BACKGROUND: Patient-controlled epidural analgesia with low concentrations of anesthetics is effective in reducing labor pain. The aim of this study was to assess and compare two ultra-low dose regimens of ropivacaine and sufentanil (0.1% ropivacaine plus 0.5 μg.ml-1 sufentanil vs. 0.06% ropivacaine plus 0.5 μg.ml-1 sufentanil) on the intervals between boluses and the duration of labor. MATERIAL AND METHODS: In this non-randomized prospective study, conducted between January and July 2010, two groups of parturients received patient-controlled epidural analgesia: Group I (n = 58; 1 mg.ml-1 ropivacaine + 0.5 μg.ml-1 sufentanil) and Group II (n = 57; 0.6 mg.ml-1 ropivacaine + 0.5 μg.ml-1 sufentanil). Rescue doses of ropivacaine at the concentration of the assigned group without sufentanil were administered as necessary. Pain, local anesthetic requirements, neuraxial blockade characteristics, labor and neonatal outcomes, and maternal satisfaction were recorded. RESULTS: The ropivacaine dose was greater in Group I (9.5 [7.7-12.7] mg.h-1 vs. 6.1 [5.1-9.8 mg.h-1], p < 0.001). A time increase between each bolus was observed in Group I (beta = 32.61 min, 95% CI [25.39; 39.82], p < 0.001), whereas a time decrease was observed in Group II (beta = -1.40 min, 95% CI [-2.44; -0.36], p = 0.009). The duration of the second stage of labor in Group I was significantly longer than that in Group II (78 min vs. 65 min, p < 0.001). CONCLUSIONS: Parturients receiving 0.06% ropivacaine exhibited less evidence of cumulative effects and exhibited faster second stage progression than those who received 0.1% ropivacaine.

The present paper presents the social and obstetric profile of women undergoing cesarean childbirth at a public maternity hospital in the interior of Sao Paulo state. This is a quantitative, retrospective, descriptive study performed using documental research. The collected data refer to the period between July and December 2005, and between January and June 2006. A total of 670 records were reviewed. A 23% rate of cesarean births was identified at the institution studied during the aforementioned period. The studied population was characterized as having a low level of education, living in a common-law relationship, and not having a paid occupation. The main indications for cesarean sections were iterativity and acute fetal distress. The findings revealed an emphasis on obstetric nursing in the low-risk normal childbirth scenario, considering the non-interventionist character inherent to their education and training.

OBJECTIVE: We sought to determine maternal and neonatal outcomes by labor onset type and gestational age. STUDY DESIGN: We used electronic medical records data from 10 US institutions in the Consortium on Safe Labor on 115,528 deliveries from 2002 through 2008. Deliveries were divided by labor onset type (spontaneous, elective induction, indicated induction, unlabored cesarean). Neonatal and maternal outcomes were calculated by labor onset type and gestational age. RESULTS: Neonatal intensive care unit admissions and sepsis improved with each week of gestational age until 39 weeks (P < .001). After adjusting for complications, elective induction of labor was associated with a lower risk of ventilator use (odds ratio [OR], 0.38; 95% confidence interval [CI], 0.28-0.53), sepsis (OR, 0.36; 95% CI, 0.26-0.49), and neonatal intensive care unit admissions (OR, 0.52; 95% CI, 0.48-0.57) compared to spontaneous labor. The relative risk of hysterectomy at term was 3.21 (95% CI, 1.08-9.54) with elective induction, 1.16 (95% CI, 0.24-5.58) with indicated induction, and 6.57 (95% CI, 1.78-24.30) with cesarean without labor compared to spontaneous labor. CONCLUSION: Some neonatal outcomes improved until 39 weeks. Babies born with elective induction are associated with better neonatal outcomes compared to spontaneous labor. Elective induction may be associated with an increased hysterectomy risk.

