John Cameron has made significant contributions to the field of Medical Physics. His contributions encompassed research and development, technical developments and education. He had a particular interest in the education of medical physicists in developing countries. Structured clinical training is also an essential component of the professional development of a medical physicist. This paper considers aspects of the clinical training and education of medical physicists in South-East Asia and the challenges facing the profession in the region if it is to keep pace with the rapid increase in the amount and technical complexity of medical physics infrastructure in the region.

Background Chronic respiratory illnesses are the most common group of childhood chronic health conditions and are overrepresented in socially isolated groups. Objective To conduct a randomized controlled pilot trial to evaluate the efficacy of Breathe Easier Online (BEO), an Internet-based problem-solving program with minimal facilitator involvement to improve psychosocial well-being in children and adolescents with a chronic respiratory condition. Methods We randomly assigned 42 socially isolated children and adolescents (18 males), aged between 10 and 17 years to either a BEO (final n = 19) or a wait-list control (final n = 20) condition. In total, 3 participants (2 from BEO and 1 from control) did not complete the intervention. Psychosocial well-being was operationalized through self-reported scores on depression symptoms and social problem solving. Secondary outcome measures included self-reported attitudes toward their illness and spirometry results. Paper-and-pencil questionnaires were completed at the hospital when participants attended a briefing session at baseline (time 1) and in their homes after the intervention for the BEO group or a matched 9-week time period for the wait-list group (time 2). Results The two groups were comparable at baseline across all demographic measures (all F < 1). For the primary outcome measures, there were no significant group differences on depression (P = .17) or social problem solving (P = .61). However, following the online intervention, those in the BEO group reported significantly lower depression (P = .04), less impulsive/careless problem solving (P = .01), and an improvement in positive attitude toward their illness (P = .04) compared with baseline. The wait-list group did not show these differences. Children in the BEO group and their parents rated the online modules very favorably. Conclusions Although there were no significant group differences on primary outcome measures, our pilot data provide tentative support for the feasibility (acceptability and user satisfaction) and initial efficacy of an Internet-based intervention for improving well-being in children and adolescents with a chronic respiratory condition. Background Despite considerable effort, most smokers relapse within a few months after quitting due to cigarette craving. The widespread adoption of mobile phones presents new opportunities to provide support during attempts to quit. Objective To design and pilot a mobile app "DistractMe" to enable quitters to access and share distractions and tips to cope with cigarette cravings. Methods A qualitative study with 14 smokers who used DistractMe on their mobiles during the first weeks of their quit attempt. Based on interviews, diaries, and log data, we examined how the app supported quitting strategies. Results Three distinct techniques of coping when using DistractMe were identified: diversion, avoidance, and displacement. We further identified three forms of engagement with tips for coping: preparation, fortification, and confrontation. Overall, strategies to prevent cravings and their effects (avoidance, displacement, preparation, and fortification) were more common than immediate coping strategies (diversion and confrontation). Tips for coping were more commonly used than distractions to cope with cravings, because they helped to fortify the quit attempt and provided opportunities to connect with other users of the application. However, distractions were important to attract new users and to facilitate content sharing. Conclusions Based on the qualitative results, we recommend that mobile phone-based interventions focus on tips shared by peers and frequent content updates. Apps also require testing with larger groups of users to assess whether they can be self-sustaining.

Porous Zr-based bulk metallic glass (PMG) with unidirectional opening pores is prepared by electrochemical etching of tungsten wires of the W/bulk metallic glass (BMG) composites. The porosity and pore size can be controlled by adjusting the tungsten wires. The PMG showed no measurable loss in thermal stability as compared to the monolithic Zr-based BMG by water quenching and is more ductile and softer than the pore-free counterpart. The specific surface area of the PMGs is calculated to be 0.65, 3.96, and 10.54 m(2)/kg for 20, 60, and 80 vol % porosity, respectively. (c) 2007 The Electrochemical Society.

Barsky, House and Kimball (2007) show that introducing durable goods into a sticky-price model leads to negative sectoral comovement of production following a monetary policy shock and, under certain conditions, to aggregate neutrality. These results appear to undermine sticky-price models. In this paper, we show that these results are not robust to two prominent and realistic features of the data, namely input-output interactions and limited mobility of productive inputs. When extended to allow for both features, the sticky-price model with durable goods delivers implications in line with VAR evidence on the effects of monetary policy shocks.

