Introdução: O tratamento das Síndromes Coronárias Agudas (SCA) sofreu várias alterações muito rápidas nos últimos anos, traduzido nas múltiplas propostas de recomendações pelo ACC/AHA/ESC, baseados na evidência clínica. Avaliamos a implementação destas recomendações, comparando uma população de doentes de 2002, com uma população de 2005. Métodos: Estudo retrospectivo de 368 doentes admitidos em 2002 e 420 doentes admitidos em 2005 por SCA (com e sem elevação do segmento ST). Analisaram-se características clínicas e estratégias de tratamento. Resultados: Não se verificaram diferenças em termos de idade, sexo masculino, factores de risco para doença coronária ou história prévia de revascularização miocárdica. Verificou-se uma redução de doentes com antecedentes de enfarte do miocárdico e insuficiência renal e aumento da apresentação como enfarte com elevação do segmento ST. O tratamento com clopidogrel (6% versus 87%), bloqueador-beta(54% versus 79%), inibidores da enzima de conversão da angiotensina (72% versus 84%) e estatinas (78% versus 91%) aumentou (para todos p<0,001). Por outro lado, verificou-se um pequeno decréscimo na utilização de ácido acetilsalicílico (98% versus 95%, p=0,039) (com maior utilização de clopidogrel) e a ticlopidina deixou de ser utilizada (46% versus 0%, p<0,001). Os antagonistas dos receptores da glicoproteína IIb/IIIa não se alteraram significativamente (66% versus 67%, p=NS). Aumentaram as intervenções coronárias percutâneas (53% versus 67%, p<0,001). Não se verificou diferença em termos de mortalidade hospitalar (8,2% versus 6,4%) e aos 30 dias (9,0% versus. 8,6%), com redução ao 1ºano de seguimento (17,1% versus 11,7%, p=0,039). As estatinas e os bloqueadores beta são preditores independentes de mortalidade, com efeito de protecção. Conclusões: Entre 2002 e 2005, o tratamento das SCA melhorou significativamente de acordo com as recomendações existentes, traduzindo-se numa melhoria da mortalidade ao 1º ano de seguimento.

Objectivo: estudar determinantes cardiovasculares condicionantes do tempo de ventilação, mortalidade e gravidade de doença em doentes admitidos numa unidade de cuidados intensivos para ventilação mecânica por exacerbação de insuficiência respiratória crónica. Desenho e local: Estudo prospectivo, com duração de 30 meses numa unidade de cuidados intensivos médico-cirúrgica com 14 camas.Material e métodos: Estudados 59 doentes com idade média de 74,7 +/- 9,7 anos, tempo médio de ventilação de 10,8 +/- 12,6 dias, APACHE II médio de 23 +/- 8,3. Avaliaram-se parâmetros ecocardiográficos (dimensões das cavidades, débito cardíaco, estudo Doppler do fluxo transvalvular mitral, estudo da veia cava inferior) e electrocardiográficos(presença de ritmo sinusal ou fibrilhação auricular) nas primeiras 24 horas de internamento na Unidade e parâmetros gasimétricos à saída. Resultados: Um tempo de ventilação mais prolongado associou-se à presença de fibrilhação auricular (p=0,027), à presença conjunta de fibrilhação auricular e uma veia cava inferior dilatada (> 20mm p=0,004) e com níveis séricos de bicarbonato> 35mEq/l na gasimetria obtida à saída (p=0,04). Verificaram-se 12 óbitos. A mortalidade associou-se à presença de dilatação do ventrículo direito (p=0,03) e a uma relação entre o ventrículo direito e o esquerdo> 0,6 (p=0,04). Conclusão: Nos doentes submetidos a ventilação mecânica por exacerbação de insuficiência respiratória crónica, a presença de fibrilhação auricular indica a possibilidade de um período de ventilação mais prolongado, em especial se houver concomitantemente uma veia cava inferior com diâmetro> 20mm. Nestes doentes, a presença de dilatação das cavidades direitas pode indicar uma probabilidade mais elevada de mortalidade.

