Nach einer hämatopoetischen Stammzelltransplantation spielt die Zuordnung hämatopoetischer Zellen zum Spender oder Empfänger für viele transplantationsbezogene Fragestellungen eine wichtige Rolle. Unter anderem ist das Persistieren von dendritischen Zellen des Empfängers, welche allogene T-Zellen des Spenders stimulieren, ein wichtiger Schritt bei der Entstehung der akuten GVHD. Aus diesem Grund wurde in dieser Arbeit die Weiterentwicklung einer Methode angestrebt, die es uns erlaubt, die Zugehörigkeit isolierter hämatopoetischer Zellen dem Spender oder dem Empfänger zuzuordnen (Chimärismusbestimmung) und gleichzeitig Aussagen über das Ursprungsgewebe und den Aktivierungszustand der Zellen machen zu können. Hierfür nutzten wir Einzelbasenpolymorphismen (SNPs). Ziel dieser Arbeit war es, einen Pool von cDNA-kodierten SNPs zu definieren, mit dem auch HLA-identische Geschwister eindeutig unteschieden werden können. rnHierfür wurden zunächst aus publizierten Datenbanken solche SNPs ausgewählt, die in kodierenden Genabschnitten konstitutiv und gewebeunabhängig auf expremierten Genen lagen und zugleich eine hohe Heterozygotenfrequenz in der europäischen Population aufwiesen. Anhand dieser Kriterien wurden mittels der NCBI-Datenbank insgesamt eine Anzahl von 208 Polymorphismen auf 150 Genen identifiziert. Anschließend erfolgte die Gestaltung von Primerpaaren zur Amplifikation der SNP-kodierenden cDNA-Abschnitte. Diese mussten mindestens eine Intron/Exon-Grenze überspannen, um genomische DNA in der PCR ausschließen zu können. Mit Hilfe der etablierten PCR-Reaktion wurden die Gene in unterschiedlichen Geweben auf ihre Expression hin überprüft. Für 45 Gene ließ sich sowohl eine entsprechende PCR etablieren als auch deren konstitutive Expression in verschiedenen hämatopoetischen Zellen nachweisen. Zur Detektion der einzelnen SNPs in der Minisequenzierung wurden Minisequenzierungs-Sonden generiert und geprüft. Im Folgenden wurden für PCR und Minisequezierung Multiplex-Reaktionen aus vier bis sechs Reaktionen zusammengestellt. Zu diesem Zweck wurden die jeweiligen Primerinteraktionen und die unterschiedlichen Basenlängen des PCR-Produktes berücksichtigt.rnVon den 45 etablierten Einzelreaktionen waren 30 für den Multiplexansatz geeignet. Unter Anwendung dieser Multiplex-Reaktionen wurden 24 HLA-identische Geschwisterpaare (Spender und Empfänger) getestet. Zur Kontrolle erfolgte zusätzlich eine konventionelle Sequenzierung der SNP-kodierenden Bereiche auf der cDNA der jeweiligen Proben. Mit Hilfe der SNP-Kombinationen und der etablierten Methodik waren wir in der Lage alle 24 untersuchten Geschwisterpaare in zwischen sechs und 18 SNP-Systemen zu unterscheiden. rnDie Möglichkeiten, die die Analysen des Chimärismus mittels SNPs auf kodierenden Bereichen der DNA mit sich bringen, liegen nicht nur in der gleichzeitigen Bestimmung der Gewebszugehörigkeit und der Detektion des bestehenden Chimärismus sowie dessen Quantifizierung unter Anwendung einer Real-time-PCR. Vielmehr ermöglicht sie auch eine Aussage über die Genexpression der untersuchten Zelle zu machen. Dies ist insbesondere dann von Interesse, wenn geringe Zellzahlen von aus Gewebe isolierten Zellen zur Verfügung stehen. Die in dieser Arbeit etablierten Ansätze werden derzeit für eine Quantifizierung mittels real-time RCR weiterentwickelt und sollen mittelfristig insbesondere für Untersuchungen des Chimärismus von dermalen und epidermalen dendritischen Zellen der Haut und anderer Zielgewebe der GvHD verwendet werden.rn

