Linoleic acid (LA) is a major constituent of low-density lipoproteins. An essential fatty acid, LA is a polyunsaturated fatty acid, which is oxidised by endogenous enzymes and reactive oxygen species in the circulation. Increased levels of low-density lipoproteins coupled with oxidative stress and lack of antioxidants drive the oxidative processes. This results in synthesis of a range of oxidised derivatives, which play a vital role in regulation of inflammatory processes. The derivatives of LA include, hydroxyoctadecadienoic acids, oxo-​octadecadienoic acids, epoxy octadecadecenoic acid and epoxy-keto-octadecenoic acids. In this review, we examine the role of LA derivatives and their actions on regulation of inflammation relevant to metabolic processes associated with atherogenesis and cancer. The processes affected by LA derivatives include, alteration of airway smooth muscles and vascular wall, affecting sensitivity to pain, and regulating endogenous steroid hormones associated with metabolic syndrome. LA derivatives alter cell adhesion molecules, this initial step, is pivotal in regulating inflammatory processes involving transcription factor peroxisome proliferator-activated receptor pathways, thus, leading to alteration of metabolic processes. The derivatives are known to elicit pleiotropic effects that are either beneficial or detrimental in nature hence making it difficult to determine the exact role of these derivatives in the progress of an assumed target disorder. The key may lie in understanding the role of these derivatives at various stages of development of a disorder. Novel pharmacological approaches in altering the synthesis or introduction of synthesised LA derivatives could possibly help drive processes that could regulate inflammation in a beneficial manner. Protein-energy wasting (PEW) is commonly seen in patients with chronic kidney disease (CKD). The condition is characterised by chronic, systemic low-grade inflammation which affects nutritional status by a variety of mechanisms including reducing appetite and food intake and increasing muscle catabolism. PEW is linked with co-morbidities such as cardiovascular disease, and is associated with lower quality of life, increased hospitalisations and a 6-fold increase in risk of death1. Significant gender differences have been found in the severity and effects of several markers of PEW. There have been limited studies testing the ability of anti-inflammatory agents or nutritional interventions to reduce the effects of PEW in dialysis patients. This thesis makes a significant contribution to the understanding of PEW in dialysis patients. It advances understanding of measurement techniques for two of the key components, appetite and inflammation, and explores the effect of fish oil, an anti-inflammatory agent, on markers of PEW in dialysis patients. The first part of the thesis consists of two methodological studies conducted using baseline data. The first study aims to validate retrospective ratings of hunger, desire to eat and fullness on visual analog scales (VAS) (paper and pen and electronic) as a new method of measuring appetite in dialysis patients. The second methodological study aims to assess the ability of a variety of methods available in routine practice to detect the presence of inflammation. The second part of the thesis aims to explore the effect of 12 weeks supplementation with 2g per day of Eicosapentaenoic Acid (EPA), a longchain fatty acid found in fish oil, on markers of PEW. A combination of biomarkers and psychomarkers of appetite and inflammation are the main outcomes being explored, with nutritional status, dietary intake and quality of life included as secondary outcomes. A lead in phase of 3 months prior to baseline was used so that each person acts as their own historical control. The study also examines whether there are gender differences in response to the treatment. Being an exploratory study, an important part of the work is to test the feasibility of the intervention, thus the level of adherence and factors associated with adherence are also presented. The studies were conducted at the hemodialysis unit of the Wesley Hospital. Participants met the following criteria: adult, stage 5 CKD on hemodialysis for at least 3 months, not expected to receive a transplant or switch to another dialysis modality during the study, absence of intellectual impairment or mental illness impairing ability to follow instructions or complete the intervention. A range of intermediate, clinical and patient-centred outcome measures were collected at baseline and 12 weeks. Inflammation was measured using five biomarkers: c-reactive protein (CRP), interleukin-6 (IL6), intercellular adhesion molecule (sICAM-1), vascular cell adhesion molecule (sVCAM-1) and white cell count (WCC). Subjective appetite was