O câncer colorretal é um dos tumores humanos mais freqüentes e a terceira causa de morte relacionada ao câncer no mundo. Apesar de importantes progressos terapêuticos, os resultados na doença avançada ainda são muito modestos. Isto deve-se ao fato de que a droga mais utilizada nesta neoplasia, o antimetabólito 5-fluorouracil (5-FU), foi desenvolvido a mais de 40 anos produzindo taxas de respostas de somente 10-15%. Recentemente, o inibidor da topoisomerase I irinotecan (CPT-11) demonstrou, em carcinoma de cólon avançado, respostas comparáveis tanto em pacientes não tratados quanto naqueles que tiveram recaída após terapia com 5-FU. Estes resultados justificam a avaliação da combinação 5-FU/CPT-11 nesta doença. Apesar das respostas dos estudos clínicos serem promissoras, a melhor seqüência de administração destes agentes ainda não foi determinada. Neste estudo, avaliamos a combinação CPT-11/5-FU quanto ao aumento da inibição do crescimento celular quando comparada com os agentes sozinhos nas linhagens celulares de carcinoma de cólon humano SW620, HT-29 e SNU-C4. Para isto, as células foram expostas às drogas sozinhas ou a várias combinações e seqüências de dose baixa e fixa (IC20) de um dos agentes na presença de dose alta e seriada do outro. As células foram avaliadas imediatamente após os tratamentos e/ou cultivadas por mais 2 dias em meio de cultura sem drogas através de coloração com sulforodamina B. As interações entre CPT-11 e 5-FU foram avaliadas por um programa de computador que permite calcular os índices de combinação (CIs) das drogas indicando sinergismo, adição, ou antagonismo (CI < 1, = 1, ou > 1, respectivamente). As respostas celulares foram relacionadas com as atividades das enzimas timidilato sintase, topoisomerase I e carboxil esterase, que foram determinadas através de ensaio que mede os sítios de ligação e atividade catalítica da enzima, ensaio de decatenação do DNA e método espectrofotométrico, respectivamente. Estando a toxicidade no DNA envolvida no mecanismo de ação das duas drogas, também relacionamos as respostas celulares com a introdução de danos ao DNA por método fluorescente. Para melhor entendermos as interações entre as drogas, examinamos os efeitos da exposição à IC20, IC50 e/ou IC80 do CPT-11 ou 5-FU por 2 h ou 24 h em alvos celulares possivelmente relacionados com a citotoxicidade destes agentes. Estes incluem: capacidade de reparo por excisão do 10 DNA, distribuição das células nas fases do ciclo celular, integridade da membrana plasmática e formação de complexos DNA-topoisomerase I. Para isto, utilizamos método de incorporação de [3H-metil]timidina, citometria de fluxo, liberação de lactato desidrogenase (LDH) no meio de cultura e ensaio de precipitação com SDS, respectivamente. Os estudos de inibição do crescimento celular revelaram valores de IC50 do 5-FU nas linhagens SW620, HT-29 e SNU-C4 de aproximadamente 15, 8 e 2 µM, respectivamente, e do CPT-11 próximos a 2, 2 e 4 µM, respectivamente. As diferentes sensibilidades ao 5-FU nas três linhagens foram determinadas principalmente pela diferença na afinidade ao substrato. As respostas comparáveis obtidas pela exposição ao CPT-11 podem ser explicadas pelo equilíbrio entre as diferentes atividades das enzimas topoisomerase I e carboxil esterase, entre as linhagens. O programa de análise da combinação das drogas mostrou adição ou sinergismo após exposição a IC20 do CPT-11 seguido do 5-FU, nas três linhagens celulares. Por outro lado, o pré-tratamento com IC20 do 5-FU antagonizou a inibição do crescimento mediada pelo CPT-11. Nenhum dos tratamentos simultâneos determinou um aumento na inibição do crescimento nas linhagens SW620 e HT-29; mas mostraram adição ou antagonismo na linhagem SNU-C4. Observamos um significativo acréscimo na introdução de danos ao DNA nas linhagens celulares SW620 e HT-29 somente quando a IC20 do CPT-11 precedeu a IC50 do 5-FU. Já na linhagem SNU-C4, não somente este tratamento, mas também a utilização simultânea dos agentes introduziu mais dano ao DNA. Os tratamentos por 2 h ou 24 h com IC20, IC50, e/ou IC80 do 5-FU ou CPT-11 