RACIONAL: O número de cirurgias ambulatoriais realizadas em hospitais, como em clínicas particulares, cresce a cada dia. Hoje, em alguns países, como a França, há predomínio das cirurgias ambulatoriais em relação às hospitalares. OBJETIVO: Avaliar retrospectivamente os pacientes operados no Serviço de Cirurgia Ambulatorial do Hospital das Clínicas da Faculdade de Medicina de Botucatu, SP. CASUÍSTICA E MÉTODO: Foram estudados retrospectivamente 437 casos clínicos de patologias orificiais, analisando a distribuição por faixa etária, por sexo, por patologias e as complicações pós-operatórias. RESULTADOS: Notou-se predomínio de pacientes com idade inferior a 45 anos (62,8%), prevalência do sexo feminino (56%), sendo a doença hemorroidária (45,1%) a principal patologia e a dor e sangramento as complicações mais freqüentes (9,8% e 7,3%). CONCLUSÕES: Os resultados satisfatórios observados demonstram a possibilidade de realização, em ambulatório, de diversos procedimentos simples em patologias anorretais freqüentes, a baixo custo e poucas complicações, sendo estas não superiores às observadas em cirurgia hospitalar.

In the last years some natural products has been described as supressors of the mutagenic process in bacteria, the antimutagenics. The literature reference that in most of the countries, the population makes use of medicinal plants. The plant Momordica charantia (Cucurbitaceae) is original from Africa being used popularly as purgative, antirheumatic and for skin problems, burns and hemorrhoids. The present work had as objective to evaluate the mutagenic and antimutagenic activities of the ethanolic extract of M. charantia in Salmonella/microsome assays using TA100, TA98 and TA102 strains. It was verified that the extract did not present mutagenic activity when evaluated in different concentrations (0.64, 1.27, 2.55 and 3.84 mg/plate) but acted as antimutagenic agent against the mutations induced by the sodium azide (TA100,-S9), 4-nitro-phenylenediamine (TA98, -S9), daunomycin (TA102, +S9) 2-anthramine (TA100 and TA98, +S9) and 2-aminofluorene (TA102, +S9). When the metabolic activation (+S9) was used, the percentage of inhibition of the mutagenicity varied in the range of 31%-96%, while in absence of metabolizing system (-S9), the maximum percentage of inhibition of the mutagenicity was 44%. In that way, we can conclude that the metabolites found in the extract has potential to protect the genetic material against the damages induced by different chemical agents.

Stryphnodendron adstringens (Mart.) Coville (barbatimão), belonging to Mimosaceae family; it is used as ornamental and the wood is used in civil buildings, edification in wet places, lathe and joinery jobs, being very used also in home-made medicine against hemorrhage, diarrhea, hemorrhoids, conjunctivitis, injury cleaning, uterus hemorrhage, ulcerous hurt and excessive oily skin. The objective of this research was to determine the allelopathic potential of an aqueous extract, boiled or not of S. adstringens (Mart.) Coville, in the Cucumis sativus germination and initial development. Thus, the aqueous vegetable extract was extracted from the shoot, which was triturated in 1L of distilled water to 100g of leaf, being the extract filtrated and separated in boiled and not boiled. The treatments used were distilled water (0%) and boiled and not boiled extracts, in the concentrations of 50 and 100%. The cucumber seeds were put to germinate in Gerbox, having filter paper as substrate, which was wet with 25mL from different treatments, in constant temperature of 25°C. The reading germination was accomplished in breaks of 24 hours, for a period of five consequently days after the beginning of the experiment, considering germinated the seeds that showed 2mm of root, approximately. To dry matter determination, the seedlings with five days after the germination were separated in shoot and root, dried during three days to a constant weight in a 60°C forced draft oven. Through results, it can be concluded that the extract of S. adstringens affected more the Cucumis sativus seedling development than the germination, and it didn't show difference if boiled or not.

Our aim is to present a modification in patient placement on surgical table for the treatment of prolapsed hemorrhoids by using PPH technique. We propose a modified jackknife position for PPH procedure where the surgeon stays between patient's legs. The modified jackknife position may provide better surgical field with better view of the dentate line which facilitates pursestring confection. In Brazil, the majority of the surgeons prefer lithotomic position for PPH technique based on Longo's first reports. In our opinion, modified jackknife position provides both better surgical field for pursestring placement and better view of the staple line after PPH firing.

