BACKGROUND AND PURPOSE: Mechanical thrombectomy is a promising new modality of interventional stroke treatment. The various devices differ with regard to where they apply force on the thrombus, taking a proximal approach such as aspiration devices or a distal approach such as basket-like devices. The study compares the in vivo effectiveness and thrombus-device interaction of these 2 approaches. METHODS: Angiography and embolization with a radioopaque whole blood thrombus was performed in 10 swine. Mechanical thrombectomy was performed in 20 cranial vessels using a proximal aspiration device (Vasco35) and a distal basket-like device (Catch) with and without proximal balloon occlusion. Fifty-six retrieval attempts were made. RESULTS: The proximal device allowed fast repeated application with a low risk of thromboembolic events (3%) and vasospasm, but it had a significantly lower success rate (39.4%) in retrieving thrombotic material than the distal device (DD) (82.6%; odds ratio, 7.3; 95% CI, 2.0 to 26.4). The compaction of the thrombus during retrieval with DD increased the risk of vessel wall irritation significantly (P<0.01) and complicated retrieval into the guiding catheter. The number of embolic events was significantly higher with DD (26%; odds ratio, 11.3; 95% CI, 1.35 to 101.6) unless proximal balloon occlusion was used. CONCLUSIONS: The proximal and the distal approaches to mechanical thrombectomy proved to be effective at achieving recanalization of cranial vessels. The proximal device is faster in application and allowed repeated attempts with a low complication rate. The DD is more successful at removing thrombotic material, but its method of application and attendant thrombus compaction increase the risk of thromboembolic events and vasospasms.

The effectiveness of low-level laser therapy in muscle regeneration is still not well known. To investigate the effects of laser irradiation during muscle healing. For this purpose, 63 rats were distributed to 3 groups: non-irradiated control group (CG); group irradiated at 10 J/cm(2) (G10); and group irradiated at 50 J/cm(2) (G50). Each group was divided into 3 different subgroups (n=7), and on days 7, 14 and 21 post-injury the rats were sacrificed. Seven days post-surgery, the CG showed destroyed zones and extensive myofibrillar degeneration. For both treated groups, the necrosis area was smaller compared to the CG. On day 14 post-injury, treated groups demonstrated better tissue organization, with newly formed muscle fibers compared to the CG. On the 21(st) day, the irradiated groups showed similar patterns of tissue repair, with improved muscle structure at the site of the injury, resembling uninjured muscle tissue organization. Regarding collagen deposition, the G10 showed an increase in collagen synthesis. In the last period evaluated, both treated groups showed statistically higher values in comparison with the CG. Furthermore, laser irradiation at 10 J/cm(2) produced a down-regulation of cyclooxygenase 2 (Cox-2) immunoexpression on day 7 post-injury. Moreover, Cox-2 immunoexpression was decreased in both treated groups on day 14. Laser therapy at both fluencies stimulated muscle repair through the formation of new muscle fiber, increase in collagen synthesis, and down-regulation of Cox-2 expression.

Alternative treatment for recurrent labial infection by herpes simplex virus (HSV) have been considered. The aim of this study was to evaluate the effectiveness of laser phototherapy in prevention and reduction of severity of labial manifestations of herpes labialis virus. Seventy-one patients, divided into experimental (n = 41) and control (n = 30) groups were followed up for 16 months. Patients in the control group were treated topically with aciclovir and patients in the experimental group were subjected to laser phototherapy (one session per week, 10 weeks): 780 nm, 60 mW, 3.0 J/cm(2) or 4.5 J/cm(2) on healthy (no HSV-1 infection) and affected (with HSV-1 infection) tissues. Patients in the experimental group presented a significant decrease in dimension of herpes labialis lesions (P = 0.013) and inflammatory edema (P = 0.031). The reduction in pain level (P = 0.051) and monthly recurrences (P = 0.076) did not reach statistical significance. This study represents an in vivo indication that this treatment should be further considered as an effective alternative to therapeutic regimens for herpes labialis lesions.