não causaram mudanças significativas na distribuição das células nas fases do ciclo celular, concentração de LDH no meio de cultura, ou formação de complexos DNA-topoisomerse I. Indicando que alterações nestes processos não estão envolvidas com os efeitos modulatórios dos pré-tratamentos na citotoxicidade das drogas. Entretanto, a incorporação de [3H-metil]timidina no DNA de células tratadas com CPT-11, aumentou em função da dose e tempo de exposição à droga. Este resultado sugere que o dano ao DNA introduzido pelo CPT-11 depende da dose e tempo de exposição podendo ser reparado por mecanismos de excisão. Somente foi observada significativa incorporação de [3H-metil]timidina em células tratadas com IC20 do 5-FU por 2 h, sugerindo que o reparo por excisão só ocorre após exposição por curto período e à doses baixas deste agente. Juntos, os resultados deste estudo mostraram que tanto os efeitos anti-proliferativos quanto a introdução de danos ao DNA pela combinação CPT-11/5-FU em dose baixa e fixa de um agente junto com dose alta do outro, nas linhagens celulares de carcinoma de cólon humano SW620, HT-29 e SNU-C4, dependem da seqüência de administração das drogas. Estes achados podem ter sido determinados pelos diferentes efeitos do tratamento com uma dose baixa dos agentes nos mecanismos de reparo por excisão do DNA.

Compounds derived from fungi has been the subject of many studies in order to broaden the knowledge of their bioactive potential. Polysaccharides from Caripia montagnei have been described to possess anti-inflammatory and antioxidant properties. In this study, glucans extracted from Caripia montagnei mushroom were chemically characterized and their effects evaluated at different doses and intervals of treatment. It was also described their action on colonic injury in the model of colitis induced by 2,4,6-trinitrobenzene sulfonic acid (TNBS), and its action on cells of the human colon carcinoma (HT-29). Compounds extracted of C. montagnei contain high level of carbohydrates (96%), low content of phenolic compounds (1.5%) and low contamination with proteins (2.5%). The (FT-IR) and (NMR) analysis showed that polysaccharides from this species of mushroom are composed of α- and β-glucans. The colonic damage was evaluated by macroscopic, histological, biochemical and immunologic analyses. The results showed a reduction of colonic lesions in all groups treated with the glucans of Caripia montagnei (GCM). GCM significantly reduced the levels of IL-6 (50 and 75 mg/kg, p < 0.05), a major inflammatory cytokine. Biochemical analyses showed that such glucans acted on reducing levels of alkaline phosphatase (75 mg/kg, p < 0.01), nitric oxide (p < 0.001), and myeloperoxidase (p < 0.001). These results were confirmed microscopically by the reduction of cellular infiltration. The increase of catalase activity suggest a protective effect of GCM on colonic tissue, confirming their anti-inflammatory potential. GCM displayed cytostatic activity against HT-29 cells, causing accumulation of cells in G1 phase, blocking the cycle cell progression. Those glucans also showed ability to modulate the adhesion of HT-29 cells to Matrigel® and reduced the oxidative stress. The antiproliferative activity against HT-29 cells displayed by GCM (p <0.001) can be attributed to its cytostatic activity and induction of apoptosis by GCM

Background: Hydroxyurea (HU), an inhibitor of ribonucleotide reductase, may potentiate the activity of 5-fluorouracil (5-FU) and folinic acid (FA) by reducing the deoxyribonucleotide pool available for DNA synthesis and repair. However as HU may inhibit the formation of 5-fluoro-2-deoxyuridine-5- monophosphate (FdUMP), one of the principal active metabolites of 5-FU, the scheduling of HU may be critical. In vitro experiments suggest that administration of HU following 5-FU, maintaining the concentration in the region of I mM for six or more hours, significantly enhances the efficacy of 5-FU. Patients and methods: 5-FU/FA was given as follows: days 1 and 2 - FA 250 mg/m 2 (max. 350 mg) over two hours followed by 5-FU 400 mg/m 2 by