INTRODUCTION: Previous cross-sectional studies have shown a high prevalence of chronic disease and disability among the elderly. Given Brazil’s rapid aging process and the obvious consequences of the growing number of old people with chronic diseases and associated disabilities for the provision of health services, a need was felt for a study that would overcome the limitations of cross-sectional data and shed some light on the main factors determining whether a person will live longer and free of disabling diseases, the so-called successful aging. The methodology of the first follow-up study of elderly residents in Brazil is presented. METHOD: The profile of the initial cohort is compared with previous cross-sectional data and an in-depth analysis of nonresponse is carried out in order to assess the validity of future longitudinal analysis. The EPIDOSO (‘Epidemiologia do Idoso’) Study conducted a two-year follow-up of 1,667 elderly people (65+), living in S. Paulo. The study consisted of two waves, each consisting of household, clinical, and biochemical surveys. RESULTS AND CONCLUSIONS: In general, the initial cohort showed a similar profile to previous cross-sectional samples in S. Paulo. There was a majority of women, mostly widows, living in multigenerational households, and a high prevalence of chronic illnesses, psychiatric disturbances, and physical disabilities. Despite all the difficulties inherent in follow-up studies, there was a fairly low rate of nonresponse to the household survey after two years, which did not actually affect the representation of the cohort at the final household assessment, making unbiased longitudinal analysis possible. Concerning the clinical and blood sampling surveys, the respondents tended to be younger and less disabled than the nonrespondents, limiting the use of the clinical and laboratory data to longitudinal analysis aimed at a healthier cohort. It is worth mentioning that gender, education, family support, and socioeconomic status were not important determinants of nonresponse, as is often the case.

ABSTRACT Objective The use of benzodiazepines (BZD) and associated factors in the elderly of Dourados, MS, Brazil was investigated. Methods This is a cross-sectional population-based study using a multistage sampling design. Participants were 1,022 individuals aged ≥ 60 years. Measurements socioeconomic status, schooling, demographic aspects, marital status, physical activity, hospitalizations, self-rated health, use of alcohol beverages and smoking were assessed by standard questionnaire. The drugs used were obtained by checking the prescription or package and classified according to the Anatomical Therapeutic Chemical Index. Chi-square test, Chi-square for linear tendency, Fisher's exact test and Poisson regression were used to assess association between variables. Results The prevalence of use of BZD was 6.5% (CI95%: 5.1-8.2%). The oldest group (PR: 1.60 for 70-79 years and PR: 1.79 for ≥ 80 years), non-use of alcohol (PR: 4.14) and use of antidepressants (PR: 8.73) were the statistically associated characteristics. Conclusions The prevalence of use of BZD in the study population was low, similar to findings of more recent Brazilian studies and lower than those obtained in international studies. The prevalence of use of BZD increased with aging and use of antidepressants.

Relaxation of the upper age limits for solid organ transplantation coupled with improvements in post-transplant survival have resulted in greater numbers of elderly patients receiving immunosuppressant drugs such as tacrolimus. Tacrolimus is a potent agent with a narrow therapeutic window and large inter- and intraindividual pharmacokinetic variability. Numerous physiological changes occur with aging that could potentially affect the pharmacokinetics of tacrolimus and, hence, patient dosage requirements. Tacrolimus is primarily metabolised by cytochrome P450 (CYP) 3A enzymes in the gut wall and liver. It is also a substrate for P-glycoprotein, which counter-transports diffused tacrolimus out of intestinal cells and back into the gut lumen. Age-associated alterations in CYP3A and P-glycoprotein expression and/or activity, along with liver mass and body composition changes, would be expected to affect the pharmacokinetics of tacrolimus in the elderly. However, interindividual variation in these processes may mask any changes caused by aging. More investigation is needed into the impact aging has on CYP and P-glycoprotein activity and expression. No single-dose, intense blood-sampling study has specifically compared the pharmacokinetics of tacrolimus across different patient age groups. However, five population pharmacokinetic studies, one in kidney, one in bone marrow and three in liver transplant recipients, have investigated age as a co-variate. None found a significant influence for age on tacrolimus bioavailability, volume of distribution or clearance. The number of elderly patients included in each study, however, was not documented and may have been only small. It is likely that inter- and intraindividual pharmacokinetic variability associated with tacrolimus increase in elderly populations. In addition to pharmacokinetic differences, donor organ viability, multiple co-morbidity, polypharmacy and immunological changes need to be considered when using tacrolimus in the elderly. Aging is associated with decreased immunoresponsiveness, a slower body repair process and increased drug adverse effects. Elderly liver and kidney transplant recipients are more likely to develop new-onset diabetes mellitus than younger patients. Elderly transplant recipients exhibit higher mortality from infectious and cardiovascular causes than younger patients but may be less likely to develop acute rejection. Elderly kidney recipients have a higher potential for chronic allograft nephropathy, and a single rejection episode can be more devastating. There is a paucity of information on optimal tacrolimus dosage and target trough concentration in the elderly. The therapeutic window for tacrolimus concentrations may be narrower. Further integrated pharmacokinetic-pharmaco-dynamic studies of tacrolimus are required. It would appear reasonable, based on current knowledge, to commence tacrolimus at similar doses as those used in younger patients. Maintenance dose requirements over the longer term may be lower in the elderly, but the increased variability in kinetics and the variety of factors that impact on dosage suggest that patient care needs to be based around more frequent monitoring in this age group.

