Purpose: To compare the ability of Subjective assessment of optic nerve head (ONH) and retinal nerve fiber layer (RNFL) by general ophthalmologists and by a glaucoma expert with objective measurements by optical coherence tomography (Stratus OCT, Carl Zeiss Meditec Inc), confocal scanning laser ophthalmoscope (HRT III; Heidelberg Engineering, Heidelberg. Germany), and scanning laser polarimetry (GDx enhanced corneal compensation; Carl Zeiss Meditec Inc, Dublin, CA) in discriminating glaucomatous and normal eyes. Methods: Sixty-one glaucomatous and 57 normal eyes or 118 subjects Were included in the study. Three independent general ophthalmologists and I glaucoma expert evaluated ONH stereo-photographs. Receiver operating characteristic curves were constructed for each imaging technique and sensitivity at fixed specificity was estimated. Comparisons or areas under these curves (aROCs) and agreement (k) were determined between stereophoto grading and best parameter from each technique. Results: Best parameter from each technique showed larger aROC (Stratus OCT RNFL 0.92; Stratus OCT ONH vertical integrated area = 0.86; Stratus OCT macular thickness = 0.82; GDx enhanced corneal compensation = 0.91, HRT3 global cup-to-disc ratio = 0.83; HRT3 glaucoma probability score numeric area score 0.83) compared with stereophotograph grading by general ophthalmologists (0.80) in separating glaucomatous and normal eyes. Glaucoma expert stereophoto grading provided equal or larger aROC (0.92) than best parameter of each computerized imaging device. Stereophoto evaluated by a glaucoma expert showed better agreement with best parameter of each quantitative imaging technique in classifying eyes either as glaucomatous or normal compared with stereophoto grading by general ophthalmologists, The combination Of Subjective assessment of the optic disc by general ophthalmologists with RNFL objective parameters improved identification of glaucoma patients in a larger proportion than the combination of these objective parameters with Subjective assessment of the optic disc by a glaucoma expert (29.5% vs. 19.7%, respectively). Conclusions: Diagnostic ability of all imaging techniques showed better performance than subjective assessment of the ONH by general ophthalmologists, but not by It glaucoma expert, Objective RNFL measurements may provide improvement in glaucoma detection when combined with subjective assessment of the optic disc by general ophthalmologists or by a glaucoma expert.

Most studies employing experimental models of unilateral glaucoma have used the normotensive contralateral eye as the normal control. However, some studies have recently reported the activation of the retinal macroglia and microglia in the uninjured eye, suggesting that the eye contralateral to experimental glaucoma should not be used as a control. This review analyzes the studies describing the contralateral findings and discusses some of the routes through which the signals can reach the contralateral eye to initiate the glial reactivation.

Purpose:To functionally and morphologically characterize the retina and optic nerve after transplantation of Brain-derived neurotrophic factor (BDNF) and Glial-derived neurotrophic factor (GDNF) secreting mesenchymal stem cells (MSCs) into glaucomatous rat eyes. Methods:Chronic ocular hypertension (COH) was induced in Brown Norway rats. Lentiviral constructs were used to transduce rat MSCs to produce BDNF, GDNF, or green fluorescent protein (GFP). The fellow eyes served as internal controls. Two days following COH induction, eyes received intravitreal injections of transduced MSCs. Electroretinography was performed to assess retinal function. Tonometry was performed throughout the experiment to monitor IOP. 42 days after MSC transplantation, rats were euthanized and the eyes and optic nerves were prepared for analysis. Results:Increased expression and secretion of BDNF and GDNF from lentiviral-transduced MSCs was verified using ELISA, and a bioactivity assay. Ratio metric analysis (COH eye/ Internal control eye response) of the Max combined response A-Wave showed animals with BDNF-MSCs (23.35 ± 5.15%, p=0.021) and GDNF-MSCs (28.73 ± 3.61%, p=0.025) preserved significantly more visual function than GFP-MSC treated eyes MSCs (18.05 ± 5.51%). Animals receiving BDNF-MSCs also had significantly better B-wave (33.80 ± 7.19%) and flicker ERG responses (28.52 ± 10.43%) than GFP-MSC treated animals (14.06 ± 12.67%; 3.52 ± 0.07%, respectively). Animals receiving GDNF-MSC transplants tended to have better function than animals with GFP-MSC transplants, but were not statistically significant (p=0.057 and p=0.0639). Conclusions:Mesenchymal stem cells are an excellent source of cells for autologous transplantation for the treatment of neurodegenerative diseases. We have demonstrated that lentiviral- transduced MSCs can survive following transplantation and preserve visual function in glaucomatous eyes. These results suggest that MSCs may be an ideal cellular vehicle for delivery of specific neurotrophic factors to the retina.

