OBJECTIVE We sought to evaluate the feasibility of k-t parallel imaging for accelerated 4D flow MRI in the hepatic vascular system by investigating the impact of different acceleration factors. MATERIALS AND METHODS k-t GRAPPA accelerated 4D flow MRI of the liver vasculature was evaluated in 16 healthy volunteers at 3T with acceleration factors R = 3, R = 5, and R = 8 (2.0 × 2.5 × 2.4 mm(3), TR = 82 ms), and R = 5 (TR = 41 ms); GRAPPA R = 2 was used as the reference standard. Qualitative flow analysis included grading of 3D streamlines and time-resolved particle traces. Quantitative evaluation assessed velocities, net flow, and wall shear stress (WSS). RESULTS Significant scan time savings were realized for all acceleration factors compared to standard GRAPPA R = 2 (21-71 %) (p < 0.001). Quantification of velocities and net flow offered similar results between k-t GRAPPA R = 3 and R = 5 compared to standard GRAPPA R = 2. Significantly increased leakage artifacts and noise were seen between standard GRAPPA R = 2 and k-t GRAPPA R = 8 (p < 0.001) with significant underestimation of peak velocities and WSS of up to 31 % in the hepatic arterial system (p <0.05). WSS was significantly underestimated up to 13 % in all vessels of the portal venous system for k-t GRAPPA R = 5, while significantly higher values were observed for the same acceleration with higher temporal resolution in two veins (p < 0.05). CONCLUSION k-t acceleration of 4D flow MRI is feasible for liver hemodynamic assessment with acceleration factors R = 3 and R = 5 resulting in a scan time reduction of at least 40 % with similar quantitation of liver hemodynamics compared with GRAPPA R = 2.

PURPOSE The aim of this work is to derive a theoretical framework for quantitative noise and temporal fidelity analysis of time-resolved k-space-based parallel imaging methods. THEORY An analytical formalism of noise distribution is derived extending the existing g-factor formulation for nontime-resolved generalized autocalibrating partially parallel acquisition (GRAPPA) to time-resolved k-space-based methods. The noise analysis considers temporal noise correlations and is further accompanied by a temporal filtering analysis. METHODS All methods are derived and presented for k-t-GRAPPA and PEAK-GRAPPA. A sliding window reconstruction and nontime-resolved GRAPPA are taken as a reference. Statistical validation is based on series of pseudoreplica images. The analysis is demonstrated on a short-axis cardiac CINE dataset. RESULTS The superior signal-to-noise performance of time-resolved over nontime-resolved parallel imaging methods at the expense of temporal frequency filtering is analytically confirmed. Further, different temporal frequency filter characteristics of k-t-GRAPPA, PEAK-GRAPPA, and sliding window are revealed. CONCLUSION The proposed analysis of noise behavior and temporal fidelity establishes a theoretical basis for a quantitative evaluation of time-resolved reconstruction methods. Grappa“ ist eine Middleware, die auf die Anbindung verschiedener Autobewerter an verschiedene E-Learning-Frontends respektive Lernmanagementsysteme (LMS) spezialisiert ist. Ein Prototyp befindet sich seit mehreren Semestern an der Hochschule Hannover mit dem LMS „moodle“ und dem Backend „aSQLg“ im Einsatz und wird regelmäßig evaluiert. Dieser Beitrag stellt den aktuellen Entwicklungsstand von Grappa nach diversen Neu- und Weiterentwicklungen vor. Nach einem Bericht über zuletzt gesammelte Erfahrungen mit der genannten Kombination von Systemen stellen wir wesentliche Neuerungen der moodle-Plugins, welche der Steuerung von Grappa aus moodle heraus dienen, vor. Anschließend stellen wir eine Erweiterung der bisherigen Architektur in Form eines neuentwickelten Grappa-php-Clients zur effizienteren Anbindung von LMS vor. Weiterhin berichten wir über die Anbindung eines weiteren Autobewerters „Graja“ für Programmieraufgaben in Java. Der Bericht zeigt, dass bereits wichtige Schritte für eine einheitliche Darstellung automatisierter Programmbewertung in LMS mit unterschiedlichen Autobewertern für die Studierenden absolviert sind. Die praktischen Erfahrungen zeigen aber auch, dass sowohl bei jeder der Systemkomponenten individuell, wie auch in deren Zusammenspiel via Grappa noch weitere Entwicklungsarbeiten erforderlich sind, um die Akzeptanz und Nutzung bei Studierenden sowie Lehrenden weiter zu steigern.

