Background A significant proportion of women who are vulnerable to postnatal depression refuse to engage in treatment programmes. Little is known about them, other than some general demographic characteristics. In particular, their access to health care and their own and their infants' health outcomes are uncharted. Methods We conducted a nested cohort case-control study, using data from computerized health systems, and general practitioner (GP) and maternity records, to identify the characteristics, health service contacts, and maternal and infant health outcomes for primiparous antenatal clinic attenders at high risk for postnatal depression who either refused (self-exclusion group) or else agreed (take-up group) to receive additional Health Visiting support in pregnancy and the first 2 months postpartum. Results Women excluding themselves from Health Visitor support were younger and less highly educated than women willing to take up the support. They were less likely to attend midwifery, GP and routine Health Visitor appointments, but were more likely to book in late and to attend accident and emergency department (A&E). Their infants had poorer outcome in terms of gestation, birthweight and breastfeeding. Differences between the groups still obtained when age and education were taken into account for midwifery contacts, A&E attendance and gestation;the difference in the initiation of breast feeding was attenuated, but not wholly explained, by age and education. Conclusion A subgroup of psychologically vulnerable childbearing women are at particular risk for poor access to health care and adverse infant outcome. Barriers to take-up of services need to be understood in order better to deliver care.

To analyze the relationship between the practice of family health team workers and women with tuberculosis, according to the family focus dimension. The participants were eight family health team workers from a city in the Joao Pessoa metropolitan area (Paraiba -PB). Data were collected through semi-structured interviews, and then subjected to thematic content analysis. The workers recognize the critical social condition that women with tuberculosis live in, as well as the prejudice they face. They suggest that these women require care based on the concept of comprehensiveness in health. They also highlight the importance of Directly Observed Therapy -Short Course, the existence of incentives, and the women's poor adherence to educational activities and groups. Few workers recognize the inclusion of relatives in the care of women with tuberculosis, which implies the need for discussion regarding this fact with the purpose of improving tuberculosis treatment effectiveness.

OBJECTIVE: To describe the initial stages of the implementation of a risk-reduction model designed by Iniciativas Sanitarias to shield women from unsafe abortion in a traditional community on the Uruguay-Brazil border. METHODS: This mixed-design study was conducted first between 22 and 26 March 2010, and then between 2 and 7 May 2011, in Rivera, Uruguay, to gather information from women seen at health centers, healthcare providers, and local policy makers before the project started and midway through the project. RESULTS: At baseline most women and providers considered abortion justifiable only on narrow grounds, yet favored the implementation of a risk-reduction model that would include preabortion as well as postabortion counseling, the former providing information on different abortion methods and their risks. By the midterm assessment, the counseling service had assisted 87 women with unwanted pregnancies. Of the 52 who came for a postabortion visit, 50 had self-administered misoprostol, with no complications. Women were highly satisfied with the counseling. At baseline, misoprostol seemed to be available from both pharmacists and informal sellers. At midterm, it was still available from informal vendors but pharmacists said they did not provide misoprostol. The risk-reduction initiative heightened public attention to the abortion issue but the controversy it generated did not seriously impede its implementation. CONCLUSION: It is feasible to implement the proposed risk-reduction model in a traditional community such as Rivera, not only in Uruguay but in any country irrespective of its abortion laws.

The issue of cultural competency in health care continues to be a priority in Australia for health and human services professionals. Cultural competence in caring for Aboriginal and Torres Strait Islander peoples is of increasing interest, and is a priority in closing the gap in health disparities between Indigenous and non-Indigenous Australians. Through a collaborative conversation, the authors draw on a case study, personal experience and the literature to highlight some of the issues associated with employing culturally appropriate, culturally safe and culturally competent approaches when caring for Aboriginal and Torres Strait Islander peoples. The intent of this article is to encourage discussion on the topic of cultural competency, and to challenge health professionals and academics to think and act on racism, colonialism, historical circumstances and the political, social, economic, and geographical realms in which we live and work, and which all impact on cultural competency.

This project involved creative artists working with older people with dementia and staff from two Belfast Health and Social Care Trust supported housing centres in a mixed programme of dance, painting, music and drama which culminated in an open workshop with relatives and friends of the tenants. The study steered away from traditional medical models of art/music/dance therapy where the participant is perceived as a ‘patient’ in favour of identifying the participant as a ‘student’ who avails of a life-long learning experience. A key premise was that access to the arts is a human right, especially in the context of advancing age and cognitive impairment. . According to one the tenants of Mullan Mews, the project served to ‘awaken - or reawaken - folk with dementia to the endless vista of possibility already in their lives if they will only look for it’. A phenomenographic analysis of video data generated by the project emphasises the importance of the individual experiences of participants in the programme. The evidence from these storylines gained strength from the development of a documentary-style film text that has proved successful in capturing and translating the live experience of the project participants into a supportive text that goes beyond the written word.