Purpose. To investigate misalignments (MAs) on retinal nerve fiber layer thickness (RNFLT) measurements obtained with Cirrus(©) SD-OCT. Methods. This was a retrospective, observational, cross-sectional study. Twenty-seven healthy and 29 glaucomatous eyes of 56 individuals with one normal exam and another showing MA were included. MAs were defined as an improper alignment of vertical vessels in the en face image. MAs were classified in complete MA (CMA) and partial MA (PMA), according to their site: 1 (superior, outside the measurement ring (MR)), 2 (superior, within MR), 3 (inferior, within MR), and 4 (inferior, outside MR). We compared RNFLT measurements of aligned versus misaligned exams in all 4 sectors, in the superior area (sectors 1 + 2), inferior area (sectors 3 + 4), and within the measurement ring (sectors 2 + 3). Results. RNFLT measurements at 12 clock-hour of eyes with MAs in the superior area (sectors 1 + 2) were significantly lower than those obtained in the same eyes without MAs (P = 0.043). No significant difference was found in other areas (sectors 1 + 2 + 3 + 4, sectors 3 + 4, and sectors 2 + 3). Conclusion. SD-OCT scans with superior MAs may present lower superior RNFLT measurements compared to aligned exams.

PURPOSE: To evaluate the sensitivity and specificity of machine learning classifiers (MLCs) for glaucoma diagnosis using Spectral Domain OCT (SD-OCT) and standard automated perimetry (SAP). METHODS: Observational cross-sectional study. Sixty two glaucoma patients and 48 healthy individuals were included. All patients underwent a complete ophthalmologic examination, achromatic standard automated perimetry (SAP) and retinal nerve fiber layer (RNFL) imaging with SD-OCT (Cirrus HD-OCT; Carl Zeiss Meditec Inc., Dublin, California). Receiver operating characteristic (ROC) curves were obtained for all SD-OCT parameters and global indices of SAP. Subsequently, the following MLCs were tested using parameters from the SD-OCT and SAP: Bagging (BAG), Naive-Bayes (NB), Multilayer Perceptron (MLP), Radial Basis Function (RBF), Random Forest (RAN), Ensemble Selection (ENS), Classification Tree (CTREE), Ada Boost M1(ADA),Support Vector Machine Linear (SVML) and Support Vector Machine Gaussian (SVMG). Areas under the receiver operating characteristic curves (aROC) obtained for isolated SAP and OCT parameters were compared with MLCs using OCT+SAP data. RESULTS: Combining OCT and SAP data, MLCs' aROCs varied from 0.777(CTREE) to 0.946 (RAN).The best OCT+SAP aROC obtained with RAN (0.946) was significantly larger the best single OCT parameter (p<0.05), but was not significantly different from the aROC obtained with the best single SAP parameter (p=0.19). CONCLUSION: Machine learning classifiers trained on OCT and SAP data can successfully discriminate between healthy and glaucomatous eyes. The combination of OCT and SAP measurements improved the diagnostic accuracy compared with OCT data alone.

