Evaluation of p24-based antiretroviral treatment monitoring in pediatric HIV-1 infection: prediction of the CD4+ T-cell changes between consecutive visits

Worldwide, 700,000 infants are infected annually by HIV-1, most of them in resource-limited settings. Care for these children requires simple, inexpensive tests. We have evaluated HIV-1 p24 antigen for antiretroviral treatment (ART) monitoring in children. p24 by boosted enzyme-linked immunosorbent assay of heated plasma and HIV-1 RNA were measured prospectively in 24 HIV-1-infected children receiving ART. p24 and HIV-1 RNA concentrations and their changes between consecutive visits were related to the respective CD4+ changes. Age at study entry was 7.6 years; follow-up was 47.2 months, yielding 18 visits at an interval of 2.8 months (medians). There were 399 complete visit data sets and 375 interval data sets. Controlling for variation between individuals, there was a positive relationship between concentrations of HIV-1 RNA and p24 (P < 0.0001). While controlling for initial CD4+ count, age, sex, days since start of ART, and days between visits, the relative change in CD4+ count between 2 successive visits was negatively related to the corresponding relative change in HIV-1 RNA (P = 0.009), but not to the initial HIV-1 RNA concentration (P = 0.94). Similarly, we found a negative relationship with the relative change in p24 over the interval (P < 0.0001), whereas the initial p24 concentration showed a trend (P = 0.08). Statistical support for the p24 model and the HIV-1 RNA model was similar. p24 may be an accurate low-cost alternative to monitor ART in pediatric HIV-1 infection.

Das Dortnunder Museum am Ostwall von Leonie Reygers

In der europäischen Museumslandschaft der Nachkriegszeit war das Dortmunder Museum am Ostwall eine feste Größe. Gründungsdirektorin Leonie Reygers holte die weite Welt in die vom Bombenkrieg verwüstete Industriestadt. Sie übernahm 1947 eine ruinöses Haus, das bereits Resultat eines Umbaus war: das 1872–75 errichtete Königliche Oberbergamt war 1911 zum Museum für Kunst und Gewerbe umgebaut worden. Reygers entwickelte die Kriegsruine als Direktorin und gestaltende Bauherrin zu einem Museum der Gegenwartskunst weiter und strebte danach, in der Kombination von Alt und Neu eine „harmonische Atmosphäre“ entstehen zu lassen. Die verschiedenen historischen Schichten wurden unter ihrer Regie zu einer neuen Einheit verschmolzen. Das aus heutiger Perspektive eher unscheinbar wirkende Haus ist eines der ganz wenigen Zeugnisse eines Museumsbaus der unmittelbaren Nachkriegszeit in Deutschland und zudem ein exemplarisches Beispiel für die architektonische Strategie des Einfügens im Umgang mit Kriegsruinen.

Narrative Deliberation? On Storytelling as a Necessary Component of Public Deliberation

This paper deals with the place of narrative, that is, storytelling, in public deliberation. A distinction is made between weak and strong conceptions of narrative. According to the weak one, storytelling is but one rhetorical device among others with which social actors produce and convey meaning. In contrast, the strong conception holds that narrative is necessary to communicate, and argue, about topics such as the human experience of time, collective identities and the moral and ethical validity of values. The upshot of this idea is that storytelling should be a necessary component of any ideal of public deliberation. Contrary to recent work by deliberative theorists, who tend to adopt the weak conception of narrative, the author argues for embracing the strong one. The main contention of this article is that stories not only have a legitimate place in deliberation, but are even necessary to formulate certain arguments in the fi rst place; for instance, arguments drawing on historical experience. This claim, namely that narrative is constitutive of certain arguments, in the sense that, without it, said reasons cannot be articulated, is illustrated by deliberative theory’s own narrative underpinnings. Finally, certain possible objections against the strong conception of narrative are dispelled.
