A study was taken to investigate the types of brassica vegetables mostly grown by smallholder farmers in two provinces of South Africa. Thirty-one smallholder vegetable farmers in the Gauteng province and Waterberg district in the Limpopo province were surveyed. In addition, the study also sought to establish the common diseases, the management strategies used and problems encountered by the farmers. Farmers were interviewed using a questionnaire with closed and open–ended questions. The results indicated that the smallholder farmers mostly grew cabbage (93.6%) as their main brassica crop followed by rape (41.2%). Thirty percent of farmers could not identify or name the predominant disease/s encountered in their fields. Major diseases encountered by farmers surveyed were an unknown disease/s (33.3%), black rot (26.7%), Alternaria leaf spot (6.7%) and white rust (6.7%). Smallholder farmers have inadequate technical information available especially relating to crop diseases, their identification and control. Farmers encountered challenges with black rot disease especially on cabbage, rape and kale and the disease was a problem during winter and summer. Generally, the smallholder farmers used crop rotation (74.2%) as a major practice to manage the diseases experienced. They rotated their brassica vegetables with other crops/vegetables like tomatoes, onions, beetroots and maize. Most of the farmers interviewed (61.3%) did not use chemicals to control diseases, whereas 38.7% of them used chemicals. This was mostly because they lacked information and knowledge, high costs associated with use of chemical fungicides and some were shifting towards organic farming. From the study it was noted that there was a need for technical support to improve farmers’ knowledge on disease identification and control within the surveyed areas.

There is a tendency to reduce ventilation rates and natural or hybrid ventilation systems to ensure the conservation of energy in school buildings. However, high indoor pollutant concentration, due to natural or hybrid ventilation systems may have a significant adverse impact on the health and academic performance of pupils and students. Reviewed evidence shows that this can be detrimental to health and wellbeing in schools because of the learner density within a small area, eventually indicating that CO2 concentrations can rise to very high levels (about 4000 ppm) in classrooms during occupancy periods. In South Africa’s naturally ventilated classrooms, it is not clear whether the environmental conditions are conducive for learning. In addition, natural ventilation will be minimized given the fact that in cold, wet or windy weather, doors and windows will commonly remain closed. Evidence from literature based studies indicates that the significance of ventilation techniques is not understood satisfactorily and additional information concerning naturally ventilated schools has to be provided for better design and policy formulation. To develop a thorough understanding of the environments in classrooms, many other parameters have to be considered as well, such as outdoor air quality, CO2 concentrations, temperature and relative humidity and safety issues that may be important drawbacks for naturally ventilated schools. The aim of this paper is to develop a conceptual understanding of methods that can be implemented to assess the effectiveness of naturally ventilated classrooms in Gauteng, South Africa. A theoretical concept with an embedded practical methodology have been proposed for the research programme to investigate the relationship between ventilation rates and learning in schools in Gauteng , a province in South Africa. It is important that existing and future school buildings must include adequate outdoor ventilation, control of moisture, and avoidance of indoor exposures to microbiologic and chemical substances considered likely to have adverse effects in South Africa. Adequate ventilation in classrooms is necessary to reduce and/or eradicate the transmission of indoor pollutants.

RATIONALE & OBJECTIVES: The food multimix (FFM)concept states that limited food resources can be combined using scientific knowledge to meet nutrient needs of vulnerable groups at low cost utilizing the ‘nutrient strengths’ of individual or candidate foods in composite recipes within a cultural context. METHODS: The method employed the food-to-food approach for recipe development using traditional food ingredients. Recipes were subjected to proximate and micronutrient analysis and optimized to meet at tleast 40% of recommended daily intakes. End products including breads, porridge and soup were developed. RESULTS: FMM products were employed in a feeding trial among 120 healthy pregnant women in Gauteng, South Africa resulting in improvements in serum iron levels from baseline values of 14.59 (=/-7.67) umol/L and 14.02 (=/-8.13) umol/L for control and intervention groups (p=0.71), to 16.03 (=/-5.67) umol/L and 18.66 (=/-9.41) umol/L (p=0.19). The increases from baseline to post-intervention were however statistically significant within groups. Similarly Mean Cell Volume values improved from baseline as well as serum ferritin and transferritin levels. CONCLUSION: The FMM concept has potential value in feeding programs for vulnerable groups including pregnant and lactating mothers.