Purpose The aim of this study was to test the correlation between Fourier-domain (FD) optical coherence tomography (OCT) macular and retinal nerve fibre layer (RNFL) thickness and visual field (VF) loss on standard automated perimetry (SAP) in chiasmal compression. Methods A total of 35 eyes with permanent temporal VF defects and 35 controls underwent SAP and FD-OCT (3D OCT-1000; Topcon Corp.) examinations. Macular thickness measurements were averaged for the central area and for each quadrant and half of that area, whereas RNFL thickness was determined for six sectors around the optic disc. VF loss was estimated in six sectors of the VF and in the central 16 test points in the VF. The correlation between VF loss and OCT measurements was tested with Spearman`s correlation coefficients and with linear regression analysis. Results Macular and RNFL thickness parameters correlated strongly with SAP VF loss. Correlations were generally stronger between VF loss and quadrantic or hemianopic macular thickness than with sectoral RNFL thickness. For the macular parameters, we observed the strongest correlation between macular thickness in the inferonasal quadrant and VF loss in the superior temporal central quadrant (rho=0.78; P<0.001) whereas for the RNFL parameters the strongest correlation was observed between the superonasal optic disc sector and the central temporal VF defect (rho=0.60; P<0.001).

PURPOSE. To evaluate the effect of disease severity on the diagnostic accuracy of the Cirrus Optical Coherence Tomograph (Cirrus HD-OCT; Carl Zeiss Meditec, Inc., Dublin, CA) for glaucoma detection. METHODS. One hundred thirty-five glaucomatous eyes of 99 patients and 79 normal eyes of 47 control subjects were recruited from the longitudinal Diagnostic Innovations in Glaucoma Study (DIGS). The severity of the disease was graded based on the visual field index (VFI) from standard automated perimetry. Imaging of the retinal nerve fiber layer (RNFL) was obtained using the optic disc cube protocol available on the Cirrus HD-OCT. Pooled receiver operating characteristic (ROC) curves were initially obtained for each parameter of the Cirrus HD-OCT. The effect of disease severity on diagnostic performance was evaluated by fitting an ROC regression model, with VFI used as a covariate, and calculating the area under the ROC curve (AUCs) for different levels of disease severity. RESULTS. The largest pooled AUCs were for average thickness (0.892), inferior quadrant thickness (0.881), and superior quadrant thickness (0.874). Disease severity had a significant influence on the detection of glaucoma. For the average RNFL thickness parameter, AUCs were 0.962, 0.932, 0.886, and 0.822 for VFIs of 70%, 80%, 90%, and 100%, respectively. CONCLUSIONS. Disease severity had a significant effect on the diagnostic performance of the Cirrus HD-OCT and thus should be considered when interpreting results from this device and when considering the potential applications of this instrument for diagnosing glaucoma in the various clinical settings. Purpose: To examine the relationship of functional measurements with structural measures. Methods: 146 eyes of 83 test subjects underwent Heidelberg Retinal Tomography (HRTIII) (disc area<2.43, mphsd<40), and perimetry testing with Octopus (SAP; Dynamic), Pulsar (PP; TOP) and Moorfields MDT (ESTA). Glaucoma was defined as progressive structural or functional loss (20 eyes). Perimetry test points were grouped into 6 sectors based on the estimated optic nerve head angle into which the associated nerve fiber bundle enters (Garway-Heath map). Perimetry summary measures (PSM) (MD SAP/ MD PP/ PTD MDT) were calculated from the average total deviation of each measured threshold from the normal for each sector. We calculated the 95% significance level of the sectorial PSM from the respective normative data. We calculated the percentage agreement with group1 (G1), healthy on HRT and within normal perimetric limits, and group 2 (G2), abnormal on HRT and outside normal perimetric limits. We also examined the relationship of PSM and rim area (RA) in those sectors classified as abnormal by MRA (Moorfields Regression Analysis) of HRT. Results: The mean age was 65 (range= [37, 89]). The global sensitivity versus specificity of each instrument in detecting glaucomatous eyes was: MDT 80% vs. 88%, SAP 80% vs. 80%, PP 70% vs. 89% and HRT 80% vs. 79%. Highest percentage agreement of HRT (respectively G1, G2, sector) with PSM were MDT (89%, 57%, nasal superior), SAP (83%, 74%, temporal superior), PP (74%, 63%, nasal superior). Globally percentage agreement (respectively G1, G2) was MDT (92%, 28%), SAP (87%, 40%) and PP (77%, 49%). Linear regression showed there was no significant trend globally associating RA and PSM. However, sectorally the supero-nasal sector had a statistically significant (p<0.001) trend with each instrument, the associated r2 coefficients are (MDT 0.38 SAP 0.56 and PP 0.39). Conclusions: There were no significant differences in global sensitivity or specificity between instruments. Structure-function relationships varied significantly between instruments and were consistently strongest supero-nasally. Further studies are required to investigate these relationships in detail.

