A shift in the activation of pulmonary macrophages characterized by an increase of IL-1, INF-alpha and IL-6 production has been induced in mice infected with Paracoccidioides brasiliensis. It is still unclear whether a functional shift in the resident alveolar macrophage population would be responsible for these observations due to the expression of cell surface molecules. We investigated pulmonary macrophages by flow cytometry from mice treated with P. brasiliensis derivatives by intratracheal route. In vivo labeling with the dye PKH26GL was applied to characterize newly recruited pulmonary macrophages from the bloodstream. Pulmonary macrophages from mice inflamed with P. brasiliensis derivatives showed a high expression of the surface antigens CD11b/CD18 and CD23 among several cellular markers. The expression of these markers indicated a pattern of activation of a subpopulation characterized as CD11b(+) or CD23(+), which was modulated in vitro by IFN-gamma and IL-4. Analysis of monocytes labelled with PKH26GL demonstrated that CD11b(+) cells did infiltrate the lung exhibiting a proinflammatoni pattern of activation, whereas CD23(+) cells were considered to be resident in the lung. These findings may contribute to better understand the pathology of lung inflammation caused by P. brasiliensis infection. (C) 2010 Elsevier GmbH. All rights reserved.

Introdução: O acidente vascular encefálico (AVE) é uma importante e frequente condição de saúde que se repercute na funcionalidade do indivíduo. No sentido de reabilitar a função perdida, é comum o recurso a intervenções de fisioterapia baseado o conceito de Bobath. Como tal, importa verificar, as modificações no âmbito do controlo postural, através da migração do centro de pressão na base de suporte, face à aplicação de uma intervenção segundo abordagem baseada no conceito de Bobath em dois indivíduos com AVE. Métodos e participantes: Foram recrutados dois indivíduos com diagnóstico de AVE num hospital da zona do grande Porto. Dados referentes ao equilíbrio estático na condição de medição “olhos abertos ou fechados” e “calçado ou descalço” foram obtidos através de plataforma de forças, antes e após uma intervenção baseado no conceito de Bobath durante 13 semanas (M0 e M1). Nestes dois momentos foram ainda avaliados a mobilidade, função cognitiva, participação, equilíbrio através do teste Timed Up & Go (TUG) e Timed Up & Go Modificado (TUGM), e das escalas Mini Mental State Examination (MMSE), Postural Assessment for Stroke Scale (PASS), Escala de Berg (EB) e Índice de Barthel Modificado (IBM). Resultados: Os participantes obtiveram em ambos os momentos pontuação máxima no MMSE. Ambos os indivíduos atingiram o valor máximo no IBM em M1 (Mo: A: 78; B: 65). Ambos os indivíduos aumentaram o score entre M0 e M1, relativamente ao PASS (A: M0:21; M1:33; B: M0: 26; M1:34) e EB (A: M0:48; M1:54; B: M0: 30; M1:50). O tempo de realização do TUG e do TUGM diminuíram entre momentos em ambos os indivíduos (respectivamente: A: 15''13'' a 13''27''; B: 24''13'' a 13''88'' e A: 19''08''' a 13''27''; B: 29''60''' a 17''64'''). A área de deslocação do centro de pressão (CP) variou entre momentos em todas as condições de avaliação, sendo menor na condição “olhos abertos e descalço” em ambos os participantes (“olhos abertos e calçado”: A: M0= 1,364, M1=2,796; B: M0=1,892, M1=2,979; “olhos abertos e descalço”: A: M0= 0,758, M1=0,727; B: M0=3,064, M1=1,952; “olhos fechados e calçado”: A: M0= 2,360, M1=2,998; B: M0=2,232, M1= 4,392; “olhos fechados e descalço”: A: M0= 1,347, M1=2,388; B: M0=1,652, M1= 1,016). O desvio padrão das deslocações anteroposteriores variou entre momentos, sendo tendencialmente maior em M1 e na condição “descalço e olhos abertos”(“olhos abertos e calçado”: A: M0= 0,201, M1=0,500; B: M0=0,252, M1=0,310; “olhos abertos e descalço”: A: M0= 0,118, M1=0,165; B: M0=0,282, M1=0,276; “olhos fechados e calçado”: A: M0= 0,308, M1=0,398; B: M0=0,274, M1= 0,471; “olhos fechados e descalço”: A: M0= 0,158 , M1=0,373; B: M0=0,230, M1= 0,172), o desvio padrão das deslocações médio-lateral seguem a mesma tendência (“olhos abertos e calçado”: A: M0= 0,370 , M1=0,473; B: M0=0,454, M1=0,517; “olhos abertos e descalço”: A: M0= 0,354, M1=0,236 ; B: M0=0,584, M1=0,381; “olhos fechados e calçado”: A: M0= 0,425, M1=0,463; B: M0=0,462, M1= 0,583; “olhos fechados e descalço”: A: M0= 0,475, M1=0,416; B: M0=0,389, M1= 0,342). A velocidade de oscilação na direcção antero – posterior variou entre momentos, sendo tendencialmente menor em M1, em ambos os participantes e em todas as condições de avaliação: “olhos abertos e calçado”: A: M0= 0,886 , M1=0,532; B: M0=2,507, M1=01,072; “olhos abertos e descalço”: A: M0= 2,562, M1=3,815 ; B: M0=4,367, M1=0,262; “olhos fechados e calçado”: A: M0= 2,689, M1=1,757; B: M0=2,821, M1= 0,769; “olhos fechados e descalço”: A: M0= 2,984, M1=2,525; B: M0=4,100, M1= 0,265), a velocidade de oscilação na direcção médio – lateral seguem a mesma tendência para as condições de “olhos abertos e calçado”: A: M0= 6,524 , M1=6,218; B: M0=0,467, M1=0,404; “olhos fechados e calçado”: A: M0= 6,387, M1=1,927; B: M0=0,351, M1= 0,505; mas a velocidade de oscilação aumenta para as condições de “olhos abertos e descalço”: A: M0= 3,108, M1=7,806 ; B: M0=1,150, M1=8,054; “olhos fechados e descalço”: A: M0= 3,444, M1=3,839; B: M0=1,434, M1= 7,891). Conclusão: Entre os dois momentos os indivíduos melhoraram a sua mobilidade, equilíbrio, participação e actividades, potencialmente devido à intervenção baseado no conceito de Bobath.

