The spatial arrangement patterns of senile plaques have been studied in 10 micron cresyl violet stained sections cut from embedded portions of 20 brain regions from SDAT brains. Two studies are reported: an initial study using the Poisson distribution and a subsequent study using pattern analysis. The initial study indicated that plaques are arranged in discrete clumps in all brain regions when examined at x100 and x400 – suggesting that both small and larger scale clumping may be present. The pattern analysis study was applied to 8 cortical regions. This technique allows a more detailed study of pattern to be made. In all regions the technique revealed that the basic pattern of plaque arrangement is the regularly spaced discrete clump – which may be present on both large and small scales.

We have studied the spatial distribution of plaques in coronal and tangential sections of the parahippocampal gyrus (PHG), the hippocampus, the frontal lobe and the temporal lobe of five SDAT patients. Sections were stained with cresyl violet and examined at two magnifications (x100 and x400). in all cases (and at both magnifications) statistical analysis using the Poisson distribution showed that the plaques were arranged in clumps (x100: V/M = 1.48 - 4.49; x400 V/M = 1.17 - 1.95). this indicates that both large scale and small scale clumping occurs. Application of the statistical techniques of pattern analysis to coronal sections of frontal and temporal cortex and PHG showed. furthermore, that both large (3200-6400 micron) and small scale (100 - 400 micron) clumps were arranged with a high degree of regularity in the tissue. This suggests that the clumps of plaques reflect underlying neural structure.

Numerous senile plaques are one of the most characteristic histological findings in SDAT brains. Large classical plaques may develop from smaller uncored forms. There is no strong evidence that, once formed, plaques disappear from the tissue. We have examined cresyl-violet stained sections of the parahippocampal gyrus (PHG), hippocampus, frontal lobe and temporal lobe of five SDAT patients. The frequency of various sizes of plaques were determined in each of these brain regions. Statistical analysis showed that the ratio of large plaques to small plaques was greater in the hippocampal formation (especially the PHG) than in the neocortex. One explanation of these results is that plaques grow more rapidly in the hippocampal formation than elsewhere. Alternatively, if the rate of plaque growth is much the same in different brain regions, the data suggest that plaques develop first in the hippocampal formation (especially the PHG) and only later spread to the neocortex. This interpretation is also consistent with the theory that the neuropathology of SDAT spreads from the olfactory cortex via the hippocampal formation to the neocortex. Further development of this technique may help identify the site of the primary lesion in SDAT.
Are there two distinct populations of cored senile plaques in senile dementia of the Alzheimer type?

The relationship between plaque diameter (PD) and core diameter (CD) was studied in four brains from each of four SDAT brains. The regions studied were parahippocampal gyrus (PHG), hippocampus, frontal and inferior temporal lobes. The largest diameters of 100 cored classical plaques and their cores were measured. CD was positively correlated with PD (Pearson's 'r' 0.4 - 0.95) in all region studied. Significant linear regressions of CD on PD with positive slopes (0.10 - 0.65) were found. Two distinct types of regression were found. Type A had a steep slope and a negative intercept on the ordinate whereas Type B had a shallow slope and a positive intercept. Both types can be found within the same brain but Type A or B predominate in a particular tissue. The data suggest that core development may occur either early or late in the development of the plaque. The two types of plaque may thus have different aetiologies. Such an interpretation is consistent with current ideas of plaque formation.

Tetrahydrobiopterin is the cofactor required for the biosynthesis of the neurotransmitters and neuromodulators dopamine, noradrenaline and serotonin. The results show that in SDAT there is decreased conversion of dihydroneopterin triphosphate to tetrahydrobiopterin. Further measurements on strictly age-matched SDAT subjects and controls have confirmed the trends in this investigation.

