PURPOSE: To retrospectively evaluate the influence of hyoid bone resection according to Sistrunk in early age due to a thyroglossal duct cyst on craniofacial growth. MATERIALS AND METHODS: We retrospectively examined 10 patients (2 females and 8 males) having had hyoid bone resection according to Sistrunk due to thyroglossal duct cysts by lateral cephalograms taken before orthodontic treatment (mean, 17.1 years; range, 8.6-31.9 years). Surgery was carried out at a mean age of 4.4 years (range, 0.37-9.8 years). All lateral cephalograms were evaluated and traced by hand. Descriptive statistics were calculated, and data from each patient were compared individually with corresponding standard values (age and gender) from Bathia and Leighton. RESULTS: With regard to sagittal parameters, the SNB angles were by trend too small and the ANB angles were too large. However, the ratio of mandibular to maxillary length showed that the patients had a mandible that was too large or maxilla that was too small. With regard to vertical parameters, large deviations from normal values in both directions (hyperdivergent to hypodivergent pattern) could be detected when we analyzed NSL/ML', NL/ML', and NSL/NL. With regard to dental parameters, the majority of the patients had retroclined upper (IsL/NL, IsL/N-A) and lower (IiL/ML, IiL/N-B) incisors. CONCLUSIONS: Several vertical and horizontal skeletal and dental cephalometric parameters were shown to be different by trend when compared with control values. A possible negative impact on craniofacial growth potential and direction as a result of hyoid resection in early age according to Sistrunk cannot be excluded.

This study analyzed the effects of unilateral detachment of the temporal muscle and coronoidotomy on facial growth in young rats. Thirty one-month-old Wistar rats were distributed into three groups: detachment, coronoidotomy and sham-operated. Under general anesthesia, unilateral detachment of the temporal muscle was performed for the detachment group, unilateral coronoidotomy was performed for the coronoidotomy group, and only surgical access was performed for the sham-operated group. The animals were sacrificed at three months of age. Their soft tissues were removed, and the mandible was disarticulated. Radiographic projections-axial views of the skulls and lateral views of hemimandibles-were taken. Cephalometric evaluations were performed, and the values obtained were submitted to statistical analyses. There was a significant homolateral difference in the length of the premaxilla, height of the mandibular ramus and body, and the length of the mandible in all three groups. However, comparisons among the groups revealed no significant differences between the detachment and coronoidotomy groups for most measurements. It was concluded that both experimental detachment of the temporal muscle and coronoidotomy during the growth period in rats induced asymmetry of the mandible and affected the premaxilla.

The aim of this article is to present the pediatric dentistry and orthodontic treatment protocol of rehabilitation of cleft lip and palate patients performed at the Hospital for Rehabilitation of Craniofacial Anomalies - University of So Paulo (HRAC-USP). Pediatric dentistry provides oral health information and should be able to follow the child with cleft lip and palate since the first months of life until establishment of the mixed dentition, craniofacial growth and dentition development. Orthodontic intervention starts in the mixed dentition, at 8-9 years of age, for preparing the maxillary arch for secondary bone graft procedure (SBGP). At this stage, rapid maxillary expansion is performed and a fixed palatal retainer is delivered before SBGP. When the permanent dentition is completed, comprehensive orthodontic treatment is initiated aiming tooth alignment and space closure. Maxillary permanent canines are commonly moved mesially in order to substitute absent maxillary lateral incisors. Patients with complete cleft lip and palate and poor midface growth will require orthognatic surgery for reaching adequate anteroposterior interarch relationship and good facial esthetics.

Placement of a single-tooth implant should be performed when a patient's facial growth has ceased. In this retrospective observational study, we evaluated if there was a difference in the timing of cessation of craniofacial growth in short, average, and long facial types. Based on the value of the angle between cranial base and mandibular plane (SN/MP angle), three groups comprising 48 subjects with short facial type (SF; SN/MP ≤28°), 77 with average facial type (AF; SN/MP ≥31.5° and ≤34.5°), and 44 with long facial type (LF; SN/MP ≥38°) were selected. Facial growth was assessed on lateral cephalograms taken at 15.4 years of age, and 2, 5, and 10 years later. Variables were considered to be stable when the difference between two successive measurements was less than 1 mm or 1°. We found no difference between facial types in the timing of cessation of facial growth. Depending on the variable, the mean age when variables became stable ranged from 18.0 years (Is-Pal in LF group) to 22.0 years (SN/MP in LF group). However, facial growth continued at the last follow-up in approximately 20% subjects. This study demonstrates that facial type is not associated with the timing of cessation of facial growth.

Growth hormone (GH) stimulates mandibular growth but its effect on the mandibular condylar cartilage is not well. understood. Objective: This study was designed to understand the influence of GH on mitotic activity and on chondrocytes maturation. The effect of GH on cartilage thickness was also determined. Design: An animal model witt differences in GH status was determined by comparing mutant Lewis dwarf rats with reduced pituitary GH synthesis (dwarf), with normal rats and dwarf animals treated with GH. Six dwarf rats were injected with GH for 6 days, while other six normal rats and six dwarf rats composed other two groups. Mandibular condylar tissues were processed and stained for Herovici's stain and immunohistochemistry, for proliferating cell nuclear antigen (PCNA) and alkaline phosphatase (ALP). Measurements of cartilage thickness as well as the numbers of immunopositive cells for each antibody were analysed by one-way analysis of variance. Results: Cartilage thickness was significantly reduced in the dwarf animals treated with GH. PCNA expression was significant lower in the dwarf rats, but significantly increased when these animals were treated with GH. ALP expression was significant higher in the dwarf animals, while it was significantly reduced in the dwarf animals treated with GH. Conclusions: The results from this study showed that GH stimulates mitotic activity and delays cartilage cells maturation in the mandibular condyte. This effect at the cellular Level may produce changes in the cartilage thickness. (C) 2004 Elsevier Ltd. All rights reserved.

