Per avaluar la utilitat dels biomarcadors en el diagnòstic etiològic de l’ictus agut, es van estudiar pacients amb ictus isquèmic agut no lacunar, en els quals es va fer una exploració cardiològica i es van determinar els biomarcadors plasmàtics seleccionats (CKMB, pèptid natriurètic cerebral(BNP), D-dímer, mioglobina i troponina I). Els pacients amb fibril•lació auricular i els pacients amb patologia embòlica en l’estudi amb ecocardiograma transtoràcic(ETT) van mostrar nivells més elevats de CKMB i BNP respecte els pacients amb ritme sinusal i estudi ETT normal, respectivament. Després de la regressió logística, CKMB i BNP es mantenien com a predictors independents de font embòlica.

Background: The aim of this study was to identify novel candidate biomarker proteins differentially expressed in the plasma of patients with early stage acute myocardial infarction (AMI) using SELDI-TOF-MS as a high throughput screening technology. Methods: Ten individuals with recent acute ischemic-type chest pain (< 12 h duration) and ST-segment elevation AMI (1STEMI) and after a second AMI (2STEMI) were selected. Blood samples were drawn at six times after STEMI diagnosis. The first stage (T(0)) was in Emergency Unit before receiving any medication, the second was just after primary angioplasty (T(2)), and the next four stages occurred at 12 h intervals after T(0). Individuals (n = 7) with similar risk factors for cardiovascular disease and normal ergometric test were selected as a control group (CG). Plasma proteomic profiling analysis was performed using the top-down (i.e. intact proteins) SELDI-TOF-MS, after processing in a Multiple Affinity Removal Spin Cartridge System (Agilent). Results: Compared with the CG, the 1STEMI group exhibited 510 differentially expressed protein peaks in the first 48 h after the AMI (p < 0.05). The 2STEMI group, had similar to 85% fewer differently expressed protein peaks than those without previous history of AMI (76, p < 0.05). Among the 16 differentially-regulated protein peaks common to both STEMI cohorts (compared with the CG at T(0)), 6 peaks were persistently down-regulated at more than one time-stage, and also were inversed correlated with serum protein markers (cTnI, CK and CKMB) during 48 h-period after IAM. Conclusions: Proteomic analysis by SELDI-TOF-MS technology combined with bioinformatics tools demonstrated differential expression during a 48 h time course suggests a potential role of some of these proteins as biomarkers for the very early stages of AMI, as well as for monitoring early cardiac ischemic recovery. Background, Cardiac complications occur commonly in vascular surgery patients. Diagnosis of cardiac complications is difficult because of the inaccuracies associated with traditional cardiac enzyme measurements. CTi, a highly sensitive and specific marker of myocardial injury, may be able to detect cardiac complications with greater ease and accuracy. Methods. The study prospectively examined 100 consecutive patients who underwent major vascular surgery between 6/7/98 and 31/12/98 at the Royal Brisbane Hospital. Daily measurements of cTi, creatine kinase (CK), creatine kinase MB (CKMB), CKMB index, renal function and haemoglobin were taken for three postoperative days. One postoperative electrocardiograph (ECC) was taken. An extensive cardiac history was taken. Intraoperative and postoperative events were recorded. Findings. There were 100 patients, 18 patients (18%) had a cTi elevation. On the basis of classical diagnostic criteria, 15 patients (15%) suffered one or more cardiac complication (either myocardial infarction, congestive cardiac failure, unstable angina or atrial fibrillation), One patient (1%) who had a cTi elevation died. CTI elevation occurred in five patients (5%) who were not diagnosed with cardiac complications based on traditional criteria. Despite not meeting specific diagnostic criteria for cardiac complications, all patients showed signs and symptoms that could be attributed to myocardial ischaemia, Every patient who developed congestive cardiac failure or atrial fibrillation had a cTi elevation. A Chi-square analysis revealed a significant association between cTi elevation and postoperative cardiac complications. Four variables contributed small but significant amounts of unique variance to the prediction of peak cTi on linear regression analysis. These were peak CKMB index, postoperative congestive cardiac failure, postoperative chest pain and postoperative cardiac complications. Conclusions. Routine cTi monitoring of postoperative vascular patients would be an effective and inexpensive way to detect patients with cardiac complications. The relationship between postoperative cTi elevation and significant coronary artery disease remains to be shown, (C) 2001 The international Society for Cardiovascular Surgery. This work describes a novel use for the polymeric film, poly(o-aminophenol) (PAP) that was made responsive to a specific protein. This was achieved through templated electropolymerization of aminophenol (AP) in the presence of protein. The procedure involved adsorbing protein on the electrode surface and thereafter electroploymerizing the aminophenol. Proteins embedded at the outer surface of the polymeric film were digested by proteinase K and then washed away thereby creating vacant sites. The capacity of the template film to specifically rebind protein was tested with myoglobin (Myo), a cardiac biomarker for ischemia. The films acted as biomimetic artificial antibodies and were produced on a gold (Au) screen printed electrode (SPE), as a step towards disposable sensors to enable point-of-care applications. Raman spectroscopy was used to follow the surface modification of the Au-SPE. The ability of the material to rebind Myo was measured by electrochemical techniques, namely electrochemical impedance spectroscopy (EIS) and square wave voltammetry (SWV). The devices displayed linear responses to Myo in EIS and SWV assays down to 4.0 and 3.5 μg/mL, respectively, with detection limits of 1.5 and 0.8 μg/mL. Good selectivity was observed in the presence of troponin T (TnT) and creatine kinase (CKMB) in SWV assays, and accurate results were obtained in applications to spiked serum. The sensor described in this work is a potential tool for screening Myo in point-of-care due to the simplicity of fabrication, disposability, short time response, low cost, good sensitivity and selectivity.