As dificuldades observadas nas negociações multilaterais têm estimulado muitos países a promover acordos regionais e bilaterais de comércio, os quais vêm assumindo crescente importância na economia mundial: metade do comércio internacional é realizado, hoje, no âmbito de acordos preferenciais. Os três grandes blocos regionais - na Europa, na América e na Ásia - estão se consolidando, com pequena participação do Brasil que se limitou ao Mercosul, de modestas dimensões para o potencial de exportações do país. O governo brasileiro vem procurando fortalecer as relações comerciais do Brasil com parceiros do terceiro mundo, e este trabalho se inscreve nesse tema. A importância relativa da África do Sul no conjunto dos países em desenvolvimento foi a motivação deste estudo, destinado a descrever e analisar as características básicas dos setores exportador e importador daquele país, comparando-as com as do Brasil. Tal comparação visa identificar prováveis complementaridades e desafios que possam subsidiar a estratégia de aproximação comercial em curso. Algumas das conclusões e recomendações são: i) o Brasil tem mais a ceder em termos de redução de alíquotas de imposto de importação, e a África do Sul em abrangência dos itens envolvidos na negociação; ii) apesar de se identificar certa possibilidade de complementaridade entre os dois países, recomenda-se que o acordo abranja o maior número possível de produtos; iii) os esforços privados dos empresários brasileiros devem se concentrar na província de Gauteng, e em KwaZulu-Natal, que respondem pela maior parte da produção exportada assim como das empresas exportadoras - já os exportadores brasileiros de bens de consumo devem dar especial atenção às províncias de Gauteng, Western Cape e Mpumalanga; iv) para não dispersar esforços, mais eficaz se os empresários brasileiros fizerem seus contactos comerciais através de entidades que congreguem as empresas sul-africanas; v) São Paulo deve concentrar o maior número de empresários interessados no acordo, embora os empresários de outros estados possam ser mais receptivos porque individualmente se voltam mais para o mercado externo do que os paulistas, como os do Espírito Santo, Pará, Mato Grosso e os da região sul, Paraná, Santa Catarina e Rio Grande do Sul.

Esta dissertação tem como intuito a análise do conteúdo noticioso dos jornais impressos (2009-2010) das cidades paraenses de Santarém e Marabá, que anualmente enfrentam inundações no período do inverno amazônico (de Dezembro a Junho). O objetivo é analisar se há Comunicação de Risco e como o risco é comunicado nos jornais locais no decorrer das etapas da Gestão de Risco. Há efetividade na Comunicação de Risco? Para isso, a metodologia foi pautada na análise do conteúdo, tendo em vista investigar a forma, conteúdo, número e memória midiática do Jornal de Santarém e Baixo Amazonas - JSBA (Santarém) e do Jornal Correio do Tocantins - CT (Marabá), na temporalidade estabelecida. O principal referencial teórico que serviu de base para pesquisa está vinculado aos conceitos de Comunicação de Risco, Análise do Conteúdo e Gestão de Risco, encontrados respectivamente nas obras de Di Giulio (2008), Bardin (2009) e Jungles (2012). Na comparação entre os jornais, observou-se que o JSBA apresentou maior quantidade de publicações e um conteúdo mais informativo do que o JCT, porém, a forma mostrou-se mais consistentes no segundo jornal. Quanto à memória midiática, o Correio do Tocantins apresentou o elemento do “esquecimento midiático”, que caracteriza a ausência de publicações sobre o tema por um longo período, enquanto que o JSBA abordou a temática em quase todos os meses. Logo, constatou-se que em 2009, os jornais faziam a comunicação do risco e não a Comunicação de Risco. Porém, após o evento extremo (inundação) vivido em 2009, no ano posterior, a Comunicação de Risco, que alerta a população antes da ocorrência do desastre, passou a ganhar seus primeiros contornos no Jornal de Santarém e Baixo Amazonas, em detrimento do Jornal Correio do Tocantins, que continuou a apresentar os mesmos resultados.