intravenous bolus (ivb) over 15 minutes and subsequently 5-FU 400 mg/m 2 infusion (ivi) over 22 hours. HU was administered on day 3 immediately after the 5-FU with 3 g ivb over 15 minutes followed by 12 g ivi over 12 hours. Results: Thirty patients were entered into the study. Median survival was nine months (range 1-51 + months). There were eight partial responses (28%, 95% CI: 13%-47%). The median duration of response was 6.5 (range 4-9 months). Grade 3-4 toxicities included neutropenia (grade 3 in eight patients and grade 4 in five), anaemia (grade 3 in one patient) and diarrhoea (grade 3 in two patients). Neutropenia was associated with pyrexia in two patients. Phlebitis at the infusion site occurred in five patients. The treatment was complicated by pulmonary embolism in one patient and deep venous thrombosis in another. Conclusion: HU administered in this schedule is well tolerated. Based on these results and those of other phase II studies, a randomised phase III study of 5-FU, FA and HU versus 5-FU and FA using the standard de Gramont schedule is recommended.

Objective To evaluate the efficacy and toxicity of Oxaliplatin and 5-Fluorouracil (5-FU)/Leucovorin (LV) combination in ovarian cancer relapsing within 2 years of prior platinum-based chemotherapy in a phase II trial. Methods Eligible patients had at least one prior platinum-based chemotherapy regimen, elevated CA-125 ≥ 60 IU/l, radiological evidence of disease progression and adequate hepatic, renal and bone marrow function. Patients with raised CA-125 levels alone as marker of disease relapse were not eligible. Oxaliplatin (85 mg/m 2) was given on day 1, and 5-Fluorouracil (370 mg/m 2) and Leucovorin (30 mg) was given on days 1 and 8 of a 14-day cycle. Results Twenty-seven patients were enrolled. The median age was 57 years (range 42-74 years). The median platinum-free interval (PFI) was 5 months (range 0-17 months) with only 30% of patients being platinum sensitive (PFI > 6 months). Six patients (22%) had two prior regimens of chemotherapy. A total of 191 cycles were administered (median 7; range 2-12). All patients were evaluable for toxicity. The following grade 3/4 toxicities were noted: anemia 4%; neutropenia 15%; thrombocytopenia 11%; neurotoxicity 8%; lethargy 4%; diarrhea 4%; hypokalemia 11%; hypomagnesemia 11%. Among 27 enrolled patients, 20 patients were evaluable for response by WHO criteria and 25 patients were evaluable by Rustin's CA-125 criteria. The overall response rate (RR) by WHO criteria was 30% (95% CI: 15- 52) [three complete responses (CRs) and three partial responses (PRs)]. The CA-125 response rate was 56% (95% CI: 37-73). Significantly, a 25% (95% CI: 9-53) radiological and a 50% (95% CI: 28-72) CA-125 response rate were noted in platinum resistant patients (PFI < 6 months). The median response duration was 4 months (range 3-12) and the median overall survival was 10 months. Conclusion Oxaliplatin and 5-Fluorouracil/ Leucovorin combination has a good safety profile and is active in platinum-pretreated advanced epithelial ovarian cancer. © 2004 Elsevier Inc. A administração intraperitoneal de 5-fluorouracil (5-FU) no pós-operatório imediato reduz a recorrência local e prolonga a sobrevida dos pacientes com câncer colônico. Contudo, esse tratamento também pode prejudicar a cicatrização das anastomoses intestinais. O objetivo deste estudo foi determinar os efeitos da quimioterapia (QT) intraperitoneal (IP) pós-operatória (PO) precoce com o 5-FU e da selagem anastomótica com o TachoSil sobre o processo de cicatrização de anastomoses colônicas. Quarenta ratos foram divididos em quatro grupos (I - IV, com dez ratos em cada) e submetidos à secção do cólon esquerdo seguida por anastomose. As anastomoses dos ratos dos grupos II e IV foram cobertas com o TachoSil. Solução salina (2 ml/dia grupos I e II) ou 5-FU (20 mg/kg/dia grupos III e IV) foi administrado por via IP uma vez ao dia, desde do procedimento cirúrgico até a morte programada dos animais no quarto dia pós-operatório. Foram realizadas medidas da pressão de ruptura e análise histopatológica