Introduction: Life expectancy is increasing and becoming a characteristic phenomenon of developed countries and, increasingly, of developing countries, such as Brazil. The aging process causes changes of some physiological functions such as loss of smell, taste, loss of appetite, among other things that end up changing the food intake of these individuals. Objectives: This study aimed to assess food consumption of the young and long-lived elderly in a city in southern Brazil. Methods: A cross-sectional survey conducted through home visits in Palmeira das Missões - RS, Brazil. The sociodemographic, anthropometrical and dietary data were collected through questionnaires and 24-hour recall. The adequacy of nutrients was assessed according to the Dietary Reference Intakes. Data were analyzed using SPSS 18.0 software. Results: The study included 424 older adults, 84,4% (n = 358) aged less than 80 years old and 15,6% (n = 66) older than 80. The intake of energy and protein was insufficient for both young elderly and the oldest. The consumption of vitamins and minerals has been insufficient in all seniors except for iron, which presented an excessive intake. There was a statistically significant difference between the elderly and oldest only for the consumption of lipids and vitamin B12. Conclusion: The majority of studies with elderly corroborate the results found in this article. An inadequate intake of nutrients can develop nutritional deficiencies, and consequently it can result in physiological and pathological changes which would compromise the functional capacity of the elderly. Energy consumption was insufficient and macronutrients were inadequate, both for the young elderly as for the oldest. Additionally, the consumption of vitamins and minerals was insufficient to everyone except the iron, which presented excessive intake for young and oldest elderly.

Introducción: La enfermedad cardiovascular es la principal causa de muerte a nivel mundial, afectando principalmente la salud pública de países pobres con economías emergentes. La transición epidemiológica en Colombia ha incrementado la proporción de pacientes ancianos con enfermedad cardiovascular y que requieren cirugía cardíaca. Sin embargo, no existe consenso sobre la conducta para la selección de pacientes añosos para este tipo de intervenciones. El objetivo de este estudio fue definir el riesgo mortalidad asociado a cirugía cardíaca en este grupo de pacientes, basados en una revisión sistemática de la literatura. Materiales y Métodos: Se diseñó una revisión sistemática empleando las plataformas PubMed (Medline), EBSCO Discovery Service, Ovid SP-EBMR, Sciverse y MDConsult. Los términos de búsqueda fueron “Aged”, “Cardiac surgery” and “Mortality”, conjugados de acuerdo con el lenguaje de cada buscador. Las publicaciones fueron seleccionadas por consenso. Los resultados se analizaron en un modelo de Mantel-Haenszel. Resultados: La búsqueda arrojó un total de 8.565 publicaciones. Los datos analizados en el modelo incluyeron 81.547 pacientes (7.855 octogenarios y 73.692 más jóvenes). El riesgo de mortalidad asociado a cirugía cardíaca en octogenarios fue de 125% (OR=2,35, IC 95% [2,15 - 2,57]). Discusión: El sometimiento de pacientes octogenarios a cirugías cardíacas mayores es una decisión que requiere un juicio clínico minucioso en el que es importante destacar que la probabilidad de un resultado francamente desfavorable es alta. Se necesitan más estudios diseñados que permitan aumentar la solidez de la evidencia actual en cuanto al riesgo aquí encontrado.