Excretion of biopterin and the related pteridines neopterin and pterin was measured in urine samples from a group of 76 male and female unipolar and bipolar depressed outpatients receiving lithium therapy, and compared to 61 male and female control subjects. The ratio of neopterin to biopterin excreted (N/B) was significantly higher in the patients than the controls. The significant positive correlation between urinary neopterin and biopterin shown by the controls was absent in the patients, indicating disrupted biosynthesis of tetrahydrobiopterin.Urinary cortisol excretion in depressed patients was similar to controls, implying normal hypothalmus-pituitary-adrenal axis function in these patients, Serum folate was shown to correlate with urinary total biopterin excretion in female unipolar patients. Two groups of elderly females with senile dementia of Alzheimer type (SDAT) were examined for urinary pteridine excretion. In the first study of 10 patients, the N/B ratio was significantly higher than in 24 controls and the ratio B/B+ N significantly lower. A second study of 30 SDAT patients and 21 controls confirmed these findings. However, neopterin correlated with biopterin in both patients and controls, indicating that the alteration in tetrahydrobiopterin metabolism may be different to that shown in depression. Lithium had no effect in vivo or in vitro on Wistar rat brain or liver biosynthesis of tetrahydrobiopterin at a range of concentrations and duration of dosing period, showing that lithium was not responsible for the lowered biopterin excretion by depressed patients. No significant effects on tetrahydrobiopterin metabolism in the rat were shown by the tricyclic antidepressant imipramine, the anticonvulsant sodium valproate, the vitamin folic acid, the anticatecholaminergic agent amethylparatyrosine, the synthetic corticosteroid dexamethasone, or stimulation of natural cortisol by immobilisation stress. Scopolamine, an ant ichol inergic drug, lowered rat brain pterin which may relate to the tetrahydrobiopterin deficits shown in SDAT.

The objective of this research was to investigate the effects of normal aging and the additional effects of chronic exposure to two experimental diets, one enriched in aluminium, the other enriched in lecithin, on aspects of the behaviour and brain histology of the female mouse. The aluminium diet was administered in an attempt to develop a rodent model of Dementia of the Alzheimer Type (DAT). With normal aging, almost all assessed aspects of behaviour were found to be impaired. As regards cognition, selective impairments of single-trial passive avoidance and Morris place learning were observed. While all aspects of open-field behaviour were impaired, the degree of impairment was directly related to the degree of motoric complexity. Deficits were also observed on non-visual sensorimotor coordination tasks and in olfactory discrimination. Histologically, neuron loss, gliosis, vacuolation and congophilic angiopathy were observed in several of the brain regions/fibre tracts believed to contribute to the control of some of the assessed behaviours. The aluminium treatment had very selective effects on both behaviour and brain histology, inducing several features observed in DAT. Behaviourally, the treatment induced impaired spatial reference memory; reduced ambulation; disturbed olfactory function and induced the premature development of the senile pattern of swimming. Histologically, significant neuron loss and gliosis were observed in the hippocampus, entorhinal cortex, amygdala, medial septum, pyriform and pr-frontal cortex. In addition, the brain distribution of congophilic angiopathy was significantly increased by the treatment. The lecithin treatment had effects on both non-cognitive and cognitive aspects of behaviour. The effects of aging on open-field ambulation and rearing were partially ameliorated by the treatment. A similar effect was observed for single-trial passive avoidance performance. Age-dependent improvements in acquisition/retention were observed in 17-23 month mice and Morris place task performance was improved in 11 and 17 month mice. Histologically, a partial sparing of neurons in the cerebellum, hippocampus, entorhinal cortex and subiculum was observed.

The pattern of senile plaques was investigated in various brain regions of six SDAT brains. In 91 pattern analyses, the regularly spaced clump was the most common pattern found in 64.8% of analyses. Clumping due to large aggregations of uncored plaques in sulci was also common. Regularly spaced clumps were equally common in the hippocampus and neocortex. The pattern of plaques varied in different tissue sections from the same brain region. Cored and uncored plaques presented a similar range of patterns but their pattern varied when they were both present in the same tissue section. Both clump diameter and the intensity of clumping were positively correlated with cored but unrelated to uncored plaque density. Plaques may develop in regular clumps on subcortical afferents and during development of the disease the clumps may spread laterally and ultimately coalesce.