Introduction: The primary somatosensory cortex (SI) contains Brodmann areas (BA) 1, 2, 3a, and 3b. Research in non-human primates showed that BAs 3b, 1, and 2 each contain one full representation of the hand with separate representations for each finger. This research also showed that the finger representation in BA3b has larger and clearer finger somatotopy than BA1 and 2. Although several efforts to map finger somatotopy in SI by fMRI have been made at 1.5 and 3T these studies have yielded variable results and were not able to detect single subject finger somatotopy, probably due to the limited spatial extent of the cortical areas representing a digit (close to the resolution in most fMRI experiments), complications due to acquisition of consistent maps for individual subjects (Schweizer et al 2008), or inter-individual variability in sulcal anatomy impeding group studies. Here, we used 7T fMRI to investigate finger somatotopy in SI, some of its functional characteristics, and its reproducibility. Methods: Eight right-handed male subjects were scanned on a 7T scanner (Siemens Medical, Germany) with an 8-channel Tx/Rx rf-coil (Rapid Biomedical, Germany). 1.3x1.3x1.3mm3 resolution fMRI data were acquired using a sinusoidal readout EPI sequence (Speck et al, 2008) and FOV=210mm, TE/TR=27ms/2.5s, GRAPPA=2. Each volume contained 28 transverse slices covering SI. A single EPI volume with 64 slices was acquired to aid coregistration. 1x1x1mm3 anatomical data were acquire using the MP2RAGE sequence (Marques et al, 2009; TE/TR/TI1,2/TRmprage=2.63ms/7.2ms/0.9,3.2s/5s). Subjects were positioned supine in the scanner with their right arm comfortably against the magnet bore. An experimenter was positioned at the entrance of the bore where he could easily reach and stroke successively the two distal phalanxes of each digit. The order of stroked digit was D1 (thumb)-D3-D5-D2-D4, with 20s ON, 10s OFF alternated. This sequence was repeated four times per run and two functional runs were acquired per subject. Realignment, smoothing (FWHM 2 mm), coregistration of the anatomical to the fMRI data and calculation of t-statistics were done using SPM8. An SI mask was obtained via an F-contrast (p<0.001) over all digits. Within the mask, voxels were labeled with the number of the digit demonstrating the highest t-value for that particular voxel. Results: For all subjects, areas corresponding to the five digits were identified in contralateral SI. BA3b showed the most consistent somatotopic finger representation (see an example in Fig.1). The five digits were localized in a consecutive order in the cortex, with D1 most anterior, inferior and distal and D5, most posterior, superior and medial (mean distance between centres of mass of digit representations ±stderr: 4.2±0.7mm; see Fig. 2). The analysis of average beta values within each finger representation region revealed the specificity of the somatotopic region to the tactile input for each tested finger (except digit 4 and 5). Five of these subjects also presented an orderly and consecutive representation of the five digits in BA1 and 2. Conclusions: Our data reveal that the increased BOLD sensitivity at 7T and the high spatial resolution used in this study allow consistent somatotopic mapping using human touch as a stimulus and that human SI contains at least three separate regions that contain five separate representations of all single contralateral fingers. Moreover, adjacent fingers were represented at adjacent cortical regions across the three SI regions. The spatial organization of SI as reflected in individual subject topography corresponds well with previous electrophysiological data in non-human primates. The small distance between digit representations highlights the need for the high spatial resolution available at 7T.