The vulnerability of subpopulations of retinal neurons delineated by their content of cytoskeletal or calcium-binding proteins was evaluated in the retinas of cynomolgus monkeys in which glaucoma was produced with an argon laser. We quantitatively compared the number of neurons containing either neurofilament (NF) protein, parvalbumin, calbindin or calretinin immunoreactivity in central and peripheral portions of the nasal and temporal quadrants of the retina from glaucomatous and fellow non-glaucomatous eyes. There was no significant difference between the proportion of amacrine, horizontal and bipolar cells labeled with antibodies to the calcium-binding proteins comparing the two eyes. NF triplet immunoreactivity was present in a subpopulation of retinal ganglion cells, many of which, but not all, likely correspond to large ganglion cells that subserve the magnocellular visual pathway. Loss of NF protein-containing retinal ganglion cells was widespread throughout the central (59-77% loss) and peripheral (96-97%) nasal and temporal quadrants and was associated with the loss of NF-immunoreactive optic nerve fibers in the glaucomatous eyes. Comparison of counts of NF-immunoreactive neurons with total cell loss evaluated by Nissl staining indicated that NF protein-immunoreactive cells represent a large proportion of the cells that degenerate in the glaucomatous eyes, particularly in the peripheral regions of the retina. Such data may be useful in determining the cellular basis for sensitivity to this pathologic process and may also be helpful in the design of diagnostic tests that may be sensitive to the loss of the subset of NF-immunoreactive ganglion cells.

Purpose: To compare the performance Glaucoma Quality of Life-15 (GQL-15) Questionnaire, intraocular pressure measurement (IOP Goldmann tonometry) and a measure of visual field loss using Moorfields Motion Displacement Test (MDT) in detecting glaucomatous eyes from a self referred population. Methods: The GQL-15 has been suggested to correlate with visual disability and psychophysical measures of visual function in glaucoma patients. The Moorfields MDT is a multi location perimetry test with 32 white line stimuli presented on a grey background on a standard laptop computer. Each stimulus is displaced between computer frames to give the illusion of "apparent motion". Participants (N=312, 90% older than 45 years; 20.5% family history of glaucoma) self referred to an advertised World Glaucoma Day (March 2009) Jules Gonin Eye Hospital, Lausanne Switzerland. Participants underwent a clinical exam (IOP, slit lamp, angle and disc examination by a general ophthalmologist), 90% completed a GQL-15 questionnaire and over 50% completed a MDT test in both eyes. Those who were classified as abnormal on one or more of the following (IOP >21 mmHg/ GQL-15 score >20/ MDT score >2/ clinical exam) underwent a follow up clinical examination by a glaucoma specialist including imaging and threshold perimetry. After the second examination subjects were classified as "healthy"(H), "glaucoma suspect" (GS) (ocular hypertension and/or suspicious disc, angle closure with SD) or "glaucomatous" (G). Results: One hundred and ten subjects completed all 4 initial examinations; of these 69 were referred to complete the 2nd examination and were classified as; 8 G, 24 GS, and 37 H. MDT detected 7/8 G, and 7/24 GS, with false referral rate of 3.8%. IOP detected 2/8 G and 8/24 GS, with false referral rate of 8.9%. GQL-15 detected 4/8 G, 16/24 GS with a false referral rate of 42%. Conclusions: In this sample of participants attending a self referral glaucoma detection event, the MDT performed significantly better than the GQL-15 and IOP in discriminating glaucomatous patients from healthy subjects. Further studies are required to assess the potential of the MDT as a glaucoma screening tool.

