Objective: To analyze how social representations of hospital and community care are structured in two groups of nursing students – 1st and 4th years. Method: Qualitative research oriented by the Theory of Social Representations. We used a questionnaire with Free Association of Words. Data were analyzed in the Software IRaMuTeQ 0.6 alpha 3. Results: We applied the method of Descending Hierarchical Classifi cation and obtained four classes. Class 4 has the largest social representation (30.41%) within the corpus. The two organizational axes are nurse and disease/patient in the central core. On the periphery are the care and help related to the nurse and the treatment and prevention associated with the disease. Conclusion: Social representations focus on disease/patient and on the role of nurses in the treatment, prevention, and care. Health promotion and the social determinants of health are absent from the social representations of students.

Este artigo apresenta aspectos da trajetória do Hospital de Caridade São Pedro de Alcântara, na cidade de Goiás, ao longo do século XIX. Instituição leiga fundada no ano de 1825, o hospital nasceu da iniciativa de um grupo local influente que reconheceu um mal social: a ausência de assistência aos destituídos e enfermos. No que se referia à assistência social, ele abarcava funções e princípios caritativos cristãos, assistindo alienados, internados em cárceres, doentes e necessitados em geral, e, com a inauguração do cemitério público, sepultava gratuitamente os indigentes.

We seek to examine the efficacy and safety of prereperfusion emergency medical services (EMS)–administered intravenous metoprolol in anterior ST-segment elevation myocardial infarction patients undergoing eventual primary angioplasty. This is a prespecified subgroup analysis of the Effect of Metoprolol in Cardioprotection During an Acute Myocardial Infarction trial population, who all eventually received oral metoprolol within 12 to 24 hours. We studied patients receiving intravenous metoprolol by EMS and compared them with others treated by EMS but not receiving intravenous metoprolol. Outcomes included infarct size and left ventricular ejection fraction on cardiac magnetic resonance imaging at 1 week, and safety by measuring the incidence of the predefined combined endpoint (composite of death, malignant ventricular arrhythmias, advanced atrioventricular block, cardiogenic shock, or reinfarction) within the first 24 hours. From the total population of the trial (N=270), 147 patients (54%) were recruited during out-of-hospital assistance and transferred to the primary angioplasty center (74 intravenous metoprolol and 73 controls). Infarct size was smaller in patients receiving intravenous metoprolol compared with controls (23.4 [SD 15.0] versus 34.0 [SD 23.7] g; adjusted difference –11.4; 95% confidence interval [CI] –18.6 to –4.3). Left ventricular ejection fraction was higher in the intravenous metoprolol group (48.1% [SD 8.4%] versus 43.1% [SD 10.2%]; adjusted difference 5.0; 95% CI 1.6 to 8.4). Metoprolol administration did not increase the incidence of the prespecified safety combined endpoint: 6.8% versus 17.8% in controls (risk difference –11.1; 95% CI –21.5 to –0.6). Out-of-hospital administration of intravenous metoprolol by EMS within 4.5 hours of symptom onset in our subjects reduced infarct size and improved left ventricular ejection fraction with no excess of adverse events during the first 24 hours.