INTRODUÇÃO: O tratamento cirúrgico para hemorroidas deve ter indicação individual e baseia-se no sintoma predominante (sangramento ou prolapso), na gravidade da doença e na presença ou ausência de componente externo (plicoma). Cabe ao cirurgião conhecer as mais variadas técnicas para que possa encontrar o tratamento mais adequado caso a caso. TÉCNICA: O procedimento THD consiste na ligadura alta seletiva e guiada por Doppler de até seis ramos arteriais submucosos que irrigam as hemorroidas, levando à sua desarterialização, associada à correção do prolapso (reparo anorretal ou lifting). Utiliza equipamento e kit especiais. CONCLUSÃO: A técnica do THD tem mostrado bons resultados iniciais. Por ser técnica cirúrgica em que se respeita a anatomia, ela age diretamente sobre a fisiopatologia da afecção e corrige suas principais consequências, ela parece bastante promissora. Sua aplicação inicial pode ser nos pacientes com doença hemorroidária de II grau, que tenham indicação de tratamento cirúrgico, e de III e IV graus, nestes últimos, podendo ser associada à ressecção de plicomas.

The treatment of hemorrhoidal disease (HD) by conventional hemorrhoidectomy is associated with significant morbidity, mainly represented by the postoperative pain and the late return to daily activities. Doppler-guided hemorrhoid artery ligation (DGHAL) is a minimal-invasive surgical treatment for HD that has been used as an alternative method in order to reduce these inconveniences. OBJECTIVE: To analyze the initial results of the DGHAL technique associated with rectal mucopexy in the treatment of HD. METHODS:Forty-two patients with stage I, III and IV hemorrhoids who were submitted to DGHAL were analyzed from December 2010 to August 2011. Eleven patients (26%) were stage II; 21 (50%), stage III; and 10 (24%), stage IV HD. All patients were operated by the same surgeon under spinal anesthesia and using the same equipment and technique to perform the procedure. The 42 patients underwent ligation of six arterial branches followed by rectal mucopexia by uninterrupted suture. Nine patients needed concomitant removal of perianal skin tag. In the postoperative, the following parameters were evaluated: pain, tenesmus, bleeding, itching, prolapse, mucus discharge and recurrence. The mean postoperative follow-up lasted four months (one to nine months). RESULTS: Tenesmus was the most common postoperative complaint for 85.7% of patients followed by pain, in 28.6%, perianal burning, in 12.3%, mucus discharge and perianal hematoma in 4.7%. Two patients had severe postoperative bleeding and required surgical haemostasis, one of which needed blood transfusion. Ninety-five percent of the patients declared to be satisfied with the method. CONCLUSION: Even though DGHAL has complications similar to those of other surgical methods, its results present less postoperative pain, allowing faster recovery and return to work. Studies with more cases and a longer follow-up are still necessary to assess the late recurrence.