Radanohjaimia käytetään radan poikkisuuntaiseen paikoitukseen sekä oskillointiin ja ne ovat osa paperinvalmistuslinjojen radanhallintajärjestelmiä. Oikein suunnitellulla ja sijoitetulla radanohjaimella voidaan estää haitallisten ratasiirtymien esiintyminen, pienentää hylkymateriaalin määrää sekä parantaa rullauksen laatua. Ohjausperiaatteet sekä niihin liittyvä laitetekniikka on tunnettu jo vuosikymmenten ajan, joskin todelliset radanohjaustarpeet ja -laitteet ovat tähän asti rajoittuneet paino-, tekstiili- ja metalliteollisuudessa käytössä oleville kapeille radoille. Tämän työn tavoitteena oli selvittää 5… 12 metriä leveälle, modernissa päällystyskoneessa kulkevalle paperiradalle soveltuvat ohjausperiaatteet sekä muodostaa radan todennäköisimpiin vauriomuotoihin perustuvat suunnittelukriteerit kahdelle erityyppiselle ohjainkonstruktiolle. Yksitelainen, taivuttava radanohjain soveltuu kohteisiin, joissa ohjainta edeltää suhteellisen pitkä vapaa ratavienti. Kaksitelainen, taittava radanohjain on puolestaan sijoitettavissa huomattavasti lyhyempään ratavientiin. Radanohjauksen teoria pohjautuu pitävän telan ja radan väliseen kohtisuoran tulokulman periaatteeseen, jonka perusteella mikä tahansa yhdensuuntaisuuspoikkeama kahden telan välillä johtaa radan poikkisuuntaiseen siirtymään. Tämän periaatteen pohjalta voidaan dynaamista ohjaustilannetta approksimoida staattisin menetelmin sekä muodostaa kireysmuutoksiin ja aaltoiluun perustuvat geometrian mitoitusperiaatteet ohjaimen ympäristön ratavienneille. Nopean ohjauksen toteutus edellyttää radanohjaimen liikkeen olevan nivelen ympäri tapahtuvaa yhdistettyä translaatio- ja rotaatioliikettä. Ohjainkonstruktiot suunnitellaan siten, että teorian mukaiset optimaaliset liikeradat toteutuvat vaaditulla ohjausnopeudella. Suunnittelua ohjaavat tuotteille asetetut lujuus- ja värähtelykriteerit sekä aiheeseen liittyvät koneturvallisuusstandardit. Konstruointi suoritetaan järjestelmällisen tuotesuunnitteluprosessin vaiheiden ja menetelmien mukaisesti.

