Patienten mit Cystischer Fibrose müssen in der Regel verschiedene Arzneimittel mehrmals täglich inhalieren. Um den hohen Zeitaufwand dafür zu reduzieren werden die Arzneimittel häufig gemischt und simultan inhaliert. Die Kenntnis der physikalisch-chemischen Kompatibilität von Mischinhalationslösungen/ -suspensionen ist deshalb von großer Bedeutung. In der vorliegenden Arbeit wurden die Kompatibilitäten von Mischungen aus Colistimethat-Inhalationslösung (mikrobiologische Wertbestimmung) mit verschiedenen Tobramycin-Inhalationslösungen, Budesonid (HPLC) mit 5,85%-iger Natriumchlorid-Lösung sowie mit Colistimethat-Inhalationslösung und Dornase alfa (SE-HPLC, SDS-PAGE, UV-Spektrometrie, T-SCX-Chromatographie) mit verschiedenen Tobramycin-Inhalationslösungen (Fluoreszenzpolarisations-Immunoassay) nachgewiesen. Durch das Mischen mit Tobramycin-Inhalationslösungen wurden die aerodynamischen Eigenschaften (FPF, MMAD, GSD) von Dornase alfa bei simultaner Verneblung nicht verändert (bestimmt mittels Kaskadenimpaktion).rnDurch die physiologischen und anatomischen Gegebenheiten, sowie die kognitiven Fähigkeiten kleiner Kinder stellt die effektive inhalative Therapie eine große Herausforderung dar. Der Respimat® bietet mit seiner langen Sprühdauer und den kleinen Aerosolpartikeln eine vielversprechende Alternative für die Anwendung bei Kleinkindern. In der vorliegenden Studie wurde untersucht ob bei Kindern unter 5 Jahren der Respimat® als Inhalationsgerät verwendet werden kann und welchen Grad an Hilfestellung sie für ein erfolgreiches Inhalationsmanöver benötigen.rnDie Ergebnisse der Handhabungsuntersuchung, sowie die Bewertung aufgezeichneter Inhalationsprofile zeigten, dass der Respimat® für Kinder < 4 Jahre nur in Kombination mit einer Inhalierhilfe wie dem AeroChamber Plus® verwendet werden sollte. Kinder im Alter von 4 Jahren sind mit entsprechender Schulung in der Lage mit dem Respimat® alleine zu inhalieren.rn

We assessed the safety and use of induced sputum (IS) in children with cystic fibrosis (CF). Forty-eight children (19 males) with CF, mean age 12.6 (range, 7.3-17.0) years and median forced expired volume in 1 sec (FEV1) 48% (range, 14-77%) predicted were recruited. Patients spontaneously expectorated sputum and then performed sputum induction by inhalation of nebulized 7% hypertonic saline. Samples were sent for bacteriological culture, and for measurement of the following inflammatory mediators: interleukin-8, myeloperoxidase, eosinophil cationic protein, and neutrophil elastase activity. FEV1 was performed before and after inhalation of hypertonic saline. There was no increase in mediator levels in IS compared to expectorated sputum (ES) samples. Only 3 patients demonstrated significant bronchoconstriction following inhalation of hypertonic saline, by the method used. From the ES samples, Pseudomonas aeruginosa was isolated in 13 patients, Staphylococcus aureus in 7 patients, Stenotrophomonas maltophilia in 1 patient, and both Pseudomonas aeruginosa and Staphylococcus aureus in 5 patients. All these organisms were found in the IS samples. However, in 2 patients whose ES grew no organisms, one patient's IS grew Pseudomonas aeruginosa, and the other patient's IS grew Staphylococcus aureus. In our study, sputum induction was safe, with no proinflammatory effect.

To investigate a possible role for human rhinovirus C in respiratory exacerbations of children with cystic fibrosis, we conducted microbiologic testing on respiratory specimens from 103 such patients in Sao Paulo, Brazil, during 2006-2007. A significant association was found between the presence of human rhinovirus C and respiratory exacerbations.