Imaging in neuroscience, clinical research and pharmaceutical trials often employs the 3D magnetisation-prepared rapid gradient-echo (MPRAGE) sequence to obtain structural T1-weighted images with high spatial resolution of the human brain. Typical research and clinical routine MPRAGE protocols with ~1mm isotropic resolution require data acquisition time in the range of 5-10min and often use only moderate two-fold acceleration factor for parallel imaging. Recent advances in MRI hardware and acquisition methodology promise improved leverage of the MR signal and more benign artefact properties in particular when employing increased acceleration factors in clinical routine and research. In this study, we examined four variants of a four-fold-accelerated MPRAGE protocol (2D-GRAPPA, CAIPIRINHA, CAIPIRINHA elliptical, and segmented MPRAGE) and compared clinical readings, basic image quality metrics (SNR, CNR), and automated brain tissue segmentation for morphological assessments of brain structures. The results were benchmarked against a widely-used two-fold-accelerated 3T ADNI MPRAGE protocol that served as reference in this study. 22 healthy subjects (age=20-44yrs.) were imaged with all MPRAGE variants in a single session. An experienced reader rated all images of clinically useful image quality. CAIPIRINHA MPRAGE scans were perceived on average to be of identical value for reading as the reference ADNI-2 protocol. SNR and CNR measurements exhibited the theoretically expected performance at the four-fold acceleration. The results of this study demonstrate that the four-fold accelerated protocols introduce systematic biases in the segmentation results of some brain structures compared to the reference ADNI-2 protocol. Furthermore, results suggest that the increased noise levels in the accelerated protocols play an important role in introducing these biases, at least under the present study conditions.

Cardiovascular disease is the leading cause of death worldwide. Within this subset, coronary artery disease (CAD) is the most prevalent. Magnetic resonance angiography (MRA) is an emerging technique that provides a safe, non-invasive way of assessing CAD progression. To generate contrast between tissues, MR images are weighted according to the magnetic properties of those tissues. In cardiac MRI, T2 contrast, which is governed by the rate of transverse signal loss, is often created through the use of a T2-Preparation module. T2-Preparation, or T2-Prep, is a magnetization preparation scheme used to improve blood/myocardium contrast in cardiac MRI. T2-Prep methods generally use a non-selective +90°, 180°, 180°, -90° train of radiofrequency (RF) pulses (or variant thereof), to tip magnetization into the transverse plane, allow it to evolve, and then to restore it to the longitudinal plane. A key feature in this process is the combination of a +90° and -90° RF pulse. By changing either one of these, a mismatch occurs between signal excitation and restoration. This feature can be exploited to provide additional spectral or spatial selectivity. In this work, both of these possibilities are explored. The first - spectral selectivity - has been examined as a method of improving fat saturation in coronary MRA. The second - spatial selectivity - has been examined as a means of reducing imaging time by decreasing the field of view, and as a method of reducing artefacts originating from the tissues surrounding the heart. Two additional applications, parallel imaging and self-navigation, are also presented. This thesis is thus composed of four sections. The first, "A Fat Signal Suppression for Coronary MRA at 3T using a Water-Selective Adiabatic T2-Preparation Technique", was originally published in the journal Magnetic Resonance in Medicine (MRM) with co-authors Ruud B. van Heeswijk and Matthias Stuber. The second, "Combined T2-Preparation and 2D Pencil Beam Inner Volume Selection", again with co-authors Ruud van Heeswijk and Matthias Stuber, was also