Objetivo Realizar una revisión sistemática de la neumonía lipoidea exógena (NLE) con el propósito de compactar y sintetizar los conocimientos fragmentados, así como el de informar el estado actual de este tema como riesgo laboral. Metodología La neumonía lipoidea es una condición poco común que resulta de la presencia de lípidos en el interior del espacio alveolar, se desconoce su actual incidencia. La neumonía lipoidea exógena se produce por aspiración o inhalación de sustancias oleosas, es una patología rara en pacientes sin enfermedades de base que predispongan a la broncoaspiración y cuando se presenta en individuos sanos se debe sospechar un origen ocupacional. Se realizó una revisión de literatura según la metodología estandarizada, incluyendo en la búsqueda: reporte de casos, descripciones de la enfermedad y el uso de técnicas diagnósticas. Las bases de datos fueron OVID además de GOOGLE ACADÉMICO; buscadores específicos: MEDLINE, CHEST, PUB MED, REDALYC, SCIELO, Europe PubMed Central (Europe PMC) y ELSEVIER. La búsqueda estuvo además, orientada por una serie de preguntas orientadoras sobre la neumonía lipoidea exógena (NLE) como riesgo laboral. Los artículos que cumplieron con los criterios de inclusión, se clasificaron de acuerdo con tipo de estudio, la calidad del artículo y finalmente se evaluaron con base en una lista de chequeo ajustada para este fin. Resultados Se seleccionó un total de 71 estudios incluyendo reporte de casos, descripciones de la enfermedad y técnicas diagnósticas; publicaciones provenientes de 21 países. Un total de 63 casos, 31 en mujeres (49,20%) y 32 en hombres (50,8%) fueron reportados; el 7,93% de los casos fueron atribuidos a la exposición de agentes en el ambiente laboral: combustibles, parafina/pintura/ pulverizaciones oleosas y diésel, ambientes propios de trabajadores de almacenes de pinturas y conductores de vehículos. Los síntomas de la neumonía lipoidea exógena (NLE) descritos fueron muy variables, puede cursar asintomática o con presencia de fiebre, disnea y tos irritativa; dolor torácico, en algunos casos hemoptisis, cianosis y pérdida de peso; El examen físico generalmente es normal, aunque puede revelar sibilancias o roncus. En pruebas de función respiratoria tales como la espirometría, se presenta un patrón restrictivo, además puede hallarse un descenso en la capacidad de difusión para el monóxido de carbono. En el hemograma puede detectarse leucocitosis con predominio de neutrófilos y aumento de la velocidad de sedimentación globular, hallazgos que también pueden ser producidos por una infección concomitante. Conclusiones Se destaca el hallazgo en la literatura de los diferentes agentes causales presentes en el ambiente laboral que pueden incidir en la aparición de la enfermedad: combustibles, parafina/pintura/ pulverizaciones oleosas y diésel, ambientes propios de trabajadores de almacenes de pinturas y conductores de vehículos. Se presentó neumonía lipoidea exógena (NLE) de tipo agudo después de 30 minutos a 24 horas de exposición masiva a aceites y sustancias oleosas, luego de de ocho meses a 9,5 años de inhalación continua secundaria. Los métodos más para de la diagnóstico enfermedad incluyen el lavado broncoalveolar (BAL), la biopsia de pulmón y la con radiografía torácica. Finalmente, es posible que en pacientes con neumonía lipoidea exógena, puedan presentarse complicaciones asociadas tales como: sobreinfección, fibrosis, retracción de las lesiones, cáncer de pulmón, bronconeumonías recurrentes e hipercalcemia.

Introduction: This study prospectively accessed the immune response to the inactivated influenza vaccine in renal transplant recipients receiving either azathioprine or mycophenolate mofetil (MMF). Side effects were investigated. Methods: Sixty-nine patients received one dose of inactivated trivalent influenza vaccine. Antihemagglutinin (HI) antibody response against each strain was measured before and one to six months after vaccination. Results: Geometric mean HI antibody titers for H1N1 and H3N2 strains increased from 2.57 and 2.44 to 13.45 (p = 0.001) and 7.20 (p < 0.001), respectively. Pre- and post-vaccination protection rates for H1N1 and H3N2 increased from 8.7% to 49.3% (p < 0.001); and 36.3% (p < 0.001) and seroconversion rates were 36% and 25.3%, respectively. There was no response to influenza B. The use of MMF reduced the H1N1 and H3N2 protection rates and the seroconversion rate for the H1N1 strain when compared with the use of azathioprine, and subjects transplanted less than 87 months also had inferior antibody response. Adverse events were mild and there were no change on renal function post-vaccination. Conclusion: Renal transplant patients vaccinated against influenza responded with antibody production for in. uenza A virus strains, but not for in. uenza B. Use of MMF and shorter time from transplantation decreased the immune response to the vaccine.