The aim of the present study was to compare quantitatively the distribution of dendritic cell subpopulations in chronic periodontitis and gingivitis. Fourteen biopsies from patients with chronic periodontitis and fifteen from patients with gingivitis were studied. An immunoperoxidase technique was used to quantify the number of Langerhans' cells (CD1a) and interstitial dendritic cells (factor XIIIa) in the oral and sulcular and junctional/pocket epithelia and in the lamina propria. A greater number of factor XIIIa+ dendritic cells in the lamina propria and CD1a+ dendritic cells in the oral epithelium were observed in gingivitis compared to the periodontitis group (p = 0.05). In the sulcular and junctional/pocket epithelia and in the lamina propria, the number of CD1a+ dendritic cells was similar in the gingivitis and periodontitis groups. In conclusion, the number of Langerhans' cells in the oral epithelium and interstitial dendritic cells in the lamina propria is increased in gingivitis compared to periodontitis, which may contribute to the different pattern of host response in these diseases.

Jorge Lobo`s disease is a chronic infection caused by the fungus Lacazia loboi endemic in South America. The infection is characterized by the appearance of parakeloidal, ulcerated or verrucous nodular or plaque-like cutaneous lesions. The histopathological aspect is characterized by poorly organized granulomas with histiocytes and multinucleated giant cells. Little is known about local immune response in lobomycosis skin lesions. Thirty-three skin biopsies from patients with Jorge Lobo`s disease were selected from Ambulatory of Dermatology, UFPA. The control group was constituted by ten biopsies from normal skin. Langerhans cells were identified by immunohistochemistry using anti-CD1a antibody (Serotec). The number of positive cells was statistically analyzed. Langerhans cells were visualized along the epidermis in biopsies from Jorge Lobo`s disease and the morphology and the number of Langerhans cells did not differ from normal skin (p > 0.05). In Jorge Lobo`s disease, this cell population probably presents some escape mechanism of the local immune system to evade the antigen presentation by those cells. (C) 2010 Published by Elsevier B.V.

Langerhans cell histiocytosis (LCH) comprises a group of disorders, the common feature of which is Langerhans cell proliferation. The clinical presentation is highly varied. The severity and prognosis of the disease are dependent on the type and extent of organ involvement. This paper reports a rare case of a four-month-old white male with unifocal LCH limited exclusively to the mandible, discussing the diagnosis, radiographic and immunohistochemical aspects, treatment and monitoring multidisciplinary of the case. The Langerhans cells (LCs) are scattered throughout the epithelium of skin and mucosa and have been associated with the graft-vs.-host disease (GVHD), which is the highest cause of morbidity and mortality in patients who underwent bone marrow transplant (BMT). This study aims at quantifying the LCs in the oral chronic GVHD (cGVHD). Microscopic sections from biopsies carried out in the buccal mucosa of 40 patients who underwent allogenic BMT and developed (20) or not (20) oral cGVHD (Groups 1 and 2, respectively) were utilised. For the control group, free surgical margins of 20 biopsies of non-inflammatory lesions in the buccal mucosa (Group 3) were used. The sections were studied in routine colouration and immunostained for CD1a. Group 1 (with cGVHD) presented a greater number of Langerhans` cells/mm(2) (50.6 +/- 37.2) when compared with the other groups (Group 2, 23.11 +/- 19.7; Group 3, 16.6 +/- 17.3). Our results suggest a greater recruitment of LCs in patients transplanted with cGVHD, probably as a result of cytokines secreted by the inflammatory cells.

Células de Langerhans (CL) são um tipo de células dendríticas que têm funções que envolvem apresentação de antígeno e a estimulação de resposta T dependente. Elas representam aproximadamente 4% das células do epitélio laríngeo. OBJETIVO: Identificar a presença de CL no epitélio das pregas vocais, comparar suas subpopulações, bem com comparar a capacidade de quatro marcadores imunoistoquímicos. FORMA DE ESTUDO: Experimental. CASUÍSTICA E MÉTODO: Seis cadáveres, 3 homens e 3 mulheres foram estudados. Foram analisadas amostras de pele e das pregas vocais coradas e imunomarcadas para vimentina, proteína S-100, CD-68 e fascina. Após análise histológica, foi realizado o teste t de Student e análise de variância no estudo estatístico. RESULTADOS E CONCLUSÕES: Foi possível identificar a presença de CL no epitélio das pregas vocais de humanos não fumantes de ambos os sexos. A fascina, a vimentina o CD-68 mostraram-se bons marcadores das CL, enquanto a proteína S-100 teve estatisticamente menor poder de marcação tanto na prega vocal (p=0,01) como na pele (p=0,02). Foi possível identificar três diferentes subpopulações de CL presentes tanto na prega vocal como na pele destes indivíduos, contudo apenas na pele observarmos maior quantidade estatisticamente significante na camada basal do epitélio.

