Temperature safety profile of laparoscopic devices: Harmonic ACE (ACE), Ligasure V (LV), and plasma trisector (PT)

Background Reports of iatrogenic thermal injuries during laparoscopic surgery using new generation vessel-sealing devices, as well as anecdotal reports of hand burn injuries during hand-assisted surgeries, have evoked questions about the temperature safety profile and the cooling properties of these instruments. Methods This study involved video recording of temperatures generated by different instruments (Harmonic ACE [ACE], Ligasure V [LV], and plasma trisector [PT]) applied according the manufacturers` pre-set settings (ACE setting 3; LV 3 bars, and the PT TR2 50W). The video camera used was the infrared Flex Cam Pro directed to three different types of swine tissue: (1) peritoneum (P), (2) mesenteric vessels (MV), and (3) liver (L). Activation and cooling temperature and time were measured for each instrument. Results The ACE device produced the highest temperatures (195.9 degrees +/- 14.5 degrees C) when applied against the peritoneum, and they were significantly higher than the other instruments (LV = 96.4 degrees +/- 4.1 degrees C, and PT = 87 degrees +/- 2.2 degrees C). The LV and PT consistently yielded temperatures that were < 100 degrees C independent of type of tissue or ""on""/ ""off"" mode. Conversely, the ACE reached temperatures higher than 200 degrees C, with a surprising surge after the instrument was deactivated. Moreover, temperatures were lower when the ACE was applied against thicker tissue (liver). The LV and PT cooling times were virtually equivalent, but the ACE required almost twice as long to cool. Conclusions The ACE increased the peak temperature after deactivation when applied against thick tissue (liver), and the other instruments inconsistently increased peak temperatures after they were turned off, requiring few seconds to cool down. Moreover, the ACE generated very high temperatures (234.5 degrees C) that could harm adjacent tissue or the surgeon`s hand on contact immediately after deactivation. With judicious use, burn injury from these instruments can be prevented during laparoscopic procedures. Because of the high temperatures generated by the ACE device, particular care should be taken when it is used during laparoscopy.

Este trabalho foi realizado com os seguintes objetivos: determinar a evolução e produção dos frutos de pepino (Cucumis sativus L.) var. Aodai em função do desenvolvimento da planta, analisar as concentrações dos nurientes no fruto no ponto de consumo e calcular as quantidades de nutrientes exportados pela colheita dos frutos. Verifica-se que as quantidades de nutrientes exportados através da colheita, por tonelada de frutos no ponto de consumo, correspondem a: K -1.617 g, N-1.050 g; P-200 g; Ca -174 g; Mg-139 g; Fe-1,91 g; Zn -1,21 g; Mn-0,98 g; B-0,87g; Cu-0,52g.

Assessment of systolic and diastolic LV function by MR myocardial tagging.

Heart failure has been divided into several different forms depending on etiology, clinical course and pathophysiology of left ventricular (LV) dysfunction. Systolic and diastolic dysfunction are characterized by a reduced cardiac output with normal (= diastolic dysfunction) or depressed (= systolic dysfunction) LV pump function. New diagnostic techniques such as magnetic resonance imaging (MRI) allow to determine noninvasively LV 3D motion by labelling specific myocardial regions (= myocardial "tagging") with a rectangular or radial grid. From the deformation of this grid rotational and translational motion of the heart can be derived. A "wringing" motion of the left ventricle has been described during systole which includes a clockwise rotation at the base and a counterclockwise rotation at the apex. During diastole, an "untwisting" motion has been demonstrated. In the normal heart, diastolic "untwisting" occurs primarily during isovolumic relaxation, analogous to the systolic "wringing" which takes place mainly during isovolumic contraction. A prolongation of the "untwisting" motion was found in the hypertrophied (aortic stenosis) and hibernating myocardium. Thus, heart failure is associated with profound alterations in the mechanical function of the heart which are manifested by changes in systolic "wringing" and diastolic "untwisting" motion.

Strategisten asiakassuhteiden johtaminen - Case: Heath Comm Oy

Tutkielman tavoitteena oli selvittää, miten yritys voi luoda itselleen ja asiakkailleen kilpailuetua johtamalla systemaattisesti asiakassuhteitaan nopeasti muuttuvassa ympäristössä. Pyrkimyksenä oli kartoittaa käytännön tekijöitä, joilla yritys voi luoda asiakkailleen ja tuotteidensa loppukäyttäjille lisäarvoa tunnistamalla sopivimmat asiakkaat ja pyrkimällä pitkiin liikesuhteisiin. Tutkielman teoreettisen osan lähdeaineistona käytettiin pääosin avainasiakasajatteluun, liikesuhteisiin ja niiden johtamiseen sekä asiakkaiden analysointiin liittyvää kirjallisuutta ja artikkeleita. Empiirisen tutkimuksen aineisto kerättiin haastattelemalla toimeksiantajayrityksen asiakkaita, johtohenkilöitä ja myynnin henkilökuntaa. Tutkimuksen lähdeaineistoa täydensi tutkijan oma työkokemus. Empiirinen tutkimus oli laadullinen case-analyysi. Asiakashaastattelujen perusteella muodostettiin käsitys pitkäaikaisiin liikesuhteisiin ja niiden johtamiseen vaikuttavista tekijöistä kohdetoimialalla. Yrityksen sisäisten keskustelujen avulla muodostettiin puolestaan käsitys yrityksen nykytoiminnasta. Vertailun pohjalta esitettiin portfolio-malli yrityksen asiakassuhteiden analysointiin sekä asiakkaan hoitosuunnitelma. Lisäksi esitettiin yleisiä toimenpide-ehdotuksia teorian ja asiakashaastattelujen pohjalta. Asiakkaan saamaan arvoon vaikuttavat tuotteiden käytettävyys ja toimintavarmuus, kommunikaation ja palvelun laatu, toimittajan riittävä asiantuntemus sekä toimitusten nopeus ja luotettavuus. Asiakkaat arvostavat erityisesti vilkasta ja avointa tiedon välitystä. Yritysten väliset sidokset voidaan nähdä asiakasosuutta kasvattavina voimina
