Este es un ensayo clínico controlado realizado en 327 pacientes ASA II, sometidas a cesárea, distribuidas aleatoriamente en dos grupos iguales para identificar las variaciones hemodinámicas y nivel metamérico, al aplicar Anestesia Intratecal con Lidocaína Isobárica al 2(AILI) comparada con Lidocaína Hiperbárica al 5(AILH), ambas a dosis de 80 mg. Resultados: la AILI, se mantuvo en determinadas metámeras sin sustancial variabilidad, comportamiento totalmente contrario a la AILH. Las diferencias estadísticas en la presión sistólica, diastólica, y frecuencia cardiaca, entre ambos tipos de anestesia no fueron significativas. La hipotensión se presentó en 32,9con AILI, y 54con AILH; siendo tratada con 2,26 mg (ñ 3.65) y 3,93 mg (ñ 4,26) de efedrina respectivamente. (p> 0.05). Con AILI se presentaron 4 casos de bloqueo raquídeo alto más dificultad respiratoria y, con AILH 3 casos, de los cuales solo 2 presentaron dificultad respiratoria. Conclusión: la AILI produce un bloqueo nervioso más predecible que la AILH. Los valores hemodinámicos y la incidencia de complicaciones, entre ambos anestésicos fueron semejantes

By using near infrared spectroscopy (NIRS) and by modifying the current Somanetics® optodes being used with the INVOS oximeter, the modified optodes are made to be fairly functional not only across the forehead, but across the hairy regions of the scalp as well. A major problem arises in the positioning of these optodes on the patients scalp and holding them in place while recording data. Another problem arises in the inconsistent repeatability of the trends displayed in the recorded data. A method was developed to facilitate the easy placement of these optodes on the patients scalp keeping in mind thepatient's comfort. The sensitivity of the optodes, too, was improved by incorporating better refined techniques for manufacturing the fiber optic brushes and fixing the same to the optode transmitting and receiving windows. The modified and improved optodes, in the single as well as in the multiplexed modes, were subjected to various tests on different areas of the brain to determine their efficiency and functionality.

Objective: The description and evaluation of the performance of a new real-time seizure detection algorithm in the newborn infant. Methods: The algorithm includes parallel fragmentation of EEG signal into waves; wave-feature extraction and averaging; elementary, preliminary and final detection. The algorithm detects EEG waves with heightened regularity, using wave intervals, amplitudes and shapes. The performance of the algorithm was assessed with the use of event-based and liberal and conservative time-based approaches and compared with the performance of Gotman's and Liu's algorithms. Results: The algorithm was assessed on multi-channel EEG records of 55 neonates including 17 with seizures. The algorithm showed sensitivities ranging 83-95% with positive predictive values (PPV) 48-77%. There were 2.0 false positive detections per hour. In comparison, Gotman's algorithm (with 30 s gap-closing procedure) displayed sensitivities of 45-88% and PPV 29-56%; with 7.4 false positives per hour and Liu's algorithm displayed sensitivities of 96-99%, and PPV 10-25%; with 15.7 false positives per hour. Conclusions: The wave-sequence analysis based algorithm displayed higher sensitivity, higher PPV and a substantially lower level of false positives than two previously published algorithms. Significance: The proposed algorithm provides a basis for major improvements in neonatal seizure detection and monitoring. Intracerebroventricular (ICV) administration of bombesin (BN) induces a syndrome characterized by stereotypic locomotion and grooming, hyperactivity and sleep elimination, hyperglycemia and hypothermia, hyperhemodynamics, feeding inhibition, and gastrointestinal function changes. Mammalian BN-like peptides (MBNs), e.g. gastrin-releasing peptide (GRP), Neuromedin C (NMC), and Neuromedin B (NMB), have been detected in the central nervous system. Radio-labeled BN binds to specific sites in discrete cerebral regions. Two specific BN receptor subtypes (GRP receptor and NMB receptor) have been identified in numerous brain regions. The quantitative 2-[14C]deoxyglucose ([14C]20G) autoradiographic method was used to map local cerebral glucose utilization (LCGU) in the rat brain following ICV injection of BN (vehicle, BN O.1Jlg, O.5Jlg). At each dose, experiments were conducted in freely moving or restrained conditions to determine whether alterations in cerebral function were the result of BN central administration, or were the result of BN-induced motor stereotypy. The anteroventral thalamic nucleus (AV) (p=O.029), especially its ventrolateral portion (AVVL) (pcerebral metabolism within parts of the anterior thalamus, which is the principle relay in the limbic circuitry. BN effects were also observed in IGr, Mi, SCh, and ME. Effects of restraint were found in LOVL, LOOM, and LG. It is suggested that increased LCGU in AV and AVVL may be the result of functional change in the limbic circuitry and the hypothalamus caused by BN receptor functional modification. In IGr, increased LCGU following BN administration is considered to be mainly the result the activation of NMB receptor, a subtype of BN receptors. In SCh, increased LCGU is believed to be caused both by BN effects on the thalamic, the hypothalamic, and the limbic functions and by activation of GRP receptor, another BN receptors subtype found in SCh. In ME, increased LCGU is suggested to be caused by BN effects on the hypothalamic functions, especially those related to the neuroendocrine functions. None of the alterations seen in these regions reflects the emission of stereotyped motor behaviors. Rather, they reflect a direct influence of BN central administration upon functioning of the cerebral regions influenced by BN administration. The restraint effects seen in LO, including LOOM and LOVL, are suggested to be the result of altered behavioral expression. The restraint effects seen in LG is suggested to be the result of reduced locomotion.

