Ocular mycoses are being reported with increasing frequency as a consequence of new medical practice and an increase in the number of immunosuppressed patients in the population, e.g.,, patients who are HIV positive and patients receiving radiation or chemotherapy. This article describes the ocular mycoses likely to be seen in such patients.

Objective: To establish risk factors for moderate and severe microbial keratitis among daily contact lens (CL) wearers in Australia. Design: A prospective, 12-month, population-based, case-control study. Participants: New cases of moderate and severe microbial keratitis in daily wear CL users presenting in Australia over a 12-month period were identified through surveillance of all ophthalmic practitioners. Case detection was augmented by record audits at major ophthalmic centers. Controls were users of daily wear CLs in the community identified using a national telephone survey. Testing: Cases and controls were interviewed by telephone to determine subject demographics and CL wear history. Multiple binary logistic regression was used to determine independent risk factors and univariate population attributable risk percentage (PAR%) was estimated for each risk factor.; Main Outcome Measures: Independent risk factors, relative risk (with 95% confidence intervals [CIs]), and PAR%. Results: There were 90 eligible moderate and severe cases related to daily wear of CLs reported during the study period. We identified 1090 community controls using daily wear CLs. Independent risk factors for moderate and severe keratitis while adjusting for age, gender, and lens material type included poor storage case hygiene 6.4× (95% CI, 1.9-21.8; PAR, 49%), infrequent storage case replacement 5.4× (95% CI, 1.5-18.9; PAR, 27%), solution type 7.2× (95% CI, 2.3-22.5; PAR, 35%), occasional overnight lens use (<1 night per week) 6.5× (95% CI, 1.3-31.7; PAR, 23%), high socioeconomic status 4.1× (95% CI, 1.2-14.4; PAR, 31%), and smoking 3.7× (95% CI, 1.1-12.8; PAR, 31%). Conclusions: Moderate and severe microbial keratitis associated with daily use of CLs was independently associated with factors likely to cause contamination of CL storage cases (frequency of storage case replacement, hygiene, and solution type). Other factors included occasional overnight use of CLs, smoking, and socioeconomic class. Disease load may be considerably reduced by attention to modifiable risk factors related to CL storage case practice.

Eye care practitioners (ECPs) would tend to agree that wearing contact lenses increases the risk for infection, but millions of patients are still fitted with lenses every year because ECPs feel that the risk is manageable and that their patients' eye health can be protected. The Fusarium and Acanthamoeba keratitis outbreaks of years past were a wake-up call to manufacturers, ECPs, and regulatory agencies that risk cannot be managed without diligence, and that the complex relationship between contact lens materials, contact lens solutions, and compliance needs to be better understood in order to optimize the efficacy of contact lens care and improve care guidelines.

OBJETIVO: Analisar o perfil dos usuários de lentes de contato entre os estudantes de medicina da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista - UNESP - Botucatu (SP), quanto à ametropia, à aquisição, aos cuidados e às complicações. MÉTODOS: Foi aplicado um questionário aos alunos do 1º ao 6º ano de Medicina da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista - UNESP - Botucatu (SP), perfazendo um estudo analítico transversal. RESULTADOS: Foram 566 entrevistados. Destes, 155 (27,4%) usavam lente de contato. Das lentes de contato usadas, 94,8% eram gelatinosas, e em 98,7% o grau era negativo. O descarte dessas lentes era mensal em 46,9% dos entrevistados e 63,9% a utilizavam, em média, 14 horas/dia. Faziam aquisição da lente de contato em óptica ou farmácia 14,1% dos estudantes. Faziam a higiene ao colocá-las e ao retirá-las dos olhos, 46,5%. As complicações referentes aos olhos vermelhos eram 51,0%; 44,5%, embaçamento; e 42,6%, sensação de corpo estranho, além de 4 estudantes relatarem ceratite. CONCLUSÕES: A maioria dos alunos utilizava lentes hidrofílicas, descarte mensal e grau negativo. A adaptação inadequada, aquisição de lentes de contato em ópticas ou farmácias e complicações graves ocorreram, mesmo nos usuários socioculturalmente considerados diferenciados. em vista disso, demanda a extrema necessidade de maior atenção na abordagem do tema em questão, a fim de prevenir o comprometimento visual permanente.

