Background The effectiveness of exercise referral schemes (ERS) is influenced by uptake and adherence to the scheme. The identification of factors influencing low uptake and adherence could lead to the refinement of schemes to optimise investment. Objectives To quantify the levels of ERS uptake and adherence and to identify factors predictive of uptake and adherence. Methods A systematic review and meta-analysis was undertaken. MEDLINE, EMBASE, PsycINFO, Cochrane Library, ISI WOS, SPORTDiscus and ongoing trial registries were searched (to October 2009) and included study references were checked. Included studies were required to report at least one of the following: (1) a numerical measure of ERS uptake or adherence and (2) an estimate of the statistical association between participant demographic or psychosocial factors (eg, level of motivation, self-efficacy) or programme factors and uptake or adherence to ERS. Results Twenty studies met the inclusion criteria, six randomised controlled trials (RCTs) and 14 observational studies. The pooled level of uptake in ERS was 66% (95% CI 57% to 75%) across the observational studies and 81% (95% CI 68% to 94%) across the RCTs. The pooled level of ERS adherence was 49% (95% CI 40% to 59%) across the observational studies and 43% (95% CI 32% to 54%) across the RCTs. Few studies considered anything other than gender and age. Women were more likely to begin an ERS but were less likely to adhere to it than men. Older people were more likely to begin and adhere to an ERS. Limitations Substantial heterogeneity was evident across the ERS studies. Without standardised definitions, the heterogeneity may have been reflective of differences in methods of defining uptake and adherence across studies. Conclusions To enhance our understanding of the variation in uptake and adherence across ERS and how these variations might affect physical activity outcomes, future trials need to use quantitative and qualitative methods.

Background A 2014 national audit used the English General Practice Patient Survey (GPPS) to compare service users’ experience of out-of-hours general practitioner (GP) services, yet there is no published evidence on the validity of these GPPS items. Objectives Establish the construct and concurrent validity of GPPS items evaluating service users’ experience of GP out-of-hours care. Methods Cross-sectional postal survey of service users (n=1396) of six English out-of-hours providers. Participants reported on four GPPS items evaluating out-of-hours care (three items modified following cognitive interviews with service users), and 14 evaluative items from the Out-of-hours Patient Questionnaire (OPQ). Construct validity was assessed through correlations between any reliable (Cochran's α>0.7) scales, as suggested by a principal component analysis of the modified GPPS items, with the ‘entry access’ (four items) and ‘consultation satisfaction’ (10 items) OPQ subscales. Concurrent validity was determined by investigating whether each modified GPPS item was associated with thematically related items from the OPQ using linear regressions. Results The modified GPPS item-set formed a single scale (α=0.77), which summarised the two-component structure of the OPQ moderately well; explaining 39.7% of variation in the ‘entry access’ scores (r=0.63) and 44.0% of variation in the ‘consultation satisfaction’ scores (r=0.66), demonstrating acceptable construct validity. Concurrent validity was verified as each modified GPPS item was highly associated with a distinct set of related items from the OPQ. Conclusions Minor modifications are required for the English GPPS items evaluating out-of-hours care to improve comprehension by service users. A modified question set was demonstrated to comprise a valid measure of service users’ overall satisfaction with out-of-hours care received. This demonstrates the potential for the use of as few as four items in benchmarking providers and assisting services in identifying, implementing and assessing quality improvement initiatives.

Background English National Quality Requirements mandate out-of-hours primary care services to routinely audit patient experience, but do not state how it should be done.

Objectives We explored how providers collect patient feedback data and use it to inform service provision. We also explored staff views on the utility of out-of-hours questions from the English General Practice Patient Survey (GPPS).

Methods A qualitative study was conducted with 31 staff (comprising service managers, general practitioners and administrators) from 11 out-of-hours primary care providers in England, UK. Staff responsible for patient experience audits within their service were sampled and data collected via face-to-face semistructured interviews.

Results Although most providers regularly audited their patients’ experiences by using patient surveys, many participants expressed a strong preference for additional qualitative feedback. Staff provided examples of small changes to service delivery resulting from patient feedback, but service-wide changes were not instigated. Perceptions that patients lacked sufficient understanding of the urgent care system in which out-of-hours primary care services operate were common and a barrier to using feedback to enable change. Participants recognised the value of using patient experience feedback to benchmark services, but perceived weaknesses in the out-of-hours items from the GPPS led them to question the validity of using these data for benchmarking in its current form.