INTRODUCTION: Transthoracic echocardiography is the method of choice for the diagnosis of cardiac myxomas, but the transesophageal approach provides a better definition of the location and characteristics of the tumor. The authors review their thirteen years' experience on the echocardiographic diagnosis of this pathology. METHODS: From 1994 to 2007, 41 cardiac tumors were diagnosed in our echocardiographic laboratory, of which 27 (65.85%) were cardiac myxomas. The exams and the patients' clinical files were retrospectively reviewed. RESULTS: Of the 27 patients, 22 (81.5%) were female, with a mean age of 62.1 +/- 13.6 years (25-84 years). The predominant clinical features were due to the obstruction caused by the tumor in more than two thirds of the patients, followed by constitutional symptoms in one third and embolic events in 30%. In the lab results, anemia was found in three patients and elevated sedimentation rate and CRP in two. In two patients the myxoma was found by chance. All the cases were of the sporadic type, although we found a prevalence of thyroid disease of 14% (4 patients). All patients underwent urgent surgical resection except one, in whom surgery was refused due to advanced age and comorbidities. The myxomas followed a typical distribution with 24 (88.8%) located in the left atrium, 18 of them attached to the atrial septum (AS) and two to the mitral valve. In one patient, the tumor involved both atria. The other two cases originated in the right atrium at the AS. Embolic phenomena were more frequent in small tumors (p = 0.027) and in those with a villous appearance (p = 0.032). Obstructive manifestations were associated with larger tumors (p = 0.046) and larger left atria (p = 0.048). In our series, there were no deaths during hospitalization or in the follow-up period of 5.2 +/- 3.7 years in 19 patients. There were two recurrences, both patients being successfully reoperated. CONCLUSION: Myxoma is the most common cardiac tumor. Transesophageal echocardiography provides excellent morphologic definition, aiding in diagnosis and follow-up. Most clinical manifestations are obstructive and are associated with larger tumors. Small tumors with a friable appearance have a higher chance of embolization. Surgical resection is usually curative and the long-term prognosis is excellent.
Encerramento Percutâneo de Shunts Interauriculares: Experiência de uma Década de um Centro Terciário

INTRODUCTION: Atrial septal defects (ASD) are among the most common congenital anomalies and account for 10% of congenital heart disease in the pediatric age-group and 30% in adults. Closure is indicated when there is evidence of hemodynamic significance or after a paradoxical embolic event. Ten years ago, percutaneous closure became the treatment of choice in our center for all patients with a clear indication and favorable anatomy. In this paper we report the experience of this first decade. OBJECTIVE: To assess the short- and long-term results of our ten-year experience with percutaneous closure of atrial septal defects. METHODS: We studied retrospectively all patients with ASD treated with a percutaneous approach between November 1998 and December 2008. The pediatric age-group consisted of patients younger than 19 years old. Demographic data, clinical indications, minor and major complication rates, success rate and long-term outcome were assessed. RESULTS: In the first ten years of experience 510 patients, of whom 166 were in the pediatric group, were treated in our center by a team of adult and pediatric cardiologists. The overall success rate of the procedure was 98% (97.5% in ASD and 99.5% in patent foramen ovale (PFO). The minor complication rate was 3% (3.4% in ASD and 2% in PFO). The most frequent complication was supraventricular tachycardia. The major complication rate was 1.2% (0.6% in ASD and 2% in PFO). Two patients developed cardiac tamponade due to hemopericardium that was resolved by pericardiocentesis, without need for surgery. One patient had an arterial pseudoaneurysm corrected by vascular surgery. There was no device embolization and no need for urgent surgery in this population. During follow-up two patients had recurrence of ischemic stroke, one had a transient ischemic attack and another had a hemorrhagic stroke. Mortality was 0.6% (0.6% in ASD and 0.5% in PFO). There were no in-hospital deaths. During follow-up there were two deaths, both in the adult group. DISCUSSION AND CONCLUSION: In this population the success rate was high and most of the complications were minor. The results of this collaboration between adult and pediatric cardiologists in the first ten years of activity confirm the safety and efficacy of percutaneous closure of septal defects, when there is careful patient selection and a standardized technique.

A hipofaringe e o esófago cervical são áreas particularmente vulneráveis a traumatismos por entubação oro-traqueal. Estes acidentes anestésicos são raros, pouco relatados e com maior incidência em situações de emergência. As perfurações faringoesofágicas podem passar despercebidas até ao aparecimento de sinais e sintomas característicos. Estes incluem dor cervical, febre, disfagia, leucocitose, enfisema subcutâneo e pneumomediastino. O tratamento cirúrgico vs conservador é controverso. É no entanto consensual que o intervalo de tempo entre o traumatismo e o diagnóstico e tratamento é, por si só, o factor mais importante para o prognóstico. Os autores apresentam um caso de uma parturiente submetida a cesariana de urgência em que ocorreu um acidente anestésico por entubação difícil, com laceração do seio piriforme e posterior formação de abcesso mediastínico.