La mesure du déjettement du tronc est un élément important pour évaluer la posture en ce qui a trait à la scoliose idiopathique. Cependant, il y a peu d'informations quant à sa mesure et les associations entre le déjettement du tronc et d'autres indicateurs ou facteurs pertinents. Les objectifs de cette étude sont : 1) d’évaluer la validité et la fiabilité de test-retest du fil à plomb pour mesurer le déjettement du tronc de C7 à S1 chez les personnes atteintes de scoliose idiopathique; et 2) d'étudier l'association entre le déjettement du tronc et les facteurs suivants : la douleur, l’angle de Cobb, le type de scoliose, la santé mentale et l’image de soi chez les personnes atteintes de scoliose idiopathique. Nous avons recruté 55 sujets âgés de 10 à 21 ans atteints de scoliose idiopathique (angle de Cobb : 15º à 60º) de la clinique de scoliose dans un hôpital pédiatrique de soins tertiaires. Le déjettement a été mesuré directement sur les sujets à deux reprises par la même physiothérapeute de même que sur les radiographies prises ce jour-là. Deux mesures ont été prises à chaque fois : une les pieds joints (FT) et l’autre en écartant les pieds (FA). Les sujets ont répondu au questionnaire adressé au patient de la Scoliosis Research Society-22, qui traite de la douleur, de l'image de soi et de la santé mentale. Le type de scoliose et l'angle de Cobb ont été mesurés sur les radiographies prises ce jour-là. Nous avons utilisé la théorie de la généralisabilité pour calculer la fidélité de test-retest pour les mesures FT et FA, en rapportant les coefficients de fiabilité (f) et les erreurs types de mesure (SEM). La validité de la mesure du fil à plomb a été calculée en comparant les mesures prises directement sur les radiographies en utilisant le coefficient de corrélation de Pearson. Les corrélations de Pearson ont été calculées entre le déjettement du tronc et la douleur, l'angle de Cobb, l'image de soi et la santé mentale. Les corrélations de Spearman ont été calculées entre le déjettement du tronc et le type de scoliose. Nous avons ensuite utilisé des modèles de régression linéaire multiple pour déterminer les associations entre ces variables. Nos résultats indiquent que les mesures de déjettement du tronc en utilisant un fil à plomb ont une forte corrélation (r = 0,87) avec la mesure obtenue par radiographie. La mesure du déjettement du tronc en utilisant un fil à plomb a démontré une excellente fidélité de test-retest (f: 0,98 pour la mesure FT et 0,99 pour la mesure FA) et les SEM étaient de 2,0 mm pour la mesure FT et 1,8 mm pour la mesure FA. Le déjettement du tronc est corrélé positivement avec l'angle de Cobb (r = 0,32, p = 0,02), mais il n'est pas corrélé à la douleur, la santé mentale, l'image de soi ou le type de scoliose. Les conclusions de notre étude ont montré que la mesure clinique du déjettement du tronc en utilisant un fil à plomb est une méthode fiable et valide et que le déjettement du tronc est associé à l'angle de Cobb. Une étude longitudinale est nécessaire pour déterminer si le déjettement du tronc est un indicateur pronostique de la progression de la scoliose. BACKGROUND: A fixed cavovarus foot deformity can be associated with anteromedial ankle arthrosis due to elevated medial joint contact stresses. Supramalleolar valgus osteotomies (SMOT) and lateralizing calcaneal osteotomies (LCOT) are commonly used to treat symptoms by redistributing joint contact forces. In a cavovarus model, the effects of SMOT and LCOT on the lateralization of the center of force (COF) and reduction of the peak pressure in the ankle joint were compared. METHODS: A previously published cavovarus model with fixed hindfoot varus was simulated in 10 cadaver specimens. Closing wedge supramalleolar valgus osteotomies 3 cm above the ankle joint level (6 and 11 degrees) and lateral sliding calcaneal osteotomies (5 and 10 mm displacement) were analyzed at 300 N axial static load (half body weight). The COF migration and peak pressure decrease in the ankle were recorded using high-resolution TekScan pressure sensors. RESULTS: A significant lateral COF shift was observed for each osteotomy: 2.1 mm for the 6 degrees (P = .014) and 2.3 mm for the 11 degrees SMOT (P = .010). The 5 mm LCOT led to a lateral shift of 2.0 mm (P = .042) and the 10 mm LCOT to a shift of 3.0 mm (P = .006). Comparing the different osteotomies among themselves no significant differences were recorded. No significant anteroposterior COF shift was seen. A significant peak pressure reduction was recorded for each osteotomy: The SMOT led to a reduction of 29% (P = .033) for the 6 degrees and 47% (P = .003) for the 11 degrees osteotomy, and the LCOT to a reduction of 41% (P = .003) for the 5 mm and 49% (P = .002) for the 10 mm osteotomy. Similar to the COF lateralization no significant differences between the osteotomies were seen. CONCLUSION: LCOT and SMOT significantly reduced anteromedial ankle joint contact stresses in this cavovarus model. The unloading effects of both osteotomies were equivalent. More correction did not lead to significantly more lateralization of the COF or more reduction of peak pressure but a trend was seen. CLINICAL RELEVANCE: In patients with fixed cavovarus feet, both SMOT and LCOT provided equally good redistribution of elevated ankle joint contact forces. Increasing the amount of displacement did not seem to equally improve the joint pressures. The site of osteotomy could therefore be chosen on the basis of surgeon's preference, simplicity, or local factors in case of more complex reconstructions.