Conclusions The lack of clarity around how out-of-hours providers should audit patient experience hinders the utility of the National Quality Requirements. Although surveys were common, patient feedback data had only a limited role in service change. Data derived from the GPPS may be used to benchmark service providers, but refinement of the out-of-hours items is needed.

The Welland Power and Supply Canal Company Limited, established in 1893 and incorporated in 1894 with a capital stock of $500,000. The aim of the company was to harness the natural water supply of the Niagara and Welland Rivers. In 1898 the Canadian Electrical News published a report by Henry Symons, QC outlining the main project of the company. This project involves the construction of a canal from the Welland River to the brow of the mountain at Thorold, a distance of 8 miles; the construction at Thorold of a power house, and from Thorold to Lake Ontario, a raceway by which to carry water into the lake. The estimate for the machinery to generate 100,000 horse power is £125,000; for transmission line to Toronto at a voltage of 10,000….The total estimate therefore amounts to £2,452,162, or roughly speaking, $12,000,000. Source: Canadian Electrical News, August 1898, p. 172. In 1899 the company officers petitioned the federal government desiring a name change to the Niagara-Welland Power Company Limited. Officers of the company were Harry Symons, President; Charles A. Hesson, Vice-President; and M.R. O’Loughlin, James B. Sheehan, James S. Haydon, Frederick K. Foster, directors; John S. Campbell, secretary-treasurer. The company’s head offices were located in St. Catharines, with a New York (City) office on Broad Street. In 1905 and 1909 the company petitioned the federal government for additional time to construct its works, which was granted. The company had until May 16, 1915 to complete construction. John S. Campbell (1860-1950) was a graduate of the University of Toronto and Osgoode Hall. During his university years John began his military career first in "K" Company, Queens Own rifles and then later as Commanding Officer of the 19th Lincoln Regiment, from 1906 to 1910. Upon his return to St. Catharines John Campbell served as secretary in the St. Catharines Garrison Club, a social club for military men begun in 1899. After being called to the Bar, he became a partner in the firm of Campbell and McCarron and was appointed to the bench in 1916, serving until retirement in 1934. Judge Campbell served as an alderman for several terms and was the mayor of St. Catharines in 1908 and 1909. He also served as the first chairman of the St. Catharines Public Utilities in 1914. John S. Campbell was married to Elizabeth Oille, daughter of Jerome B. and Charlotte (St. John) Oille. The family home "Cruachan" was located at 32 Church St.

Crowdsourcing in recent times has become popular among not-for-profits as a means of eliciting members of the public to contribute to activities that would normally have been carried out by staff or by contracting external expertise. The GLAM (galleries, libraries, archives, museums) sector does have a history of involving online volunteers (e.g. reviewing books). Extending that tradition, some GLAM institutions are engaging in crowdsourcing projects to enhance and enrich their collections. But what motivates the public to participate in these crowdsourcing activities? Understanding the unique motivations of participants is needed to establish a motivational framework for GLAM organisations in their not-for-profit context. We present findings from a study of the motivational factors affecting participation in the Australian Newspapers Digitisation Program (ANDP) by the National Library of Australia (NLA). Based on motivational theories and frameworks the study shows that the participants are motivated by a complex framework of personal, collective and external factors. Participants were highly intrinsically motivated, but valued altruistic and community motivations as well. Community and external factors played a vital role in their continued involvement. The paper concludes with a conceptual framework of the motivational factors for crowdsourcing participants in a GLAM context based on the motivational dynamics observed in the ANDP case.

This paper presents an analysis of the role and experiences of the teacher practitioner within the partnership arrangement in the Bachelor of Learning Management (BLM) degree at the Rockhampton campus of Central Queensland University (CQU). The paper begins by discussing the concept of partnerships and the background to current pre-service teacher education programs in Australia. It then introduces the BLM, documenting the experiences of partner-teachers involved in the program and analysing this model of partnership. The paper concludes with some suggestions for improving teacher-lecturer partnerships.