Aim To evaluate the use of pulse oximetry as a test for pulp vitality, by comparing in the same patient, the levels of oxygen saturation of the index finger and of the maxillary central incisor and canine teeth without clinically detectable pulp inflammation. Methodology Seventeen male and female patients aged between 26 and 38 years participated and a total of 32 maxillary central incisor and 32 canine teeth were analysed. Selection criteria required the teeth to have healthy crowns, or with restorations no more than 2 mm in diameter and no clinical and radiographical signs or symptoms of pulp or periapical inflammatory changes. The negative control group consisted of 10 root filled teeth. Measurements were first taken from the index finger of patients. Their teeth were then subjected to a thermal test with refrigerant gas and then to a vitality test with pulse oximetry. Data were analysed by Pearson`s and paired t-tests. Results There were no significant statistical correlations between blood oxygen levels in the index finger and in the teeth of the patient (P > 0.05). There was a statistically significant difference in the oxygen levels between the two tooth groups studied and the index finger (P <= 0.002). Mean oxygen values in the index finger of patients were 95% (SD = 1.6), oxygen values in the maxillary central incisor were 91.29% (SD = 2.61) and mean oxygen values in maxillary canine were 90.69% (SD = 2.71). Conclusion The method determined consistently the level of blood oxygen saturation of the pulp in maxillary central incisor and canine teeth and can therefore be used for pulp vitality testing. Further studies are required to assess the effectiveness and validity of pulse oximetry in determining pulp vitality in traumatized teeth.

Seed extracts of Aphanamixis polystachya Wall et Parker (pithraj) and Azadirachta indica A. Juss (neem) were evaluated for their in vivo and in vitro toxicity to Nephotettix cincticeps Uhler (rice green leafhopper). Crude extracts from both plants showed toxicity to leafhopper. Among them, the methanol extract of pithraj (MCX) was most toxic and showed 95% mortality effects at 72 h after treatment (HAT), followed by neem (74%). When LD50's were compared, it was found that the neem extract possessed the highest toxicity (LD50 16.59 μg/insect) at 72 HAT. Both the pithraj (MCX) and neem extracts showed their enzyme inhibition effectiveness against rice green leafhopper. The highest inhibition rate (IR) was caused by neem (60%) at the concentration of 2.0 mg/ml, followed by MCX (47%). The lowest IR50 value (0.97 mg/ml) was observed in neem at 30 min.

The effectiveness of methylene blue (MB) combined with pyrimethamine (PYR), chloroquine (CQ) or quinine (Q) was examined in a classical four-day suppressive test against a causative agent of rodent malaria, Plasmodium berghei. A marked potentiation was observed when MB was administered at a non-curative dose of 15 mg/kg/day in combination with PYR (0.19 mg/kg/day) or Q (25 mg/kg/day). No synergy was found between MB (15 mg/Kg) and CQ (0.75 mg/Kg). Our results suggest that the combination of MB with PYR or Q may improve the efficacy of these currently used antimalarial drugs.

The present paper describes the effect of metals ions on the in vitro availability of enoxacin (a second generation quinolone antibiotic) owing to drug-metal interaction. These interaction studies were performed at 37 °C in different pH environments simulating human body compartments and were studied by UV spectroscopic technique. In order to determine the probability of these reactions different kinetic parameters (dissolution constants (K) and free energy change (ΔG)) for these reactions were also calculated. It is proposed that the structure of enoxacin contains various electron donating sites which facilitate its binding with metallic cations forming chelates. Hence taking food products, nutritional supplements or multivitamins containing multivalent cations at the same time as enoxacin, could reduce the absorption of the drug into the circulation and thus would decrease the effectiveness of the drug. In addition, the MIC of enoxacin for various microorganisms before and after interaction with metal ions was calculated which in most cases was increased which possibly could impair the clinical efficacy of the drug.

OBJECTIVE: to compare the effects of low intensity laser therapy on in vitro bacterial growth and in vivo in infected wounds, and to analyze the effectiveness of the AsGa Laser technology in in vivo wound infections. METHODS: in vitro: Staphylococcus aureus were incubated on blood agar plates, half of them being irradiated with 904 nm wavelength laser and dose of 3J/cm2 daily for seven days. In vivo: 32 male Wistar rats were divided into control group (uninfected) and Experimental Group (Infected). Half of the animals had their wounds irradiated. RESULTS: in vitro: there was no statistically significant variation between the experimental groups as for the source plates and the derived ones (p>0.05). In vivo: there was a significant increase in the deposition of type I and III collagen in the wounds of the infected and irradiated animals when assessed on the fourth day of the experiment (p=0.034). CONCLUSION: low-intensity Laser Therapy applied with a wavelength of 904nm and dose 3J/cm2 did not alter the in vitro growth of S. aureus in experimental groups; in vivo, however, it showed significant increase in the deposition of type I and III collagen in the wound of infected and irradiated animals on the fourth day of the experiment.