The field of action for rehabilitation is that of making use of the patient's maximum functional capacity with the purpose of adapting to life in relation to the environment. Rehabilitation must commence immediately, although it may be in different forms from the acute phase to sequelae. It is considered appropriate to call the physiatrist as soon as the neurologic condition has stabilised. A list is made of the measures to be taken for rehabilitation in the acute phase and sequelae, and the composition of the rehabilitation team is described. In what concerns location, where to rehabilitate the patient? The group of ambulatory patients should have their rehabilitation as outpatients. Our experience with house calls is briefly described. The group of patients who cannot walk, those that present an eminently motor condition, with the possibility of being able to walk, should be with their families, with transport provided to health and rehabilitation centres. The second group, with the capacity of walking within a reasonable time, especially if with multiple associated problems such as impaired communication, should be hospitalised in a rehabilitation department. The third group consists of severely handicapped patients, for whom a solution must be found that provides life with a minimum of dignity in centres or homes. From among the measures to be introduced, we point out following: acquisition of transport for patients who must travel, as outpatients, to the department; providing family doctors with complete freedom to refer their patients to rehabilitation centres.

BACKGROUND: In ST-segment elevation myocardial infarction (STEMI) patients treated with primary angioplasty, neutrophil response and its prognostic significance are not entirely understood. METHODS: We retrospectively studied 305 consecutive and non-selected STEMI patients. They were divided into three groups according to the maximum neutrophil percentage in the first 48 hours. We compared baseline demographic characteristics, coronary disease risk factors, cardiac history, clinical presentation, therapeutics administered and clinical evolution. We then assessed survival in the three groups and determined predictors of 30-day mortality. Group 1 (G1) had a mean age of 57 +/- 14 years and showed mean neutrophilia of 73.3%, Group 2 (G2) 61 +/- 13 years and 79.9%, and Group 3 (G3) 66 +/- 13 years and 84.2%. We compared outcomes and 30-day mortality between the groups. RESULTS: Mean age rose with increased neutrophil response. There were no statistically significant baseline differences between the groups except for more smokers in Groups 1 and 2, and more patients presenting with Killip class > or = 2 and fewer with uncomplicated evolution in Group 3. During 30-day follow-up there were 19 deaths (G1=1, G2=3 and G3=15). In univariate analysis mortality predictors were age > or = 75 years, anterior STEMI, maximum creatinine kinase > or = 2500 UI/L, culprit lesion in proximal anterior descending artery, incomplete revascularization, Killip > or = 2 at presentation, and being in G3. After multivariate regression analysis independent predictors were age > or = 75 years, incomplete revascularization and being in G3. CONCLUSION: In myocardial infarction patients undergoing mechanical revascularization, an intense neutrophil response (routinely, easily and inexpensively assessed) is related to worse short-term prognosis.

A Síndroma de Stevens-Johnson (SSJ), a Síndroma de Sobreposição (SS) e a Necrólise Tóxica Epidérmica (NTE) são emergências médicas raras, mas com elevadas morbilidade e mortalidade. A literatura referente às características destas doenças em Portugal é muito escassa. Procedeu-se à análise dos registos clínicos dos 20 doentes internados na Unidade de Queimados (UQ) do Hospital de São José nos últimos 15 anos com o diagnóstico de SSJ, SS ou NTE. A maior parte das toxidermias foi do tipo NTE (65%), seguida do SS (25%) e do SSJ (10%). A idade média foi 57,1 ± 19,0 anos. A duração média do internamento foi de 12,6 ± 7,8 dias. A mortalidade foi de 50%, sendo significativamente maior que os 16,4% de mortalidade global registada na UQ no mesmo período (p < 0,01). A área de superfície corporal total envolvida foi de 43,9 ± 28,6 %. O agente causal mais frequentemente implicado foi o alopurinol (35%), seguido da exposição à luz ultravioleta e metoxipsoraleno (15%). Catorze doentes (70%) foram tratados com corticóides nos primeiros dias de internamento, enquanto seis doentes (30%) foram tratados conservadoramente. A mortalidade foi menor nos doentes tratados com corticóides (42,8% vs 66,7%), embora esta diferença não fosse estatisticamente significativa. As taxas de infecção também não diferiram significativamente nos dois grupos. O SCORTEN nas primeiras 24 horas demonstrou ser um bom preditor de mortalidade. São necessários mais estudos para tentar reduzir a mortalidade nestas doenças.