measured using the first question from the Appetite and Dietary Assessment (ADAT) tool and VAS for measurements of hunger, desire to eat and fullness. A novel feature of the study was the assessment of the appetite peptides leptin, ghrelin and peptide YY as biomarkers of appetite. Nutritional status/inflammation was assessed using the Malnutrition-Inflammation Score (MIS) and the Patient-Generated Subjective Global Assessment (PG-SGA). Dietary intake was measured using 3-day records. Quality of life was measured using the Kidney Disease Quality of Life Short Form version 1.3 (KDQOL-SF™ v1.3 © RAND University), which combines the Short-Form 36 (SF36) with a kidney-disease specific module2. A smaller range of these variables was available for analysis during the control phase (CRP, ADAT, dietary intake and nutritional status). Statistical analysis was carried out using SPSS version 14 (SPSS Inc, Chicago IL, USA). Analysis of the first part of the thesis involved descriptive and bivariate statistics, as well as Bland-Altman plots to assess agreement between methods, and sensitivity analysis/ROC curves to test the ability of methods to predict the presence of inflammation. The unadjusted (paired ttests) and adjusted (linear mixed model) change over time is presented for the main outcome variables of inflammation and appetite. Results are shown for the whole group followed by analyses according to gender and adherence to treatment. Due to the exploratory nature of the study, trends and clinical significance were considered as important as statistical significance. Twenty-eight patients (mean age 61±17y, 50% male, dialysis vintage 19.5 (4- 101) months) underwent baseline assessment. Seven out of 28 patients (25%) reported sub-optimal appetite (self-reported as fair, poor or very poor) despite all being well nourished (100% SGA A). Using the VAS, ratings of hunger, but not desire to eat or fullness, were significantly (p<0.05) associated with a range of relevant clinical variables including age (r=-0.376), comorbidities (r=-0.380) nutritional status (PG-SGA score, r=-0.451), inflammatory markers (CRP r=-0.383; sICAM-1 r=-0.387) and seven domains of quality of life. Patients expressed a preference for the paper and pen method of administering VAS. None of the tools (appetite, MIS, PG-SGA, albumin or iron) showed an acceptable ability to detect patients who are inflamed. It is recommended that CRP should be tested more frequently as a matter of course rather than seeking alternative methods of measuring inflammation. 27 patients completed the 12 week intervention. 20 patients were considered adherent based on changes in % plasma EPA, which rose from 1.3 (0.94)% to 5.2 (1.1)%, p<0.001, in this group. The major barriers to adherence were forgetting to take the tablets as well as their size. At 12 weeks, inflammatory markers remained steady apart from the white cell count which decreased (7.6(2.5) vs 7.0(2.2) x109/L, p=0.058) and sVCAM-1 which increased (1685(654) vs 2249(925) ng/mL, p=0.001). Subjective appetite using VAS increased (51mm to 57mm, +12%) and there was a trend towards reduction in peptide YY (660(31) vs 600(30) pg/mL, p=0.078). There were some gender differences apparent, with the following adjusted change between baseline and week 12: CRP (males -3% vs females +17%, p=0.19), IL6 (males +17% vs females +48%, p=0.77), sICAM-1 (males -5% vs females +11%, p=0.07), sVCAM-1 (males +54% vs females +19%, p=0.08) and hunger ratings (males 20% vs females -5%, p=0.18). On balance, males experienced a maintainence or reduction in three inflammatory markers and an improvement in hunger ratings, and therefore appeared to have responded better to the intervention. Compared to those who didn’t adhere, adherent patients maintained weight (mean(SE) change: +0.5(1.6) vs - 0.8(1.2) kg, p=0.052) and fat-free mass (-0.1 (1.6) vs -1.8 (1.8) kg, p=0.045). There was no difference in change between the intervention and control phase for CRP, appetite, nutritional status or dietary intake. The thesis makes a significant contribution to the evidence base for understanding of PEW in dialysis patients. It has advanced knowledge of methods of assessing inflammation and appetite. Retrospective ratings of hunger on a VAS appear to be a valid method of assessing appetite although samples which include patients with very poor appetite are required to confirm this. Supplementation with fish oil appeared to improve subjective appetite and dampen the inflammatory response. The effectiveness of the intervention is influenced by gender and adherence. Males appear to be more responsive to the primary outcome variables than females, and the quality of response is improved with better adherence. These results provide evidence to support future interventions aimed at reducing the effects of PEW in dialysis patients.