Esse estudo teve por objetivo analisar as dinâmicas de mudanças organizacionais transcorridas em um estabelecimento de saúde. Conduzido através metodologia de estudo de caso descritivo, teve como campo de pesquisa o Instituto de Hematologia do Estado do Rio de Janeiro. Buscando conhecer o papel da preparação para acreditação na dinâmica da mudança em uma organização de saúde essa pesquisa foi assim estruturada: abordagem dos problemas encontrados nas mudanças organizacionais em estabelecimentos de saúde; quadro teórico estruturado com uma revisão de literatura; embasamento da metodologia aplicada, com definição de instrumentos de coletas de dados, de material e atores implicados no levantamento para realização da análise qualitativa. O estudo analisou as seguintes variáveis: a natureza da mudança focalizando a extensão, ritmo e trajetória; as estratégias de ação, contemplando as situações de adesões e resistência e a concepção, verificando se as mudanças foram indutivas ou dedutivas. O resultado demonstrou que a preparação para acreditação naquele hospital, proporcionou mudanças com movimentos lentos, mas com continuidade em todos os setores do estabelecimento. Foi identificada participação mais ativa de um grupo de profissionais identificados como facilitadores, funcionando como multiplicadores. A abrangência das estratégias aplicadas foram desde as reuniões em assembléias gerais, á formação de grupos de estudo por setores para entendimento do manual de padrões de acreditação. Foram realizados processos internos de auto-avaliação com base no manual de acreditação. Em relação á concepção, o processo de mudança foi motivado pela determinação da direção do hospital para obtenção do certificado de acreditação internacional. Quanto á resistência e adesão, o estudo demonstrou que a participação de uma grande maioria dos profissionais foi motivada pelo desejo de aprender e desenvolver novas práticas que proporcionasse a melhoria da qualidade da assistência. A análise de dados aponta certa resistência da categoria médica no início do processo. Do ponto de vista organizacional, foram criadas novas estruturas. A conclusão do estudo: O processo de preparação para acreditação na unidade de saúde estudada demonstrou ser um instrumento capaz de promover mudanças em organizações de saúde.

O uso das informações e indicadores provenientes do Sistema de Informações Hospitalares do SUS (SIH/SUS), tanto para análise de situação de saúde da população como para análise do desse, SUS, é cada vez mais frequente. Tal sistema é, desde sua concepção, fortemente influenciado pelas políticas públicas na área de atenção à saúde, como as definidas pelas Normas Operacionais e os incentivos e restrições a determinadas práticas. Alterações na operação do sistema introduzem descontinuidades e vieses nas informações, provocando eventuais imprecisões ou mesmo distorções nos resultados da extração de dados do sistema. Para que se possa avaliar o resultados de políticas, a situação da assistência à saúde ou as condições de saúde de uma população, é necessário, portanto, que se tenha uma visão clara e objetiva de quais informações são disponíveis, a sua evolução e como utiliza-las, considerando devidamente as influências exógenas e endógenas do sistema. O presente estudo está estruturado de acordo com o contexto do Sistema de Informações Hospitalares. Como componente da Previdência Social, é estudada a criação do Sistema de Assistência médico-Hospitalar da Previdência Social (SAMHPS), as suas origens e seus eixos estruturantes, assim como a sua expansão para a rede filantrópica e de ensino, com a ampliação de sua cobertura. Já no contexto do Sistema Único de Saúde (SUS), é estudada a incorporação do SAMHPS ao SUS, levando à criação do SIH/SUS, com a expansão para a rede pública e a sua universalização. A influência das políticas de saúde é analisada a partir da implantação de incentivos e restrições que afetam a assistência hospitalar e seu reflexo nas informações do SIH/SUS. A forma de categorização e a identificação dos prestadores são também examinadas, tendo em vista a sua importância na análise e determinação de políticas de saúde. Outro aspecto que é analisado é a forma de apropriação das informações do diagnóstico que levou à internação: a adoção da 10 Revisão da Classificação Internacional de Doenças, o caso específico das causas externas e a implantação da Tabela de Compatibilidade entre Procedimentos e Diagnósticos. Para identificar as mudanças políticas e operacionais do SUS, da regulamentação da assistência hospitalar e do SIH/SUS, foi pesquisada sua legislação Leis, Decretos, Normas Operacionais, Portarias, Instruções e Manuais. O relacionamento entre as informações e as políticas é analisado identificando a implantação destas políticas e verificando o efeito sobre os indicadores da assistência hospitalar obtidos do SIH/SUS. Como conclusão, foi visto que análises que utilizem as séries históricas devem, obrigatoriamente, levar em consideração as modificações, tanto do SUS como do SIH/SUS, para que possam chegar a conclusões mais precisas. Descontinuidades nas séries históricas efetivamente mostram modificações das políticas e da operação do sistema. A facilidade de acesso, a disponibilidade, a oportunidade e rapidez de atualização das informações do SIH/SUS são fatores positivos do sistema; é possível analisar o efeito de determinada ação pouco após a sua implantação. A análise das informações do diagnóstico denota a necessidade de treinamento dos codificadores no uso da CID-10 em morbidade e de uma ampla revisão da Tabela de Compatibilidade entre Diagnóstico Principal e o Procedimento Realizado.