INTRODUCTION: Carotid body tumours (CBT) are neoplasms that develop from paragangionic cells of this structure. They are rare, with an estimated incidence of 1:30000 and can be associated with other neuro-endocrine neoplasia. The authors report their experience in the management of the disease, in the last 10 years. MATERIAL AND METHODS: Eight patients (with eight tumours) were treated, all submitted to tumour resection. 75% were female and the mean age was 56 years. We report a 12,5% incidence of neurological sequelae from surgery, and no mortality. In the follow-up (which varied between 1 and 10 years), no local or contralateral recurrence or metastasis were registered. Also, we did not found family cases of this disease. CONCLUSIONS: The authors noticed an unusually high proportion of female patients. The tumour resection was curative in all patients, with a rate of neurological complications inferior to that reported in other published series. These neurological sequelae were reported in patients with large tumours, thus reinforcing the outmost importance of an early diagnosis. Pre-operative selective embolization of these tumours can be helpful in the resection of large tumours, allowing a potential reduction in neurological complications.

The Ross procedure has been used in children and young adults for aortic valve replacement and the correction of complex obstruction syndromes of the left ventricular outflow tract. We report the mid-term results of the Ross procedure in a single institution and performed by the same surgical team. Population: Between March 1999 and December 2005, 18 patients were operated on using the Ross procedure. The mean age at the time of surgery was 12 years, being 12 patients male (67%). The primary indication for surgery was isolated aortic valve disease, being the predominant abnormality in 58% of cases aortic regurgitation and in 42% left ventricular outflow tract obstruction. Associated lesions included sub-aortic membrane in 3 patients (16%), small VSD in 2 patients (11%), bicuspid aortic valve in 4 patients (22%) and severe left ventricular dysfunction and mitral valve regurgitation in 1 patient (6%). Ten of the 18 patients (56%) had been submitted to previous surgical procedures or percutaneous interventions. Results: Early post-operative mortality was not seen, but two patients (11%), had late deaths, one due to endocarditis, a year after the Ross procedure, and the other due to dilated cardiomiopathy and mitral regurgitation. The shortest time of follow-up is 6 months and the longest 72 months (median 38 months). Of the 16 survivors, 14 patients are in class I of the NYHA and 2 in class II, without significant residual lesions or need for re-intervention. The 12 patients with more than a year of follow up revealed normal coronary perfusion in all patients and no segmental wall motion abnormalities. Nevertheless, two of the 12 patients developed residual dynamic obstruction of LVOT and in three patients aortic regurgitation of a mild to moderate degree was evident. Significant gradients were not verified in the RVOT. Conclusions: The Ross procedure, despite its complexity, can be undertaken with excellent immediate results. Aspects such as the dilation of the neo aortic root and homograft evolution can not be considered in a study of this nature, seeing that the mean follow up time does not exceed 5 years.