Le glaucome représente la première cause de cécité irréversible à l’échelle mondiale. C’est une maladie neuro-dégénérative caractérisée traditionnellement par une pression intraoculaire (PIO) élevée, un dommage du nerf optique et un défaut du champ visuel correspondant. En fait, la PIO élevée constitue le facteur de risque central associé au développement du glaucome. Cependant, en dépit d’un contrôle adéquat de la PIO, la maladie continue à progresser chez certains patients. Cela montre qu’il existe d’autres facteurs impliqués dans la pathogenèse du glaucome. Des études récentes indiquent qu’un dérèglement de l’oxygène est associé à son développement. En utilisant une nouvelle technologie multi-spectrale capable de mesurer la saturation en oxygène (SaO2) dans les structures capillaires de la rétine, cette étude tentera de déterminer si un état d’oxygénation anormal pourrait se retrouver à la papille optique des patients souffrant de glaucome. Une meilleure compréhension du rôle de l’oxygène pourrait aider à améliorer le pronostic du glaucome. Les résultats de l’étude indiquent que le facteur de position (supérieure, temporale et inférieure de la papille optique) n’a aucun effet sur la mesure SaO2 ainsi que sa variabilité chez les patients normaux. La comparaison de la SaO2 entre les sujets normaux et glaucomateux ne montre pas de différence statistiquement significative. En conclusion, la SaO2 «normale» mesurée dans les yeux glaucomateux n'exclut pas nécessairement que l'hypoxie ne soit pas impliquée dans la pathogenèse. Au moment de l’étude, la PIO était bien contrôlée par des médicaments topiques, ce qui pourrait influencer l’oxygénation à la papille optique.

Justification: Le glaucome entraîne une perte progressive de la vision causée par la détérioration du nerf optique. Le glaucome est répandu dans le monde et cause la cécité dans environ sept millions de personnes. Le glaucome touche plus de 400 000 Canadiens et sa prévalence augmente avec le vieillissement de la population.1,2 Il s'agit d'une maladie chronique surnoise dont les symptômes se manifestent uniquement lors des stades avancés et qui peuvent mener à la cécité. Présentement, le seul moyen possible d’arrêter la progression du glaucome au stade initial est de diminuer la pression intra-oculaire (PIO). Les analogues de prostaglandines (APG) topiques sont fréquemment utilisées comme traitement de première ligne. Cependant, la recherche démontre que cette classe de médicaments peut changer certaines propriétés de la cornée, et possiblement influencer la mesure de la PIO.3 Objectif: À déterminer si l'utilisation d'APG affecte les propriétés biomécaniques de la cornée. La conclusion sera basée sur l'analyse intégrée des résultats obtenus de l'analyseur Reichert oculaire Réponse (ORA), la tonométrie par applanation de Goldmann (TAG) et la pachymétrie ultrasonographique. Le deuxième objectif potentiel de cette étude est de déterminer la corrélation, le cas échéant, entre les propriétés biomécaniques de la cornée, l'épaisseur de la cornée centrale (ECC) et la PIO chez les patients subissant un traitement d’APG topique. L'hypothèse principale de cette étude est que l’APG influence les propriétés de la cornée telles que l'épaisseur centrale, l'élasticité et la résistance. Patients et méthodes : Soixante-dix yeux de 35 patients, âgés de 50-85 ans, atteints de glaucome à angle ouvert (GAO) et traités avec APG topique ont été examinés. Seulement les sujets avec une réfraction manifeste entre -6,00 D et +4,25 D ont été inclus. Les critères d'exclusion sont: patients avec n'importe quelle autre maladie de la cornée de l’œil, telles que la dystrophie endothéliale de Fuch’s et kératocône, ou tout antécédent de traumatisme ou d'une chirurgie de la cornée, ainsi que le port de lentilles de contact. Nous avons demandé aux patients atteints du glaucome qui ont des paramètres stables