published in the journal MRM. The third, "A cylindrical, inner volume selecting 2D-T2-Prep improves GRAPPA-accelerated image quality in MRA of the right coronary artery", written with co-authors Jerome Yerly and Matthias Stuber, has been submitted to the "Journal of Cardiovascular Magnetic Resonance", and the fourth, "Combined respiratory self-navigation and 'pencil-beam' 2D-T2 -Prep for free-breathing, whole-heart coronary MRA", with co¬authors Jerome Chaptinel, Giulia Ginami, Gabriele Bonanno, Simone Coppo, Ruud van Heeswijk, Davide Piccini, and Matthias Stuber, is undergoing internal review prior to submission to the journal MRM. -- Les maladies cardiovasculaires sont la cause principale de décès dans le monde : parmi celles-ci, les maladies coronariennes sont les plus répandues. L'angiographie par résonance magnétique (ARM) est une technique émergente qui fournit une manière sûre, non invasive d'évaluer la progression de la coronaropathie. Pour obtenir un contraste entre les tissus, les images d'IRM sont pondérées en fonction des propriétés magnétiques de ces tissus. En IRM cardiaque, le contraste en T2, qui est lié à la décroissance du signal transversal, est souvent créé grâce à l'utilisàtion d'un module de préparation T2. La préparation T2, ou T2-Prep, est un système de préparation de l'aimantation qui est utilisé pour améliorer le contraste entre le sang et le myocarde lors d'une IRM cardiaque. Les méthodes de T2-Prep utilisent généralement une série non-sélective d'impulsions de radiofréquence (RF), typiquement [+ 90°, 180°, 180°, -90°] ou une variante, qui bascule l'aimantation dans le plan transversal, lui permet d'évoluer, puis la restaure dans le plan longitudinal. Un élément clé de ce processus est la combinaison des impulsions RF de +90° et -90°. En changeant l'une ou l'autre des impulsions, un décalage se produit entre l'excitation du signal et de la restauration. Cette fonction peut être exploitée pour fournir une sélectivité spectrale ou spatiale. Dans cette thèse, les deux possibilités sont explorées. La première - la sélectivité spectrale - a été examinée comme une méthode d'améliorer la saturation de la graisse dans l'IRM coronarienne. La deuxième - la sélectivité spatiale - a été étudiée comme un moyen de réduire le temps d'imagerie en diminuant le champ de vue, et comme une méthode de réduction des artefacts provenant des tissus entourant le coeur. Deux applications supplémentaires, l'imagerie parallèle et la self-navigation, sont également présentées. Cette thèse est ainsi composée de quatre sections. A artrite psoriásica (APs) é uma doença articular sistêmica e polimórfica de apresentação e curso clínico variáveis, associada a comorbidades importantes como diabetes mellitus, hipertensão arterial e dislipidemia. Para o diagnóstico precoce da doença é necessário alto grau de suspeita clínica, sobretudo quando as manifestações cutâneas são sutis e pouco definidas. Doença erosiva progressiva pode ocorrer em até metade dos pacientes, associada a alterações anatômicas e funcionais em cerca de 20%, de modo que o prognóstico da APs permanece obscuro, especialmente se diagnóstico e tratamento forem tardios. Fundamentados em ampla revisão da literatura (PubMed e Lilacs) e experiência dos nossos serviços, novos conceitos de imunogenética, fisiopatologia, aspectos clínicos e terapêuticos serão discutidos. Fatores que reduzem a qualidade e a expectativa de vida dos pacientes e novas diretrizes que norteiam um tratamento mais precoce e efetivo serão enfatizados. O controle do processo inflamatório, especialmente nas formas axiais e entesíticas da APs, tornou-se possível graças à introdução dos medicamentos biológicos anti-TNF. Finalmente, o papel do GRAPPA (Group for Research and Assessment of Psoriasis and Psoriatic Arthritis) deve ser ressaltado, já que promove reuniões e estudos conjuntos entre reumatologistas e dermatologistas no sentido de fornecer evidências científicas para as amplas mudanças no manejo clínico e terapêutico de pacientes com APs

