In the last decade, population ageing has been registered as a global phenomenon. A relation exists between falling and ageing, since falling frequency increases significantly with age. In fact, one in three older adult falls annually. Although ageing is generically associated with decrease and degeneration of psychological and physical functions, it is still not common for the correct identification of risk factors to lead to a clinical prognosis of the elder being in risk of falling. Therefore, the goal of this review article is to identify, categorise and analyse typical ageing and fall factors mentioned in the literature as well as to quantify the number of times they were referenced. The research considered hundreds of publications, but analysis was then restricted to the 87 most pertinent articles written in English and published in journals or scientific magazines between 1995 and 2010. We concluded that falls among older adults can be characterised by the following: anatomic characteristics and physiological consequences of ageing; the pathologies that induce falls, which can be neurological, musculoskeletal, cardiovascular and other diseases; causes and risk factors of falls that can be behavioural, biological, environmental or socio-economic; type of physical consequences of falls, including fractures, bruises, injuries or other physical consequences; and strategies to prevent, mitigate or rehabilitate, which can be of a physical, environmental or behavioural nature.

Objective: To determine the accuracy of the Timed Up and Go Test (TUGT) for screening the risk of falls among community-dwelling elderly individuals. Method: This is a prospective cohort study with a randomly by lots without reposition sample stratified by proportional partition in relation to gender involving 63 community-dwelling elderly individuals. Elderly individuals who reported having Parkinson's disease, a history of transitory ischemic attack, stroke and with a Mini Mental State Exam lower than the expected for the education level, were on a wheelchair and that reported a single fall in the previous six months were excluded. The TUGT, a mobility test, was the measure of interested and the occurrence of falls was the outcome. The performance of basic activities of daily living (ADL) and instrumental activities of daily living (IADL) was determined through the Older American Resources and Services, and the socio-demographic and clinical data were determined through the use of additional questionnaires. Receiver Operating Characteristic Curves were used to analyze the sensitivity and specificity of the TUGT. Results: Elderly individuals who fell had greater difficulties in ADL and IADL (p<0.01) and a slower performance on the TUGT (p=0.02). No differences were found in socio-demographic and clinical characteristics between fallers and non- fallers. Considering the different sensitivity and specificity, the best predictive value for discriminating elderly individuals who fell was 12.47 seconds [(RR= 3.2) 95% CI: 1.3- 7.7]. Conclusions: The TUGT proved to be an accurate measure for screening the risk of falls among elderly individuals. Although different from that reported in the international literature, the 12.47 second cutoff point seems to be a better predictive value for Brazilian elderly individuals.

Introdução: As quedas são um dos mais sérios problemas de saúde pública associado à idade. Objectivo: Analisar a influência de um programa de exercícios específicos, realizado no domicílio, em idosos, na força muscular dos membros inferiores, no equilíbrio, no medo de cair e na autonomia. Métodos: Foi realizado um estudo quase-experimental, com a duração de 3 meses, em dois grupos de mulheres idosas, o grupo experimental (n=6), que foi submetido ao programa de exercícios e o grupo controlo (n=6), que apenas recebia visitas mensais para ser submetido às avaliações. Para avaliar os factores de risco de queda foram utilizados o Five-Times-Sit-to-Stand-Test para avaliar a força muscular dos membros inferiores, o Functional Reach Test para avaliar o equilíbrio, a Falls Efficacy Scale para avaliar o medo de cair e a Medida de Independência Funcional para avaliar a autonomia das participantes. Resultados: Verificou-se uma melhor prestação do grupo experimental na realização do FTSST, conseguindo melhorar cerca de 9 segundos em média ao longo do estudo. Os resultados obtidos através do FRT demonstraram uma melhoria da média e mediana entre o 1º e 2º momento de avaliação, nos dois grupos, piorando progressivamente após esse momento. Inicialmente, o número de participantes com medo de cair era superior no grupo experimental (66,7%), sendo apenas 33% no final do estudo, verificando-se um aumento na média das pontuações no grupo experimental até ao 3º momento de avaliação,. Através dos resultados da MIF verificou-se que os dois grupos se encontravam na classificação de independência completa, não se tendo observado influência do programa de exercícios na independência das idosas. Conclusão: O exercício físico específico melhorou a força muscular dos membros inferiores e o medo de cair.