This study has as general object: to verify as the health professionals that work in different hospitals evaluate the hospital services; and specific objectives: identify how health professionals evaluate the work conditions in different hospitals and verify the central nucleus and the peripheral elements of the social representations elaborated by these professionals about these institution. This is a descriptive study realized in Natal, capital of Rio Grande do Norte. Were collected 213 questionnaires. Concerning the evaluation of the health services, high averages had been verified in the philanthropic hospital, for example, quality of the customer service. At the state hospital was observed lowest ones: respect to the privacy of the patients. Similar results were found about work conditions. It was observed as central nucleus the categories Overcrowded and Humanized care, in the state and philanthropic hospital respectively, and as peripheral elements Low wages and Overcrowded. Being thus the conflicts in this scene are inevitable because of the poor structure of some public hospitals, however, these problems can be foresee and solvable if the hospital has a free expression channel accessible to all agents. The evaluation has to become integrant part of the culture of the organization, a time that this will guide the steps in direction to the best quality in the hospital assistance

This is an exploratory and descriptive study that was jointly carried out by Nursing Care and Occupational Therapy as part of a Research Project that intended to prepare children for elective surgery at the University of São Paulo's Hospital de Reabilitação de Anomalias Craniofaciais. Objective: using toys as a therapeutic resource for relieving the child's real and unconscious tensions concerning hospitalization for surgical treatment at the HRAC - USP. Method: 44 children participated in the study. An observation form was used to collect data and it was applied at two separate times: the first time was the day before the surgery was to take place and the second on the day of the surgery just before the event. Twenty one variables were elaborated by the researchers to categorize behaviors regarding hospitalization. The resources used were: storytelling, dramatization and demonstration of nursing interventions with puppets dressed in surgical garb (gloves, surgical gown, mask and cap) with medical equipment commonly used in hospital. Results: of the 21 variables analyzed, 8 showed statistically significant differences on the McNemar Test (p<0.05). Conclusion: interactive play enables hospitalized children to interact in the hospital environment, so that they can express feelings and emotions and it contributes to humanized hospital assistance.

Introduction The use of oral rehydration solutions in our context remains limited. This study was conducted to analyze the rate of this use in Douala, Cameroon and thereby determine the factors associated with it. Method A cross-sectional survey was administered to parents of children aged five years and younger during a six-month study period. The studied variables focused on the socio-demographic data of the population, data on diarrhoea and its severity, data on oral rehydration salts (ORS), and data related to other interventions for diarrhoea. The chi-square test was used to qualify associations between variables, with the significance level being set at 5%. Results Overall, 672 people agreed to participate in the study. Among them, 418 (62.2%) correctly defined diarrhoea. When their children develop diarrhoea, the majority of the parents (348, 51.8%) reported seeking hospital assistance before any intervention, while 225 parents (33.5%) preferred the use of ORS first. Four hundred seventy-five parents (70.7%) had heard of ORS and among them 313 (65.9%) had actually given ORS to their children as treatment during these children’s most recent episodes of diarrhoea. Of the parents who had given their children ORS, 217 (69.3%) knew how to prepare it, and 122 (39.0%) knew how to administer it. One hundred thirty-five parents (20.1%) had administered metronidazole to treat their children’s diarrhoea. The age of the children, the parents’ level of education, and the number of children in the household significantly influenced the use or non-use of ORS (respectively, p < 0.001, p = 0.003 and p < 0.0001). Rehydration was correctly identified by 234 parents (34.8%) as the purpose of administering ORS. Conclusion The knowledge and the use of ORS in diarrhoea by the study sample was insufficient. The role of ORS was poorly known. Awareness campaigns can be carried out in order to improve the use of this effective intervention for diarrhoea.