VILLA “CAPELLO - MORA”: PROGETTO DI RESTAURO E RIFUNZIONALIZZAZIONE Il restauro è da intendere come un intervento diretto sull’opera, e anche come sua eventuale modifica, condotta sempre sotto un rigoroso controllo tecnico-scientifico e storico-critico, se parliamo di conservazione, intendiamo l’operare in un intento di salvaguardia e di prevenzione, da attuare proprio per evitare che si debba poi intervenire con il restauro, che comprende un evento traumatico per il manufatto. Un seconda parola chiave in questo discorso è la “materia” il restauro interviene sulla materia di un monumento e questa costituisce il tramite dei valori culturali antichi, la sua conservazione e il suo restauro garantisce la trasmissione anche dei significati estetici, storici simbolici del costruito. Ma certamente influisce il tempo sulle cose per cui il progetto di restauro non può astenersi dall’intervenire, in una logica di minimo intervento, di reversibilità, di facile lettura. Il concetto di nuovo in un opera antica, concetto che a parere personale, pare centrare in pieno il problema. Il nuovo infatti “deve avere carattere di autonomia e di chiara leggibilità: come l’<> di Boito deve essere inequivocabilmente opera nuova, come prodotto figurativo e materiale autonomo, chiara ed inequivocabile espressione <>”. Ne deriva riassumendo che oggi l’obbiettivo deve essere quello di conservare da un lato senza non sotrarre altra materia alla fabbrica e di valorizzare, ossia aggiungere, nuove “presenze” di cultura contemporanea. Per questo si parlerà di progetto di restauro e rifunzionalizzazione. La fabbrica ha subito nel corso dell’ultimo decennio una serie di “rovinose” manomissioni, che a differenza di quelle operate in tempi più antichi (coincidenti con esigenze funzionali corrispondenti alla logica dell’adattare) appaiano ben più gravi, e contribuiscono a peggiorare la lettura del fabbricato. Il Veneto e soprattutto la zona intorno a Bassano del Grappa presenta una infinità di dimore padronali casini di caccia, resti di antiche residenze (colombare, oratori, ecc.) risalenti al periodo di maggior fioritura della residenza di “Villa” della Serenissima. Nel caso specifico di studio , quindi della rifunzionalizzazione dell’edificio, la domanda sorge spontanea; Come ristabilire un senso a questi spazi? E nell’ipotesi di poter realmente intervenire che cosa farne di questi oggetti?. E ultimo ma non ultimo in che modo poter ristabilire un “dialogo” con l’edificio in una lettura corretta del suo significato non solo per quel che riguarda la materia ma anche per ciò che ci trasmette nel “viverlo”, nell’usufruirne nel fatto stesso di poterlo vedere, passandoci davanti. tà. Lidea si forma prima ancora da un esigenza del territorio, il comune di Cassola dove ha sede la Villa,pur avendo un discreto numero di abitanti e collocandosi in posizione nevralgica in quanto molto vicino al centro diBasssano (ne è quasi la promulgazione), non possiede uno spazio espositivo /rappresentativo, un edificio a carattere pubblico, sede di “eventi” culturali locali e non. Villa Capello –Mora, potrebbe rispondere bene a questo tipo di utilizzo. Si è deciso di pensare ad un luogo a carattere espositivo che possa funzionare durante tutto l’anno e nei periodi etsivi includa anche il giardino esterno. Il progetto muove da due principi il principio di “reversibilità” e quello del “minimo intervento” sottolinenando volutamente che siano gli ogetti esposti e lo spazio dei locali a fare da protagonisti, Punti chiave nell’interpretazione degli spazi sono stati i percorsi, la “narrazione” degli oggetti esposti avviene per momenti e viene rimarcata nell’allestimento tramite i materiali i colori e le superfici espositive, perché nel momento in cui visito una mostra o un museo, è come se stessi vivendo la narrazione di un qualcosa, oggetto semplice od opera complessa che sia inteso nell’ambito museale-espositivo esso assume una capacità di trasmissione maggiore