La méthylation de l'ADN est l'une des modifications épigénétiques au niveau des îlots CpG. Cette modification épigénétique catalysée par les ADN méthyltransférases (DNMTs) consiste en la méthylation du carbone 5' d’une cytosine ce qui aboutit à la formation de 5-méthylcytosine. La méthylation de l'ADN est clairement impliquée dans l'inactivation des gènes et dans l'empreinte génétique. Elle est modulée par la nutrition, en particulier par les donneurs de méthyle et par une restriction protéique. Ces modifications épigénétiques persistent plus tard dans la vie et conduisent au développement de nombreuses pathologies telles que le syndrome métabolique et le diabète de type 2. En fait, de nombreux gènes clés subissent une modification de leur état de méthylation en présence des composants du syndrome métabolique. Cela montre que la méthylation de l'ADN est un processus important dans l'étiologie du syndrome métabolique. Le premier travail de ce doctorat a porté sur la rédaction d’un article de revue qui a examiné le cadre central du syndrome métabolique et analyser le rôle des modifications épigénétiques susceptibles d'influer sur l'apparition du stress oxydant et des complications cardiométaboliques. D’autre part, les cellules intestinales Caco-2/15, qui ont la capacité de se différencier et d’acquérir les caractéristiques physiologiques de l'intestin grêle, ont été utilisées et traitées avec du Fer-Ascorbate pour induire un stress oxydant. Le Fer-Ascorbate a induit une augmentation significative de l’inflammation et de la peroxydation des lipides (malondialdehyde) ainsi que des altérations de de la défense antioxydante (SOD2 et GPx) accompagnées de modifications épigénétiques. De plus, la pré-incubation des cellules avec de la 5-aza-2'-désoxycytidine, un agent de déméthylation et/ou l’antioxydant Trolox a normalisé la défense antioxydante, réduit la peroxydation des lipides et prévenu l'inflammation. Ce premier travail a démontré que les modifications du redox et l’inflammation induites par le Fer-Ascorbate peuvent impliquer des changements épigénétiques, plus particulièrement des changements dans la méthylation de l’ADN. Pour mieux définir l’impact du stress oxydant au niveau nutritionnel, des cochons d’Inde âgés de trois jours ont été séparés en trois groupes : 1) Témoins: alimentation régulière; 2) Nutrition parentérale (NP) 3) H2O2 : Témoins + 350 uM H2O2. Après quatre jours, pour un groupe, les perfusions ont été stoppées et les animaux sacrifiés pour la collecte des foies. Pour l’autre groupe d’animaux, les perfusions ont été arrêtées et les animaux ont eu un accès libre à une alimentation régulière jusqu'à la fin de l’étude, huit semaines plus tard où ils ont été sacrifiés pour la collecte des foies. Ceci a démontré qu’à une semaine de vie, l'activité DNMT et les niveaux de 5'-méthyl-2'-désoxycytidine étaient inférieurs pour les groupes NP et H2O2 par rapport aux témoins. A neuf semaines de vie, l’activité DNMT est restée basse pour le groupe NP alors que les niveaux de 5'-méthyl-2'-désoxycytidine étaient plus faibles pour les groupes NP et H2O2 par rapport aux témoins. Ce travail a démontré que l'administration de NP ou de H2O2, tôt dans la vie, induit une hypométhylation de l'ADN persistante en raison d'une inhibition de l'activité DNMT. Finalement, des souris ayant reçu une diète riche en gras et en sucre (HFHS) ont été utilisées comme modèle in vivo de syndrome métabolique. Les souris ont été nourris soit avec un régime standard chow (témoins), soit avec une diète riche en gras et en sucre (HFHS) ou avec une diète HFHS en combinaison avec du GFT505 (30 mg/kg), un double agoniste de PPARα et de PPARδ, pendant 12 semaines. La diète HFHS était efficace à induire un syndrome métabolique étant donnée l’augmentation du poids corporel, du poids hépatique, des adiposités viscérales et sous-cutanées, de l’insensibilité à l’insuline, des lipides plasmatiques et hépatiques, du stress oxydant et de l’inflammation au niveau du foie. Ces perturbations étaient accompagnées d’une déficience dans l’expression des gènes hépatiques PPARα et PPARγ concomitant avec une hyperméthylation de leurs promoteurs respectifs. L’ajout de GFT505 à la diète HFHS a empêché la plupart des effets cardiométaboliques induits par la diète HFHS via la modulation négative de l’hyperméthylation des promoteurs, résultant en l’augmentation de l’expression des gènes hépatiques PPARα et PPARγ. En conclusion, GFT505 exerce des effets métaboliques