The potential interactive effects of iron (Fe) limitation and Ocean Acidification in the Southern Ocean (SO) are largely unknown. Here we present results of a long-term incubation experiment investigating the combined effects of CO2 and Fe availability on natural phytoplankton assemblages from the Weddell Sea, Antarctica. Active Chl a fluorescence measurements revealed that we successfully cultured phytoplankton under both Fe-depleted and Fe-enriched conditions. Fe treatments had significant effects on photosynthetic efficiency (Fv/Fm; 0.3 for Fe-depleted and 0.5 for Fe-enriched conditions), non-photochemical quenching (NPQ), and relative electron transport rates (rETR). pCO2 treatments significantly affected NPQ and rETR, but had no effect on Fv/Fm. Under Fe limitation, increased pCO2 had no influence on C fixation whereas under Fe enrichment, primary production increased with increasing pCO2 levels. These CO2-dependent changes in productivity under Fe-enriched conditions were accompanied by a pronounced taxonomic shift from weakly to heavily silicified diatoms (i.e. from Pseudo-nitzschia sp. to Fragilariopsis sp.). Under Fe-depleted conditions, this functional shift was absent and thinly silicified species dominated all pCO2 treatments (Pseudo-nitzschia sp. and Synedropsis sp. for low and high pCO2, respectively). Our results suggest that Ocean Acidification could increase primary productivity and the abundance of heavily silicified, fast sinking diatoms in Fe-enriched areas, both potentially leading to a stimulation of the biological pump. Over much of the SO, however, Fe limitation could restrict this possible CO2 fertilization effect.