Le diabète est un syndrome métabolique caractérisé par une hyperglycémie chronique due à un défaut de sécrétion de l’insuline, de l’action de l’insuline (sensibilité), ou une combinaison des deux. Plus d'un million de canadiens vivent actuellement avec le diabète. La prévalence de cette maladie est au moins trois fois plus élevée chez les autochtones que dans la population canadienne en général. Notre équipe vise à étudier les effets potentiellement antidiabétiques de certaines plantes médicinales utilisées par les Cris d'Eeyou Istchee (Baie James, Québec) où l’adhérence aux traitements médicamenteux est faible, en partie à cause de la déconnection culturelle de ces derniers. Grâce à une approche ethnobotanique, notre équipe a identifié 17 plantes médicinales utilisées par cette population pour traiter des symptômes du diabète. Parmi ces plantes, l’extrait éthanolique de Rhododendron groenlandicum (Thé du Labrador) a montré un fort potentiel antidiabétique chez plusieurs lignées cellulaires, notamment les adipocytes (3T3-L1). Cette plante induit la différenciation adipocytaire probablement par l’activation du peroxisome proliferator-activated receptor gamma (PPAR γ). Cette stimulation améliore la résistance à l’insuline et constitue un mécanisme privilégié pour une classe de médicaments antidiabétiques, les thiazolidinediones. Le but de la présente étude est de valider l’efficacité et l’innocuité de R. groenlandicum in vivo, dans un modèle animal de résistance à l’insuline, d’élucider les mécanismes par lesquels cet extrait exerce ses effets antidiabétiques et d’identifier les principes actifs responsables de son activité. L'isolation et l'identification des constituants actifs ont été réalisées à l’aide d'une approche de fractionnement guidé par bioessai; en l'occurrence, l'adipogénèse. Cette approche, réalisée dans la lignée adipocytaire 3T3-L1, a pour but de mesurer leur teneur en triglycérides. Des études in vivo ont été réalisées sur le modèle de souris DIO (diet induced obesity). L'extrait éthanolique du R. groenlandicum a été incorporé à la nourriture grasse (35% d’apport calorique lipidique) à trois doses différentes (125, 250 et 500 mg / kg) sur une période de 8 semaines. Des tissus cibles de l’insuline (foie, muscle squelettique et tissus adipeux) ont été récoltés afin de faire des analyses d’immunobuvardage de type western. La quercétine, la catéchine et l’épicatéchine ont été identifiées comme étant les composés actifs responsables de l'effet antidiabétique du R. groenlandicum. Seules la catéchine et l’épicatéchine activent l’adipogénèse uniquement à forte concentration (125-150 M), tandis que la quercétine l’inhibe. L’étude in vivo a montré que le traitement avec R. groenlandicum chez les souris DIO réduit le gain de poids de 6%, diminue l'hyperglycémie de 13% et l’insulinémie plasmatique de 65% et prévient l’apparition des stéatoses hépatiques (diminution de 42% de triglycéride dans le foie) sans être toxique. Les analyses d’immunobuvardage ont montré que R. groenlandicum stimule la voie de l’insuline via la phosphorylation de l’Akt et a augmenté le contenu protéique en Glut 4 dans les muscles des souris traitées. Par contre, dans le foie, le R. groenlandicum passerait par deux voies différentes, soit la voie insulino-dépendante par l’activation de l’AKT, soit la voie insulino-indépendante par la stimulation de l’AMPK. L’amélioration observée des stéatoses hépatiques chez les souris DIO traitées, a été confirmée par une baisse du facteur de transcription, SREBP-1, impliqué dans la lipogénèse de novo, ainsi qu’une diminution de l’inflammation hépatique (diminution de l’activité d’IKK α/β). En conclusion, l’ensemble de ces résultats soutiennent le potentiel thérapeutique de Rhododendron groenlandicum et de ses composants actifs dans le traitement et la prévention du diabète de type 2. Nous avons validé l'innocuité et l'efficacité de cette plante issue de la médecine traditionnelle Cri, qui pourrait être un traitement alternatif du diabète de type 2 dans une population ayant une faible adhérence au traitement pharmacologique existant.