O presente estudo tem como objeto as representações mentais de mulheres produzidas pelas enfermeiras obstétricas na assistência ao parto. Os objetivos foram: Discutir as representações mentais das mulheres assistidas pelas enfermeiras sobre o parto e a prática obstétrica; Discutir o habitus da enfermeira obstétrica percebido pela mulher e Analisar as relações de poder simbólico entre os agentes envolvidos no processo de parturição. Trata-se de uma pesquisa qualitativa que utilizou a história oral temática e o teste de associação de palavras como técnicas de coleta de dados. Para a análise do material, utilizamos o método da análise de conteúdo de Bardin. A fim de dar sustentação teórica ao estudo, adotamos os conceitos de: campo, capitais, habitus, poder simbólico, trocas linguísticas, identidade e representações mentais, desenvolvidos por Pierre Bourdieu. Os resultados encontrados foram agrupados em duas categorias: As representações mentais das mulheres sobre o parto e a prática obstétrica: as percepções construídas e desconstruídas com o processo de parturição e O habitus da enfermeira obstétrica percebido pelas mulheres durante o processo de parturição: o poder simbólico destas agentes na construção de uma nova demanda social para o campo obstétrico. A primeira categoria apresentou as representações construídas pela socialização e as transformações das representações mentais das mulheres consequente à interação com a enfermeira no campo obstétrico. Neste sentido, as percepções das mulheres sobre o parto e a prática obstétrica confirmaram a forte influência do modelo tecnocrático nos depoimentos. Além disso, a prática humanizada da enfermeira contribuiu para a construção de uma nova visão de mundo nas mulheres pesquisadas que provocou um confronto entre suas representações mentais. A segunda categoria desvelou a identidade da enfermeira obstétrica percebida pelas mulheres através dos atributos profissionais e dos sinais distintivos que resultaram na associação de estereótipos a estas agentes. Ainda revelou que a manifestação do poder simbólico da enfermeira obstétrica foi percebido de diversas formas: pelo efeito de mobilização na mulher, em associação com as tecnologias de cuidado e através do fortalecimento da mulher para o parto. Concluímos que no contato com a mulher, a enfermeira exerceu um poder simbólico por estar em melhores posições no campo obstétrico. Tal fato, para as mulheres, resultou em transformações das suas representações mentais em relação ao processo de parturição, o que contribuiu para a construção de uma nova demanda para o campo obstétrico. Por outro lado, a enfermeira obstétrica, ao ser reconhecida pelas usuárias estudadas, fortaleceu a posição de sua prática no campo.

This research aims to measure the energy spending in parturient women of low gestation risk. Participants were selected randomly and submitted to fasting (n=15; Group I) or honey ingestion (n = 15; Group II). Data were collected by means of capillary blood values and heart frequency monitoring. The paired t-test with a 5% significance level and Tukey's method were used in statistical analysis. The results showed that honey ingestion did not promote an overload in the mother's glucose; the lactate response demonstrated that the substrate offered was well used; the cardiorespiratory rate demonstrated good performance for both groups; the total energy spent during labor demonstrated that carbohydrate ingestion exerts significant influence, improving maternal anaerobic performance; the group which remained in fasting presented, immediately after labor, higher levels of lactate, showing the organism's efforts to compensate for the energy spent.

Superstitions are found everywhere in our lives, and medicine, a profession that is prides itself on an evidence-based approach to treatment, is not exempt. A superstition that pervades the labor and delivery floor is that it is busier during certain phases of the lunar cycle, specifically the full moon. Although some studies have demonstrated an increase in deliveries that are related to the lunar cycle, there has been disagreement about when, in the lunar cycle, the peak volume occurs. Front to the divergence of the existent results in the literature to relate the events of the lunar cycle with deliveries, the aim of this review was to accomplish the literature in the attempt of explaining this popular culture with base in the results presented by different researchers.

Objective. To identify reasons why women look for early health care attention at the end of pregnancy. Methodology. Qualitative study based in socio-existential phenomenology proposed by Alfred Schütz. Nine pregnant women who consulted to the maternity service without being in real labor were interviewed. Results. The following analysis categories emerged: Having different symptoms or signals from the ones they had felt or been informed about, not being able to identify real labor, fear to fetal complications for being close to the probable labor date, feeling calmer when being seen by professionals of the institution, lack of problem solving by other services of the health institution, fearing out of hospital delivery, imitating real labor, looking for tranquility in the idealization of labor and modifying behaviors to guarantee attention. Conclusion. Women are insecure and fear because of a fragmented health care, where they do not perceive themselves linked to the prenatal control program anymore, and at the same time they don’t find the health care assistance they want.

O artigo apresenta o perfil social e obstétrico das mulheres submetidas ao parto cesárea em uma maternidade pública do interior do Estado de São Paulo. Trata-se de estudo quantitativo, retrospectivo, do tipo descritivo, com pesquisa documental. Os dados coletados referem-se ao período entre os meses de junho e dezembro de 2005, e janeiro a junho de 2006. Foram consultados 670 prontuários. A taxa de parto cesárea na instituição-campo foi calculada em 23% para o período supracitado. Foram características da população estudada a baixa escolaridade formal, a união consensual e o trabalho não remunerado. As principais indicações para as cesarianas foram a iteratividade e o sofrimento fetal agudo. Como desdobramento dos achados está a ênfase pela valorização da enfermagem obstétrica no cenário de atendimento ao parto normal de baixo risco, considerando seu caráter não-intervencionista inerente à sua formação.