et qui utilisent l’APG dans les deux yeux de cesser l’APG dans l'œil moins affecté par la PIO, et de continuer l’utilisation d’APG dans l'œil contralatéral. Le meilleur œil est défini comme celui avec moins de dommage sur le champ visuel (CV) (déviation moyenne (DM), le moins négatif) ou une PIO maximale historique plus basse si la DM est égale ou encore celui avec plus de dommage sur la tomographie par cohérence optique (TCO, Cirrus, CA) ou la tomographie confocale par balayage laser (HRT, Heidelberg, Allemagne). Toutes les mesures ont été prises avant la cessation d’APG et répétées 6 semaines après l’arrêt. Les patients ont ensuite recommencé l’utilisation d’APG et toutes les mesures ont été répétées encore une fois après une période supplémentaire de 6 semaines. Après commencer ou de changer le traitement du glaucome, le patient doit être vu environ 4-6 semaines plus tard pour évaluer l'efficacité de la goutte.4 Pour cette raison, on été décidé d'utiliser 6 semaines d'intervalle. Toutes les mesures ont été effectuées à l’institut du glaucome de Montréal par le même technicien, avec le même équipement et à la même heure de la journée. L'œil contralatéral a servi comme œil contrôle pour les analyses statistiques. La tonométrie par applanation de Goldmann a été utilisée pour mesurer la PIO, la pachymétrie ultrasonographique pour mesurer l'ECC, et l’ORA pour mesurer les propriétés biomécaniques de la cornée, incluant l'hystérèse cornéenne (HC). L’hypothèse de l'absence d'effet de l'arrêt de l’APG sur les propriétés biomécaniques a été examiné par un modèle linéaire à effets mixtes en utilisant le logiciel statistique R. Les effets aléatoires ont été définies à deux niveaux: le patient (niveau 1) et l'œil de chaque patient (niveau 2). Les effets aléatoires ont été ajoutés au modèle pour tenir compte de la variance intra-individuelle. L’âge a également été inclus dans le modèle comme variable. Les contrastes entre les yeux et les temps ont été estimés en utilisant les valeurs p ajustées pour contrôler les taux d'erreur internes de la famille en utilisant multcomp paquet dans R. Résultats: Une augmentation statistiquement significative due l 'HC a été trouvée entre les visites 1 (sur APG) et 2 (aucun APG) dans les yeux de l'étude, avec une moyenne (±erreur standard) des valeurs de 8,98 ± 0,29 mmHg et 10,35 ± 0,29 mmHg, respectivement, correspondant à une augmentation moyenne de 1,37 ± 0,18 mmHg (p <0,001). Une réduction significative de 1,25 ± 0,18 mmHg (p <0,001) a été observée entre les visites 2 et 3, avec une valeur moyenne HC finale de 9,09 ± 0,29 mmHg. En outre, une différence statistiquement significative entre l’oeil d’étude et le contrôle n'a été observée que lors de la visite 2 (1,01 ± 0,23 mmHg, p <0,001) et non lors des visites 1 et 3. Une augmentation statistiquement significative du facteur de résistance conréen (FRC) a été trouvée entre les visites 1 et 2 dans les yeux de l'étude, avec des valeurs moyennes de 10,23 ± 0,34 mmHg et 11,71 ± 0,34 mmHg, respectivement. Le FRC a ensuite été réduit de 1,90 ± 0,21 mmHg (p <0,001) entre les visites 2 et 3, avec une valeur moyenne FRC finale de 9,81 ± 0,34 mmHg. Une différence statistiquement significative entre l’oeil d’étude et le contrôle n'a été observée que lors de la visite 2 (1,46 ± 0,23 mmHg, p <0,001). Une augmentation statistiquement significative de l'ECC a été trouvée entre les visites 1 et 2 dans les yeux de l'étude, avec des valeurs moyennes de 541,83 ± 7,27 µm et 551,91 ± 7,27 µm, respectivement, ce qui correspond à une augmentation moyenne de 10,09 ± 0,94 µm (p <0,001). L'ECC a ensuite diminué de 9,40 ± 0,94 µm (p <0,001) entre les visites 2 et 3, avec une valeur moyenne finale de 542,51 ± 7,27 µm. Une différence entre l’étude et le contrôle des yeux n'a été enregistré que lors de la visite 2 (11,26 ± 1,79 µm, p <0,001). De même, on a observé une augmentation