Up to one third of the general population suffers from symptoms caused by hemorrhoids. Conservative treatment comes first unless the patient presents with an acute hemorrhoidal prolapse or a thrombosis. A fiber enriched diet is the primary treatment option, recommended in the perioperative period as well as a long-term prophylaxis. A timely limited application of topical ointments or suppositories and/or flavonoids are further treatment options. When symptoms persist interventional procedures for grade I-II hemorrhoids, and surgery for grade III-IV hemorrhoids should be considered. Rubber band ligation is the interventional treatment of choice. A comparable efficacy using sclerosing or infrared therapy has not yet been demonstrated. We therefore do not recommend these treatment options for the cure of hemorrhoids. Self-treatment by anal insertion of bougies is of lowrisk and may be successful, particularly in the setting of an elevated sphincter pressure. Anal dilation, sphincterotomy, cryosurgery, bipolar diathermy, galvanic electrotherapy, and heat therapy should be regarded as obsolete given the poor or missing data reported for these methods. For a long time, the classic excisional hemorrhoidectomy was considered to be the gold standard as far as surgical procedures are concerned. Primary closure (Ferguson) seems to be superior compared to the "open" version (Milligan Morgan) with respect to postoperative pain and wound healing. The more recently proposed stapled hemorrhoidopexy (Longo) is particularly advisable for circular hemorrhoids. Compared to excisional hemorrhoidectomy the Longo-operation is associated with reduced postoperative pain, shorter operation time and hospital stay as well as a faster recovery, with the disadvantage though of a higher recurrence rate. Data from Hemorrhoidal Artery Ligation (HAL)-, if appropriate in combination with a Recto-Anal Repair (HAL/RAR)-, demonstrates a similar trend towards a better tolerance of the procedure at the expense of a higher recurrence rate. These relatively "new" procedures equally qualify for the treatment of grade III and IV hemorrhoids, and, in the case of stapled hemorrhoidopexy, may even be employed in the emergency situation of an acute anal prolapse. While under certain circumstances different treatment options are equivalent, there is a clear specificity with respect to the application of those procedures in other situations. The respective pros and cons need to be discussed separately with every patient. According to their own requirements a treatment strategy has to be defined according to their individual requirements.

Detailed data on the health status of Saudi women are lacking. This cross sectional study attempts to provide a comprehensive description of the health status of Saudi females between the ages of 15-45 residing in Yanbu Al-Siniyah. The purpose is to assess women's needs for health services. The health status indicators are chronic tracer conditions, reported symptoms and multidimensional functioning levels. The generic functioning instrument of the Medical Outcome Study was used to estimate physical, social, and role functioning; degree of pain and health perceptions. The information was obtained by interviewing subjects and abstracting facts from their medical records. The results show functioning scores are in the "well health" range for physical, social, role and pain. Crowding and education have an equal or stronger effect of reducing functioning levels than the diagnosed tracer conditions. The highest prevalence conditions having a definite functional impact and diagnosed adequately in primary care are anemia, urinary tract infection, hemorrhoids, rheumatoid arthritis, caries and gingivitis. Reported symptoms strongly reducing function levels in this study are dyspnea, heart pain, incontinence, eye and skin problems, and joint ache. The impact of the reliability and validity of the measures used and other limitations of the results are discussed. Finally, some policy implications and suggestions for future study are presented. ^

Objetivo: Evaluación de la eficacia analgésica para el dolor de la episiotomía entre el paracetamol y el Ibuprofeno, en las primeras 42 horas postparto. Método: Estudio cuasi-experimental (prospectivo y simple ciego) en mujeres que dieron a luz en el HOSPITAL UNIVERSITARIO CENTRAL DE ASTURIAS (OVIEDO), excluyendo alérgicas, patologías asociadas ó aquellas que el idioma impidiese un correcto entendimiento. Dos grupos: 1) Paracetamol 1 gr; 2) Ibuprofeno 600 mg. Tamaño de muestra: 110 por grupo para alcanzar mínimo de 80. Variable principal: grado de dolor según puntuación de escala (0 a 3). Otras variables: edad de paciente, semanas de gestación, peso neonatal, paridad, inicio del parto, anestesia epidural, tipo de parto, desgarro, inflamación y enrojecimiento, hematoma, hemorroides, necesidad de sondaje evacuador, aplicación de hielo y solicitud de analgesia. Tamaño final de la muestra: 88 grupo paracetamol y 97 grupo ibuprofeno. La escala de dolor se midió a las 2 horas postparto (previo al tratamiento) y, posteriormente, cada 8 hasta 42 horas. Se realizó análisis descriptivo y comparación entre grupos. Resultados: No encontramos diferencias significativas en la escala de dolor entre ambos fármacos, ni en los subgrupos analizados, salvo en el subgrupo de partos eutócicos, donde el ibuprofeno fue superior al paracetamol. En el global de la serie, el grupo de paracetamol solicitó hielo y otra medicación con mayor frecuencia que el grupo de ibuprofeno. Conclusiones: El ibuprofeno 600 mg y el paracetamol de 1 gr obtienen una respuesta similar en las primeras 42 horas postparto, si bien el ibuprofeno parece tener algunas ventajas adicionales.