OBJETIVE: This study aimed to assess the practices of pharmacists in Hospital Care. Method - we interviewed 20 pharmacists from the Pharmacy Division by applying a structured instrument, in September 2005. This instrument addressed aspects related to the main activities at the Hospital Pharmacy, which were assessed according to indicators organized into five areas: sector management, hospital pharmacotechniques, committee activities, information and pharmacotherapeutic follow-up, as well as teaching and research activities.RESULTS: the Pharmacy Division considered all structural aspects under analysis as essential for the good development and application of its services. We found that some essential services, such as the Medication Information Service and Pharmacotherapeutic Follow-up, were absent. Pharmacist professionals were dissatisfied about human resource and physical structure dimensioning, and they presented as not very active in terms of Pharmaceutical Care.CONCLUSION: Results indicate that care is still centered on the drug, with few clinical activities. We suggest reformulations in service management, particularly in the management of pharmacists.

Aim: To determine whether telephone support using an evidence-based protocol for chronic heart failure (CHF) management will improve patient outcomes and will reduce hospital readmission rates in patients without access to hospital-based management programs. Methods: The rationale and protocol for a cluster-design randomised controlled trial (RCT) of a semi-automated telephone intervention for the management of CHF, the Chronic Heart-failure Assistance by Telephone (CHAT) Study is described. Care is coordinated by trained cardiac nurses located in Heartline, the national call center of the National Heart Foundation of Australia in partnership with patients’ general practitioners (GPs). Conclusions: The CHAT Study model represents a potentially cost-effective and accessible model for the Australian health system in caring for CHF patients in rural and remote areas. The system of care could also be readily adapted for a range of chronic diseases and health systems. Background & aim: This paper describes nutrition care practices in acute care hospitals across Australia and New Zealand. Methods: A survey on nutrition care practices in Australian and New Zealand hospitals was completed by Directors of dietetics departments of 56 hospitals that participated in the Australasian Nutrition Care Day Survey 2010. Results: Overall 370 wards representing various specialities participated in the study. Nutrition risk screening was conducted in 64% (n=234) of the wards. Seventy nine percent(n=185) of these wards reported using the Malnutrition Screening Tool, 16% using the Malnutrition Universal Screening Tool (n=37), and 5% using local tools (n=12). Nutrition risk rescreening was conducted in 14% (n=53) of the wards. More than half the wards referred patients at nutrition risk to dietitians and commenced a nutrition intervention protocol. Feeding assistance was provided in 89% of the wards. “Protected” meal times were implemented in 5% of the wards. Conclusion: A large number of acute care hospital wards in Australia and New Zealand do not comply with evidence-based practice guidelines for nutritional management of malnourished patients. This study also provides recommendations for practice.

The Australasian Nutrition Care Day Survey (ANCDS) reported two-in-five patients in Australian and New Zealand hospitals consume ≤50% of the offered food. The ANCDS found a significant association between poor food intake and increased in-hospital mortality after controlling for confounders (nutritional status, age, disease type and severity)1. Evidence for the effectiveness of medical nutrition therapy (MNT) in hospital patients eating poorly is lacking. An exploratory study was conducted in respiratory, neurology and orthopaedic wards of an Australian hospital. At baseline, 24-hour food intake (0%, 25%, 50%, 75%, 100% of offered meals) was evaluated for patients hospitalised for ≥2 days and not under dietetic review. Patients consuming ≤50% of offered meals due to nutrition-impact symptoms were referred to ward dietitians for MNT with food intake re-evaluated on day-7. 184 patients were observed over four weeks. Sixty-two patients (34%) consumed ≤50% of the offered meals. Simple interventions (feeding/menu assistance, diet texture modifications) improved intake to ≥75% in 30 patients who did not require further MNT. Of the 32 patients referred for MNT, baseline and day-7 data were available for 20 patients (68±17years, 65% females, BMI: 22±5kg/m2, median energy, protein intake: 2250kJ, 25g respectively). On day-7, 17 participants (85%) demonstrated significantly higher consumption (4300kJ, 53g; p<0.01). Three participants demonstrated no improvement due to ongoing nutrition-impact symptoms. “Percentage food intake” was a quick tool to identify patients in whom simple interventions could enhance intake. MNT was associated with improved dietary intake in hospital patients. Further research is needed to establish a causal relationship.