significative de la PIO entre les visites 1 et 2 dans les yeux de l'étude, avec des valeurs moyennes de 15,37 ± 0,54 mmHg et 18,37 ± 0,54 mmHg, respectivement, ce qui correspond à une augmentation moyenne de 3,0 ± 0,49 mmHg (p <0,001). Une réduction significative de 2,83 ± 0,49 mmHg (p <0,001) a été observée entre les visites 2 et 3, avec une valeur moyenne de la PIO finale de 15,54 ± 0,54 mmHg. L’oeil de contrôle et d’étude ne différaient que lors de la visite 2 (1,91 ± 0,49 mmHg, p <0,001), ce qui confirme l'efficacité du traitement de l’APG. Lors de la visite 1, le biais de la PIO (PIOcc - PIO Goldmann) était similaire dans les deux groupes avec des valeurs moyennes de 4,1 ± 0,54 mmHg dans les yeux de contrôles et de 4,8 ± 0,54 mmHg dans les yeux d’études. Lors de la visite 2, après un lavage de 6 semaines d’APG, le biais de la PIO dans l'œil testé a été réduit à 1,6 ± 0,54 mmHg (p <0,001), ce qui signifie que la sous-estimation de la PIO par TAG était significativement moins dans la visite 2 que de la visite 1. La différence en biais PIO moyenne entre l'étude et le contrôle des yeux lors de la visite 2, en revanche, n'a pas atteint la signification statistique (p = 0,124). On a observé une augmentation peu significative de 1,53 ± 0,60 mmHg (p = 0,055) entre les visites 2 et 3 dans les yeux de l'étude, avec une valeur de polarisation finale de la PIO moyenne de 3,10 ± 0,54 mmHg dans les yeux d'études et de 2,8 ± 0,54 mmHg dans les yeux de contrôles. Nous avons ensuite cherché à déterminer si une faible HC a été associée à un stade de glaucome plus avancé chez nos patients atteints du glaucome à angle ouvert traités avec l’APG. Lorsque l'on considère tous les yeux sur l’APG au moment de la première visite, aucune association n'a été trouvée entre les dommages sur le CV et l'HC. Cependant, si l'on considère seulement les yeux avec un glaucome plus avancé, une corrélation positive significative a été observée entre la DM et l'HC (B = 0,65, p = 0,003). Une HC inférieure a été associé à une valeur de DM de champ visuelle plus négative et donc plus de dommages liés au glaucome. Conclusions : Les prostaglandines topiques affectent les propriétés biomécaniques de la cornée. Ils réduisent l'hystérèse cornéenne, le facteur de résistance cornéen et l'épaisseur centrale de la cornée. On doit tenir compte de ces changements lors de l'évaluation des effets d’APG sur la PIO. Plus de recherche devrait être menées pour confirmer nos résultats. De plus, d’autres études pourraient être réalisées en utilisant des médicaments qui diminuent la PIO sans influencer les propriétés biomécaniques de la cornée ou à l'aide de tonomètre dynamique de Pascal ou similaire qui ne dépend pas des propriétés biomécaniques de la cornée. En ce qui concerne l'interaction entre les dommages de glaucome et l'hystérésis de la cornée, nous pouvons conclure qu' une HC inférieure a été associé à une valeur de DM de CV plus négative. Investigou-se, clinicamente o resultado da ablação uveal intravítrea em 13 olhos cegos de cães com glaucoma crônico unilateral. Os olhos acometidos foram submetidos à ablação uveal intravítrea, por meio de injeção na câmara vítrea de 0,5ml de sulfato de gentamicina (40mg/ml) associado a 0,3ml de fosfato de dexametasona (4mg/ml). As variáveis clinicas oftálmicas foram quali-quantificadas em escores, por até 48 semanas do pós-operatório; além de aspectos relacionados à dor, como variações do apetite e peso corporal. Nos sinais clínicos, de secreção ocular, blefaroespasmo, quemose, hifema e pigmentação, neovascularização, pannus e variações de apetite e peso corporal, não se notaram diferenças significativas entre os momentos. A ablação uveal intravítrea diminuiu a hiperemia conjuntival, porém acarretou aumento de opacidade corneana. A associação da ablação com antiinflamatórios tópico e sistêmico indicou não se tratar de procedimento doloroso.