positifs en améliorant le syndrome métabolique induit par l'alimentation HFHS via des modifications épigénétiques des gènes PPARs. Ensemble, les travaux de cette thèse ont démontré que le stress oxydant provenant de la nutrition induit d’importants changements épigénétiques pouvant conduire au développement du syndrome métabolique. La nutrition apparait donc comme un facteur crucial dans la prévention de la reprogrammation fœtale et du développement du syndrome métabolique. Puisque les mécanismes suggèrent que le stress oxydant agit principalement sur les métabolites du cycle de la méthionine pour altérer l’épigénétique, une supplémentation en ces molécules ainsi qu’en antioxydants permettrait de restaurer l’équilibre redox et épigénétique.

L'athérosclérose est une maladie inflammatoire chronique caractérisée par l'accumulation de cholestérol dans la paroi artérielle et associée à une réponse immunitaire anormale dans laquelle les macrophages jouent un rôle important. Récemment, il a été démontré que les vaisseaux lymphatiques jouent un rôle primordial dans le transport inverse du cholestérol (Martel et al. JCI 2013). L’objectif global de mon stage de maîtrise a été de mieux caractériser la dysfonction lymphatique associée à l’athérosclérose, en étudiant de plus près l’origine physiologique et temporelle de ce mauvais fonctionnement. Notre approche a été d’étudier, depuis l’initiation de l’athérosclérose jusqu’à la progression d’une lésion athérosclérotique tardive, la physiologie des deux constituants principaux qui forment les vaisseaux lymphatiques : les capillaires et collecteurs lymphatiques. En utilisant comme modèle principal des souris Ldlr-/-; hApoB100+/+, nous avons pu démontrer que la dysfonction lymphatique est présente avant même l’apparition de l’athérosclérose, et que cette dysfonction est principalement associée avec un défaut au niveau des vaisseaux collecteurs, limitant ainsi le transport de la lymphe des tissus périphériques vers le sang. De plus, nous avons démontré pour la première fois l’expression du récepteur au LDL par les cellules endothéliales lymphatiques. Nos travaux subséquents démontrent que ce défaut de propulsion de la lymphe pourrait être attribuable à l’absence du récepteur au LDL, et que la dysfonction lymphatique observée précocement dans l’athérosclérose peut être limitée par des injections systémiques de VEGF (vascular endothelial growth factor) –C. Ces résultats suggèrent que la caractérisation fonctionnelle de la capacité de pompage des vaisseaux collecteurs serait une condition préalable à la compréhension de l'interaction entre la fonction du système lymphatique et la progression de l'athérosclérose. Ultimement, nos travaux nous ont amené à considérer de nouvelles cibles thérapeutiques potentielles dans la prévention et le traitement de l’athérosclérose.

Introducción: La enfermedad coronaria es la principal causa de muerte en Colombia y el mundo en personas mayores de 45 años (1, 2). Con la variación en los perfiles epidemiológicos se espera que aumente su prevalencia y costo, disminuyendo la mortalidad. En el estudio diagnóstico de enfermedad coronaria, se realizan múltiples pruebas paraclínicas, dentro de estas, el ecocardiograma es muy utilizado y sus recomendaciones de uso se basan en estudios de hace más de 10 años (3). Metodología: Se realizó un estudio de prueba diagnóstica para determinar las características operativas del ecocardiograma transtorácico para el diagnóstico de enfermedad coronaria frente al cateterismo cardiaco en pacientes con dolor torácico. Resultados: Se analizaron 290 pacientes con una edad promedio de 67 años y 68% hombres; el 74.1% tenía hipertensión arterial y el 24.5% de diabetes mellitus. En el 61% de los pacientes se evidenció enfermedad coronaria por angiografía. La sensibilidad del ecocardiograma transtorácico para enfermedad coronaria fue del 70%, especificidad del 55%, valor predictivo negativo del 54% y valor predictivo positivo del 71%; con una efectividad del 65% y una capacidad predictora del 60.6% (p=0.02). Discusión: La anatomía coronaria fue similar a la descrita previamente, siendo las lesiones multivaso presente en un 41% y la lesión de un vaso fue del 22% (4, 5). Teniendo en cuenta la sensibilidad del 70% y especificidad del 55%, la facilidad de acceso, características no invasivas y el bajo costo; nos permiten recomendar utilizarlo como guía diagnóstica en el contexto del dolor torácico en el servicio de urgencias.