Enquadramento - O envolvimento paterno no trabalho de parto tem suscitado o interesse de vários profissionais de saúde, nomeadamente dos enfermeiros obstetras, dado que os benefícios da presença paterna no contexto de parto ainda não são clara e totalmente assumidos por todos os profissionais. Por outro lado reconhece-se que o pai é o acompanhante ideal da grávida durante o trabalho de parto, pelo seu apoio e lugar que ocupa na tríade. Objetivos – Identificar os benefícios e constrangimentos do envolvimento paterno durante o trabalho de parto e parto. Método – Efetuada revisão integrativa da literatura sobre os “Benefícios e constrangimentos do envolvimento paterno durante o trabalho de parto e parto” nas bases de dados: PubMed, LILACS, SciELO, Repositórios Institucionais e Bibliotecas Digitais e EBSCO Host, elaborados entre 2000 e 2014. Selecionaram-se 19 artigos que obedeceram aos critérios de inclusão deste estudo. Dos 19 artigos selecionados, 4 são revisões sistemáticas da literatura, 13 são estudos qualitativos e 2 são estudos quantitativos. Resultados – Evidenciou-se um “novo” pai, que acompanha a grávida desde a gravidez, e que esteve presente no trabalho de parto. É dada grande relevância aos Enfermeiros Especialistas em Saúde Materna e Obstetrícia, pelo seu papel na inclusão do pai na sala de parto e desmitificação de tabus. Após esta pesquisa constatou-se que, ainda há lacunas relativamente à presença do pai parto. São muitos os benefícios do envolvimento paterno no parto, mas ainda são visíveis constrangimentos por parte do pai relativamente ao contexto de parto. Conclusões - A presença do pai na sala de parto permite estreitar os laços mais íntimos, consolidando a união familiar e proporcionando bem-estar à grávida. Realça-se o papel dos profissionais de saúde na integração do pai na maternidade. Caminhamos para um contexto de parto mais alargado e do qual já não faz parte apenas a grávida e o seu filho, mas no qual o pai é também elemento chave de todo o processo. Background: Fetal scalp lactate testing has been shown to be as useful as pH with added benefits. One remaining question is What level of lactate should trigger intervention in the first stage of labour?' Aims: This study aimed to establish the lactate level in the first stage of labour that indicates the need for intervention to ensure satisfactory outcomes for both babies and mothers. Methods: A prospective study at Mater Mothers' Hospital, Brisbane, Australia, a tertiary referral centre. One hundred and forty women in labour, with non-reassuring fetal heart rate traces, were tested using fetal blood scalp sampling of 5 mu L of capillary blood tested on an Accusport (Boeringer, Mannheim, East Sussex, UK) lactate meter. Decision to intervene in labour was based on clinical assessment plus a predetermined cut off. Main outcome measures were APGAR scores, cord arterial pH, meconium stained liquor and Intensive Care Nursery admission. Results: Two-graph receiver operating characteristic (TG-ROC) analysis showed optimal specificity, and sensitivity for predicting adverse neonatal outcomes was a scalp lactate level above 4.2 mmol/L. Conclusions: Fetal blood sampling remains the standard for further investigating-non-reassuring cardiotocograph (CTG) traces. Even so, it is a poor predictor of fetal outcomes. Scalp lactate has been shown to be at least as good a predictor as scalp pH, with the advantages of being easier, cheaper and with a lower rate of technical failure. Our study, found that a cut off fetal scalp lactate level of 4.2 mmol/L, in combination with an assessment of the entire clinical picture, is a useful tool in identifying those women who need intervention.