P>Background: We aimed to evaluate the ascorbic acid concentration in secondary aqueous humour (AH) from glaucomatous patients and to compare it with primary AH from primary open-angle glaucoma patients and non-glaucomatous patients. Methods: Primary AH samples were prospectively obtained from clinically uncontrolled primary open-angle glaucoma patients and senile cataract patients (controls) prior to trabeculectomy and cataract surgery. Secondary AH samples were obtained from eyes with previous intraocular surgery, prior to trabeculectomy or cataract surgery. AH (0.1 mL) was aspirated by inserting a 26-gauge needle into the anterior chamber just before surgery and then immediately stored at -80 degrees C. The ascorbic acid concentration was determined in a masked fashion by high-pressure liquid chromatography. Results: A total of 18 patients with senile cataract, 16 glaucomatous patients with primary AH (no previous intraocular surgery) and 11 glaucomatous patients with secondary AH (previous intraocular surgery) were included. There was no difference in mean age between groups (P = 0.15). The mean +/- standard deviation concentration of ascorbic acid in the secondary AH from glaucomatous patients (504 +/- 213 mu mol/L [95% confidence interval {CI}, 383-624]) was significantly lower than the concentration of ascorbic acid found in the primary aqueous of primary open-angle glaucoma (919 +/- 427 mu mol/L [95% CI, 709-1128]) and control patients (1049 +/- 433 mu mol/L [95% CI, 848-1249]; P < 0.01, Kruskal-Wallis test). Conclusions: The ascorbic acid concentration in secondary AH of glaucomatous patients was approximately twofold lower in comparison with primary AH of glaucomatous and cataract patients. The implications of a reduced concentration of ascorbic acid in the secondary AH deserve further investigation.

Background: We aimed to investigate the performance of five different trend analysis criteria for the detection of glaucomatous progression and to determine the most frequently and rapidly progressing locations of the visual field. Design: Retrospective cohort. Participants or Samples: Treated glaucoma patients with =8 Swedish Interactive Thresholding Algorithm (SITA)-standard 24-2 visual field tests. Methods: Progression was determined using trend analysis. Five different criteria were used: (A) =1 significantly progressing point; (B) =2 significantly progressing points; (C) =2 progressing points located in the same hemifield; (D) at least two adjacent progressing points located in the same hemifield; (E) =2 progressing points in the same Garway-Heath map sector. Main Outcome Measures: Number of progressing eyes and false-positive results. Results: We included 587 patients. The number of eyes reaching a progression endpoint using each criterion was: A = 300 (51%); B = 212 (36%); C = 194 (33%); D = 170 (29%); and E = 186 (31%) (P = 0.03). The numbers of eyes with positive slopes were: A = 13 (4.3%); B = 3 (1.4%); C = 3 (1.5%); D = 2 (1.1%); and E = 3 (1.6%) (P = 0.06). The global slopes for progressing eyes were more negative in Groups B, C and D than in Group A (P = 0.004). The visual field locations that progressed more often were those in the nasal field adjacent to the horizontal midline. Conclusions: Pointwise linear regression criteria that take into account the retinal nerve fibre layer anatomy enhances the specificity of trend analysis for the detection glaucomatous visual field progression.