This work analyzed the histopathology and epidermal Langerhans cells (LC) of Montenegro skin test (MST) in patients with American tegumentary leishmaniasis (ATL) in order to in situ characterize and compare the immunological reaction of the two major clinical forms of ATL, localized cutaneous leishmaniasis (LCL) and mucocutaneous leishmaniasis (MCL). MST histopathology of both LCL and MCL showed superficial and deep perivascular inflammatory infiltrate composed mainly of lymphocytes and histiocytes. Epidermal LC population was higher in MST biopsies taken from LCL patients when compared to MCL group, at 48 and 72 hours after antigen inoculation. Increased number of epidermal LC displayed in MST biopsies of LCL patients represents specific cellular immunity against parasites. The decrease of LC in MST biopsies of MCL patients does not necessarily indicate a worse specific cellular immunity in this clinical form of leishmaniasis.

The predominant clinical and radiological features of Langerhans' cell histiocytosis (LCH) in children are due to osseous involvement. Extra-osseous disease is far less common, occurring in association with bone disease or in isolation; nearly all anatomical sites may be affected and in very various combinations. The following article is based on a multicentre review of 31 children with extra-osseous LCH. The objective is to summarise the diverse possibilities of organ involvement. The radiological manifestations using different imaging modalities are rarely pathognomonic on their own. Nevertheless, familiarity with the imaging findings, especially in children with systemic disease, may be essential for early diagnosis.

BackgroundPulmonary Langerhans cell histiocytosis (PLCH) is a rare disorder characterised by granulomatous proliferation of CD1a-positive histiocytes forming granulomas within lung parenchyma, in strong association with tobacco smoking, and which may result in chronic respiratory failure. Smoking cessation is considered to be critical in management, but has variable effects on outcome. No drug therapy has been validated. Cladribine (chlorodeoxyadenosine, 2-CDA) down-regulates histiocyte proliferation and has been successful in curbing multi-system Langerhans cell histiocytosis and isolated PLCH.Methods and patientsWe retrospectively studied 5 patients (aged 37¿55 years, 3 females) with PLCH who received 3 to 4 courses of cladribine therapy as a single agent (0.1 mg/kg per day for 5 consecutive days at monthly intervals). One patient was treated twice because of relapse at 1 year. Progressive pulmonary disease with obstructive ventilatory pattern despite smoking cessation and/or corticosteroid therapy were indications for treatment. Patients were administered oral trimethoprim/sulfamethoxazole and valaciclovir to prevent opportunistic infections. They gave written consent to receive off-label cladribine in the absence of validated treatment.ResultsFunctional class dyspnea improved with cladribine therapy in 4 out of 5 cases, and forced expiratory volume in 1 second (FEV1) increased in all cases by a mean of 387 ml (100¿920 ml), contrasting with a steady decline prior to treatment. Chest high-resolution computed tomography (HRCT) features improved with cladribine therapy in 4 patients. Hemodynamic improvement was observed in 1 patient with pre-capillary pulmonary hypertension. The results suggested a greater treatment effect in subjects with nodular lung lesions and/or thick-walled cysts on chest HRCT, with diffuse hypermetabolism of lung lesions on positron emission tomography (PET)-scan, and with progressive disease despite smoking cessation. Infectious pneumonia developed in 1 patient, with later grade 4 neutrocytopenia but without infection.DiscussionData interpretation was limited by the retrospective, uncontrolled study design and small sample size.ConclusionCladribine as a single agent may be effective therapy in patients with progressive PLCH.