Cathepsin S is a protease important in major histocompatibility complex (MHC) class II antigen presentation and also in degrading the extracellular matrix. Studies, most of them experimental, have shown that cathepsin S is involved in different pathological conditions such as obesity, inflammation, atherosclerosis, diabetes, and cancer.    The overall hypothesis of this report is that high levels of circulating cathepsin S, is a biomarker that reflects pathology induced by inflammation and obesity. The overall aim of this report was to investigate possible associations between circulating cathepsin S, inflammation, glucometabolic disturbance, and its associated diseases in the community. As cathepsin S appears to be a novel risk marker for several pathological conditions, we also wanted to examine the effect of dietary intervention on circulating cathepsin S concentrations.    This thesis is based on data from three community-based cohorts, the Uppsala longitudinal study of adult men (ULSAM), the prospective investigation of the vasculature in Uppsala seniors (PIVUS), and a post-hoc study from the randomized controlled NORDIET trial.    In the first study, we identified a cross-sectional positive association between serum cathepsin S and two markers of cytokine-mediated inflammation, CRP and IL-6. These associations were similar in non-obese individuals. In longitudinal analyses, higher cathepsin S at baseline was associated with higher CRP and IL-6 levels after six years of follow-up. In the second study, we identified a cross-sectional association between increased serum levels of cathepsin S and reduced insulin sensitivity. These associations were similar in non-obese individuals. No significant association was observed between cathepsin S and insulin secretion. In longitudinal analysis, higher cathepsin S levels were associated with an increased risk of developing diabetes during the six-year follow-up. In the third study, we found that higher serum levels of cathepsin S were associated with increased mortality risk. Moreover, in the ULSAM cohort, serum cathepsin S was independently associated with cause-specific mortality from cardiovascular disease and cancer. In the fourth study, we identified that adherence to an ad libitum healthy Nordic diet for 6 weeks slightly decreased the levels of plasma cathepsin S in normal or marginally overweight individuals, relative to the control group. Changes in circulating cathepsin S concentrations were correlated with changes in body weight, LDL-C, and total cholesterol.    Conclusion: This thesis shows that circulating cathepsin S is a biomarker that independently reflects inflammation, insulin resistance, the risk of developing diabetes, and mortality risk. Furthermore, a Nordic diet moderately reduced cathepsin S levels in normal-weight and overweight men and women. This effect may be partially mediated by diet-induced weight loss and possibly by reduced LDL-C concentrations.

OBJECTIVE: This study examines the effects of malondialdehyde (MDA) and uric acid on insulin resistance and atherogenicity in subjects with and without mood disorders, the metabolic syndrome (MetS) and tobacco use disorder (TUD). METHODS: We included 314 subjects with depression and bipolar depression, with and without the MetS and TUD and computed insulin resistance using the updated homeostasis model assessment (HOMA2IR) and atherogenicity using the atherogenic index of plasma (AIP), that is log10 (triglycerides/high density lipoprotein (HDL) cholesterol. RESULTS: HOMA2IR is correlated with body mass index (BMI) and uric acid levels, but not with mood disorders and TUD, while the AIP is positively associated with BMI, mood disorders, TUD, uric acid, MDA and male sex. Uric acid is positively associated with insulin and triglycerides and negatively with HDL cholesterol. MDA is positively associated with triglyceride levels. Comorbid mood disorders and TUD further increase AIP but not insulin resistance. Glucose is positively associated with increasing age, male gender and BMI. DISCUSSION: The results show that mood disorders, TUD and BMI together with elevated levels of uric acid and MDA independently contribute to increased atherogenic potential, while BMI and uric acid are risk factors for insulin resistance. The findings show that mood disorders and TUD are closely related to an increased atherogenic potential but not to insulin resistance or the MetS. Increased uric acid is a highly significant risk factor for insulin resistance and increased atherogenic potential. MDA, a marker of lipid peroxidation, further contributes to different aspects of the atherogenic potential. Mood disorders and TUD increase triglyceride levels, lower HDL cholesterol and are strongly associated with the atherogenic, but not insulin resistance, component of the MetS.