Objective: Recent evidence suggests that cortical activity associated with voluntary movement is relatively shifted from medial to lateral premotor areas in Parkinson's disease. This shift occurs bilaterally even for unilateral responses. It is not clear whether the shift in processing reflects an overall change in movement strategy, thereby involving alternate cortical areas, or reflects a compensatory change whereby, given the appropriate conditions, less impaired cortical areas are able to provide a similar function in compensation for those areas which are more impaired. This issue was examined in patients with hemi-Parkinson's disease, in whom basal ganglia impairment is most pronounced in one hemisphere. Methods: Fourteen patients with hemi-Parkinson's disease and 15 age-matched control subjects performed a Go/NoGo finger movement task and the contingent negative variation (CNV) was recorded from 21 scalp positions. Results and conclusions: Maximal CNV amplitudes were found over central medial regions for control subjects, but were shifted more frontally for Parkinson's disease patients, reduced in amplitude over the midline and lateralized towards the side ipsilateral to the greatest basal ganglia impairment. This shift in cortical activity from medial to lateral areas in Parkinson's disease patients appears to reflect a compensatory mechanism operating predominantly on the side of greatest basal ganglia impairment. Cortical activity associated with voluntary movement is shifted from medial to lateral premotor areas in Parkinson's disease. This occurs bilaterally, even for unilateral movements. We have used both EEG and MEG to further investigate medial and lateral premotor activity in patients with hemi-Parkinson's disease, in whom basal ganglia impairment is most pronounced in one hemisphere. The CNV, recorded from 21 scalp positions in a Go/NoGo task, was maximal over central medial regions in control subjects. For hemi-Parkinson's disease subjects, activity was shifted more frontally, reduced in the midline and lateralised towards the side of greatest basal ganglia impairment. With 143 channel whole-scalp magneto encephalography (MEG) we are further examining asymmetries in supplementary motor/premotor cortical activity prior to self-paced voluntary movement. In preliminary results, one hemi-Parkinson's disease patient with predominantly left-side symptoms showed strong medial activity consistent with a dominant source in the left supplementary motor area (SMA). Three patients showed little medial activity, but early bilateral sources within lateral premotor cortex. Results suggest greater involvement of lateral premotor rather than the SMA prior to movement in Parkinson's disease and provide evidence for asymmetric function of the SMA in hemi- Parkinson's disease, with reduced activity on the side of greatest basal ganglia deficit.

There is growing interest in understanding the role of the non-injured contra-lateral hemisphere in stroke recovery. In the experimental field, histological evidence has been reported that structural changes occur in the contra-lateral connectivity and circuits during stroke recovery. In humans, some recent imaging studies indicated that contra-lateral sub-cortical pathways and functional and structural cortical networks are remodeling, after stroke. Structural changes in the contra-lateral networks, however, have never been correlated to clinical recovery in patients. To determine the importance of the contra-lateral structural changes in post-stroke recovery, we selected a population of patients with motor deficits after stroke affecting the motor cortex and/or sub-cortical motor white matter. We explored i) the presence of Generalized Fractional Anisotropy (GFA) changes indicating structural alterations in the motor network of patientsâeuro? contra-lateral hemisphere as well as their longitudinal evolution ii) the correlation of GFA changes with patientsâeuro? clinical scores, stroke size and demographics data iii) and a predictive model.

BACKGROUND: Resting-state functional magnetic resonance imaging (fMRI) enables investigation of the intrinsic functional organization of the brain. Fractal parameters such as the Hurst exponent, H, describe the complexity of endogenous low-frequency fMRI time series on a continuum from random (H = .5) to ordered (H = 1). Shifts in fractal scaling of physiological time series have been associated with neurological and cardiac conditions. METHODS: Resting-state fMRI time series were recorded in 30 male adults with an autism spectrum condition (ASC) and 33 age- and IQ-matched male volunteers. The Hurst exponent was estimated in the wavelet domain and between-group differences were investigated at global and voxel level and in regions known to be involved in autism. RESULTS: Complex fractal scaling of fMRI time series was found in both groups but globally there was a significant shift to randomness in the ASC (mean H = .758, SD = .045) compared with neurotypical volunteers (mean H = .788, SD = .047). Between-group differences in H, which was always reduced in the ASC group, were seen in most regions previously reported to be involved in autism, including cortical midline structures, medial temporal structures, lateral temporal and parietal structures, insula, amygdala, basal ganglia, thalamus, and inferior frontal gyrus. Severity of autistic symptoms was negatively correlated with H in retrosplenial and right anterior insular cortex. CONCLUSIONS: Autism is associated with a small but significant shift to randomness of endogenous brain oscillations. Complexity measures may provide physiological indicators for autism as they have done for other medical conditions.