A novel reusable molecularly imprinted polymer (MIP) assembled on a polymeric layer of carboxylated poly(vinyl chloride) (PVCsingle bondCOOH) for myoglobin (Myo) detection was developed. This polymer was casted on the gold working area of a screen printed electrode (Au-SPE), creating a novel disposable device relying on plastic antibodies. Electrochemical impedance spectroscopy (EIS), cyclic voltammetry (CV) and Fourier transform infrared spectroscopy (FTIR) studies confirmed the surface modification. The MIP/Au-SPE devices displayed a linear behaviour in EIS from 0.852 to 4.26 μg mL−1, of positive slope 6.50 ± 1.48 (kΩ mL μg−1). The limit of detection was 2.25 μg mL−1. Square wave voltammetric (SWV) assays were made in parallel and showed linear responses between 1.1 and 2.98 μg mL−1. A current decrease was observed against Myo concentration, producing average slopes of −0.28 ± 0.038 μA mL μg−1. MIP/Au-SPE also showed good results in terms of selectivity. The error% found for each interfering species were 7% for troponin T (TnT), 11% for bovine serum albumin (BSA) and 2% for creatine kinase MB (CKMB), respectively. Overall, the technical modification over the Au-SPE was found a suitable approach for screening Myo in biological fluids.

PURPOSE: To evaluate the efficacy of a systematic model of care for patients with chest pain and no ST segment elevation in the emergency room. METHODS: From 1003 patients submitted to an algorithm diagnostic investigation by probability of acute ischemic syndrome. We analyzed 600 ones with no elevation of ST segment, then enrolled to diagnostic routes of median (route 2) and low probability (route 3) to ischemic syndrome. RESULTS: In route 2 we found 17% acute myocardial infarction and 43% unstable angina, whereas in route 3 the rates were 2% and 7%, respectively. Patients with normal/non--specific ECG had 6% probability of AMI whereas in those with negative first CKMB it was 7%; the association of the 2 data only reduced it to 4%. In patients in route 2 the diagnosis of AMI could only be ruled out with serial CKMB measurement up to 9 hours, while in route 3 it could be done in up to 3 hours. Thus, sensitivity and negative predictive value of admission CKMB for AMI were 52% and 93%, respectively. About one-half of patients with unstable angina did not disclose objective ischemic changes on admission. CONCLUSION: The use of a systematic model of care in patients with chest pain offers the opportunity of hindering inappropriate release of patients with ACI and reduces unnecessary admissions. However some patients even with normal ECG should not be released based on a negative first CKMB. Serial measurement of CKMB up to 9 hours is necessary in patients with medium probability of AMI.