Fruto das alterações demográficas nas últimas décadas, verifica-se um duplo envelhecimento: na base e no topo, advindo daí a necessidade de respostas sociais. Os Centros de Dia são estruturas semi-institucionalizadas que surgem enquadradas numa política de 'envelhecer em casa' sendo, por isso, uma verdadeira alternativa ao internamento permanente. O objectivo geral deste estudo passa por avaliar, na perspectiva do utente, a dinâmica institucional dos Centros de Dia e o seu contributo para a melhoria da sua qualidade de vida. O estudo é sustentando por uma amostra de 48 utentes de IPSS's e pela análise dos níveis gerais de satisfação, serviços disponibilizados, actividades propostas e graus de participação dos utentes. Como instrumento de recolha de dados, usou-se uma versão adaptada do 'Questionário do Utente', do protocolo de avaliação de Centros de Dia. Os resultados apontam para respostas favoráveis a uma boa socialização, muito próximas da concordância na autonomia e bom nível de satisfação relativamente à satisfação global. Nas actividades de vida diária, exceptuando o tomar banho, todos os outros itens obtiveram resposta ‘sem ajuda’. Relativamente à intensidade de utilização dos Centros de Dia registámos um valor elevado. O principal motivo de adesão ao programa é a necessidade de companhia.

Demographic and epidemiological transformations have led to an increase in elderly populations in the world, and chronic diseases become the main health problem in this population, with consequences for the independence and autonomy, and interfering in the lifestyle and daily activities, and may decrease the welfare and quality of life. So, there is an urgent need for multidisciplinary research on the quality of life, understood as a multidimensional and subjective concept, as well as the associated factors, such as health habits, presence of chronic conditions and functional capacity. Thus, In qualitative terms, the Article 1 provides an assessment and perception of the elderly about their quality of life. Article 2, in turn, presents the results of more extensive quantitative research, which can be seen that age, presence of chronic diseases and depression were associated with the quality of life. Thus, we discuss the need for action was planning and health strategies, with interdisciplinary approach, considering the environmental context and reality of family elders, promoting quality in the process of aging

Introduction: The SPPB provides information about physical function and is a predictor of adverse events in the elderly. Frailty is a multidimensional syndrome that increases susceptibility to diseases and disability. However it may be possible to prevent or postpone frailty if is identified early. Our objective is to analyze SPPB s ability in screening for frailty a community-dwelling young elderly from cities with distinct socioeconomic conditions. Methods: Data were originated from community dwelling adults (65-74 years old) in Canada (Saint Bruno; n = 60) and Brazil (Santa Cruz; n = 64). SPPB was used to assess physical performance. Frailty was defined as the presence of ≥ 3 of these criteria: weight loss, exhaustion, weakness, mobility limitation and low physical activity. One point was given for each criterion met, totalizing a frailty score ranged from 0 to 5. The Linear Regression and Receiver Operating Characteristics analyses were performed to evaluate the SPPB s screening ability. Results: Mean age was 69.48, 10.0% of the Saint Bruno s sample and 28.1% of Santa Cruz s were frail (p = 0.001), the SPPB score means were 9.6 and 8.5 respectively (p = 0.01). SPPB correlated with the frailty score (R2 = 0.33), with better results for Saint Bruno. A cutoff of 9 in SPPB had good sensitivity and specificity in discriminating frail from non frail in Saint Bruno (AUC = 0.81) but showed fair results in Santa Cruz (AUC = 0.61). Conclusion: The SPPB has moderate ability in predicting frailty among older adult s population, and is an useful test to identify people with good functionality and low frailty when SPPB scores are ≥9

