complication of Group A streptococcal infections. Seventeen-year-old male was admitted with palpable purpura in the limbs, ankle swelling, microscopic hematuria and elevated inflammation markers. After short remission period, an extensive exudative pleural effusion was identified. High anti-streptococcal antibodies titers were found. This report illustrates an atypical and rare presentation of a post-streptococcal syndrome.

This in situ study investigated, using scanning electron microscopy, the effect of stimulated saliva on the enamel surface of bovine and human substrates submitted to erosion followed by brushing abrasion immediately or after one hour. During 2 experimental 7-day crossover phases, 9 previously selected volunteers wore intraoral palatal devices, with 12 enamel specimens (6 human and 6 bovine). In the first phase, the volunteers immersed the device for 5 minutes in 150 ml of a cola drink, 4 times a day (8h00, 12h00, 16h00 and 20h00). Immediately after the immersions, no treatment was performed in 4 specimens (ERO), 4 other specimens were immediately brushed (0 min) using a fluoride dentifrice and the device was replaced into the mouth. After 60 min, the other 4 specimens were brushed. In the second phase, the procedures were repeated but, after the immersions, the volunteers stimulated the salivary flow rate by chewing a sugar-free gum for 30 min. Enamel superficial alterations of all specimens were then evaluated using a scanning electron microscope. Enamel prism core dissolution was seen on the surfaces submitted to erosion, while on those submitted to erosion and to abrasion (both at 0 and 60 min) a more homogeneous enamel surface was observed, probably due to the removal of the altered superficial prism layer. For all the other variables - enamel substrate and salivary stimulation -, the microscopic pattern of the enamel specimens was similar.

McCune-Albright syndrome is characterized by the triad café-au-lait cutaneous spots, polyostotic fibrous dysplasia and endocrinopathies. This article presents two cases of McCune-Albright syndrome in a middle-aged woman and a young girl. Both patients presented café-au-lait spots on the face and other parts of the body and expansion of the mandible with radiopaque-radiolucent areas with ground-glass radiographic appearance, and were diagnosed as having fibrous dysplasia and endocrine disorders. The patient of Case 1 had fibrous dysplasia on the upper and lower limbs, thorax, face and cranium, early puberty, hyperglycemia, hyperthyroidism and high serum alkaline phosphatase levels. The patient of Case 2 presented lesions on the upper limbs and evident endocrine disorders. In both cases presented in this article, the initial exam was made because of the mandibular lesion. However, a diagnosis of fibrous dysplasia must lead to investigation of the involvement of other bones, characterizing polyostotic fibrous dysplasia, which is manifested in a number of diseases. An accurate differential diagnosis is mandatory to determine the best treatment approach for each case.

Background and Purpose: Chronic unilateral hematuria is characterized by intermittent or continuous gross hematuria that cannot be diagnosed using standard radiology and hematology methods. In the past, it was managed with partial or total nephrectomy. In the age of minimally invasive procedures, however, endoscopy has enabled more accurate diagnosis and management. We analyzed our experience with transurethral ureterorenoscopy using a flexible ureteroscope to determine the feasibility and success of endoscopic management of renal hematuria. Patients and Methods: We reviewed the records of 13 patients who presented with chronic unilateral hematuria, in whom radiologic and laboratory tests failed to reveal the source of bleeding. In the cases in which the lesion was identified, after complete inspection of the collecting systems, the bleeding site was treated ureteroscopically with a holmium: yttrium-aluminum-garnet ( YAG) laser. Results: Follow-up ranged from 4 to 60 months ( mean 26 mos). During the follow-up of the 13 patients, 11 remained symptom-free, with only one session of flexible ureterorenoscopy necessary. Relapse occurred in two patients after 4 months and 6 months, respectively; during a second session of flexible ureteroscopy, the bleeding site was successfully identified and cauterized with a holmium: YAG laser. No surgical complications occurred. Conclusions: Conservative treatment of patients with chronic unilateral hematuria should always be considered. Laser ureteroscopic treatment is an excellent method and should be considered as the first option for the management of chronic unilateral hematuria.

In this work, we report a systematic investigation of upconversion losses and their effects on fluorescence quantum efficiency and fractional thermal loading in Nd(3+)-doped fluoride glasses. The energy transfer upconversion (gamma(up)) parameter, which describes upconversion losses, was experimentally determined using different methods: thermal lens (TL) technique and steady state luminescence (SSL) measurements. Additionally, the upconversion parameter was also obtained from energy transfer models and excited state absorption measurements. The results reveal that the microscopic treatment provided by the energy transfer models is similar to the macroscopic ones achieved from the TL and SSL measurements because similar gamma(up) parameters were obtained. Besides, the achieved results also point out the migration-assisted energy transfer according to diffusion-limited regime rather than hopping regime as responsible for the upconversion losses in Nd-doped glasses. The solvation effect of the ionic liquid 1-N-butyl-3-methylimidazolium hexafluorophosphate on nucleophilic substitution reactions of halides toward the aliphatic carbon of methyl p-nitrobenzenesulfonate (pNBS) was investigated by computer simulations. The calculations were performed by using a hybrid quantum-mechanical/molecular-mechanical (QM/MM) methodology. A semiempirical Hamiltonian was first parametrized on the basis of comparison with ab initio calculations for Cl(-) and Br(-) reaction with pNBS at gas phase. In condensed phase, free energy profiles were obtained for both reactions. The calculated reaction barriers are in agreement with experiment. The structure of species solvated by the ionic liquid was followed along the reaction progress from the reagents, through the transition state, to the final products. The simulations indicate that this substitution reaction in the ionic liquid is slower than in nonpolar molecular solvents proper to significant stabilization of the halide anion by the ionic liquid in comparison with the transition state with delocalized charge. Solute-solvent interactions in the first solvation shell contain several hydrogen bonds that are formed or broken in response to charge density variation along the reaction coordinate. The detailed structural analysis can be used to rationalize the design of new ionic liquids with tailored solvation properties. (c) 2008 American Institute of Physics.