das anastomoses. A perda relativa de peso foi significativamente maior nos animais do grupo III comparado a todos os demais grupos (p=0,0004). Não houve diferença significativa entre os grupos no que se refere à presença de fístulas, coleções perianastomóticas, sinais de dilatação intestinal pré-anastomótica ou aderências pós-operatórias. A pressão de ruptura foi significativamente menor no grupo III comparada a todos os demais grupos (p=0,001). A neoangiogênese foi significativamente menor no grupo III comparada aos grupos I e II (p=0,05). A infiltração fibroblástica foi significativamente maior no grupo I e em comparação ao grupo III (p=0,035). Não ocorreu diferença significativa entre os grupos no que concerne à presença de infiltração de células inflamatórias e deposição de colágeno. Os dados obtidos permitem concluir que a QT IP precoce com 5-FU afetou negativamente a fase inicial da cicatrização de anastomoses colônicas. Contudo, a selagem com o TachoSil foi capaz de reverter alguns dos efeitos adversos decorrentes da QT.

Chemotherapy response rates for advanced colorectal cancer remain disappointingly low, primarily because of drug resistance, so there is an urgent need to improve current treatment strategies. To identify novel determinants of resistance to the clinically relevant drugs 5-fluorouracil (5-FU) and SN38 (the active metabolite of irinotecan), transcriptional profiling experiments were carried out on pretreatment metastatic colorectal cancer biopsies and HCT116 parental and chemotherapy-resistant cell line models using a disease-specific DNA microarray. To enrich for potential chemoresistance-determining genes, an unsupervised bioinformatics approach was used, and 50 genes were selected and then functionally assessed using custom-designed short interfering RNA(siRNA) screens. In the primary siRNA screen, silencing of 21 genes sensitized HCT116 cells to either 5-FU or SN38 treatment. Three genes (RAPGEF2, PTRF, and SART1) were selected for further analysis in a panel of 5 colorectal cancer cell lines. Silencing SART1 sensitized all 5 cell lines to 5-FU treatment and 4/5 cell lines to SN38 treatment. However, silencing of RAPGEF2 or PTRF had no significant effect on 5-FU or SN38 sensitivity in the wider cell line panel. Further functional analysis of SART1 showed that its silencing induced apoptosis that was caspase-8 dependent. Furthermore, silencing of SART1 led to a downregulation of the caspase-8 inhibitor, c-FLIP, which we have previously shown is a key determinant of drug resistance in colorectal cancer. This study shows the power of systems biology approaches for identifying novel genes that regulate drug resistance and identifies SART1 as a previously unidentified regulator of c-FLIP and drug-induced activation of caspase-8. Introduction: 5-Fluorouracil (5-FU) is considered to be the backbone of colorectal cancer (CRC) systemic therapy since the great majority of recommended regimens include its administration. A clinical picture consisting of chest pain, sometimes cardiac enzyme elevation, electrocardiogram abnormalities consistent with myocardial ischemia, and normal coronary angiogram associated with 5-FU administration have been infrequently reported. The clinical dilemma is: Which chemotherapy regimen should we use in CRC patients with a previous acute coronary syndrome (ACS) associated with 5-FU? Case Report: We describe the case of a 55-year-old otherwise healthy woman with metastatic colon adenocarcinoma who presented an ACS probably secondary to arterial vasospasm while receiving continuous intravenous 5-FU infusion (mFOLFOX6 regimen). After the ACS, the patient was treated with raltitrexate plus oxaliplatin (TOMOX) and subsequently with irinotecan plus cetuximab with no other cardiac event. Conclusion: The risk of cardiotoxicity associated with 5-FU is low but real. The probable mechanism is arterial vasospasm, as suggested by our case report. Both the use of the TOMOX regimen and irinotecan plus cetuximab seems to be safe regimens to be considered in this clinical scenario. © 2009 Humana Press Inc.