Evidence from both clinical and experimental studies demonstrates that mild hypothermia prevents encephalopathy and brain edema in acute liver failure (ALF). As part of a series of studies to elucidate the mechanism(s) involved in this protective effect, groups of rats with ALF resulting from hepatic devascularization were maintained at either 37°C (normothermic) or 35°C (hypothermic), and neurological status was monitored in relation to cerebrospinal fluid (CSF) concentrations of ammonia and lactate. CSF was removed via implanted cisterna magna catheters. Mild hypothermia resulted in a delay in onset of encephalopathy and prevention of brain edema; CSF concentrations of ammonia and lactate were concomitantly decreased. Blood ammonia concentrations, on the other hand, were not affected by hypothermia in ALF rats. These findings suggest that brain edema and encephalopathy in ALF are the consequence of ammonia-induced impairment of brain energy metabolism and open the way for magnetic resonance spectroscopic monitoring of cerebral function in ALF. Mild hypothermia could be beneficial in the prevention of severe encephalopathy and brain edema in patients with ALF awaiting liver transplantation.

Dans les neurones et les cellules vasculaires cérébrales, les dérivés réactifs de l’oxygène jouent un double rôle puisqu’ils peuvent avoir à la fois des effets bénéfiques, à faibles concentrations, et des effets délétères, à des concentrations élevées. Chez la souris, la circulation cérébrale se distingue des autres lits vasculaires puisque le peroxyde d’hydrogène (H2O2) est le principal médiateur endothélial relaxant endogène. L’objectif de notre première étude a été de caractériser l’implication physiologique du H2O2 dérivé de la eNOS dans la fonction endothéliale cérébrale de la souris. Nous avons voulu identifier les mécanismes impliqués dans la dilatation induite par l’augmentation de débit intra-luminal (flow-mediated dilation, FMD). La FMD est la réponse à un stimulus physiologique endothélial la plus représentative de la situation in vivo. Nous avons démontré que le H2O2, et non le monoxyde d’azote (NO), dérivant de l’activation de la eNOS cérébrale, est le principal médiateur de la FMD. Cependant, nous connaissons très peu de données sur l’évolution de la voie du H2O2 au cours du vieillissement qu’il soit associé ou non aux facteurs de risque pour les maladies cardiovasculaires. Au cours du vieillissement, au niveau périphérique, les facteurs endothéliaux constricteurs ou dilatateurs évoluent en fonction de l’augmentation de stress oxydant. La présence de facteurs de risque pour les maladies cardiovasculaires, telle que l’hypercholestérolémie, pourrait accentuer l’augmentation du stress oxydant et ainsi accélérer la dysfonction endothéliale. Au niveau cérébral, très peu de données sont disponibles. Dans le cadre de notre deuxième étude, nous avons émis l’hypothèse qu’un débalancement des facteurs endothéliaux pourrait être à l’origine (1) de la dysfonction endothéliale cérébrale observée au cours du vieillissement et (2) de la dysfonction endothéliale précoce qui apparaît en présence d’athérosclérose. Nos résultats ont montré que l’augmentation de stress oxydant associée au vieillissement conduit à une libération endogène accrue de TXA2 qui diminue la voie du H2O2 au niveau cérébral et, par conséquent, réduit la dilatation dépendante de l’endothélium. De plus, la présence d’athérosclérose accélère l’apparition de la dysfonction endothéliale cérébrale. Le rôle clé joué par le stress oxydant a été confirmé par un traitement préventif avec l’antioxydant catéchine qui a permis de renverser tous les effets délétères de l’athérosclérose sur les fonctions endothéliales cérébrales. Finalement, la dysfonction endothéliale cérébrale précoce, associée avec l’athérosclérose, pourrait non seulement augmenter l’incidence de développer des accidents vasculaires cérébraux (AVC) mais aussi induire une diminution du débit sanguin cérébral et, ultimement, affecter les fonctions neuronales. Dans le cadre de notre troisième étude, nous avons émis l’hypothèse que l’augmentation de stress oxydant est associée avec une diminution du débit sanguin cérébral et un déclin subséquent des fonctions cognitives. Nous avons utilisé des souris athérosclérotiques âgées de 3 mois que nous avons soumises, ou pas, à un traitement chronique à la catéchine. Nos travaux montrent qu’un traitement préventif avec la catéchine peut prévenir les effets néfastes de l’athérosclérose sur la FMD, le débit sanguin et le déclin des fonctions cognitives qui est normalement associé au vieillissement. Nos résultats ont permis de distinguer l’effet du vieillissement des effets de l’athérosclérose sur les fonctions vasculaires cérébrales. Le traitement préventif avec la catéchine a eu des effets bénéfiques marqués sur la fonction endothéliale cérébrale, le débit sanguin cérébral et les fonctions cognitives, démontrant le rôle clé de l’environnement redox dans la régulation des fonctions cérébrales.