BACKGROUND: The prevalence of protein-energy malnutrition in older adults is reported to be as high as 60% and is associated with poor health outcomes. Inadequate feeding assistance and mealtime interruptions may contribute to malnutrition and poor nutritional intake during hospitalisation. Despite being widely implemented in practice in the United Kingdom and increasingly in Australia, there have been few studies examining the impact of strategies such as Protected Mealtimes and dedicated feeding assistant roles on nutritional outcomes of elderly inpatients. AIMS: The aim of this research was to implement and compare three system-level interventions designed to specifically address mealtime barriers and improve energy intakes of medical inpatients aged ≥65 years. This research also aimed to evaluate the sustainability of any changes to mealtime routines six months post-intervention and to gain an understanding of staff perceptions of the post-intervention mealtime experience. METHODS: Three mealtime assistance interventions were implemented in three medical wards at Royal Brisbane and Women's Hospital: AIN-only: Additional assistant-in-nursing (AIN) with dedicated nutrition role. PM-only: Multidisciplinary approach to meals, including Protected Mealtimes. PM+AIN: Combined intervention: AIN + multidisciplinary approach to meals. An action research approach was used to carefully design and implement the three interventions in partnership with ward staff and managers. Significant time was spent in consultation with staff throughout the implementation period to facilitate ownership of the interventions and increase likelihood of successful implementation. A pre-post design was used to compare the implementation and nutritional outcomes of each intervention to a pre-intervention group. Using the same wards, eligible participants (medical inpatients aged ≥65 years) were recruited to the preintervention group between November 2007 and March 2008 and to the intervention groups between January and June 2009. The primary nutritional outcome was daily energy and protein intake, which was determined by visually estimating plate waste at each meal and mid-meal on Day 4 of admission. Energy and protein intakes were compared between the pre and post intervention groups. Data were collected on a range of covariates (demographics, nutritional status and known risk factors for poor food intake), which allowed for multivariate analysis of the impact of the interventions on nutritional intake. The provision of mealtime assistance to participants and activities of ward staff (including mealtime interruptions) were observed in the pre-intervention and intervention groups, with staff observations repeated six months post-intervention. Focus groups were conducted with nursing and allied health staff in June 2009 to explore their attitudes and behaviours in response to the three mealtime interventions. These focus group discussions were analysed using thematic analysis. RESULTS: A total of 254 participants were recruited to the study (pre-intervention: n=115, AIN-only: n=58, PM-only: n=39, PM+AIN: n=42). Participants had a mean age of 80 years (SD 8), and 40% (n=101) were malnourished on hospital admission, 50% (n=108) had anorexia and 38% (n=97) required some assistance at mealtimes. Occasions of mealtime assistance significantly increased in all interventions (p<0.01). However, no change was seen in mealtime interruptions. No significant difference was seen in mean total energy and protein intake between the preintervention and intervention groups. However, when total kilojoule intake was compared with estimated requirements at the individual level, participants in the intervention groups were more likely to achieve adequate energy intake (OR=3.4, p=0.01), with no difference noted between interventions (p=0.29). Despite small improvements in nutritional adequacy, the majority of participants in the intervention groups (76%, n=103) had inadequate energy intakes to meet their estimated energy requirements. Patients with cognitive impairment or feeding dependency appeared to gain substantial benefit from mealtime assistance interventions. The increase in occasions of mealtime assistance by nursing staff during the intervention period was maintained six-months post-intervention. Staff focus groups highlighted the importance of clearly designating and defining mealtime responsibilities in order to provide adequate mealtime care. While the purpose of the dedicated feeding assistant was to increase levels of mealtime assistance, staff indicated that responsibility for mealtime duties may have merely shifted from nursing staff to the assistant. Implementing the multidisciplinary interventions empowered nursing staff to "protect" the mealtime from external interruptions, but further work is required to empower nurses to prioritise mealtime activities within their own work schedules. Staff reported an increase in the profile of nutritional care on all wards, with additional non-nutritional benefits noted including improved mobility and functional independence, and better identification of swallowing difficulties. IMPLICATIONS: The PhD research provides clinicians with practical strategies to immediately introduce change to deliver better mealtime care in the hospital setting, and, as such, has initiated local and state-wide roll-out of mealtime assistance programs. Improved nutritional intakes of elderly inpatients was observed; however given the modest effect size and reducing lengths of hospital stays, better nutritional outcomes may be achieved by targeting the hospital-to-home transition period. Findings from this study suggest that mealtime assistance interventions for elderly inpatients with cognitive impairment and/or functional dependency show promise.