Este estudo objetivou verificar a compreensão das experiências dos familiares em relação ao cuidado com a saúde bucal das crianças. É estudo qualitativo, realizado em 2007, em distrito de saúde do município de Ribeirão Preto, SP, com 12 cuidadores. Utilizou-se referencial teórico da vulnerabilidade e a perspectiva hermenêutica. Três categorias empíricas foram elaboradas: os significados do cuidado com a saúde bucal, em busca das causas e da prevenção de agravos bucais e a realidade dos serviços de saúde bucal. Entre outros elementos potencializadores da vulnerabilidade infantil aos agravos bucais, emergiu a supervalorização da causalidade biológica, do atendimento de alta complexidade e da odontologia estética e, entre os protetores, a valorização do saber popular e a integração de ações e conhecimentos profissionais. Aponta-se para a revisão das estratégias de prevenção e promoção de saúde bucal, fornecendo elementos para auxiliar os serviços de saúde a reorganizarem o cuidado com a saúde bucal de crianças.

O objeto deste estudo são as repercussões do estoma intestinal por Câncer na promoção da saúde sexual de mulheres. A investigação sobre a promoção da saúde sexual da mulher com estoma torna-se instigante frente à condição imposta pela cirurgia, em interface com os constructos sócio-histórico-culturais relacionados aos papéis sociais, os quais podem influenciar na forma como as mulheres promovem sua saúde sexual. Esta pesquisa teve por objetivos: conhecer os aspectos biológicos, psicológicos e socioculturais anteriores e posteriores à confecção do estoma intestinal definitivo em mulheres; analisar as repercussões do estoma na promoção da saúde sexual de mulheres; e propor estratégias de cuidar de enfermagem às mulheres com estoma para autopromoção da saúde sexual, considerando a Teoria de Promoção da Saúde de Nola Pender. Trata-se de uma pesquisa com abordagem qualitativa, tendo como sujeitos 14 mulheres com estomia intestinal definitiva, após terem sido acometidas por Câncer. O referencial teórico-metodológico utilizado foi a Promoção da Saúde de Nola Pender, o qual, a partir da identificação dos fatores biopsicossociais e comportamentais, busca incentivar atitudes saudáveis, visando ao bem-estar como proposta de promoção da saúde. O cenário foi o Centro Municipal de Reabilitação Oscar Clark, localizado no município do Rio de Janeiro. Para a produção dos dados foi realizada a técnica de entrevista semiestruturada, utilizando um roteiro pré-elaborado, com base no diagrama de Nola Pender. A análise de conteúdo dos discursos obtidos permitiu criar três categorias: a) perfil sociocultural, psicobiológico e comportamental de mulheres com estoma: uma caracterização antes e após a cirurgia; b) conhecimentos, influências e sentimentos da mulher com estoma sobre a promoção da saúde sexual após a cirurgia; c) resultado do comportamento para promoção da saúde sexual após o estoma: um processo em construção. Os fatores determinantes do comportamento para a promoção da saúde sexual envolveram as condições biológicas, especialmente em decorrência dos efeitos colaterais da radioterapia, além de um processo complexo permeado por fatores sociais, incluindo o estigma, as desigualdades de gênero, as relações de poder, dentre outros valores que norteiam o comportamento humano. As mudanças experienciadas requereram das mulheres o desenvolvimento de estratégias de enfrentamento à nova situação, com a necessidade de adaptações comportamentais para a vivência e promoção da saúde sexual. Tais adaptações refletiram em um aprendizado pontual, da ordem do vivido, o qual perpassou pela falta de orientação em saúde e pelas questões socioculturais. Com isso, a vivência da sexualidade foi considerada a principal barreira para a promoção da saúde sexual. Já os comportamentos direcionados à prevenção de agravos à saúde sexual foram percebidos como as ações que mais proporcionam benefícios. Neste cenário, a consulta de enfermagem apresenta-se como um instrumento relevante na assistência clínica-educativa. Este estudo contribui para um aprofundamento do conhecimento acerca da promoção da saúde sexual de mulheres com estoma e sinaliza propostas para a atuação do enfermeiro na assistência a essas pessoas.

We investigated the temporal relationship between lifestyle and mental health among 564 midlife women. The mental health measured included anxiety, depression, and mental well-being; the lifestyle measures included body mass index (BMI), exercise, smoking, alcohol use, and caffeine consumption. We found that BMI was positively related with mental well-being (r = .316, p = .009); smokers had lower mental well-being than nonsmokers (β = 6.725, p = .006), and noncaffeine drinkers had higher mental well-being (β = 5, p = .023). Past alcohol-drinkers had less anxiety than nondrinkers (β = 1.135, p = .04). Therefore, lifestyle is predictive of mental health among midlife and older women.