La Vitamine K (VK) est largement reconnue pour son rôle dans la coagulation sanguine toutefois, de plus en plus de travaux indiquent son implication dans la fonction cérébrale. La VK est requise pour l'activation de différentes protéines, par exemple la protéine Gas6, et la ménaquinone-4 (MK-4), le principal vitamère K dans le cerveau, est impliquée dans le métabolisme des sphingolipides. Dans un rapport précédent, nous avons montré qu'un régime alimentaire faible en VK tout au long de la vie était associé à des déficits cognitifs chez des rats âgés. La warfarine sodique est un puissant antagoniste de la VK qui agit en bloquant le cycle de la VK, provoquant un «déficit relatif de VK » au niveau cellulaire. À la lumière du rôle émergent de la VK dans le cerveau, la warfarine pourrait représenter un facteur de risque pour la fonction cérébrale. Ce travail est donc pertinente en raison de la forte proportion d'adultes traîtés à la warfarine sodique. Dans la présente étude, 14 rats mâles Wistar ont été traités avec 14 mg de warfarine/kg /jour (dans l'eau potable) et des injections sous-cutanées de VK (85 mg/kg), 3x/sem, pendant 10 semaines. Quatorze rats témoins ont été traités avec de l'eau normale et injectés avec une solution saline. Les rats ont été soumis à différents tests comportementaux après quoi les niveaux de phylloquinone, MK-4, sphingolipides (cérébroside, sulfatide, sphingomyéline, céramide et gangliosides), et les sous-types de gangliosides (GT1b, GD1a, GM1, GD1b), ont été évalués dans différentes régions du cerveau. Comparativement aux rats du groupe contrôle, les rats traités à la warfarine présentaient des latences plus longues au test de la piscine de Morris (p <0,05) ainsi qu'une hypoactivité et un comportement exploratoire plus faible au test de « l’open field » (p <0,05). Le traitement par warfarine a également entraîné une diminution spectaculaire du niveau de MK-4 dans toutes les régions du cerveau (p <0,001), une altération des concentrations de sphingolipidiques, en particulier dans le cortex frontal et le mésencéphale (p <0,05), et une perte de différences régionales sphingolipidiques, notamment pour les gangliosides. Le traitement par warfarine a été associé à un niveau inférieur de GD1a dans l'hippocampe et un niveau supérieur de GT1b dans le cortex préfrontal et le striatum. En conclusion, la déficience en VK induite par warfarine altère les niveaux de VK et sphingolipides dans le cerveau, avec de potentiels effets néfastes sur les fonctions cérébrales.

As empresas brasileiras tendem a colocar apenas ações preferenciais no mercado. Este trabalho discute algumas das conseqüências dessa prática, procurando demonstrar que esta afugenta os investidores de longo prazo que podem exercer uma dupla função: monitorar algumas atividades da empresa coibindo certas práticas indesejadas da direção e estabilizar o mercado reduzindo as chances de takeovers hostis, mas não dos takeovers sob os quais o monitoramento ocorre, levando em consideração sua natureza de bem público e o problema de reputação, que tipicamente levam a uma oferta subótima desse tipo de atividade. Também é analisado como a presença dos investidores de longo prazo afeta a ação dos arbitradores que procuram extrair sinais do mercado para auferir ganhos com preços fora dos fundamentos.