Protein-energy malnutrition is a syndrome in which anaemia together with multivitamin and mineral deficiency may be present. The pathophysiological mechanisms involved have not, however, yet been completely elucidated. The aim of the present study was to evaluate the pathophysiological processes that occur in this anaemia in animals that were submitted to protein-energy malnutrition, in particular with respect to Fe concentration and the proliferative activity of haemopoietic cells. For this, histological, histochemical, cell culture and immunophenotyping techniques were used. Two-month-old male Swiss mice were submitted to protein-energy malnutrition with a low-protein diet (20g/kg) compared with control diet (400 g/kg). When the experimental group had attained a 20% loss of their original body weight, the animals from both groups received, intravenously, 20IU erythropoietin every other day for 14 d. Malnourished animals showed a decrease in red blood cells, Hb concentration and reticulocytopenia, as well as severe bone marrow and splenic atrophy. The results for serum Fe, total Fe-binding capacity, transferrin and erythropoietin in malnourished animals were no different from those of the control animals. Fe reserves in the spleen, liver and bone marrow were found to be greater in the malnourished animals. The mixed colony-forming unit assays revealed a smaller production of granulocyte-macrophage colony-forming units, erythroid burst-forming units, erythroid colony-forming units and CD45, CD117, CD119 and CD71 expression in the bone marrow and spleen cells of malnourished animals. These findings suggest that, in this protein-energy malnutrition model, anaemia is not caused by Fe deficiency or erythropoietin deficiency, but is a result of ineffective erythropoiesis.

Perilipin-1 surrounds lipid droplets in both adipocytes and in atheroma plaque foam cells and controls access of lipases to the lipid core. In hemodialysis (HD) patients, dyslipidemia, malnutrition, inflammation and atherosclerosis are common. Thirty-six HD patients and 28 healthy volunteers were enrolled into the study. Ten HD patients suffered from coronary heart disease (CHD). Perilipin-1, triglycerides, total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol (HDL-C), body mass index, albumin, geriatric nutritional risk index, normalized protein catabolic rate, interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were measured. Perilipin-1 did not differ between HD patients and healthy volunteers. IL-6 and TNF-α were higher in HD patients. The evaluated nutritional markers and the markers of inflammation did not differ between HD patients with high perilipin-1 levels and HD patients with low perilipin-1 levels. Regarding the lipid profile, only HDL-C differed between HD patients with high perilipin-1 levels and HD patients with low perilipin-1 levels, and it was higher in the first subgroup. Perilipin-1 was significantly higher in HD patients without CHD. Perilipin-1 is detectable in the serum of HD patients and it is associated with increased HDL-C and decreased incidence of CHD.