A inserção do psicólogo no campo da saúde vem crescendo de forma considerável no âmbito nacional, principalmente nas últimas duas décadas. Está concentrada aí, atualmente, a maior oferta de vagas em concursos públicos para psicólogos, bem como uma ampliação de oportunidades na rede privada. Trata-se de um cenário bastante amplo, que inclui a atenção básica (postos de saúde, Programa de Saúde da Família, entre outros), a assistência às demandas de saúde mental (Centros de Atenção Psicossocial, rede ambulatorial e hospitais psiquiátricos), além de institutos, hospitais especializados e hospitais gerais (HGs). Em franco processo de estruturação, o campo da psicologia na saúde é marcado por pluralidades de papéis, demandas e histórias, principalmente no que tange ao contexto do HG, onde a especificidade da psicologia permanece até hoje um tanto quanto enigmática. A escassez dos dados sobre a entrada de psicólogos em HGs no Rio de Janeiro apontou o caminho que nossas buscas deveriam assumir. Assim, o objeto desta tese é discutir alguns dos diferentes fazeres do psicólogo no HG. Para tanto, optamos por um percurso que inevitavelmente nos levou a uma perspectiva histórica. Na busca de elementos que pudessem contar quando e como a psicologia se tornou uma especialidade presente nos hospitais gerais, procuramos os agentes precursores desta empreitada, aqueles que participaram ativamente da construção desse espaço como um dos lugares da psicologia na saúde. Foram realizadas cinco entrevistas com personagens dessa história no contexto fluminense, psicólogas que, cada uma em seu hospital e em um dado momento histórico, tiveram papel decisivo para que a psicologia se tornasse uma especialidade legitimada nesse cenário. Tivemos ainda como fontes de pesquisa alguns relatórios, conversas de bastidores, visitas a alguns HGs e a própria vivência da autora desta tese como psicóloga vinculada à Secretaria Estadual de Saúde e Defesa Civil do Rio de Janeiro (SESDEC/RJ). Verificamos que a história da psicologia nos HG no Rio de Janeiro é significativamente recente, oficializando-se em 1990 com a realização do primeiro concurso público com provimento de vagas para psicólogos em HGs vinculados à SES/RJ (atual SESDEC). Outros concursos ampliaram as equipes e a configuração de uma rotina na qual a palavra de ordem passou a ser a atenção ao risco psicossocial. Até esse ponto, no entanto, foram travadas muitas batalhas: entre os psicólogos e as demais equipes, entre os novos psicólogos e as direções das suas unidades de saúde, entre psicólogos estatutários e psicólogos prestadores, entre psicólogos e a própria psicologia. Nossa empreitada privilegiou a narrativa desses personagens, as histórias por eles contadas, que nos auxiliaram na composição pregressa e atual da psicologia no HG no Rio de Janeiro. São memórias que contam das dificuldades e das vitórias de cada um desses momentos, na tentativa de se definir o quase indefinível lugar da psicologia no HG. Nosso trabalho pretendeu construir um dos possíveis mapas desse campo, objetivando, com isso, fornecer subsídios para que psicólogos possam ampliar seus estilos e coletivos de pensamento em prol de um exercício profissional ético e responsável.