Importance of the field: The use of topical agents poses unique and challenging hurdles for drug delivery. Topical steroids effectively control ocular inflammation, but are associated with the well-recognized dilemma of patient compliance. Although administration of topical antimicrobials as prophylaxis is acceptable among ophthalmologists, this common practice has no sound evidence base Developing a new antimicrobial agent or delivery strategy with enhanced penetration by considering the anatomical and physiological constraints exerted by the barriers of the eye is not a commonly perceived strategy. Exploiting the permeability of the sclera, subconjunctival routes may offer a promising alternative for enhanced drug delivery and tissue targeting.Area covered in this review: Ocular drug delivery strategies were reviewed for ocular inflammation and infections clinically adopted for newer class of antimicrobials, which use a multipronged approach to limit risks of endophthalmitis.What the reader will gain: The analysis substantiates a new transscleral drug delivery therapeutic approach for cataract surgery.Take home message: A new anti-inflammatory and anti-infective paradigm that frees the patient from the nuisance of topical therapeutics is introduced, opening a large investigative avenue for future improved therapies.

The contact lens industry has evolved and now provides many choices, including continuous wear, overnight orthokeratology, frequent-replacement lenses, daily-disposable lenses, and many alternatives in systems of care and maintenance. Epidemiologic studies to date have shown that how a lens is worn, particularly if worn overnight, can increase the risk of microbial keratitis. However, the risk of silicone hydrogel contact lenses worn on a continuous-wear basis has been evaluated only recently. This article summarizes the recent research data on extended-wear silicone hydrogel lenses and discusses the challenges of early evaluations of silicone hydrogel lens safety. Finally, the relevance of this information is discussed to practitioners and contact lens wearers making choices about the risks and benefits of different products and how they are used.

Most women acquire genital high risk human papillomavirus (HPV) infection during their lifetime, but seldom the infection persists and leads to cervical cancer. However, currently it is not possible to identify the women who will develop HPV mediated cervical cancer and this often results to large scale follow-up and overtreatment of the likely spontaneously regressing infection. Thus, it is important to obtain more information on the course of HPV and find markers that could help to identify HPV infected women in risk for progression of cervical lesions and ultimately cancer. Nitric oxide is a free radical gas that takes part both in immune responses and carcinogenesis. Nitric oxide is produced also by cervical cells and therefore, it is possible that cervical nitric oxide could affect also HPV infection. In the present study, including 801 women from the University of Helsinki between years of 2006 and 2011, association between HPV and cervical nitric oxide was evaluated. The levels of nitric oxide were measured as its metabolites nitrate and nitirite (NOx) by spectrophotometry and the expression of nitric oxide producing enzymes endothelial and inducible synthases (eNOS, iNOS) by Western blotting. Women infected with HPV had two-times higher cervical fluid NOx levels compared with non-infected ones. The expression levels of both eNOS and iNOS were higher in HPV-infected women compared with non-infected. Another sexually transmitted disease Chlamydia trachomatis that is an independent risk factor for cervical cancer was also accompanied with elevated NOx levels, whereas vaginal infections, bacterial vaginosis and candida, did not have any effect on NOx levels. The meaning of the elevated HPV related cervical nitric oxide was evaluated in a 12 months follow-up study. It was revealed that high baseline cervical fluid NOx levels favored HPV persistence with OR 4.1. However, low sensitivity (33%) and high false negative rate (67%) restrict the clinical use of the current NOx test. This study indicated that nitric oxide favors HPV persistence and thus it seems to be one of the cofactor associated with a risk of carcinogenesis.