Les effets bénéfiques des lipoprotéines de haute densité (HDL) contre l'athérosclérose ont été attribués, en grande partie, à leur composante protéique majeure, l'apolipoprotéine A-I (apoA-I). Cependant, il y a des indications que l'apoA-I peut être dégradée par des protéases localisées dans les plaques athérosclérotiques humaines, ce qui pourrait réduire l'efficacité des thérapies basées sur les HDL sous certaines conditions. Nous décrivons ici le développement et l'utilisation d'une nouvelle sonde bioactivatable fluorescente dans le proche infrarouge, apoA-I-Cy5.5, pour l'évaluation des activités protéolytiques spécifiques qui dégradent l'apoA-I in vitro, in vivo et ex vivo. La fluorescence basale de la sonde est inhibée par la saturation du fluorophore Cy5.5 sur la protéine apoA-I, et la fluorescence émise par le Cy5.5 (proche infrarouge) est révélée après clivage de la sonde. La protéolyse in vitro de l'apoA-I par des protéases a montré une augmentation de la fluorescence allant jusqu'à 11 fois (n=5, P ≤ 0.05). En utilisant notre nouvelle sonde apoA-I-Cy5.5 nous avons pu quantifier les activités protéolytiques d'une grande variété de protéases, incluant des sérines (chymase), des cystéines (cathepsine S), et des métalloprotéases (MMP-12). En outre, nous avons pu détecter l'activation de la sonde apoA-I-Cy5.5 sur des sections d'aorte de souris athérosclérotiques par zymographie in situ et avons observé qu'en présence d'inhibiteurs de protéases à large spectre, la sonde pourrait être protégée des activités protéolytiques des protéases (-54%, n=6, P ≤ 0,001). L'infusion in vivo de la sonde apoA-I-Cy5.5 dans les souris athérosclérotiques (Ldlr -/--Tg (apoB humaine)) a résulté en utilisant un système d'imagerie moléculaire combinant la fluorescence moléculaire tomographique et la résonance magnétique,en un signal de fluorescence dans l'aorte plus important que celui dans les aortes des souris de type sauvage C57Bl/6J (CTL). Les mesures in vivo ont été confirmées par l'imagerie ex vivo de l'aorte qui a indiqué une augmentation de 5 fois du signal fluorescent dans l'aorte des souris ATX (n=5) par rapport à l'aorte des souris (n=3) CTL (P ≤ 0,05). L'utilisation de cette sonde pourrait conduire à une meilleure compréhension des mécanismes moléculaires qui sous-tendent le développement et la progression de l'athérosclérose et l'amélioration des stratégies thérapeutiques à base de HDL.

The objective of this clinical study was to evaluate the effectiveness of the toothbrushing with and without fluoride and the daily fluoride rinse (NaF 0.05%) on produced white spot, in vivo. This was a clinical study, controlled, randomized and triple blind. Thirty patients were selected for orthodontics reasons from Orthodontics Specialization Course at the Brazilian Dental Association - Section of Rio Grande do Norte. In this study it was used 4 bicuspid upper and lower. They had orthodontic reason for extractions, in 35 days, at least. The sample had one hundred and twenty teeth that received orthodontic bands. The bands were fixed with polycarboxylate cement, and there was a space standardized between bands and one surface of teeth. The four bicuspid of each patients were randomized and nominated as A, B, C and D. These nominations determinated the sequence of the extractions and what was done in each tooth. All the patients had been submitted to the toothbrushing with or without fluoride for 35 days. After this period, the A tooth of each patient was extracted to serve as control. The others teeth (B, C and D) were extracted one by each week. The entire sample was analyzed through the clinical examination and by laser fluorescence (DIAGNOdent®) in three different times: before orthodontic bands, 28 days after fixed and then removed the bands and, the last one, 07 days after one of the three treatments (toothbrushing with or without fluoride, tooth paste with fluoride and mouth rinse with fluoride). At the beginning all groups (A, B, C and D) had the same conditions, no significant difference was found. The same situation was found in a clinical examination. The results of the DIAGNOdent® for the groups that used tooth paste without fluoride, with fluoride and mouth rinse with fluoride, after 28 days, there was no significant difference. Clinically, the white spot was formed in all teeth after 28 days. When it was compared the three treated groups, the group without fluoride in tooth paste had worst result than the others groups. But there was no significant association between the number of active and inactive white spots and the type of treatment that the teeth had received. The demineralization of the enamel surface, under the orthodontic bands, it happened in a few weeks. The exposition of the white spots in oral environmental resulted in an improvement, but it was not enough to return to the values from the base line, either for the toothbrushing and/or the use of fluorite mouth rinse. Mouth rinse and toothpaste with fluoride have showed to reduce the incidence of demineralization in the enamel, but none seems to be superior to another one in an in vivo study