Abnormalities of calcium and vitamin D metabolism in cystic fibrosis (CF) are well documented. We tested the hypothesis that alterations in calcium metabolism are related to vitamin D deficiency, and that bone resorption is increased relative to accretion in patients with CF. Calcitropic hormones, electrolytes, osteocalcin (OC) and bone alkaline phosphatase (BAP), (markers of bone mineralisation), urinary deoxypyridinoline [total (t) Dpd, a marker of bone resorption] and lumbar spine bone mineral density (LS BMD), expressed as a z-score, were measured in 149 (81 M) CF and 141 (61 M) control children aged 5.3-10.99 years, adolescents aged 11-17.99 years and adults aged 18-55.9 years. Cystic fibrosis (CF) patients with Burkholderia cepacia complex (Bcc) pulmonary infections have high morbidity and mortality. The aim of this study was to compare different methods for identification of Bcc species isolated from paediatric CF patients. Oropharyngeal swabs from children with CF were used to obtain isolates of Bcc samples to evaluate six different tests for strain identification. Conventional (CPT) and automatised (APT) phenotypic tests, polymerase chain reaction (PCR)-recA, restriction fragment length polymorphism-recA, recAsequencing, and matrix-assisted laser desorption ionization-time of flight (MALDI-TOF) were applied. Bacterial isolates were also tested for antimicrobial susceptibility. PCR-recA analysis showed that 36 out of the 54 isolates were Bcc. Kappa index data indicated almost perfect agreement between CPT and APT, CPT and PCR-recA, and APT and PCR-recA to identify Bcc, and MALDI-TOF and recAsequencing to identify Bcc species. The recAsequencing data and the MALDI-TOF data agreed in 97.2% of the isolates. Based on recA sequencing, the most common species identified were Burkholderia cenocepacia IIIA (33.4%),Burkholderia vietnamiensis (30.6%), B. cenocepaciaIIIB (27.8%), Burkholderia multivorans (5.5%), and B. cepacia (2.7%). MALDI-TOF proved to be a useful tool for identification of Bcc species obtained from CF patients, although it was not able to identify B. cenocepacia subtypes.

BACKGROUND: Newborn screening (NBS) for Cystic Fibrosis (CF) has been introduced in many countries, but there is no ideal protocol suitable for all countries. This retrospective study was conducted to evaluate whether the planned two step CF NBS with immunoreactive trypsinogen (IRT) and 7 CFTR mutations would have detected all clinically diagnosed children with CF in Switzerland. METHODS: IRT was measured using AutoDELFIA Neonatal IRT-Kit in stored NBS cards. RESULTS: Between 2006 and 2009, 66 children with CF were reported, 4 of which were excluded for various reasons (born in another country, NBS at 6 months, no informed consent). 98% (61/62) had significantly higher IRT compared to matched control group. There was one false negative IRT result in an asymptomatic child with atypical CF (normal pancreatic function and sweat test). CONCLUSIONS: All children but one with atypical CF would have been detected with the planned two step protocol.

BACKGROUND: The potential effects of ionizing radiation are of particular concern in children. The model-based iterative reconstruction VEO(TM) is a technique commercialized to improve image quality and reduce noise compared with the filtered back-projection (FBP) method. OBJECTIVE: To evaluate the potential of VEO(TM) on diagnostic image quality and dose reduction in pediatric chest CT examinations. MATERIALS AND METHODS: Twenty children (mean 11.4 years) with cystic fibrosis underwent either a standard CT or a moderately reduced-dose CT plus a minimum-dose CT performed at 100 kVp. Reduced-dose CT examinations consisted of two consecutive acquisitions: one moderately reduced-dose CT with increased noise index (NI = 70) and one minimum-dose CT at CTDIvol 0.14 mGy. Standard CTs were reconstructed using the FBP method while low-dose CTs were reconstructed using FBP and VEO. Two senior radiologists evaluated diagnostic image quality independently by scoring anatomical structures using a four-point scale (1 = excellent, 2 = clear, 3 = diminished, 4 = non-diagnostic). Standard deviation (SD) and signal-to-noise ratio (SNR) were also computed. RESULTS: At moderately reduced doses, VEO images had significantly lower SD (P < 0.001) and higher SNR (P < 0.05) in comparison to filtered back-projection images. Further improvements were obtained at minimum-dose CT. The best diagnostic image quality was obtained with VEO at minimum-dose CT for the small structures (subpleural vessels and lung fissures) (P < 0.001). The potential for dose reduction was dependent on the diagnostic task because of the modification of the image texture produced by this reconstruction. CONCLUSIONS: At minimum-dose CT, VEO enables important dose reduction depending on the clinical indication and makes visible certain small structures that were not perceptible with filtered back-projection.