This research project focused primarily on assessing the impact toughness of the weld and the base material of a steel pipe API 5L X70 submerged arc welded, used to conduct remote oil and gas (linepipes). The analysis followed strictly the Specification for Line Pipe - API 5L Standard, regarding the removal of the specimens of regions-of-proof-long section of the pipe, at 90o and 180o from the welded joint, and mechanical properties of toughness and Charpy-V, both the joint welded as the base material. Specimens of steel tube supplied by Tenaris Confab-SA were sized for tensile and Charpy-V, according to ASTM E 8M and ASTM E23, respectively. The result obtained showed that the API X70 steel tube has high Charpy-V toughness, near to each other at both 90o and 180o from the welded joint of the tube, and both higher than the weld metal. Microstructural and microhardness analysis complemented the present study

Two infants are described who presented in the neonatal period with a direct hyperbilirubinemia. This was initially presumed to be because of the diagnosis of gastroschisis and the prolonged use of parenteral nutrition. However, both infants were eventually found to have an associated choledochal cyst. The cases are a novel association not previously reported and should heighten the awareness that anatomical causes of a direct hyperbilirubinemia need to be ruled out in all neonates.

PURPOSE The purpose of this study was to investigate whether there are microbiological differences in bacterial samples collected from labial piercings made of different materials. METHODS Sterile piercings of 4 materials were randomly allocated to 80 pierced subjects. After 2 weeks, microbiologic samples were collected and processed by checkerboard DNA-DNA hybridization methods. Wilcoxon signed ranks and Mann-Whitney tests were used for statistical analysis (adjustment for multiple comparisons). RESULTS There were no statistically significant differences between material groups in relation to baseline data. In samples from stainless steel piercings, the total microbial load was significantly higher than the other materials (P<.05). Ten (mainly periopathogenic) species were found at significantly higher levels (P<.001) on steel than on polypropylene and/or polytetrafluoroethylene piercings. CONCLUSIONS Labial piercings made of stainless steel could promote the development of a pathogenic biofilm.

OBJECTIVES To evaluate risk factors for urethral recurrence (UR) in women with neobladder. MATERIAL AND METHODS From 1994 to 2011, 297 women (median age = 54y; interquartile range: 47-57) underwent radical cystectomy with ileal neobladder for bladder cancer in 4 centers. None of the patients had bladder neck involvement at preoperative assessment. Univariable and multivariable analyses were used to estimate recurrence-free survival and overall survival. The median follow-up was 64 months (interquartile range: 25-116). RESULTS Of the 297 patients, 81 developed recurrence (27%). The 10- and 15-year recurrence-free survival rates were 66% and 66%, respectively. The 10- and 15-year overall survival rates were 57% and 55%, respectively. UR occurred in 2 patients (0.6%) with solitary urethral, 4 (1.2%) with concomitant urethral and distant recurrence, and 1 with concomitant urethral and local recurrence (0.3%). Bladder tumors were located at the trigone in 27 patients (9.1%). None of these patients developed UR. Lymph node tumor involvement was present in 60 patients (20.2%). On univariable and multivariable analyses, pathologic tumor and nodal stage were independent predictors for the overall risk of recurrence. UR was associated with a positive final urethral margin status (P<0.001) whereas no significant associations were found for carcinoma in situ, pathologic tumor and nodal stage, and bladder trigone involvement. CONCLUSIONS In this series, only 0.6% of women developed solitary UR. A positive final urethral margin was associated with an increased risk of UR. Women with involvement of the bladder trigone were not at higher risk of UR.

Technical complications in implant prosthetic cases represent a major challenge in dentistry. This case report describes minimally invasive management to recover an implant with a fractured remnant of a zirconia abutment, including provisional rehabilitation during a sequential treatment protocol in the esthetic zone. A patient was treated with a screw-retained one-piece implant-supported reconstruction made of a customized zirconia abutment with direct ceramic veneering in the maxillary right central incisor position. During the prosthetic try-in, a fracture in the apical portion of the abutment was evident. The first rescue attempt led to fracture of the retrieval instrument. Immediately, an individualized wired construction was applied to bond the existing fractured reconstruction to the neighboring teeth to maintain the peri-implant mucosal architecture. Because the implant screw canal was blocked, a customized round bur had to be manufactured and was placed in the implant axis with a specific bracket tool from the service set to protect the interior implant threads. Then, the drills of the service set were guided by the newly created access to remove the fractured remnants. The implant screw was retapped and the area rinsed with chlorhexidine solution. All remnants were removed without the need for surgical intervention. Neither the implant connection nor the bone-to-implant interface was damaged. The stepwise treatment approach with the customized round bur combined with the system-specific drills of the service set saved the blocked implant so that the patient could be successfully rehabilitated with a new implant reconstruction.