The objective of this work was to evaluate the effect of the variables number of recipients, synchronization protocol, reproductive efficiency indicators and pregnancy cost, in the economic effectiveness of in vivo and in vitro bovine embryo production. A simulation application was elaborated to allow the user to insert the input variable parameters. A basic scenario, from the efficiency traditional rates of in vivo (ET) and in vitro production (IVP) techniques of bovine embryos, was introduced in the software as a criterion to compare the results. This software was able to reproduce both ET and IVP scenarios. The embryo production was simulated through stochastic simulation. The optimal number of recipients using sensitivity analysis was determined. The net present value and cost per pregnancy were used as a decision parameter. The synchronization for fixed-time embryo transfer decreased the recipient idleness and, consequently, the final cost of pregnancy, in comparison to the traditional methodology. Foetal sexing must be associated to IVP of bovine embryos. In addition, the optimal recipient number per donor is variable and depends on data inserted in the system.

The present study was designed to investigate the effectiveness of different ultrasonic instruments on the root surface. Fourteen patients with 35 single root teeth designated for extraction were recruited to the present study. Teeth were assigned to four experimental groups: group 1, piezoelectric ultrasonic device; group 2, magnetostrictive ultrasonic device; group 3, hand instrumentation; and group 4, untreated teeth (control). After instrumentation, the teeth were extracted and the presence of residual deposits (roughness and root surfaces characteristics) were analyzed. The results showed that residual deposits were similar in all tested groups: piezoelectric, 8.7%; magnetostrictive, 9.7%; hand instrumentation, 11.1% and control, 76.4%. There were statistically significant differences between control and all the experimental groups (p < 0.0001). With respect to roughness parameters evaluation, R(a) and R(z) of the roots treated with the different instruments showed a similar pattern (p > 0.05), but for R(t) and R(y), a significant difference was observed (p < 0.05) among hand instrumentation and ultrasonic devices. SEM analysis revealed a similar root surface pattern for the ultrasonic devices, but curettes showed many instrumental scratches, deep gouges, and a relatively large amount of dentin was removed. Within the limits of the study, although the instruments produced similar results, root surfaces instrumentated with curettes were rougher and had more root surface tissue removed than with the ultrasonic device.

The aim of this study was to evaluate the antimicrobial effect of biomechanical preparation using different irrigating solutions. Seventy-eight root canals from premolars of four dogs were used. After experimental induction of periapical lesions, the root canals were prepared using the following solutions for irrigation: Group 1) 2.5% sodium hypochlorite (NaOCl); Group 2) 2% chlorhexidine (CHX); Group 3) saline solution and Group 4) control group with no biomechanical preparation. The microbiological evaluation of the root canals was performed by counting the colony forming units (CFUs) using different culture mediums. Two absorbent paper cones were used in each root canal in order to collect the microbiological samples before, and thirty days after the biomechanical preparation. The culture plates were incubated in aerobic, anaerobic and microaerophilic environment. Statistical evaluation was carried out using analysis of variance, Tukey and Student tests. The results demonstrated that there was reduction in the number of microorganisms in the NaOCl and CHX groups (p<0.05). There was greater effectiveness in the chlorhexidine group. The group that used saline solution and the control group presented an increased number of microorganisms. It can be concluded that the use of antimicrobial irrigating solutions during biomechanical preparation promotes the reduction of endodontic microbiota. However, a considerable number of microorganisms were still observed.