Introduction: Patients with Cystic fibrosis (CF) are more susceptible to pathogens like P. aeruginosa (PA). PA primo-infection requires particular attention as failure in eradication is associated with accelerated lung deterioration. The main aim of this study is to assess the rate of PA eradication according to our particular protocol with inhaled tobramycin and oral ciprofloxacin, as there is no consensus in the literature on what eradication protocol is optimal. Methods: Retrospective single centre study with data analysis from June 1st 2007 to June 1st 2011 of patients with PA primo-infection exclusively treated by 3 x 28 days of inhaled tobramycin and oral ciprofloxacin for the first and last 21 days. Success in eradication is defined by ≥ 3 negative bacteriologies for 6 months after the beginning of the protocol. If ≥ 1 bacteriology is positive, we consider the eradication as a failure. Results: Out of 41 patients, 18 followed the eradication protocol and were included in our analysis (7 girls (38.9%) and 11 boys (61.1%)). Boys had 12 primo-infections and girls had 8. Among these 20 primo-infections, 16 (80%) had an overall success in eradication and 4 (20%) a failure. There was no significant statistical differences in age between these groups (t-test = 0.07, p = 0.94), nor for FEV1% (t-test = 0.96, p = 0.41) or BMI (t-test = 1.35, p = 0.27). Rate of success was 100% for girls and 66.6% for boys. Conclusion: Our protocol succeeded in an overall eradication rate of 80%, without statistical significant impact on FEV1 % and BMI values. However, there was a sex difference with eradication rates in girls (100%) and boys (66.6%). A sex difference has not yet been reported in the literature. This should be evaluated in further studies.

We investigated the clinical impact of human coronaviruses (HCoV) OC43, 229E, HKU1 and NL63 in pediatric patients with cystic fibrosis (CF) during routine and exacerbation visits. A total of 408 nasopharyngeal aspirate samples were obtained from 103 patients over a 1-year period. Samples positive for HCoV were submitted for nucleotide sequencing to determine the species. Nineteen samples (4.65%) were positive for HCoV, of which 8 were positive for NL63, 6 for OC43, 4 for HKU1, and 1 for 229E. Identification of HCoV was not associated with an increased rate of respiratory exacerbations, but NL63-positive patients had higher exacerbation rates than patients who were positive for other HCoV species.

Die pulmonale Applikation von Arzneimitteln hat in den vergangenen Jahren zu- rnnehmend an Bedeutung gewonnen und wird vor allem in der lokalen Therapie bei Asthma, COPD und Mukoviszidose genutzt. Als neues innovatives Inhalationsgerät befindet sich der Respimat® seit 2004 auf dem Arzneimittelmarkt und ist bisher nur für Erwachsene zugelassen. Daher wurden im Rahmen dieser Arbeit die Anwendbarkeit und die Modifikation des Respimats® bei Kindern untersucht. Zusammenfassend zeigen die Ergebnisse, dass Kinder ab einem Alter von fünf Jahren fähig sind suffizient mit dem Respimat® zu inhalieren und gute Werte für die estimierte Intrathorakaldosis zu erreichen. Besonders Kinder von 9-12 Jahren und Patienten mit DPI- oder Autohaler®-Inhalationserfahrung erreichten aufgrund eines hohen Atemflusses eine niedrigere estimierte Intrathorakaldosis als es das Potenzial des Respimats® erlaubt hätte. Durch den Einbau eines höheren internen Widerstands in den Orginalrespimat® wurde der Atemfluss signifikant verlangsamt und die Intrathorakaldosis erhöht. Des Weiteren wurden in der vorliegenden Arbeit die physikalisch-chemische Kompatibilität und die aerodynamischen Eigenschaften von Mischinhalationslösung/-suspensionen untersucht. In der Praxis mischen Muko-viszidose-Patienten oftmals mehrere Fertigarzneimittel und inhalieren diese simultan. Dadurch wird der Zeitaufwand der täglichen Inhalation reduziert und die Compliance sowie die Lebensqualität verbessert. Die 4er Kombination aus Colistimethat mit konservierungsmittelfreier Salbutamolsulfat, Ipratropiumbromidinhalationslösungen und Fluticason-17-propionatinhalationssuspension erwies sich als physikalisch-chemisch kompatibel. Die entwickelte mikrobiologische Wertbestimmungsmethode erwies sich als robust, präzise, gut reproduzierbar und erfüllte alle Anforderungen des statistischen Modells. Weiterhin wurden die aerodynamischen Eigenschaften einer praxisrelevanten 3er Mischinhalationssuspension aus Salbutamolsulfat, Ipratropiumbromid und Fluticason-17-propionat im Vergleich zu deren konsekutiven Vernebelung mittels Kaskadenimpaktion betrachtet. Die Charakteristik des erzeugten Aerosols änderte sich für simultan vernebeltes Salbutamolsulfat und Ipratropiumbromid nicht, wohingegen Fluticason-17-propionat bei simultaner Vernebelung eine Reduktion des MMADs, der delivered dose und der recovery aufwies. Dennoch ist die inhalative Glukokortikoiddosis ausreichend hoch um eine effektive antiinflammatorische Wirkung zu gewährleisten. Daher kann die simultane Inhalation der 3er Kombination empfohlen werden und somit die Compliance und die Lebensqualität von Mukoviszidosepatienten erhöht werden.rn