Encontra-se bem estabelecido que, na abordagem terapêutica do enfarte agudo do miocárdio (EAM), a trombólise e a angioplastia coronária percutânea (ACTP) permitem diminuir a mortalidade e melhorar o prognóstico, sendo esse benefício tanto maior quanto menor o tempo decorrido entre o evento isquémico e o procedimento terapêutico. No entanto, não está ainda estabelecido qual o impacto que o atraso da implementação dessas terapêuticas tem no desenvolvimento de taquidisritmias ventriculares e por consequência no resultado da alternância da onda T(TWA), que representa um método reconhecido para avaliação da vulnerabilidade ventricular às referidas arritmias. Objectivo: Analisar os resultados da TWA por microvoltagem numa população submetida a ACTP na sequência de EAM e avaliar a influência do timing de revascularização miocárdica nos resultados da TWA. Métodos: Estudámos 79 doentes (67 do sexo masculino; 57 ± 11 anos) consecutivos, admitidos por EAM e sujeitos a revascularização miocárdica por ACTP durante o internamento. A TWA foi avaliada utilizando um aparelho da HeartTwave System (Cambridge Heart, Inc., Bedford, Massachusetts) nos 30 dias pós-EAM. Durante a realização de uma prova de esforço em tapete rolante com protocolo manual, com o objectivo de elevar a frequência cardíaca até aos 110 batimentos por minuto, realizou-se o registo electrocardiográfico através da aplicação de sete eléctrodos standard e outros sete eléctrodos sensores de alta-resolução, especialmente concebidos para redução do «ruído», dispostos segundo as derivações ortogonais de Frank(X,Y, Z). A TWA foi considerada positiva quando se verificou a presença de alternância da onda T de magnitude ³ 1,9 μV de forma consistente e mantida (> 1 minuto), com início para frequências cardíacas < 110 batimentos/minuto ou quando esta alternância se verificou em doentes em repouso. Foi considerada negativa quando se conseguiu obter dados sem artefactos durante pelo menos um minuto, com frequências > 105 batimentos/minuto, sem atingir critério de positividade e indeterminada se não podia ser classificada como positiva ou negativa. Foram excluídos doentes com EAM ou revascularização miocárdica (cirúrgica ou percutânea) prévios, os que apresentavam insuficiência cardíaca congestiva, com fibrilhação auricular, > 10 extrassístoles por minuto, pacemaker definitivo, bradicárdia < 40 bpm, complexos QRS com duração > 130 ms ou sob terapêutica anti-arrítmica. Considerámos como marcador de risco para a ocorrência de eventos arrítmicos ventriculares malignos a presença de TWA positiva ou indeterminada. (TWA «nãonegativa»). Os resultados da TWA foram comparados entre o grupo de doentes submetidos a ACTP nas primeiras 24 horas pós-EAM (Grupo A; n = 45) e o grupo de doentes submetidos a ACTP > 24 horas pós-EAM. (Grupo B; n = 34) Resultados: A TWA foi positiva em 16 doentes (20,2%) e negativa em 56 (70,9%). Em 7 casos (8,9%), o resultado do teste foi considerado indeterminado. A TWA foi «não-negativa» em 29,1% da população. No grupo A a TWA foi “não-negativa” em 9 doentes (20 %) (6 com TWA positiva e três com TWA indeterminada) e negativa em 36 doentes (80 %) e no grupo B foi «não-negativa» em 14 doentes (41%) (10 com TWA positiva e quatro com TWA indeterminada) e negativa em 20 (59 %) (p < 0,05). Não se encontraram diferenças entre os dois grupos no que respeita à fracção de ejecção ventricular esquerda. No seguimento até aos 60 dias após a alta hospitalar não foram documentados eventos arrítmicos ventriculares, síncopes ou óbito. Foram reinternados cinco doentes (7 %) por recorrênciade angor. Conclusões: Numa população de sobreviventes de EAM encontrámos uma prevalência de TWA não negativa de 29 %, apesar da revascularização miocárdica com ACTP. A ACTP, quando efectuada nas primeiras 24 horas após o início do EAM, reduz de forma significativa o número de doentes com TWA não negativa,sugerindo que esta intervenção precoce poderá baixar o risco arrítmico destes doentes e influenciar favoravelmente o prognóstico pós-EAM. O impacto da morte súbita na mortalidade pós-EAM justifica estudos prospectivos de maiores dimensões

INTRODUCTION: Peripheral embolism is frequently related to a cardiac source of embolism. Transesophageal echocardiography (TEE) is a useful tool for identifying such sources. OBJECTIVES: Our laboratory has gained wide experience in TEE, with a large number of exams performed to search for a cardiac source of embolism. We therefore thought it would be useful to present our experience in the last 12 years following the introduction of the technique. METHODS: This was a retrospective study of 1110 consecutive patients undergoing TEE to search for a cardiac source of embolism, after an embolic event and a transthoracic echocardiogram. RESULTS: The patients' mean age was 53 +/- 14 years, 52% male. There was peripheral embolism in 5% of cases and cerebral embolism in the remainder. The exam identified a potential embolic source in 35.6% of cases, the most frequent diagnoses being intracardiac shunt at the atrial level (9.5%), atrial septal aneurysm (ASA) (6.6%), intracardiac thrombi (6.4%) and atherosclerotic plaques in the thoracic aorta (9.6%). The presence of ASA was frequently associated with patent foramen ovale (27%), which was more frequent in younger patients. Overall, we identified a cardiac source of embolism more often in elderly patients, with a predominance of atherosclerotic plaques in the aorta. ETE was more frequently diagnostic in patients with peripheral embolism, but there were no differences in terms of etiology. CONCLUSIONS: TEE is very useful to search for cardiac sources of embolism, especially in younger patients, in whom causes potentially treatable surgically or percutaneously can be identified. In elderly patients, therapeutic strategy will probably not be changed by the findings (mostly thrombi and atherosclerotic plaques). The presence of ASA and embolic events makes it essential to perform a thorough search by TEE for intracardiac shunts, which are frequently associated.