Introduction : La scaphocéphalie est la craniosynostose monosuturaire la plus commune (1/2000). Celle-ci est causée par la fusion prématurée de la suture sagittale. Une chirurgie corrective de la voûte crânienne peut être effectuée dans la première année de vie de l’enfant. Il n’existe actuellement aucune donnée précise dans la littérature scientifique étudiant l’occlusion chez les patients scaphocéphales, ainsi que les impacts potentiels de la chirurgie de la voûte crânienne sur celle-ci. Objectifs : L’objectif primaire de cette étude est de décrire et comparer la malocclusion dento-squelettique d’un groupe de patients scaphocéphales à une population pédiatrique normale. L’objectif secondaire est d’évaluer la différence au niveau de l’occlusion entre un sous-groupe de patients scaphocéphales ayant eu une chirurgie corrective de la voûte crânienne et un sous-groupe ne l’ayant pas eu. Méthodologie : Quatre-vingt-onze patients scaphocéphales (2-11 ans; 71 garçons) de la banque de données de la Clinique de Craniofacial du CHU Ste-Justine ont formé le groupe expérimental. Tous les patients ont eu un examen orthodontique complet et ont été suivis. Parmi ceux-ci, quarante-quatre avaient eu une chirurgie corrective de la voûte crânienne et quarante-sept n’en avaient pas eu, mais étaient suivis régulièrement à la clinique. Trente-huit (33 garçons; 17 opérés) de ces patients ont eu des radiographies céphalométriques latérales et parmi ceux-ci, un certain nombre ont reçus des suivis de croissance radiologiques. Résultats : Les valeurs cliniques de la classification dentaire, ainsi que la mesure du surplomb horizontal, ont indiqué une augmentation de la prévalence de malocclusions de classe II chez les enfants scaphocéphales. Par contre, les valeurs céphalométriques indicatrices de malocclusion squelettique de classe II (ex. : N-A perp HP, N-B perp HP, N-Pog perp HP, Wits, N-A-Pog) sont demeurées dans les limites de la normale. Certaines valeurs céphalométriques présentent une différence statistiquement significative entre les patients opérés et non opérés (ANS-PNS t2, p=0.025; /1-FH t2, p=0.028), mais ces variations individuelles ne sont pas reliées à la scaphocéphalie. Conclusion : Les enfants scaphocéphales présentent cliniquement davantage de malocclusions de classe II que les enfants normaux. Par contre, les valeurs radiologiques antéro-postérieures et transverses demeurent dans les limites de la normale. La chirurgie corrective de la voûte crânienne n’affecte également pas l’occlusion chez ces patients.

OBJETIVO: Apontar as possíveis alterações orofaciais decorrentes do sintoma obstrução nasal em pacientes portadores de doenças alérgicas de vias aéreas superiores, por meio de revisão de literatura. FONTES DE DADOS: Levantamento bibliográfico utilizando bancos de dados eletrônicos, como Medline, Ovid, SciELO e Lilacs, com as palavras-chave asthma, rhinitis e mouth breathing, abrangendo os 30 últimos anos. Foram incluídos artigos de revisão, estudos observacionais e ensaios clínicos. SÍNTESE DOS DADOS: A obstrução nasal é encontrada freqüentemente em doenças alérgicas de vias aéreas, como rinite e asma. A respiração bucal decorrente da obstrução nasal pode interferir de maneira direta no desenvolvimento infantil, com alterações no crescimento do crânio e orofacial, na fala, na alimentação, na postura corporal, na qualidade do sono e no desempenho escolar. CONCLUSÕES: Devido à variedade de alterações orofaciais encontradas na criança respiradora bucal decorrente de obstrução nasal por doenças alérgicas de vias aéreas, é necessário realizar diagnóstico e tratamento precoces por uma equipe multidisciplinar, composta por médico, ortodontista e fonoaudiólogo, contemplando a visão de uma via respiratória única, que traz conseqüências ao crescimento e desenvolvimento do sistema motor oral.

Dentre os diversos meios de se determinar o grau de maturação esquelética do paciente destaca-se o Método de Lamparski modificado por Hassel e Farman, em 1995, que propuseram a identificação do estágio da maturação por meio das modificações anatômicas das 2ª, 3ª e 4ª vértebras cervicais. Cientes das qualidades advogadas ao método citado surgiu o interesse em avaliar sua reprodutibilidade com o intuito de divulgá-lo e incorporá-lo como um elemento no diagnóstico e auxiliar no prognóstico dos tratamentos das más oclusões. A amostra constou de 100 telerradiografias em norma lateral de pacientes triados para tratamento ortodôntico na Faculdade de Odontologia de Araçatuba - UNESP nos períodos de 2000 e 2001. Foram incluídos pacientes de ambos os gêneros na faixa etária de 6 a 16 anos e a média de 9 anos e 7 meses. Três examinadores devidamente calibrados realizaram a avaliação das radiografias classificando-as em escores de 1 a 6. Após a análise dos resultados, os mesmos foram tabulados e submetidos ao coeficiente Kappa de concordância para avaliação inter e intra-examinador concluindo, dessa forma, a reprodutibilidade do referido método. O método de determinação da maturação esquelética por meio das vértebras cervicais mostrou-se reproduzível na avaliação do estágio em que o indivíduo se encontra na curva de crescimento.