OBJETIVO: Verificar o efeito prognóstico das alterações de quatro marcadores de lesão miocárdica (CKMB atividade/massa e troponinas T e I) pós- stent em relação à ocorrência de morte, infarto e novos procedimentos de revascularização em um ano e determinar a incidência e as variáveis preditoras de suas elevações. MÉTODOS: Em 199 pacientes tratados por stents em artérias naturais foram mensurados os quatro marcadores em três períodos: pré-procedimento, 6-8h e 14-18h pós-stent. Curvas de sobrevivência e a regressão logística de Cox determinaram o impacto prognóstico das alterações na ocorrência de eventos cardíacos em um ano. RESULTADOS: A incidência das alterações dos 4 marcadores pós-stent é relativamente freqüente (6,1% a 32,8%), associa-se à maior complexidade das lesões tratadas (longas e angulações >45º) e ao desenvolvimento de intercorrências durante o procedimento (oclusão de ramos secundários, presença de angina e alterações eletrocardiográficas). A sobrevivência livre de infarto e nova revascularização foi significativamente menor somente nos pacientes com elevação da CKMB atividade pós-procedimento em comparação àqueles sem esta alteração (60% x 85,08%, p=0,025). Influenciaram a sobrevivência livre de eventos o diabetes mellitus (OR: 2,27, p=0,0256), pré-dilatação com balão (OR: 3,16, p=0,0082) e elevação da CKMB atividade pós-procedimento (OR: 3,64, p=0,0162). CONCLUSÃO: A recomendação da monitoração sistemática da CKMB atividade pós-stent coronário baseia-se na compreensão do seu comportamento clínico e laboratorial, nos resultados dos estudos que relacionam a elevação de seus níveis pós-procedimento à pior evolução tardia e ao baixo custo do exame.

OBJETIVO: Avaliar, através da evolução dos novos marcadores bioquímicos de injúria cardíaca, se a cardioversão elétrica (CVE) causa lesão miocárdica. MÉTODOS: Foram avaliados 76 pacientes (P) submetidos a CVE eletiva de fibrilação atrial ou flutter atrial. Medidas de creatinafosfoquinase (CPK), CKMB-atividade e dosagem de CKMB-massa (M), mioglobina e troponina I cardíaca (cTnI) foram determinadas antes e após 6 e 24 horas da CVE. RESULTADOS: A CVE resultou um sucesso em 58 P (76,3%). A carga cumulativa (CC) foi de até 350 joules (J) em 36 P, de 500 a 650 J em 20 P e de 900 a 960 J em 20 P, com energia média aplicada de 493 J (± 309). A cTnI permaneceu dentro da normalidade nos 76 P. Com o aumento da CC, ocorreu elevação de CPK (> valor de p = 0,007), CKMB-atividade (> valor de p = 0,002), CKMB-M (> valor de p = 0,03) e mioglobina (> valor de p = 0,015). Correlação positiva foi observada entre a CC e picos de CPK (r = 0,660; p < 0,001), CKMB-atividade (r = 0,429; p < 0,0001), CKMB-M (r = 0,265; p = 0,02) e mioglobina (r = 0,684; p < 0,0001). Correlação também positiva ocorreu entre o número de choques e picos de CPK (r = 0,770; p < 0,001), CKMB-atividade (r = 0,642; p < 0,0001), CKMB-M (r = 0,430; p < 0,0001) e mioglobina (r = 0,745; p < 0,0001). CONCLUSÃO: A CVE não causa lesão miocárdica detectável pela dosagem da cTnI. Elevações de CPK, CKMB-atividade, CKMB-M e mioglobina são decorrentes de lesão do músculo esquelético, estando correlacionadas positivamente com a CC aplicada ou com o número de choques.