Les chutes chez les personnes âgées représentent un problème important de santé publique. Des études montrent qu’environ 30 % des personnes âgées de 65 ans et plus chutent chaque année au Canada, entraînant des conséquences néfastes sur les plans individuel, familiale et sociale. Face à une telle situation la vidéosurveillance est une solution efficace assurant la sécurité de ces personnes. À ce jour de nombreux systèmes d’assistance de services à la personne existent. Ces dispositifs permettent à la personne âgée de vivre chez elle tout en assurant sa sécurité par le port d'un capteur. Cependant le port du capteur en permanence par le sujet est peu confortable et contraignant. C'est pourquoi la recherche s’est récemment intéressée à l’utilisation de caméras au lieu de capteurs portables. Le but de ce projet est de démontrer que l'utilisation d'un dispositif de vidéosurveillance peut contribuer à la réduction de ce fléau. Dans ce document nous présentons une approche de détection automatique de chute, basée sur une méthode de suivi 3D du sujet en utilisant une caméra de profondeur (Kinect de Microsoft) positionnée à la verticale du sol. Ce suivi est réalisé en utilisant la silhouette extraite en temps réel avec une approche robuste d’extraction de fond 3D basée sur la variation de profondeur des pixels dans la scène. Cette méthode se fondera sur une initialisation par une capture de la scène sans aucun sujet. Une fois la silhouette extraite, les 10% de la silhouette correspondant à la zone la plus haute de la silhouette (la plus proche de l'objectif de la Kinect) sera analysée en temps réel selon la vitesse et la position de son centre de gravité. Ces critères permettront donc après analyse de détecter la chute, puis d'émettre un signal (courrier ou texto) vers l'individu ou à l’autorité en charge de la personne âgée. Cette méthode a été validée à l’aide de plusieurs vidéos de chutes simulées par un cascadeur. La position de la caméra et son information de profondeur réduisent de façon considérable les risques de fausses alarmes de chute. Positionnée verticalement au sol, la caméra permet donc d'analyser la scène et surtout de procéder au suivi de la silhouette sans occultation majeure, qui conduisent dans certains cas à des fausses alertes. En outre les différents critères de détection de chute, sont des caractéristiques fiables pour différencier la chute d'une personne, d'un accroupissement ou d'une position assise. Néanmoins l'angle de vue de la caméra demeure un problème car il n'est pas assez grand pour couvrir une surface conséquente. Une solution à ce dilemme serait de fixer une lentille sur l'objectif de la Kinect permettant l’élargissement de la zone surveillée.

Syftet med studien var att undersöka hur fallfrekvensen var fördelad mellan män och kvinnor, hur antalet fall var fördelade i åldersgrupperna över respektive under 80 år, när de flesta fallolyckor i det egna hemmet inträffade och om fallen medförde några konsekvenser.Studien var en retrospektiv registerstudie och inkluderade fall hos personer i eget boende med trygghetslarm och/eller hjälp av nattpatrull. Fallolyckorna registrerades på ett instrument som var speciellt utvecklat för projektet. Data från en sex månaders studie med totalt 510 fall av 213 individer analyserades. Resultatet visade att det var främst äldre personer över 80 år som föll och som hamnade på sjukhus. Fallen inträffade oftast förmiddagar och eftermiddagar. Få fall inträffade mellan klockan 00-09. Kvinnorna föll framförallt under december månad. Antal fall som ledde till sjukhusvård var 4.5 % och av dessa var nästan alla individer över 80 år och flertalet var kvinnor. Av de fall som ledde till sjukhusvård inträffade 39 % vid första och enda fallet. Då många vårdtagare föll ett flertal gånger och antalet fall kunde vara mycket högt bör de fallförebyggande åtgärderna ses över. Downton fallriskindex skulle kunna vara ett komplement till övriga fallpreventioner i hemmet. Studien ger användbar data med möjlighet att till exempel utforma hemtjänstens schema utifrån studieresultatet och på så sätt minska antalet fallolyckor i hemmen.

Introduction: Falls among older adults is a public health problem, therefore it is necessary preventive actions, however the adherence is the major problem faced by practitioners and researchers working on falls prevention programs. Objective: To evaluate the variables related to the adherence to falls prevention programs among the elderly enrolled in a Basic Health Unit (BHU). Methods: Was performed an observational cross-sectional analytical study. All elderly registered in a BHU and able to ambulate independently were invited to participate in a falls prevent program. The Elderly who Adhered to the Program (EAP) were evaluated at BHU; and the Elderly Not Adhered to the Program (ENAP) were identified and assessed at home. The assessment for both groups was performed using an evaluation form containing personal data, measures and clinical scales to assess cognitive status, balance, mobility, fear of falling, handgrip strength. Data were analyzed with SPSS 20.0. In addition to this assessment, the ENAP underwent a semi structured interview, in which we used the qualitative approach based on the figure of the Collective Subject Discourse. Results: The study included 222 elderly, 111 EAP and 111ENAP, most aged between 70 and 79 years (48.2%), female (68.5%), married (52.3%) and illiterate (47.7%). Consolidated as protective factors for adherence, worst rates of physical activity (p = 0.001), balance (p = 0.010) and cognition (p = 0.007). The interview of ENAP identified two themes: "Local implementation of programs for the prevention of falls" and "Relationship between BHU and the elderly health care," and found that the elderly who did not adhere were unable to displace and did not mention that primary care programs are related to health care in elderly. Conclusions: Elderly who do not adhere to the program differ from elderly who adhere as worst indices of cognition, balance and physical activity which implies greater risk of falling; and they were unable to participate in falls prevention program and by to be caregiver and showed displacement difficult