Destructive leadership behaviour often results in damage to the organisations that the individual is entrusted to lead. Although accurately pinpointing the type of destructive behaviour is difficult, this article seeks to offer suggestions as to why leaders spiral into such unattractive behaviour. After reviewing the literature, this paper highlights four drivers for destructive ways that people act based on detailed qualitative scenarios that involve how those who experienced such behaviour reacted and felt. The study reveals a noticeable human experience from which nobody can escape, and offers understanding of the study participants’ experiences. Out of respect to the participants, the authors keep their identity anonomous. We drew our subjects from a cross-section of organisations that function internationally within one area of the manufacturing industry. The article presents a model comprising two dimensions: 1) the leader’s attitude to the organisation he or she leads and 2) adequacy of his or her leadership capabilities. The models offer us understanding of the drivers of the destructive actions that the leader exhibits. Understanding allows us to provide managers with tactical methods to protect them against destructive behaviour and help them lessen the worst aspects of destructive behaviour in both their colleagues and themselves.

BACKGROUND: In some Western countries, more and more patients seek initial treatment even for minor injuries at emergency units of hospitals. The initial evaluation and treatment as well as aftercare of these patients require large amounts of personnel and logistical resources, which are limited and costly, especially if compared to treatment by a general practitioner. In this study, we investigated whether outsourcing from our level 1 trauma center to a general practitioner has an influence on patient satisfaction and compliance. METHODS: This prospective, randomized study, included n = 100 patients who suffered from a lateral ankle ligament injury grade I-II (16, 17). After radiological exclusion of osseous lesions, the patients received early functional treatment and were shown physical therapy exercises to be done at home, without immobilization or the use of stabilizing ortheses. The patients were randomly assigned into two groups of 50 patients each: Group A (ER): Follow-up and final examination in the hospital's emergency unit. Group B (GP): Follow-up by general practitioner, final examination at hospital's emergency unit. The patients were surveyed regarding their satisfaction with the treatment and outcome of the treatment. RESULTS: Female and male patients were equally represented in both groups. The age of the patients ranged from 16 - 64 years, with a mean age of 34 years (ER) and 35 years (GP). 98% (n = 98) of all patients were satisfied with their treatment, and 93% (n = 93) were satisfied with the outcome. For these parameters no significant difference between the two groups could be noted (p = 0.7406 and 0.7631 respectively). 39% of all patients acquired stabilizing ortheses like ankle braces (Aircast, Malleoloc etc.) on their own initiative. There was a not significant tendency for more self-acquired ortheses in the group treated by general practicioners (p = 0,2669). CONCLUSION: Patients who first present at the ER with a lateral ankle ligament injury grade I-II can be referred to a general practitioner for follow-up treatment without affecting patient satisfaction regarding treatment and treatment outcome.

The cyclic peptide Melanin Concentrating Hormone (MCH) is known to control a large number of brain functions in mammals such as food intake and metabolism, stress response, anxiety, sleep/wake cycle, memory, and reward. Based on neuro-anatomical and electrophysiological studies these functions were attributed to neuronal circuits expressing MCHR1, the single MCH receptor in rodents. In complement to our recently published work (1) we provided here new data regarding the action of MCH on ependymocytes in the mouse brain. First, we establish that MCHR1 mRNA is expressed in the ependymal cells of the third ventricle epithelium. Second, we demonstrated a tonic control of MCH-expressing neurons on ependymal cilia beat frequency using in vitro optogenics. Finally, we performed in vivo measurements of CSF flow using fluorescent micro-beads in wild-type and MCHR1-knockout mice. Collectively, our results demonstrated that MCH-expressing neurons modulate ciliary beating of ependymal cells at the third ventricle and could contribute to maintain cerebro-spinal fluid homeostasis.

The present study used functional magnetic resonance imaging to demonstrate that performance of visual spatial and visual nonspatial working memory tasks involve the same regions of the lateral prefrontal cortex when all factors unrelated to the type of stimulus material are appropriately controlled. These results provide evidence that spatial and nonspatial working memory may not be mediated, respectively, by mid-dorsolateral and mid-ventrolateral regions of the frontal lobe, as widely assumed, and support the alternative notion that specific regions of the lateral prefrontal cortex make identical executive functional contributions to both spatial and nonspatial working memory.