The elderly population growth in Brazil and in the world is an incontestable reality, arising from a significant declining rate in mortality and fertility, resulting from the remarkable improvements in the quality of life of the people. Associated with the introduction of new technologies in the medical area, these issues have been highly contributing for the increase of the population longevity. The numbers of the elderly in Brazil and in the world show female population predominance within the aging segment, a phenomenon known as the feminisation of the old-age . Aging, therefore, is nowadays one of the primary issues and that has been mobilizing the Brazilian s society, since the development raised from this new reality brought with it enormous challenges and complex social implications which are already felt in the daily lives of the societies. My work experience among some age groups has shown that aging is a much differentiated process which has instigated my interest in understanding why aging has to face an overwhelming and painful experience for some people and enriched and full of signification for others. Overviewing, this research aimed to understand the aging development through the analysis of the processes of signification and production of meaning that permeate the aging and the subjective well-being of three aged women participants of the Project Health and Citizenship on Third Age /CEFET-RN, that evidenced attitudes and behaviors concerning the integration and the activity toward the elderly population. The methodological strategy used was History of Life, starting from collecting data based on deep interviews. The analysis of data evidenced that the elderly well-being is a unique and distinct meaningful experience for each person, concerning each story of their lives within differentiated social, cultural, economic context, from this perspective one can demystify the concept of that aging occurs in a homogeneous way for everyone, everywhere and with the same rhythm. The narratives presented in this research showed the human development as being a dialectic, discursive and interactionist process which extends throughout the adult life and continues to the aging life. The guarantee that development and aging are a parallel phenomenon always in interdependence on the preceding phases of live, are corroborated within the studies. This present study confirms that the elderly population can be a phase of growth, personal realization and continued development, without disrespecting the heterogeneous and the subjectivity of the person who ages. Notifying those healthy and well-succeeded aging experiences, this issue has the intention to contribute to demystify the concept of aging as a social problem, illness needed to be treated, and the stereotypy of the elderly being dependent and unproductive

Tendo-se em conta a expectativa de vida cada vez mais alta, vários estudos têm sido desenvolvidos de modo a contribuir para a melhoria da qualidade de vida na terceira idade. Objetivou-se conhecer a opinião dos idosos de um município de porte médio do interior paulista sobre o que é qualidade de vida, através de uma pergunta aberta que foi incluída como parte de um inquérito populacional sobre estilo e qualidade de vida. A análise da referida questão foi realizada através do método de Análise de Conteúdo. Posteriormente calcularam-se as freqüências das categorias obtidas, agrupadas segundo o método de Ward, e em seguida os idosos foram agrupados segundo o método de k-médias. Os resultados indicaram a existência de três grupos de idosos segundo sua definição de qualidade de vida: o primeiro valorizou a questão afetiva e a família; o segundo priorizou a obtenção do prazer e conforto; o terceiro poderia ser sintetizado como o idoso que identifica a qualidade de vida colocando em prática o seu ideário de vida. Sugere-se que, na implementação de ações, se objetive melhorar a qualidade de vida do idoso, levando-se em conta as magnitudes e as diferenças de cada grupo.

Este estudo relata a percepção de cuidadores de idosos que atuam em três instituições de amparo ao idoso na cidade de Araçatuba, São Paulo. Foram abordadas questões em relação ao envelhecimento, motivações pessoais dos cuidadores em buscar trabalho com essa parcela da população, relacionamento com o idoso, dificuldades durante o trabalho e a satisfação encontrada junto ao idoso. Realizou-se entrevista com os cuidadores, e o material verbal coletado foi analisado utilizando a técnica do Discurso do Sujeito Coletivo. Pôde-se perceber, através dos relatos, o envolvimento emocional do cuidador com seu trabalho, a sobrecarga física e emocional à qual está exposto. Relatos a respeito do envelhecimento também puderam ser coletados, além de questões de ordem social, que motivaram o cuidador a buscar esse tipo de trabalho. Os diversos olhares do cuidador nos impulsionam na busca de estratégias de capacitação teórica e suporte psicológico a esse grupo, pensando no seu bem-estar e no bem-estar do idoso.

Objective: To analyze the effects of a water-based exercise program on peak torque (PT) and rate of torque development (RTD) during maximal voluntary ballistic isometric contractions of the lower limb muscles and the performance of a number of functional tests in the elderly. Method: Thirty-seven elderly were randomly assigned to water-based training (3 d/wk for 12 wk) or a control group. Extensor and flexor PT and RTD of the ankle, knee, and hip joints and functional tests were evaluated before and after training. Results: PT increased after training for the hip flexors (18%) and extensors (40%) and the plantar-flexor (42%) muscles in the water-based group. RTD increased after training for the hip-extensor (10%), knee-extensor (11%), and ankle plantar-flexor (27%) muscles in the water-based group. Functional tests also improved after training in the water-based group (p < .05). Conclusion: The water-based program improved PT and RTD and functional performance in the elderly.