The decline in frontal cognitive functions contributes to alterations of gait and increases the risk of falls in patients with dementia, a category which included Alzheimer's disease (AD). The objective of the present study was to compare the gait parameters and the risk of falls among patients at different stages of AD, and to relate these variables with cognitive functions. This is a cross-sectional study with 23 patients with mild and moderate AD. The Clinical Dementia Rating was used to classify the dementia severity. The kinematic parameters of gait (cadence, stride length, and stride speed) were analyzed under two conditions: (a) single task (free gait) and (b) dual task (walking and counting down). The risk of falls was evaluated using the Timed Up-and-Go test. The frontal cognitive functions were evaluated using the Frontal Assessment Battery (FAB), the Clock Drawing Test (CDT) and the Symbol Search Subtest. The patients who were at the moderate stage suffered reduced performance in their stride length and stride speed in the single task and had made more counting errors in the dual task and still had a higher fall risk. Both the mild and the moderate patients exhibited significant decreases in stride length, stride speed and cadence in the dual task. Was detected a significant correlation between CDT, FAB, and stride speed in the dual task condition. We also found a significant correlation between subtest Similarities, FAB and cadence in the dual task condition. The dual task produced changes in the kinematic parameters of gait for the mild and moderate AD patients and the gait alterations are related to frontal cognitive functions, particularly executive functions.

Suite à une recension des écrits sur les soins d’hygiène des personnes atteintes de démence de type Alzheimer (DTA), force est de constater la rareté, voire l’absence de recherches sur la question des soins d’hygiène (SH) dispensés à domicile par les conjointes. Pourtant, la conjointe à domicile est confrontée aux mêmes difficultés que les intervenants des établissements de santé et doit ainsi faire face aux comportements, parfois difficiles de la personne atteinte. Dans la pratique, les infirmières questionnent peu les difficultés rencontrées par les aidantes au moment des soins d’hygiène, ce qui permet difficilement de prendre conscience de leurs sentiments d’isolement et de détresse et de les réduire, le cas échéant. Pour pallier cette lacune, la présente recherche vise à comprendre l’expérience que vivent des conjointes d’hommes atteints de DTA lorsqu’elles leur dispensent des SH à domicile. Un second objectif vise à identifier les dimensions qui sont associées à cette expérience. Une approche de recherche qualitative est utilisée. Les participantes ont été recrutées par l’entremise des Sociétés Alzheimer de Laval et des Laurentides. La stratégie de collecte des données a impliqué la réalisation de deux entrevues individuelles, face à face, de même que l’administration d’un court questionnaire portant sur les aspects sociodémographiques. La démarche retenue pour l’analyse de l’ensemble de données est inspirée de l’approche proposée par Miles et Huberman (2003). Les résultats permettent d’abord de mettre en évidence des profils de conjointes qui, tout en étant variables, présentent certaines similarités. Eu égard aux SH, toutes ont à consacrer des efforts soutenus, quotidiens et intenses. Elles doivent faire montre de patience et compter sur des capacités personnelles les amenant à ressentir des sentiments positifs malgré les situations difficiles. Les résultats montrent par ailleurs que la dispensation des SH provoque aussi des sentiments négatifs associés au fardeau que ces soins impliquent. Les conjointes mettent toutefois en œuvre une variété de stratégies d’adaptation au stress telles la résolution de problème, la recherche de soutien social et le recadrage. Cinq ensembles de dimensions personnelles et contextuelles sont associés à l’expérience des aidantes : 1) En ce qui a trait aux caractéristiques personnelles des conjointes, l’avancement en âge et l’état de santé physique ou psychologique influencent négativement l’expérience lors des SH. Par contre, les ressources personnelles intrinsèques (acceptation de la réalité, capacité de trouver un sens à l’évènement, habileté à improviser et sens de l’humour) sont utilisées de manière naturelle ou acquise; 2) Les caractéristiques personnelles du conjoint (année du diagnostic, pertes d’autonomie et troubles de comportement) affectent négativement l’expérience vécue; 3) La relation conjugale présente un intérêt important puisqu’il apparaît qu’une relation conjugale pré-diagnostic positive semble favoriser des sentiments positifs chez la conjointe en ce qui a trait aux SH; 4) Les dimensions familiales ont un impact favorable, étant donné le soutien psychologique reçu de la famille; 5) Les dimensions macro-environnementales, incluant l’aide reçue du réseau informel plus large de même que du réseau formel, ainsi que les divers aménagements matériels de l’environnement physique du couple, ressortent enfin comme ayant un impact positif. Au terme de l’analyse des résultats, l’auteure est en mesure de proposer une synthèse de l’expérience des conjointes. La discussion aborde quatre enjeux qui se dégagent des résultats observés : impacts des difficultés rencontrées lors des SH dans la décision d’hébergement, réticence des aidantes à faire appel aux ressources du réseau formel pour obtenir de l’aide eu égard aux SH, importance des ressources personnelles des aidantes et potentiel de l’approche relationnelle humaine (human caring) pour faire face aux défis que pose la dispensation des SH.