Objective: Investigate the influence of apprehensive gait on activation and cocontraction of lower limb muscles of younger and older female adults. Methods: Data of 17 younger (21.47±2.06 yr) and 18 older women (65.33±3.14. yr) were considered for this study. Participants walked on the treadmill at two different conditions: normal gait and apprehensive gait. The surface electromyographic signals (EMG) were recorded during both conditions on: rectus femoris (RF), vastus lateralis (VL), vastus medialis (VM), biceps femoris (BF), tibialis anterior (TA), gastrocnemius lateralis (GL), and soleus (SO). Results: Apprehensive gait promoted greater activation of thigh muscles than normal gait (F=5.34 and p=0.007, for significant main effect of condition; RF, p=0.002; VM, p<0.001; VL, p=0.003; and BF, p=0.001). Older adults had greater cocontraction of knee and ankle stabilizer muscles than younger women (F=4.05 and p=0.019, for significant main effect of groups; VM/BF, p=0.010; TA/GL, p=0.007; and TA/SO, p=0.002). Conclusion: Apprehensive gait promoted greater activation of thigh muscles and older adults had greater cocontraction of knee and ankle stabilizer muscles. Thus, apprehensive gait may leads to increased percentage of neuromuscular capacity, which is associated with greater cocontraction and contribute to the onset of fatigue and increased risk of falling in older people. © 2013 Elsevier Ltd.

Introdução: Estudos recentes têm mostrado que as quedas são a causa externa de morte mais importante entre idosos, podendo levar a hospitalização, lesões, dependência e aumento nos custos dos serviços sociais e de saúde. O comprometimento da mobilidade funcional é um importante fator de risco para quedas, mas aspectos sociais, ambientais e comportamentais também podem influenciar nesse evento. Objetivo: Identificar os aspectos socioeconômicos e contextuais associados com a mobilidade funcional e quedas em idosos residentes no município de São Paulo. Métodos: Foram utilizados os dados do Estudo Saúde, Bem-Estar e Envelhecimento (SABE), uma amostra representativa para os indivíduos com idade igual ou superior a 60 anos do município de São Paulo, em 2010. As variáveis dependentes do estudo foram a ocorrência de alguma queda no último ano e o comprometimento da mobilidade funcional, mensurada pelo teste Timed Up and Go (TUG). Fatores individuais (estado marital, raça/cor, anos de estudo e percepção de suficiência de renda) e contextuais (Índice de Gini, área verde/ habitante, taxa de homicídio e percentual de domicílios em favelas) foram analisados por modelos logísticos multiníveis. Resultados: De 1.190 idosos inclusos, 29 por cento relataram ter caído no último ano e 46 por cento apresentaram comprometimento da mobilidade funcional. Os fatores individuais socioeconômicos não apresentaram associação com a ocorrência de queda, mas ter 8 anos ou mais de anos de estudo foi um fator protetor para comprometimento da mobilidade em todos os modelos testados (OR: 0,56). Morar em subprefeituras com taxa de homicídio moderada apresentou associação com chance aumentada de cair (OR: 1.51, 95 por cento IC: 1.09-2.07). Moderada área verde se associou com maior chance de cair entre os indivíduos com 80 anos e mais (OR:2,63, 95 por cento IC: 1.23-5.60). Conclusão: Os resultados estão de acordo com a literatura em relação à associação das características do bairro de residência com quedas e mobilidade funcional em idosos. Estratégias voltadas para prevenção de quedas e de dificuldade na mobilidade funcional devem considerar aspectos sociais e ambientais de locais públicos. The aim of the present study was to identify factors associated with the occurrence of falls among elderly adults in a population-based study (ISACamp 2008). A population-based cross-sectional study was carried out with two-stage cluster sampling. The sample was composed of 1,520 elderly adults living in the urban area of the city of Campinas, São Paulo, Brazil. The occurrence of falls was analyzed based on reports of the main accident occurred in the previous 12 months. Data on socioeconomic/demographic factors and adverse health conditions were tested for possible associations with the outcome. Prevalence ratios (PR) were estimated and adjusted for gender and age using the Poisson multiple regression analysis. Falls were more frequent, after adjustment for gender and age, among female elderly participants (PR = 2.39; 95% confidence interval (95% CI) 1.47 - 3.87), elderly adults (80 years old and older) (PR = 2.50; 95% CI 1.61 - 3.88), widowed (PR = 1.74; 95% CI 1.04 - 2.89) and among elderly adults who had rheumatism/arthritis/arthrosis (PR = 1.58; 95% CI 1.00 - 2.48), osteoporosis (PR = 1.71; 95% CI 1.18 - 2.49), asthma/bronchitis/emphysema (PR = 1,73; 95% CI 1.09 - 2.74), headache (PR = 1.59; 95% CI 1.07 - 2.38), mental common disorder (PR = 1.72; 95% CI 1.12 - 2.64), dizziness (PR = 2.82; 95% CI 1.98 - 4.02), insomnia (PR = 1.75; 95% CI 1.16 - 2.65), use of multiple medications (five or more) (PR = 2.50; 95% CI 1.12 - 5.56) and use of cane/walker (PR = 2.16; 95% CI 1.19 - 3,93). The present study shows segments of the elderly population who are more prone to falls through the identification of factors associated with this outcome. The findings can contribute to the planning of public health policies and programs addressed to the prevention of falls.