Over the past decade, teaching and learning in virtual worlds has been at the forefront of many higher education institutions around the world. The DEHub Virtual Worlds Working Group (VWWG) consisting of Australian and New Zealand higher education academics was formed on 2009. These educators are investigating the role that virtual worlds play in the future of education and actively changing the direction of their own teaching practice and curricula. 47 academics reporting on 28 Australian higher education institutions present an overview of how they have changed directions through the effective use of virtual worlds for diverse teaching and learning activitiessuch as business scenarios and virtual excursions, role-play simulations, experimentation and language development. The case studies offer insights into the ways in which institutions are continuing to change directions in their teaching to meet changing demands for innovative teaching, learning and research in virtual worlds. This paper highlights the ways in which the authors are using virtual worlds to creat opportunities for rich, immersive and authentic activities that would be difficult or not possible to achieve through traditional approaches

Perícia é a capacidade de aplicar habilidades intelectuais para resolver problemas em domínios estratégicos, com um desempenho e qualidade de solução superior à média dos profissionais da área. Ampliar a compreensão do que é a perícia fornece suporte e justificativas para a proposição de novos recursos para aquisição e modelagem de conhecimento na área da Engenharia de Conhecimento. Esta tese apresenta os resultados de um estudo sobre a perícia em Geologia, em especial numa aplicação em Petrografia Sedimentar. A tarefa em questão é especialmente significativa porque, ao contrário das tarefas típicas, cujo estudo tem levado ao desenvolvimento de diversas metodologias de aquisição de conhecimento, essa tarefa aplica primariamente raciocínio baseado na análise de imagens e, secundariamente, busca e métodos analíticos para interpretar os objetos da perícia (no caso, rochas-reservatório de petróleo). O objetivo deste projeto de tese é a identificação dos recursos cognitivos aplicados por especialistas na solução de problemas, que são essencialmente de reconhecimento visual e a representação do que é reconhecido. A interpretação dessas habilidades fornece fundamentos para a proposta de novos recursos para aquisição e modelagem, e posterior desenvolvimento de sistemas especialistas para interpretação de rochas. Também contribuem para o tratamento da perícia em outros campos que possuam o mesmo caráter de reconhecimento visual tal como a interpretação de rochas. O estudo foi desenvolvido em duas fases. Na primeira, o conhecimento foi eliciado de um especialista em Petrografia Sedimentar e estruturado, utilizando técnicas tradicionais de aquisição de conhecimento. A segunda fase envolveu o desenvolvimento de um experimento com dezenove geólogos com diferentes níveis de perícia, para identificar os tipos de conhecimentos que suportam a perícia e quais os métodos de solução que são aplicados nos altos níveis da perícia. O estudo das habilidades cognitivas demonstrou que especialistas em Petrografia sedimentar desenvolvem uma grande variedade de formas mentais e hierarquias que diferem daquelas normalmente descritas na literatura da área. Especialistas retém ainda um grande conjunto de abstrações simbólicas de imagens, denominados aqui de pacotes visuais. Os pacotes visuais possuem importante papel na indexação das estruturas mentais e na condução do processo de inferência. As representações são tipicamente associadas com seus próprios métodos de solução de problemas adequados à complexidade da tarefa de caracterização de reservatórios. A aplicação desses recursos faz parte do conjunto de conhecimentos tácitos dos especialistas. A associação de grafos de conhecimento e a análise de casos mostrou-se, neste trabalho, um método adequado para a externalizar e adquirir o conhecimento declarativo e as relações causais, as quais não são evidenciadas com as técnicas de aquisição de conhecimento tradicionais. Métodos de solução de problemas, por sua vez, foram eliciados com o auxílio das bibliotecas de solução de problemas disponíveis na literatura e grafos de conhecimento. O modelo de representação, aqui proposto, expressa o conhecimento em dois níveis: o nível da externalização, compatível com o conhecimento de um intermediário em Petrografia Sedimentar, e o nível da inferência, que modela o conhecimento tácito do especialista. Esta tese apresenta de forma inédita o conceito de pacote visual como uma primitiva de representação e um conjunto de métodos de solução de problemas adequados à interpretação de rochas.