A quimioterapia antitumoral convencional, baseada na aplicação da dose máxima tolerável de drogas antineoplásicas, está comumente associada com mielossupressão e causa diminuição da viabilidade e número de células dendríticas (DCs), principais células apresentadoras de antígenos (APCs) do sistema imune. Uma alternativa para limitar os efeitos tóxicos desses agentes é a quimioterapia metronômica, baseada na administração das drogas em doses baixas e frequentes que, entre outros aspectos, melhora a eficácia antitumoral pela inibição da angiogênese no sítio do tumor. Estudos prévios do grupo indicam que doses ultrabaixas de determinados agentes antineoplásicos modulam positivamente o estado de ativação e as funções das células dendríticas, sugerindo que a associação entre vacinas de DCs e quimioterapia metronômica possibilitaria a estimulação da resposta imune, ao mesmo tempo em que as células tumorais sofrem o efeito dos agentes antineoplásicos em doses ultrabaixas. Assim, hipotetizamos que o uso de 5-Fluorouracil e Leucovorina, drogas de escolha para o tratamento do câncer colorretal, em dose efetiva mínima ou dose ultrabaixa não tóxica, poderia modular positivamente as DCs humanas e potencializar a imunogenicidade de células tumorais, além de torná-las mais sensíveis à ação de linfócitos T citotóxicos. Desse modo, o presente estudo tem o objetivo de avaliar o efeito in vitro da dose efetiva mínima e dose não tóxica de 5-Fluorouracil e Leucovorina sobre as funções de células dendríticas humanas e sobre a imunogenicidade de células de câncer colorretal humano da linhagem HT-29. Com esse propósito as DCs humanas foram sensibilizadas com lisatos de células tumorais pré tratadas com as doses previamente determinadas das drogas e avaliadas quanto à capacidade de apresentação de antígenos e indução de linfócitos T citotóxicos... 5-Fluorouracil (5-FU) is one of the most widely used drugs for treatment of cancers, including breast cancer that exhibits its anticancer activity by inhibiting DNA synthesis and also incorporated into DNA and RNA. The objective of this investigation was to find out the total nucleotide metabolism genes regulated by 5-FU in breast cancer cell line. The breast cancer cell line MCF-7 was treated with the drug 5-FU. To analyze the expression of genes, we have conducted the experiment using 1.7k and 19k human microarray slide and confirmed the expression of genes by semiquantitative reverse transcription-polymerase chain reaction. The expression of 44 genes involved in the nucleotide metabolism pathway was quantified. Of these 44 genes analyzed, transcription of 6 genes were upregulated and 9 genes were downregulated. Earlier studies revealed that the transcription of genes for key enzymes like thymidylate synthase, thymidinekinase, and dihydropyrimidine dehydrogenase are regulated by 5-FU. This study identified some novel genes like thioredoxin reductase, ectonucleotide triphosphate dephosphorylase, and CTP synthase are regulated by 5-FU. The data also reveal large-scale perturbation in transcription of genes not involved directly in the known mechanism of action of 5-FU.

5-Fluorouracil (5FU), an analogue of uracil, was found to inhibit the production of infectious particles of rinderpest virus (RPV) in Vero cells (African green monkey kidney cells) by 99%, at a concentration of 1 μg/ml. The levels of individual mRNA specific for five of the virus genes were also reduced drastically, while the level of mRNA for a cellular housekeeping gene—glyceraldehyde-3-phosphate dehydrogenase (GAPDH)—was unaltered by fluorouracil treatment of infected cells. Both virus RNA and protein synthesis showed inhibition in a dose-dependent manner. The virions which budded out of 5-fluorouracil-treated cells also contained reduced amounts of virus proteins compared with virus particles from untreated cells.

