Il presente lavoro nasce dall’obiettivo di individuare strumenti statistici per indagare, sotto diversi aspetti, il flusso di lavoro di un Laboratorio di Anatomia Patologica. Il punto di partenza dello studio è l’ambiente di lavoro di ATHENA, software gestionale utilizzato nell’Anatomia Patologica, sviluppato dalla NoemaLife S.p.A., azienda specializzata nell’informatica per la sanità. A partire da tale applicativo è stato innanzitutto formalizzato il workflow del laboratorio (Capitolo 2), nelle sue caratteristiche e nelle sue possibili varianti, identificando le operazioni principali attraverso una serie di “fasi”. Proprio le fasi, unitamente alle informazioni addizionali ad esse associate, saranno per tutta la trattazione e sotto diversi punti di vista al centro dello studio. L’analisi che presentiamo è stata per completezza sviluppata in due scenari che tengono conto di diversi aspetti delle informazioni in possesso. Il primo scenario tiene conto delle sequenze di fasi, che si presentano nel loro ordine cronologico, comprensive di eventuali ripetizioni o cicli di fasi precedenti alla conclusione. Attraverso l’elaborazione dei dati secondo specifici formati è stata svolta un’iniziale indagine grafica di Workflow Mining (Capitolo 3) grazie all’ausilio di EMiT, un software che attraverso un set di log di processo restituisce graficamente il flusso di lavoro che li rappresenta. Questa indagine consente già di valutare la completezza dell’utilizzo di un applicativo rispetto alle sue potenzialità. Successivamente, le stesse fasi sono state elaborate attraverso uno specifico adattamento di un comune algoritmo di allineamento globale, l’algoritmo Needleman-Wunsch (Capitolo 4). L’utilizzo delle tecniche di allineamento applicate a sequenze di processo è in grado di individuare, nell’ambito di una specifica codifica delle fasi, le similarità tra casi clinici. L’algoritmo di Needleman-Wunsch individua le identità e le discordanze tra due stringhe di caratteri, assegnando relativi punteggi che portano a valutarne la similarità. Tale algoritmo è stato opportunamente modificato affinché possa riconoscere e penalizzare differentemente cicli e ripetizioni, piuttosto che fasi mancanti. Sempre in ottica di allineamento sarà utilizzato l’algoritmo euristico Clustal, che a partire da un confronto pairwise tra sequenze costruisce un dendrogramma rappresentante graficamente l’aggregazione dei casi in funzione della loro similarità. Proprio il dendrogramma, per la sua struttura grafica ad albero, è in grado di mostrare intuitivamente l’andamento evolutivo della similarità di un pattern di casi. Il secondo scenario (Capitolo 5) aggiunge alle sequenze l’informazione temporale in termini di istante di esecuzione di ogni fase. Da un dominio basato su sequenze di fasi, si passa dunque ad uno scenario di serie temporali. I tempi rappresentano infatti un dato essenziale per valutare la performance di un laboratorio e per individuare la conformità agli standard richiesti. Il confronto tra i casi è stato effettuato con diverse modalità, in modo da stabilire la distanza tra tutte le coppie sotto diversi aspetti: le sequenze, rappresentate in uno specifico sistema di riferimento, sono state confrontate in base alla Distanza Euclidea ed alla Dynamic Time Warping, in grado di esprimerne le discordanze rispettivamente temporali, di forma e, dunque, di processo. Alla luce dei risultati e del loro confronto, saranno presentate già in questa fase le prime valutazioni sulla pertinenza delle distanze e sulle informazioni deducibili da esse. Il Capitolo 6 rappresenta la ricerca delle correlazioni tra elementi caratteristici del processo e la performance dello stesso. Svariati fattori come le procedure utilizzate, gli utenti coinvolti ed ulteriori specificità determinano direttamente o indirettamente la qualità del servizio erogato. Le distanze precedentemente calcolate vengono dunque sottoposte a clustering, una tecnica che a partire da un insieme eterogeneo di elementi individua famiglie o gruppi simili. L’algoritmo utilizzato sarà l’UPGMA, comunemente applicato nel clustering in quanto, utilizzando, una logica di medie pesate, porta a clusterizzazioni pertinenti anche in ambiti diversi, dal campo biologico a quello industriale. L’ottenimento dei cluster potrà dunque essere finalmente sottoposto ad un’attività di ricerca di correlazioni utili, che saranno individuate ed interpretate relativamente all’attività gestionale del laboratorio. La presente trattazione propone quindi modelli sperimentali adattati al caso in esame ma idealmente estendibili, interamente o in parte, a tutti i processi che presentano caratteristiche analoghe.

Three-dimensional (3D) models of teeth and soft and hard tissues are tessellated surfaces used for diagnosis, treatment planning, appliance fabrication, outcome evaluation, and research. In scientific publications or communications with colleagues, these 3D data are often reduced to 2-dimensional pictures or need special software for visualization. The portable document format (PDF) offers a simple way to interactively display 3D surface data without additional software other than a recent version of Adobe Reader (Adobe, San Jose, Calif). The purposes of this article were to give an example of how 3D data and their analyses can be interactively displayed in 3 dimensions in electronic publications, and to show how they can be exported from any software for diagnostic reports and communications among colleagues.