A study was designed to characterize a carbapenem-resistant Klebsiella pneumoniae (KPSA01) isolated from a patient in Gauteng, South Africa without recent travel outside South Africa. Molecular characterization was done using isoelectric focusing, polymerase chain reaction and sequencing for bla(VIM), bla(IMP), bla(NDM), bla(CTX-Ms), bla(OXAs), bla(TEMs), and bla(SHV), plasmid-mediated quinolone resistance determinants, multilocus sequencing typing, plasmid replicon typing, and addiction factors. KPSA01 produced VIM-1 and belonged to the newly described sequence type ST569. The plasmid that harboured bla(VIM) typed within the narrow host range IncF replicon group, contained the aadA1 gene cassette, and tested positive for the vagCD and ccdAB addiction systems. This is the first report of VIM-1-producing K. pneumoniae outside Europe. It is important that surveillance studies be undertaken in Africa to determine if VIM-1-producing K. pneumoniae are present in significant numbers.

OBJECTIVES: To assess paediatric antiretroviral treatment (ART) outcomes and their associations from a collaborative cohort representing 20% of the South African national treatment programme. DESIGN AND SETTING: Multi-cohort study of 7 public sector paediatric ART programmes in Gauteng, Western Cape and KwaZulu-Natal provinces. SUBJECTS: ART-naive children (< or = 16 years) who commenced treatment with > or = 3 antiretroviral drugs before March 2008. OUTCOME MEASURES: Time to death or loss to follow-up were assessed using the Kaplan-Meier method. Associations between baseline characteristics and mortality were assessed with Cox proportional hazards models stratified by site. Immune status, virological suppression and growth were described in relation to duration of ART. RESULTS: The median (interquartile range) age of 6 078 children with 9 368 child-years of follow-up was 43 (15 - 83) months, with 29% being < 18 months. Most were severely ill at ART initiation. More than 75% of children were appropriately monitored at 6-monthly intervals with viral load suppression (< 400 copies/ml) being 80% or above throughout 36 months of treatment. Mortality and retention in care at 3 years were 7.7% (95% confidence interval 7.0 - 8.6%) and 81.4% (80.1 - 82.6%), respectively. Together with young age, all markers of disease severity (low weight-for-age z-score, high viral load, severe immune suppression, stage 3/4 disease and anaemia) were independently associated with mortality. CONCLUSIONS: Dramatic clinical benefit for children accessing the national ART programme is demonstrated. Higher mortality in infants and those with advanced disease highlights the need for early diagnosis of HIV infection and commencement of ART.

BACKGROUND As access to antiretroviral therapy (ART) expands, increasing numbers of older patients will start treatment and need specialised long-term care. However, the effect of age in ART programmes in resource-constrained settings is poorly understood. The HIV epidemic is ageing rapidly and South Africa has one of the highest HIV population prevalences worldwide. We explored the effect of age on mortality of patients on ART in South Africa and whether this effect is mediated by baseline immunological status. METHODS In this retrospective cohort analysis, we studied HIV-positive patients aged 16-80 years who started ART for the first time in six large South African cohorts of the International Epidemiologic Databases to Evaluate AIDS-Southern Africa collaboration, in KwaZulu-Natal, Gauteng, and Western Cape (two primary care clinics, three hospitals, and a large rural cohort). The primary outcome was mortality. We ascertained patients' vital status through linkage to the National Population Register. We used inverse probability weighting to correct mortality for loss to follow-up. We estimated mortality using Cox's proportional hazards and competing risks regression. We tested the interaction between baseline CD4 cell count and age. FINDINGS Between Jan 1, 2004, and Dec 31, 2013, 84,078 eligible adults started ART. Of these, we followed up 83,566 patients for 174,640 patient-years. 8% (1817 of 23,258) of patients aged 16-29 years died compared with 19% (93 of 492) of patients aged 65 years or older. The age adjusted mortality hazard ratio was 2·52 (95% CI 2·01-3·17) for people aged 65 years or older compared with those 16-29 years of age. In patients starting ART with a CD4 count of less than 50 cells per μL, the adjusted mortality hazard ratio was 2·52 (2·04-3·11) for people aged 50 years or older compared with those 16-39 years old. Mortality was highest in patients with CD4 counts of less than 50 cells per μL, and 15% (1103 of 7295) of all patients aged 50 years or older starting ART were in this group. The proportion of patients aged 50 years or older enrolling in ART increased with successive years, from 6% (290 of 4999) in 2004 to 10% (961 of 9657) in 2012-13, comprising 9% of total enrolment (7295 of 83 566). At the end of the study, 6304 (14%) of 44,909 patients still alive and in care were aged 50 years or older. INTERPRETATION Health services need reorientation towards HIV diagnosis and starting of ART in older individuals. Policies are needed for long-term care of older people with HIV.
