ABSTRACT: Neuropathy is a cause of significant disability in patients with Fabry disease, yet its diagnosis is difficult. In this study we compared the novel noninvasive techniques of corneal confocal microscopy (CCM) to quantify small-fiber pathology, and non-contact corneal esthesiometry (NCCA) to quantify loss of corneal sensation, with established tests of neuropathy in patients with Fabry disease. Ten heterozygous females with Fabry disease not on enzyme replacement therapy (ERT), 6 heterozygous females, 6 hemizygous males on ERT, and 14 age-matched, healthy volunteers underwent detailed quantification of neuropathic symptoms, neurological deficits, neurophysiology, quantitative sensory testing (QST), NCCA, and CCM. All patients with Fabry disease had significant neuropathic symptoms and an elevated Mainz score. Peroneal nerve amplitude was reduced in all patients and vibration perception threshold was elevated in both male and female patients on ERT. Cold sensation (CS) threshold was significantly reduced in both male and female patients on ERT (P < 0.02), but warm sensation (WS)and heat-induced pain (HIP) were only significantly increased in males onERT (P<0.01). However, corneal sensation assessed withNCCAwas significantly reduced in female (P < 0.02) and male (P < 0.04) patients on ERT compared with control subjects. According to CCM, corneal nerve fiber and branch density was significantly reduced in female (P < 0.03) and male (P < 0.02) patients on ERT compared with control subjects. Furthermore, the severity of neuropathic symptoms and the neurological component of the Mainz Severity Score Index correlated significantly with QSTand CCM. This study shows that CCM and NCCA provide a novel means to detect early nerve fiber damage and dysfunction, respectively, in patients with Fabry disease.

We study electronic transport across a helical edge state exposed to a uniform magnetic ((B) over right arrow) field over a finite length. We show that this system exhibits Fabry-Perot-type resonances in electronic transport. The intrinsic spin anisotropy of the helical edge states allows us to tune these resonances by changing the direction of the (B) over right arrow field while keeping its magnitude constant. This is in sharp contrast to the case of nonhelical one-dimensional electron gases with a parabolic dispersion, where similar resonances do appear in individual spin channels (up arrow and down arrow) separately which, however, cannot be tuned by merely changing the direction of the (B) over right arrow field. These resonances provide a unique way to probe the helical nature of the theory. We study the robustness of these resonances against a possible static impurity in the channel.

This paper presents the design and characterization of a fiber Fabry-Perot interferometer (FFPI) acoustic wave detector with its Q point being stabilized actively. The relationship between the reflectivity of the F-P cavity facets and cavity length was theoretically analyzed, and high visibility of 100% was realized by optimized design of the F-P cavity. To prevent the drifting of the Q point, a new stabilization method by actively feedback controlling of the diode laser is proposed and demonstrated, indicating the method is simple and easy operating. Measurement shows that good tracing of Q point was effectively realized. (c) 2008 Elsevier B.V. All rights reserved.

It is demonstrated with powerful evidence that the extraordinary transmission of a metallic grating is undoubtedly due to the excitation of standing surface plasma waves in the Fabry-Perot like resonator. This is the first time that the strong standing waves set up in the groove of a sub-wavelength double-layer grating (SWDG) for the surface plasma waves have been reported. Moreover, about 90% transmission is gained with an SWDG, more easily fabricated than ordinary metallic gratings, in the first peak of transmission spectrum.

O acesso ao tratamento da Doença de Fabry (DF) no sistema público de saúde nacional. Trata-se de uma pesquisa exploratória de corte transversal, centrada em elementos qualitativos, realizada com os profissionais e os pacientes portadores da DF, no Ambulatório de Genética Clínica do Hospital Universitário Gaffrée e Guinle - HUGG. Utilizou-se como coleta de dados a entrevista aberta e semiestruturada. Seu objetivo maior é identificar os aspectos bioéticos envolvidos no acesso ao tratamento da DF no SUS, e para tal buscamos tornar manifestos os argumentos morais dos profissionais do ambulatório, acerca da existência de uma política pública para o tratamento das doenças raras no SUS. A Bioética Principialista de Beauchamp e Childress, em seus princípios prima-facie: o respeito pela autonomia; a não maleficência; a beneficência e a justiça, é tomada como fundamentação teórica deste estudo. O tratamento dos dados se deu por meio do método de análise de conteúdo, de Bardin. A pesquisa contempla o percurso histórico das principais política públicas de saúde, e seus movimentos em direção à criação do SUS, e a integração dos hospitais universitários ao sistema público de saúde, em seus marcos legais. Ela também enfoca a mobilização da sociedade política e organizada em busca da materialização política pública de atenção às Doenças Raras. O estudo constatou que os princípios de Justiça e da Beneficência emergiram espontaneamente, e por vezes implicitamente, na fala do sujeitos da pesquisa, em suas justificativas morais para a criação de uma política pública para Doenças Raras. Ademais, é delineado o curso da doença na família, haja vista tratar-se de doença hereditária. Assinala-se de que modo a DF chegou do acaso a estas pessoas, e como estas chegaram ao diagnóstico e tratamento.