PURPOSE: The aim of this study was to determine occupational facial fractures in central Switzerland. Concomitant injuries were also studied. MATERIALS AND METHODS: The Department of Cranio-Maxillofacial Surgery at the University Hospital in Berne provides a 24-hour maxillofacial trauma service for its population (1.6 million). The present study was comprised of 42 patients (8.4% of treated maxillofacial injuries) with occupational maxillofacial fractures registered at this unit between 2000 and 2002. Information on the topic of occupation, the cause of the accidents, and the topographic location of the fractures was analyzed. RESULTS: The mean age of the patients was 44.4 years, with a male to female ratio of 41:1. Sixty-nine percent of the injuries occurred in farm and forestry workers and in construction laborers during the summertime (33%). Workers in these occupations carried a 127-fold (farm and forestry workers) and a 44-fold (construction laborers) higher risk of incurring maxillofacial fractures than did service and office workers. Injuries were most frequently (43%) caused by a thrown, projected, or falling object. Eighty-two percent of the fractures occurred in the midface region and at the skull base. Fifty-nine percent of the patients had concomitant injuries. In 69%, surgery was necessary, the mean duration of their hospital stay being 4.8 days. CONCLUSION: The probability of sustaining work-related maxillofacial traumata is correlated to the nature of the occupation. Farm and forestry workers are at the highest risk, most frequently injured by being struck by an object or an animal. The introduction of personalized safety measures should become obligatory in high-risk occupations.

OBJECTIVES To present the development of an experimental model in rats for translational expansive tooth movement. SETTING AND SAMPLE Section of Periodontology at Department of Dentistry Aarhus University. Twenty male Wistar rats in two pilot experimental settings plus seven animals without any intervention serving as controls. MATERIAL AND METHODS The second molar (group P1) or the second and third molar (group P2) in the maxillae of the animals were moved buccally using transpalatal β-titanium springs. In the group P2, two spring types (high force and low force) and two preangulations (0° passive or 30° torsion moment) were tested. The amount and type of tooth movement achieved and the resulting skeletal effect were assessed on microCT images, histological analysis was performed on few selected specimens. RESULTS Expansive translational root movement amounting half a tooth width was achieved. Comparison of the amount of tooth movement at the right and left side of the maxilla showed that the expansion was rather symmetrical in the P2 group. Skeletal widening of the maxilla contributed in the P2 group to approximately one-third of the total root movement, whereas two-thirds were dental movement. CONCLUSION With the model used in the P2 group, further research on translational expansive tooth movement and its effect on the periodontium can be pursued. In models for orthodontic expansion, it is strongly recommended to separately evaluate skeletal and dental effects.

Histomorphometric evaluation of the buccal aspects of periodontal tissues in rodents requires reproducible alignment of maxillae and highly precise sections containing central sections of buccal roots; this is a cumbersome and technically sensitive process due to the small specimen size. The aim of the present report is to describe and analyze a method to transfer virtual sections of micro-computer tomographic (CT)-generated image stacks to the microtome for undecalcified histological processing and to describe the anatomy of the periodontium in rat molars. A total of 84 undecalcified sections of all buccal roots of seven untreated rats was analyzed. The accuracy of section coordinate transfer from virtual micro-CT slice to the histological slice, right-left side differences and the measurement error for linear and angular measurements on micro-CT and on histological micrographs were calculated using the Bland-Altman method, interclass correlation coefficient and the method of moments estimator. Also, manual alignment of the micro-CT-scanned rat maxilla was compared with multiplanar computer-reconstructed alignment. The supra alveolar rat anatomy is rather similar to human anatomy, whereas the alveolar bone is of compact type and the keratinized gingival epithelium bends apical to join the junctional epithelium. The high methodological standardization presented herein ensures retrieval of histological slices with excellent display of anatomical microstructures, in a reproducible manner, minimizes random errors, and thereby may contribute to the reduction of number of animals needed.

The purpose of this long-term follow-up study was twofold-firstly, to assess prevalence of relapse after treatment of deep bite malocclusion and secondly, to identify risk factors that predispose patients with deep bite malocclusion to relapse. Sixty-one former patients with overbite more than 50% incisor overlap before treatment were successfully recalled. Clinical data, morphometrical measurements on plaster casts before treatment, after treatment and at long-term follow-up, as well as cephalometric measurements before and after treatment were collected. The median follow-up period was 11.9 years. Patients were treated by various treatment modalities, and the majority of patients received at least a lower fixed retainer and an upper removable bite plate during retention. Relapse was defined as increase in incisor overlap from below 50% after treatment to equal or more than 50% incisor overlap at long-term follow-up. Ten per cent of the patients showed relapse to equal or larger than 50% incisor overlap, and their amount of overbite increase was low. Among all cases with deep bite at follow-up, gingival contact and palatal impingement were more prevalent in partially corrected noncompliant cases than in relapse cases. In this sample, prevalence and amount of relapse were too low to identify risk factors of relapse.