RESUMO Objetivos: A presente investigação teve como principais objetivos descrever a qualidade subjetiva do sono e as perturbações do sono e analisar a intensidade dos sintomas depressivos e dos sentimentos de solidão em idosos institucionalizados; comparar estes dados com um grupo de idosos não institucionalizados e analisar a relação entre estas variáveis nos dois grupos. Métodos: Este estudo insere-se no Projeto Trajetórias do Envelhecimento de Idosos em Resposta Social de onde foi retirada uma amostra de cento e quarenta idosos sem défice cognitivo, com 70 institucionalizados e 70 não institucionalizados emparelhados por idade, sexo, escolaridade e estado civil. A média de idades foi de 76,58 (DP = 6,10), incluindo 104 mulheres e 36 homens. Como instrumentos foram utilizados um Questionário Sociodemográfico, o Questionário sobre o Sono na Terceira Idade, a Escala Geriátrica de depressão e a Escala de Solidão da Universidade da Califórnia, Los Angeles. Resultados: Verificou-se que os idosos institucionalizados apresentavam mais sentimentos de solidão do que os não institucionalizados. Contudo, não se verificaram diferenças entre os dois grupos em relação aos sintomas depressivos, qualidade subjetiva do sono ou perturbações do sono, com algumas exceções: os idosos residentes na comunidade mostraram ter a perceção de demorar mais tempo a adormecer, de acordar mais cedo e de ter mais pesadelos. Através de uma análise correlacional verificou-se, na amostra global, que quanto pior a qualidade subjetiva do sono mais sintomas depressivos se observavam e quanto mais sintomas depressivos, mais sentimentos de solidão, não havendo, contudo, relação entre o sono e a solidão. Conclusões: Concluímos que a situação de institucionalização se acompanha de mais sentimentos de solidão, mas não de sintomas depressivos ou de pior qualidade de sono. Por esse motivo, sugere-se que se desenvolvam programas de intervenção dirigidos à solidão em idosos institucionalizados. ABSTRACT Aims: The main objectives of this investigation were to describe the subjective quality of sleep and sleep disorders and analyze the intensity of depressive and loneliness symptoms in institutionalized elderly people; compare these data against a non-institutionalized elderly people subsample and analyze the relationship between these variables in both subsamples. Method: This study is part of Trajectories of Elderly Aging in Social Response Project from which a sample of one hundred and forty elderly people with no cognitive impairment was taken, with 70 institutionalized and 70 non-institutionalized matched by age, sex, education, and marital status. The average age was 76.58 (SD = 6.10), including 104 women and 36 men. The tools used for this analysis were a sociodemographic questionnaire, the Questionnaire About Sleep in the Older Adults, Geriatric Depression Scale, and the Loneliness Scale of the University of California, Los Angeles. Results: The study confirmed that institutionalized elderly people had more feelings of loneliness than those non-institutionalized. However, there were no differences between the two subsamples regarding depressive symptoms and subjective sleep quality or sleep disturbances, with some exceptions: Elderly people living in the community showed to have the perception of taking more time to fall asleep, waking up earlier, and having more nightmares. Through a correlational analysis it was found, in both subsamples, that the worse the subjective sleep quality the more depressive symptoms were observed; and the more depressive symptoms, the more feelings of loneliness, despite of not existing a relation between sleep and loneliness. Conclusions: We concluded that institutionalization is linked to more feelings of loneliness but not to depressive symptoms nor to worse quality of sleep. For this reason, it is suggested that intervention programs are developed with a focus on elderly institutionalized populations.

To investigate the prevalence of urinary incontinence among elderly people living in São Paulo, Brazil and their associated risk factors. The Pan-American Health Organization and World Health Organization coordinated a multicenter study named Health, Wellbeing and Aging (SABE Study) in elderly people (over 60 years old) living in seven countries in Latin America and the Caribbean. In Brazil, the study was carried out in São Paulo in the year 2000. The total Brazilian sample included 2,143 people. The prevalence of self reported urinary incontinence was 11.8% among men and 26.2% for women. It was verified that among those reporting urinary incontinence, 37% also reported stroke and 34% depression. It was found that the greater the dependence that the elderly people presented, the greater the prevalence of urinary incontinence. The associated factors found were depression (odds ratio = 2.49), female (2.42), advanced age (2.35), important functional limitation (2.01). Urinary incontinence is a highly prevalent symptom among the elderly population of the municipality of São Paulo, especially among women. The adoption of preventive measures can reduce the negative effects of urinary incontinence.