rispetto a qundo lo stesso ogetto si trova in un contesto differente, la luce e la sua disposizione nello spazio, fanno sì che il racconto sia narrato, bene o male, ancora più importante è lo sfondo su cui si staglia l’opera, che può essere chiuso, come una serie di scatole dentro la scatola (edificio) oppure aperto, con singole e indipendenti pannellature dove l’edificio riveste il carattere proprio di sfondo. Scelta la seconda delle due ipotesi, si è voluto rimarcare nella composizione degli interni i momenti della narrazione, così ad esempio accedendo dall’ingresso secondario (lato nord-ovest) al quale si è dato l’ingresso alla mostra, -superata la prima Hall- si viene, catapultati in uno spazio porticato in origine aperto e che prevediamo chiuso da una vetrata continua, portata a debita distanza dal colonnato. Questo spazio diventa la prima pagina del testo, una prima pagina bianca, uno spazio libero, di esposizione e non di relax e di presentazione dell’evento, uno spazio distributivo non votato solo a questo scopo, ma passibile di trasformazione. A rimarcare il percorso una pavimentazione in cemento lisciato, che invita l’accesso alle due sale espositive a sinista e a destra dell’edificio. Nell’ala a sinistra (rispetto al nord) si apre la stanza con camino seicentesco forse un tempo ad uso cucina? Sala che ospita dei pannelli a sospesi a muro e delle teche espositive appese a soffitto. L’ala di destra, diametralmente opposta, l’unica che al piano terra mantiene la pavimentazione originale, stabiliva il vero atrio d’entrata, nel XVII sec. riconoscibile da quattro colonne tuscaniche centrali a reggere un solaio ligneo, in questa stanza il percorso segnato a terra torna nella disposizione dei pannelli espositivi che si dispongono in un a formare un vero corridoio, tra uno e l’atro di questi pannelli degli spazi sufficienti a intravedere le colonne centrali, i due lati dello stretto percorso non sono paralleli e aprono in senso opposto a due restanti spazi dell’intero salone, La sala suddivisa così in queste tre parti - una di percorrenza due di esposizione - , risulta modificata nella lettura che ne annulla l’originale funzione. Questa nuova “forma”, non vuole essere permanente infatti i pannelli non sono fissi. la narrazione avviene così per momenti, per parti, , che non devono necessariamente far comprendere il “tutto”, ma suggerirlo. Così come per il percorso che non è obbligato ma suggerito. A terminare il piano altre due sale di minore dimensione e di modesto carattere, nelle quali sono presenti pannellature a parete come nella prima sala. L’allestimento prosegue al piano primo a cui si accede tramite due rampe, trattate nel progetto difformemente, in base al loro stato e al loro significato, la rampa più recente (XIX sec) rompe lo spazio al piano nobile stravolgendo l’ingresso a questo salone, che originariamente avveniva salendo dalla scala principale e oltrepassando un disimpegno. La scala ottocentesca introduce direttamente all’ambiente che ora risulta liberato dalle superfetazioni dello scorso secolo, mostrandosi come era stato previsto in origine. Questa rottura dello spazio viene marcata nel progetto da un volume che “imprigiona” la rampa e segna l’inizio della sala espositiva, la sala delle architetture. Al centro della sala in un gioco di pieni e vuoti i pannelli, espositivi, distaccati di poco gli uni dagli altri questa volta non a incorniciare delle colonne ma a richiamo delle pitture murali che raffiguranti una loggia alludono ad una lettura per “parti” e scandiscono lo spazio in una “processione” di eventi. A seguito del “gioco” sono presenti in senso ortogonale ai pannelli sopradescritti ulteriori pannellature appese ad una struttura metallica indipendente dalle capriate lignee. Tale struttura assume ad una duplice funzione, funge sia da irrigidimento della struttura (edificio) che da supporto alla pannellatura, ad essa infatti è collegata una fune, che entrerà in tensione, quando il pannello verrà ad appendersi.