OBJETIVO: Testar os valores diagnóstico e prognóstico imediatos da proteína C-reativa (PCR) nos pacientes admitidos na sala de emergência (SE) com dor torácica (DT) e sem elevação do segmento ST no eletrocardiograma (ECG). MÉTODOS: De janeiro de 2002 a dezembro de 2003, 980 pacientes consecutivos foram atendidos com DT suspeita de síndrome coronariana aguda na SE (idade = 64,9 ± 14,3 anos, homens = 55%, diabéticos = 18%, ECG normal = 84%). Dosou-se a PCR na admissão, a creatinofosfoquinase MB fração massa (CKMB) e a troponina I seriadas, além de se registrar ECG seriados. As medidas da PCR foram padronizadas (PCR-p) pelo valor do limite superior da normalidade (LSN) do teste utilizado (3,0 mg/L para a PCR de alta sensibilidade-PCR-AS e 0,1 mg/dl para PCR titulada-PCR-t). RESULTADOS: Foi diagnosticado infarto agudo do miocárdio (IAM) em 125 pacientes, e seus valores para a PCR-p foram 1,31 ± 2,90 (mediana = 0,47) versus 0,79 ± 1,39 (0,30) nos sem IAM (p = 0,031). A PCR-p > 1,0 apresentou sensibilidade de 30%, especificidade de 80,4%, valores preditivos positivo e negativo de 6,1% e de 96,7%, para o diagnóstico de IAM. Houve quarenta eventos cardíacos intra-hospitalares (óbitos = dezesseis, revascularizações de urgência = 22, IAM = dois). No 1º quartil da PCR-p (< 0,10) registraram-se três eventos, enquanto no 4º quartil (> 0,93) ocorreram quinze eventos (p = 0,003). Na regressão logística foram preditores independentes para eventos cardíacos a insuficiência ventricular esquerda, o sexo masculino e a PCR-p > 0,32, com razão de chances de 7,6, 2,8 e 2,2, respectivamente. CONCLUSÃO: Nos pacientes atendidos com DT na SE, a PCR-p: 1) Não foi um bom marcador de IAM, apesar de um valor normal praticamente afastar esse diagnóstico; 2) Um valor superior a um terço do seu limite superior da normalidade (LSN) (>1 mg/L da PCR-AS ou >0,33 mg/dl da PCR-t) foi preditor de eventos cardíacos adversos intra-hospitalares.

FUNDAMENTO: O infarto do miocárdio perioperatório (IMPO) é uma complicação da cirurgia de revascularização miocárdica (CRM) com potencial impacto prognóstico. A cintilografia miocárdica (CM) com pirofosfato marcado com tecnécio-99m é utilizada no diagnóstico de IMPO, mas demonstra limitada sensibilidade para lesões subendocárdicas. A ressonância magnética cardiovascular (RMC), por sua vez, detém alta acurácia para a detecção de necrose miocárdica. OBJETIVO: Comparar a RMC e a CM para a detecção de IMPO após CRM. MÉTODOS: Foram estudados 24 pacientes portadores de doença arterial coronária crônica, com a técnica de realce tardio pela RMC e com a CM, antes e depois da CRM, analisando-se o surgimento de áreas de necrose miocárdica perioperatória (IMPO). Mensuraram-se também marcadores bioquímicos de lesão miocárdica (CKMB e troponina I), antes e depois da cirurgia. RESULTADOS: Dezenove pacientes completaram o estudo. Desses, 6 (32%) apresentaram IMPO à RMC, e 4 (21%) à CM (p = NS). Dos 323 segmentos do ventrículo esquerdo avaliados, 17 (5,3%) exibiram necrose perioperatória à RMC, e 7 (2,2%) à CM (p = 0,013). Observou-se moderada concordância entre os métodos (kappa = 0,46), havendo divergência, quanto ao diagnóstico de IMPO, em 4 (21%) casos, a maioria com pequenas áreas de necrose perioperatória à RMC, não visualizadas à CM. Em todos os casos com IMPO à RMC, houve elevação significativa de CKMB e troponina I. CONCLUSÃO: Houve moderada concordância diagnóstica entre os métodos para a detecção de IMPO, mas a RMC permitiu a visualização de pequenas áreas de necrose miocárdica perioperatória, não identificadas pela CM e associadas à elevação de marcadores bioquímicos de lesão miocárdica.

Doxorubicin (DOX) is an anthracycline antibiotic with a broad antitumor spectrum. However, the clinical use of DOX is limited because of its cardiotoxicity, a dose-dependent effect. Colloidal drug delivery systems, such as microemulsions (MEs), allow the incorporation of drugs, modifying the pharmacokinetic (PK) profile and toxic effects. In this study, we evaluated the PK profile and cardiotoxicity of a new DOX ME (DOX-ME). The PK profile of DOX-ME was determined and compared with that of the conventional DOX after single-dose administration (6mg/kg, intravenous) in male Wistar rats (n = 12 per group). The cardiotoxicity of DOX formulations was evaluated by serum creatine kinase MB (CKMB) activity in both animal groups before and after drug administration. The plasma DOX measurements were performed by high-performance liquid chromatography with fluorescence detection, and the CKMB levels were assayed using the CKMB Labtest® kit. The ME system showed a significant increase in plasma DOX concentrations and lower distribution volume when compared with conventional DOX. Serum CKMB activity increased after conventional DOX administration but was unchanged in the DOX-ME group. These results demonstrate modifications in drug access to susceptible sites using DOX-ME. DOX-ME displayed features that make it a promising system for future therapeutic application. © 2012 Wiley Periodicals, Inc.