L'athérosclérose est une maladie inflammatoire chronique caractérisée par l'accumulation de cholestérol dans la paroi artérielle et associée à une réponse immunitaire anormale dans laquelle les macrophages jouent un rôle important. Récemment, il a été démontré que les vaisseaux lymphatiques jouent un rôle primordial dans le transport inverse du cholestérol (Martel et al. JCI 2013). L’objectif global de mon stage de maîtrise a été de mieux caractériser la dysfonction lymphatique associée à l’athérosclérose, en étudiant de plus près l’origine physiologique et temporelle de ce mauvais fonctionnement. Notre approche a été d’étudier, depuis l’initiation de l’athérosclérose jusqu’à la progression d’une lésion athérosclérotique tardive, la physiologie des deux constituants principaux qui forment les vaisseaux lymphatiques : les capillaires et collecteurs lymphatiques. En utilisant comme modèle principal des souris Ldlr-/-; hApoB100+/+, nous avons pu démontrer que la dysfonction lymphatique est présente avant même l’apparition de l’athérosclérose, et que cette dysfonction est principalement associée avec un défaut au niveau des vaisseaux collecteurs, limitant ainsi le transport de la lymphe des tissus périphériques vers le sang. De plus, nous avons démontré pour la première fois l’expression du récepteur au LDL par les cellules endothéliales lymphatiques. Nos travaux subséquents démontrent que ce défaut de propulsion de la lymphe pourrait être attribuable à l’absence du récepteur au LDL, et que la dysfonction lymphatique observée précocement dans l’athérosclérose peut être limitée par des injections systémiques de VEGF (vascular endothelial growth factor) –C. Ces résultats suggèrent que la caractérisation fonctionnelle de la capacité de pompage des vaisseaux collecteurs serait une condition préalable à la compréhension de l'interaction entre la fonction du système lymphatique et la progression de l'athérosclérose. Ultimement, nos travaux nous ont amené à considérer de nouvelles cibles thérapeutiques potentielles dans la prévention et le traitement de l’athérosclérose.

O contingente de idosos iniciando tratamento de diálise vem aumentado de maneira importante desde o ano de 2000, conforme mostrado pelo United States Renal Data System. Dados nacionais demonstram que cerca de 31,5% dos pacientes em diálise tem idade acima de 65 anos, segundo o Censo Brasileiro de Diálise de 2011. Estes dados alertam para a importância no cuidado da saúde do idoso em diálise. O cuidado nutricional desse grupo merece especial atenção, uma vez que já foi demonstrado que a prevalência de desnutrição energético-proteica nos pacientes idosos em diálise é significantemente maior do que a de adultos jovens em diálise. Nesse sentido, faz-se importante desenvolver estudos com instrumentos que avaliem o estado nutricional desse grupo. Os questionários de avaliação subjetiva global de 7 pontos (ASG-7p) e o malnutrition inflammation score (MIS) têm sido bastante empregados em estudos incluindo pacientes em diálise. Contudo, estes instrumentos estão originalmente na língua Inglesa, e para o seu uso em nosso meio, é necessário a tradução por meio da adaptação transcultural. Porém, até o momento, não há estudos que tenham realizado a tradução transcultural desses questionários para a língua Portuguesa. Deste modo, o objetivo deste estudo foi realizar a adaptação transcultural para o português de dois instrumentos de avaliação do estado nutricional em pacientes idosos em tratamento crônico de hemodiálise (HD). O estudo é do tipo observacional e seccional. A adaptação transcultural consistiu na tradução para a língua Portuguesa da ASG-7p e MIS pelo método da retrotradução, avaliação da equivalência semântica, confiabilidade e validade de construto. Para tanto, 101 pacientes idosos em HD foram avaliados. Ambos questionários apresentaram alto grau de similaridade semântica. A consistência interna apresentou um valor de α de Cronbach para a ASG-7p de 0,72 (satisfatório) e para o MIS de 0,53 (não satisfatório). A ASG-7p apresentou discreta reprodutibilidade intra-avaliador e moderada reprodutibilidade interavaliador; e para o MIS tanto a reprodutibilidade inter quanto intra-avaliador foi indicativo de forte reprodutibilidade. Referente à validade de construto, ao avaliar o estado nutricional pela ASG-7p notou-se que os pacientes classificados como bem nutridos apresentaram medidas de peso corporal, adequação da dobra cutânea tricipital, percentual de gordura corporal, circunferência da panturrilha e creatinina sérica significantemente maiores do que de pacientes desnutridos. Para o MIS, ao comparar os grupo de bem nutridos com o de desnutridos, a albumina sérica, ângulo de fase, percentual de gordura corporal, a adequação da força de preensão manual e da dobra cutânea tricipital e a circunferência da panturrilha apresentaram diferença significante entre os grupos. Em conclusão, os questionários ASG-7p e MIS traduzidos para o português podem ser utilizados na população de idosos em HD.