Depuis quelques années, il y a émergence de souches de Salmonella enterica sérovar Typhimurium multirésistantes causant une septicémie et la mort chez le porc. Ceci constitue un problème majeur pour l’industrie porcine et possiblement pour la santé publique. L’objectif de ce projet était de comparer et de caractériser une souche capable de causer une septicémie chez le porc et une souche commensale, en observant l’interaction avec des cellules épithéliales, des macrophages humains et d’identifier des gènes exprimés par les souches septicémiques et les souches commensales. Tout d’abord, l’infection de cellules épithéliales permet d’observer l’adhérence et l’invasion des bactéries, pour ainsi mettre en évidence la capacité des souches à coloniser le tractus gastro-intestinal. La souche commensale possède un pouvoir d’adhésion supérieur à la souche septicémique. Par la suite, l’infection de macrophages permet de caractériser le niveau de phagocytose et de survie. L’importance de la survie dans les macrophages pourrait permettre de faire un lien avec la septicémie. Toutefois, aucune différence n’est observable dans les conditions qui ont été testé. Ensuite, la technique SCOTS (Selective Capture of Transcribed Sequences) est utilisée pour capturer des gènes uniques à la souche septicémique et un autre SCOTS est fait pour capturer les gènes spécifiques à la souche commensale. Finalement, les gènes sont clonés, leur spécificité face aux souches est analysé par dot blot et ils sont identifiés par séquençage suivient d’une analyses bioinformatiques. Les gènes identifiés par SCOTS, lors des captures pour la souche septicémique et la souche commensale, se trouvent à être des gènes communs aux Salmonella. Toutefois, la différence de pathologie causée par les deux souches, n’est peut-être pas l’acquisition de nouveaux gènes, mais plutôt une différence d’expression entre les deux souches.

Background and Objective: This study evaluated the prevalence and the molecular diversity of Archaea in the subgingival biofilm samples of subjects with peri-implantitis. Material and Methods: Fifty subjects were assigned into two groups: Control (n = 25), consisting of subjects with healthy implants; and Test (n = 25), consisting of subjects with peri-implantitis sites, as well as a healthy implant. In the Test group, subgingival biofilm samples were taken from the deepest sites of the diseased implant. In both groups, subgingival biofilm was collected from one site with a healthy implant and from one site with a periodontally healthy tooth. DNA was extracted and the 16S ribosomal RNA gene was amplified with universal primer pairs for Archaea. Amplified genes were cloned and sequenced, and the phylotypes were identified by comparison with known 16S ribosomal RNA sequences. Results: In the Control group, Archaea were detected in two and three sites of the implant and the tooth, respectively. In the Test group, Archaea were detected in 12, 4 and 2 sites of diseased implants, healthy implants and teeth, respectively. Diseased implants presented a significantly higher prevalence of Archaea in comparison with healthy implants and natural teeth, irrespective of group. Over 90% of the clone libraries were formed by Methanobrevibacter oralis, which was detected in both groups. Methanobacterium congelense/curvum was detected in four subjects from the Test group and in two subjects from the Control group. Conclusion: Although M. oralis was the main species of Archaea associated with both healthy and diseased implant sites, the data indicated an increased prevalence of Archaea in peri-implantitis sites, and their role in pathogenesis should be further investigated.

A canaliculite é uma afecção rara da via lacrimal. Os autores apresentam uma série de cinco casos tratados na Faculdade de Medicina de Botucatu-UNESP, comentam os achados à luz da literatura pertinente e tecem considerações sobre o tratamento efetuado, ressaltando que a canaliculotomia pode ter bons resultados mesmo que o canalículo não receba suturas ou moldes.

The polysaccharides beta-glucans occur as a principal component of the cellular walls. Some microorganisms, such as yeast and mushrooms, and also cereals such as oats and barley, are of economic interest because they contain large amounts of beta-glucans. These substances stimulate the immune system, modulating humoral and cellular immunity, and thereby have beneficial effect in fighting infections (bacterial, viral, fungal and parasitic). beta-Glucans also exhibit hypocholesterolemic and anticoagulant properties. Recently, they have been demonstrated to be anti-cytotoxic, antimutagenic and anti-tumorogenic, making them promising candidate as pharmacological promoters of health. (C) 2008 Elsevier B.V. The purpose of this study was to report a case of external ophthalmomyiasis caused by Cochliomyia hominivorax in a 10 years-old child, which presented acute preseptal celulitis and had 17 worms removed from the lacrimal canaliculus and conjunctival cul de sac, bilaterally, with resolution. The authors discuss the diagnosis and treatment of this nosological entity.