OBJECTIVES: The aim of this single-blind randomized crossover study was to evaluate specific effects of manual acupuncture on central and vegetative nervous system activity measured by quantitative electroencephalography (qEEG) and heart rate variability (HRV). DESIGN: Twenty (20) healthy volunteers (mean: 25.2 +/- 3.6 years) were monitored simultaneously using a qEEG system and a 12-channel electrocardiogram recorder during verum acupuncture (VA) at acupuncture point Large Intestine 4 (Hegu) (LI4) or placebo acupuncture (PA) at a sham point. RESULTS: In the EEG conduction of the occipital area, needle stimulation in VA increased alpha1-frequency significantly, and the ratio alpha1/theta was shifted to the benefit of alpha1 over all electrodes. The HRV parameters showed a significant increase of the low frequency/high frequency (HF) ratio during the first minute of stimulation in VA, indicating an initial increase of sympathetic activation. However, an increase of HF power in the minute after stimulation followed by a decrease in heart rate suggests delayed vagal activation. De qi (a sensation that is typical of acupuncture needling) occurred in 16 subjects during VA and in 9 volunteers during PA (80% versus 45%). CONCLUSIONS: Manual stimulation on LI4 seems to lead to specific changes in alpha EEG-frequency and in HRV parameters. A linear relationship between the HRV parameters and the alpha EEG band might point to a specific modulation of cerebral function by vegetative effects during acupuncture.

In acute neuroinflammatory states such as meningitis, neutrophils cross the blood-brain barrier (BBB) and contribute to pathological alterations of cerebral function. The mechanisms that govern neutrophil migration across the BBB are ill defined. Using live-cell imaging, we show that LPS-stimulated BBB endothelium supports neutrophil arrest, crawling, and diapedesis under physiological flow in vitro. Investigating the interactions of neutrophils from wild-type, CD11a(-/-), CD11b(-/-), and CD18(null) mice with wild-type, junctional adhesion molecule-A(-/-), ICAM-1(null), ICAM-2(-/-), or ICAM-1(null)/ICAM-2(-/-) primary mouse brain microvascular endothelial cells, we demonstrate that neutrophil arrest, polarization, and crawling required G-protein-coupled receptor-dependent activation of β2 integrins and binding to endothelial ICAM-1. LFA-1 was the prevailing ligand for endothelial ICAM-1 in mediating neutrophil shear resistant arrest, whereas Mac-1 was dominant over LFA-1 in mediating neutrophil polarization on the BBB in vitro. Neutrophil crawling was mediated by endothelial ICAM-1 and ICAM-2 and neutrophil LFA-1 and Mac-1. In the absence of crawling, few neutrophils maintained adhesive interactions with the BBB endothelium by remaining either stationary on endothelial junctions or displaying transient adhesive interactions characterized by a fast displacement on the endothelium along the direction of flow. Diapedesis of stationary neutrophils was unchanged by the lack of endothelial ICAM-1 and ICAM-2 and occurred exclusively via the paracellular pathway. Crawling neutrophils, although preferentially crossing the BBB through the endothelial junctions, could additionally breach the BBB via the transcellular route. Thus, β2 integrin-mediated neutrophil crawling on endothelial ICAM-1 and ICAM-2 is a prerequisite for transcellular neutrophil diapedesis across the inflamed BBB.

Multiple system atrophy (MSA) is a rare neurodegenerative disorder associated with parkinsonism, ataxia, and autonomic dysfunction. Its pathology is primarily subcortical comprising vacuolation, neuronal loss, gliosis, and α-synuclein-immunoreactive glial cytoplasmic inclusions (GO). To quantify cerebellar pathology in MSA, the density and spatial pattern of the pathological changes were studied in α-synuclein-immunolabelled sections of the cerebellar hemisphere in 10 MSA and 10 control cases. In MSA, densities of Purkinje cells (PC) were decreased and vacuoles in the granule cell layer (GL) increased compared with controls. In six MSA cases, GCI were present in cerebellar white matter. In the molecular layer (ML) and GL of MSA, vacuoles were clustered, the clusters exhibiting a regular distribution parallel to the edge of the folia. Purkinje cells were randomly or regularly distributed with large gaps between surviving cells. Densities of glial cells and surviving neurons in the ML and surviving cells and vacuoles in the GL were negatively correlated consistent with gliosis and vacuolation in response to neuronal loss. Principal components analysis (PCA) suggested vacuole densities in the ML and vacuole density and cell losses in the GL were the main source of neuropathological variation among cases. The data suggest that: (1) cell losses and vacuolation of the GCL and loss of PC were the most significant pathological changes in the cases studied, (2) pathological changes were topographically distributed, and (3) cerebellar pathology could influence cerebral function in MSA via the cerebello-dentato-thalamic tract.

The goal of the present work is to develop some strategies based on research in neurosciences that contribute to the teaching and learning of mathematics. The interrelationship of education with the brain, as well as the relationship of cerebral structures with mathematical thinking was discussed. Strategies were developed taking into consideration levels that include cognitive, semiotic, language, affect and the overcoming of phobias to the subject. The fundamental conclusion was the imperative educational requirement in the near future of a new teacher, whose pedagogic formation must include the knowledge on the cerebral function, its structures and its implications to education, as well as a change in pedagogy and curricular structure in the teaching of mathematics.