Depuis ces deux dernières décennies, des efforts considérables en psychologie cognitive et neuropsychologie ont été déployés pour mieux comprendre les dynamiques entre les différents systèmes cognitifs (fonctions exécutives, mémoire, langage, etc.). L’observation d’une co-existence entre des atteintes en rétention à court terme verbale (RCTv) et en langage dans plusieurs pathologies a conduit certains chercheurs à concevoir une alternative à la conceptualisation classique de la RCTv, telle que proposée par le modèle initial de la mémoire de travail de Baddeley (1986). Particulièrement, les travaux sur l’aphasie ont conduit à l’émergence d’une approche psycholinguistique de la RCTv, postulant que le traitement et le maintien des mots font appel à des processus communs. Cette approche, et particulièrement le modèle d’activation interactive (N. Martin & Gupta, 2004; N. Martin & Saffran, 1997), prédit que les capacités en RCTv sont notamment modulées par les caractéristiques linguistiques engagées durant l’épreuve, variant selon la nature des items verbaux et la modalité de rappel, ainsi que par les compétences linguistiques des individus. L’objectif de la présente thèse était de tester ces prédictions à partir d’une exploration des relations entre le traitement sémantique des mots et la RCTv chez l’adulte sain (article 1) et dans la démence de type Alzheimer (DTA) (article 2). Dans le premier article, deux expériences mettent en évidence l’influence des liens associatifs entre les mots sur les capacités à maintenir temporairement des informations verbales. Les participants ont tendance à faussement reconnaître des mots associés aux mots de la liste, reflet d’une activation du réseau sémantique durant la rétention temporaire. Cette contribution sémantique est accentuée en situation de suppression articulatoire, une condition qui empêche le participant de répéter les listes durant la tâche. Les résultats du second article indiquent que la modalité de réponse module différemment la performance en RCTv chez les participants âgés sans atteinte neurologique et ceux atteints de la DTA. Ces données en RCTv sont compatibles avec les atteintes spécifiques du traitement du mot, également trouvées chez le groupe avec DTA. Les implications théoriques et cliniques de ces résultats sont discutées. Les limites et perspectives futures sont également abordées.

Previous research demonstrates that dementia of the Alzheimer type (DAT) is characterised by deficits of episodic memory, especially in the acquisition of new material. As well as this deficit in acquisition, some researchers have also argued for a deficit in consolidation in DAT. We examined acquisition and consolidation by measuring the intertrial gained and lost access in DAT, Mild Cognitive Impairment (MCI) and controls. We report findings from a study of clinical data based on assessment of patients using three free recall trials of a word list. We found that both DAT and MCI groups showed a deficit in acquisition and consolidation of items between trials relative to controls. Moreover, the DAT group was significantly impaired relative to the MCI group for both acquisition and consolidation. Correlations within each group showed that there were strong relationships between intertrial measures and standard measures of memory function. Importantly in no group was there a significant correlation between our measures of acquisition and consolidation: we argue that these measures reflect different underlying processes, and the failure to consolidate in DAT and MCI is not related to the deficit in acquisition. Finally, we showed strong correlations between our measure and dementia severity, suggesting that acquisition and consolidation both get worse as the dementia progresses.