5-fluorouracil (FUra) has been shown to modulate the aminoacylation function of rat liver tRNA. The present study was aimed at studying the structure-function relationship of FUra-substituted tRNA. Male Wistar rats (2-3 month old) were given a single i.p. injection of FUra at 50, 250, or 500 mg/kg body wt. and FUra-substituted total liver tRNA, i.e. tRNA(FUra50, 250, and 500, respectively, were isolated 3 h later. Normal tRNA (tRNA(N)) was isolated from saline-treated control rats. Thermal denaturation studies showed higher melting temperatures for tRNA(FUra) compared to tRNA(N). Heat denaturation followed by renaturation of total tRNA did not affect the activity of tRNA(N) and tRNA(FUra50), where as tRNA(FUra250 and 500) lost 35% and 72% of activity, respectively, compared to the corresponding group of non-denatured tRNA. Antibodies specific to rat liver tRNA recognized normal and FUra-substituted tRNA in the order of tRNA(N) > tRNA(FUra50) > or = tRNA(FUra250) > tRNA(FUra500) in an avidin-biotin micro-enzyme linked immunosorbant assay. tRNA(N) or tRNA(FUra50) preincubated with tRNA antiserum showed 74% and 59% of aminoacylation activity, respectively, compared to that of corresponding tRNA preincubated with normal rabbit IgG. However, activities of similarly treated tRNA(FUra250 and 500) were not affected. The observations of possible changes in the secondary structure of rat liver tRNA upon incorporation of FUra are discussed.

he ultrastructure of purified rinderpest virus and intracellular viral nucleocapsids from infected vero cells treated with a subtoxic dose of 5-fluorouracil (5-Fu) (1 mug/ml), has been analysed by transmission electron microscopy, and compared with that of normal virus particle and nucleocapsids. The results reveal dramatic alterations in the structure of both virions and nucleocapsids. The surface glycoprotein projection of virions was not seen or present at a much reduced level. The intracellular nucleocapsids showed pronounced structural changes,with respect to size, shape and fine structure. The length of treated nucleocapsids is much smaller as compared to the control. The central hollow core is missing in case of drug-treated nucleocapsid and the herring bone structure is replaced by a 'beads on string' structure. The presence of N protein, which is a major structural component of nucleocapsids was seen in 5-Fu-treated cells, but it was associated with a predominantly diffused form of nucleocapsids as seen by immunoelectron microscopy. We report here the first definitive and visual evidence of altered structure of virions and their nucleocapsids after 5-Fu treatment

Folate-targeted poly[(p-nitrophenyl acrylate)-co-(N-isopropylacrylamide)] nanohydrogel (F-SubMG) was loaded with 5-fluorouracil (5-FU) to obtain low (16.3 +/- 1.9 mu g 5-FU/mg F-SubMG) and high (46.8 +/- 3.8 mu g 5-FU/mg F-SubMG) load 5-FU-loaded F-SubMGs. The complete in vitro drug release took place in 8 h. The cytotoxicity of unloaded F-SubMGs in MCF7 and HeLa cells was low; although it increased for high F-SubMG concentration. The administration of 10 mu M 5-FU by 5-FU-loaded F-SubMGs was effective on both cellular types. Cell uptake of F-SubMGs took place in both cell types, but it was higher in HeLa cells because they are folate receptor positive. After subcutaneous administration (28 mg 5-FU/kg b.w.) in Wistar rats, F-SubMGs were detected at the site of injection under the skin. Histological studies indicated that the F-SubMGs were surrounded by connective tissue, without any signs of rejections, even 60 days after injection. Pharmacokinetic study showed an increase in MRT (mean residence time) of 5-FU when the drug was administered by drug-loaded F-SubMGs.