A Organização Mundial de Saúde destaca a deambulação e a adoção de posições verticais pela parturiente, como práticas demonstradamente benéficas, devendo ser estimuladas pela enfermeira obstetra na condução do trabalho de parto e parto normal, em situações de baixo risco obstétrico. O projeto preconizou a promoção da deambulação e o incentivo à adoção de posições verticais na fase ativa do trabalho de parto. As intervenções de enfermagem na promoção destas práticas foram avaliadas na equipa de enfermeiros obstetras do Serviço de Urgência Obstétrica e Ginecológica do Centro Hospitalar Barreiro-Montijo, Entidade Pública Empresarial e num grupo de parturientes, através da aplicação de questionários. As necessidades dos enfermeiros obstetras e das parturientes determinaram a realização de ações formativas e educacionais e a elaboração de materiais informativos de suporte à prática assistencial. A avaliação do projeto demonstrou que ainda existem práticas a serem melhoradas, sendo a sua continuidade vantajosa para profissionais e parturientes; ABSTRACT: TITLE: Move to Better Birth - Freedom of Movement and Upright Positions in First Stage of Labor. The World Health Organization highlights ambulation and adoption of vertical position by the woman, as demonstrated beneficial practices and should be encouraged by obstetric nurses in the conduct of obstetric labor and normal delivery, in low risk situations. The project advocated for the promotion of ambulation and encouragement for the adoption of vertical positions in the active phase of labor. Nursing interventions to promote these practices were evaluated in the obstetric nurses team of Obstetric and Gynecological Emergency Department from Hospital Center Barreiro-Montijo, Business Public Entity, and in a group of women in labor, through the use of questionnaires. The needs of obstetric nurses and women in labor determined the realization of training and educacional activities and the development of information materials to support the care practice. The project evaluation demonstrates that there are still practices to be improved, and its continuity being advantageous for professionals and women in labor.

There is currently much interest in the appropriate use of obstetrical technology, cost containment and meeting consumers' needs for safe and satisfying maternity care. At the same time, there has been an increase in professionally unattended home births. In response, a new type of service, the out-of-hospital childbearing center (CBC) has been developed which is administratively and structurally separate from the hospital. In the CBC, maternity care is provided by certified nurse-midwives to carefully screened low risk childbearing families in conjunction with physician and hospital back-up.^ It was the purpose of this study to accomplish the following objectives: (1) To describe in a historical prospective study the demographic and medical-obstetric characteristics of patients laboring in eleven selected out-of-hospital childbearing centers in the United States from May 1, 1972, to December 15, 1979. Labor is defined as the onset of regular contractions as determined by the patient. (2) To describe any differences between those patients who require transfer to a back-up hospital and those who do not. (3) To describe administrative and service characteristics of eleven selected out-of-hospital childbearing centers in the United States. (4) To compare the demographic and medical-obstetric characteristics of women laboring in eleven selected out-of-hospital childbearing centers with a national sample of women of similar obstetric risk who according to birth certificates delivered legitimate infants in a hospital setting in the United States in 1972.^ Research concerning CBCs and supportive to the development of CBCs including studies which identified factors associated with fetal and perinatal morbidity and mortality, obstetrical risk screening, and the progress of technological development in obstetrics were reviewed. Information concerning the organization and delivery of care at each selected CBC was also collected and analyzed.^ A stratified, systematic sample of 1938 low risk women who began labor in a selected CBC were included in the study. These women were not unlike those described previously in small single center studies reported in the literature. The mean age was 25 years. Sixty-three per cent were white, 34 per cent Hispanic, 88 per cent married, 45 per cent had completed at least two years of college, nearly one-third were professionals and over a third were housewives. . . . Self-hypnosis was taught to 87 obstetric patients (HYP) and was not taught to 56 other patients (CNTRL), all delivered by the same family physician, in order to determine whether the use of self-hypnosis by low-risk obstetric patients leads to fewer technologic interventions during their deliveries or greater satisfaction of parturients with their delivery experience or both. The outcomes of the deliveries of these two groups were compared, and the HYP group was compared to 352 low-risk patients delivered by other family physicians at the same hospital (WCH). Questionnaires were mailed postpartum to 156 patients, all delivered by the same family physician, to determine satisfaction with delivery using the Labor and Delivery Satisfaction Index (LADSI). The hypnosis group showed a significant reduction in the number of epidurals (11.4% less than CNTRL and 17.9% less than WCH, p < 0.05) and the use of intravenous lines (18.5% less for both, p < 0.05). The number of episiotomies was significantly less in the HYP group compared to WCH (15.9%, p < 0.05) and 11.5% less when compared to CNTRL. The tear rate was not statistically different. Combined use of the intervention triad (epidural–forceps–episiotomy) was less for HYP than for CNTRL (15.8% less) and WCH (10.2% less, p < 0.05). More deliveries were done in the labor room with HYP than CNTRL (21%, p < 0.05). The second stage was shortened by 10 min (HYP vs CNTRL). Overall satisfaction of HYP and CNTRL patients was similar and generally favorable.