INTRODUCTION: The use of drug-eluting stents in the context of mechanical reperfusion following ST-segment elevation myocardial infarction (MI) was initially viewed with concern. The main fear was that the drugs' action in unstable lesions could increase the risk of thrombotic stent occlusion. Furthermore, there was no evidence that the proven benefit of reduced instent restenosis could be extended to such patients, since they were excluded from the initial clinical trials. OBJECTIVES: To assess the safety and long-term clinical outcomes of the use of drug-eluting stents in primary angioplasty. METHODS: The first 100 consecutive and non-selected patients admitted for MI and treated by primary angioplasty with drug-eluting stent implantation in the target lesion were analyzed retrospectively. The efficacy and safety of the procedure, in-hospital clinical evolution and the occurrence of major adverse cardiac events in the first year were assessed. RESULTS: Patients' mean age was 58.2 +/- 11.5 years, and 78 were male. The success rate of primary angioplasty was 99%. Stents coated with sirolimus were used in 67 patients, paclitaxel in 19 and dexamethasone in 16. In-hospital mortality was 3%. The follow-up rate at 12 months was 98%. During this period, the rate of target vessel revascularization was 1% (with no patient requiring target lesion revascularization), MI 2%, and overall mortality 3.9%. Fourteen patients had clinical indication for repeat coronary angiography, which showed no significant in-stent restenosis. One event was considered to be due to acute stent thrombosis. The incidence of major adverse events was 5.9%. CONCLUSION: The use of drug-eluting stents in MI patients undergoing primary mechanical revascularization is safe and is associated with a reduced incidence of major adverse events, thrombosis and clinical restenosis at one year.

Percutaneous transluminal coronary angioplasty (PTCA) with stent placement is widely used to achieve myocardial revascularization in patients with symptomatic ischemic heart disease and significant coronary artery stenosis. Drug-eluting stents are used in most patients undergoing percutaneous angioplasty. Stent thrombosis is an uncommon but serious complication, manifested mostly by sudden death or acute ST-elevation myocardial infarction. The authors report the case of a 68-year-old patient with acute anterior ST-elevation myocardial infarction. Five years previously, she had had a similar presentation and underwent primary angioplasty of the left anterior descending artery with implantation of a drug-eluting stent. The patient was discharged under antithrombotic therapy. She discontinued antiplatelet therapy and two days later suffered an acute anterior myocardial infarction. Primary angioplasty revealed stent thrombosis.

Introdução: O tratamento cirúrgico mini-invasivo da incontinência urinária de esforço feminina com próteses suburetrais aplicadas por via transobturadora é consensualmente aceite na actualidade. O objectivo deste estudo é avaliar eficácia e segurança das próteses suburetrais transobturadoras a médio prazo, comparando a abordagem outside-in com a inside-out. Material e Métodos: Estudo retrospectivo de 298 doentes com diagnóstico de incontinência urinária de esforço submetidas entre 2003 e 2006 a cirurgia por via transobturadora. Destas doentes 113 mulheres realizaram a abordagem outside-in e 185 a abordagem insideout. Resultados: A média etária das doentes foi de 57.2 ± 10.3 anos, em que 69.1% se encontravam na menopausa. A paridade média foi de 2.2 ± 1.1. A técnica outside-in foi utilizada com maior frequência em associação com outra(s) cirurgia(s) do pavimento pélvico (83.2% versus 37.8% na técnica inside-out). O tempo médio de follow-up foi de 14.35 ± 13.75 meses nas doentes com prótese aplicada outside-in e de 11.79 ± 10.39 meses no grupo inside-out. Os resultados globais de eficácia obtidos foram idênticos nos dois grupos com taxas de cura e cura ou melhoria, respectivamente, de 76.9% e 92.9% no grupo outside-in e de 82.7% e 93.5% no grupo inside-out (diferença não significativa). A duração média da cirurgia para incontinência urinária de esforço isolada foi significativamente menor no grupo inside-out (14.77 ± 5.37 minutos versus 21.21 ± 7.48 minutos, p < 0.05). No pós-operatório a frequência de incontinência por imperiosidade de novo e de erosões da prótese foi idêntica em ambos as técnicas, contudo as erosões foram mais frequentes nas próteses microporosas do que nas macroporosas (p < 0.05). Discussão e conclusões: As próteses transobturadoras usadas no tratamento da I.U.E. são eficazes e seguras. As taxas de cura e melhoria são elevadas, com diferenças não significativas em função da técnica utilizada. A técnica inside-out associa-se a um significativo menor tempo cirúrgico.
