OBJECTIVES: To compare the efficacy and safety of infliximab-biosimilar with other biological drugs for the treatment of active ankylosing spondylitis (AS). METHODS: Systematic literature review for randomized controlled trials (RCTs) with adalimumab, etanercept, golimumab, infliximab and infliximab-biosimilar in AS was performed and indirect meta-analysis (Bayesian mixed treatment comparison) was carried out. The proportion of patients reaching 20 % improvement by the assessment of Spondyloarthritis International Society response criteria (ASAS20) at weeks 12 and 24 was used as efficacy endpoints, and the occurrence of serious adverse events at week 24 was applied to compare the safety of the biologicals. RESULTS: Altogether, 13 RCTs, identified by the systematic literature search, were included in the analysis. Results on the ASAS20 efficacy endpoint were reported for week 12 in 12 RCTs involving 2,395 patients, and for week 24 in 5 RCTs comprising 1,337 patients. All the five biological agents proved to be significantly superior to placebo. Infliximab showed the highest odds ratio (OR) of 7.2 (95 % CI 3.68-13.19) compared to placebo, followed by infliximab-biosimilar with OR 6.25 (95 % CI 2.55-13.14), both assessed at week 24. No significant difference was found between infliximab-biosimilar and other biological treatments regarding their efficacy and safety. CONCLUSIONS: This is the first study which includes a biosimilar drug in the meta-analysis of biological treatments in AS. The results have proven the similar efficacy and safety profile of infliximab-biosimilar treatment compared to other biologicals.

OBJECTIVE: The aim of this meta-analysis was to compare the efficacy and safety of infliximab-biosimilar and other available biologicals for the treatment of rheumatoid arthritis (RA), namely abatacept, adalimumab, certolizumab pegol, etanercept, golimumab, infliximab, rituximab and tocilizumab. METHODS: A systematic literature review of MEDLINE database until August 2013 was carried out to identify relevant randomized controlled trials (RCTs). Bayesian mixed treatment comparison method was applied for the pairwise comparison of treatments. Improvement rates by the American College of Rheumatology criteria (ACR20 and ACR50) at week 24 were used as efficacy endpoints, and the occurrence of serious adverse events was considered to assess the safety of the biologicals. RESULTS: Thirty-six RCTs were included in the meta-analysis. All the biological agents proved to be superior to placebo. For ACR20 response, certolizumab pegol showed the highest odds ratio (OR) compared to placebo, OR 7.69 [95 % CI 3.69-14.26], followed by abatacept OR 3.7 [95 % CI 2.17-6.06], tocilizumab OR 3.69 [95 % CI 1.87-6.62] and infliximab-biosimilar OR 3.47 [95 % CI 0.85-9.7]. For ACR50 response, certolizumab pegol showed the highest OR compared to placebo OR 8.46 [3.74-16.82], followed by tocilizumab OR 5.57 [95 % CI 2.77-10.09], and infliximab-biosimilar OR 4.06 [95 % CI 1.01-11.54]. Regarding the occurrence of serious adverse events, the results show no statistically significant difference between infliximab-biosimilar and placebo, OR 1.87 [95 % CI 0.74-3.84]. No significant difference regarding efficacy and safety was found between infliximab-biosimilar and the other biological treatments. CONCLUSION: This is the first indirect meta-analysis in RA that compares the efficacy and safety of biosimilar-infliximab to the other biologicals indicated in RA. We found no significant difference between infliximab-biosimilar and other biological agents in terms of clinical efficacy and safety.

Objective: The objective of the study is to explore preferences of gastroenterologists for biosimilar drugs in Crohn’s Disease and reveal trade-offs between the perceived risks and benefits related to biosimilar drugs. Method: Discrete choice experiment was carried out involving 51 Hungarian gastroenterologists in May, 2014. The following attributes were used to describe hypothetical choice sets: 1) type of the treatment (biosimilar/originator) 2) severity of disease 3) availability of continuous medicine supply 4) frequency of the efficacy check-ups. Multinomial logit model was used to differentiate between three attitude types: 1) always opting for the originator 2) willing to consider biosimilar for biological-naïve patients only 3) willing to consider biosimilar treatment for both types of patients. Conditional logit model was used to estimate the probabilities of choosing a given profile. Results: Men, senior consultants, working in IBD center and treating more patients are more likely to willing to consider biosimilar for biological-naïve patients only. Treatment type (originator/biosimilar) was the most important determinant of choice for patients already treated with biologicals, and the availability of continuous medicine supply in the case biological-naïve patients. The probabilities of choosing the biosimilar with all the benefits offered over the originator under current reimbursement conditions are 89% vs 11% for new patients, and 44% vs 56% for patients already treated with biological. Conclusions: Gastroenterologists were willing to trade between perceived risks and benefits of biosimilars. The continuous medical supply would be one of the major benefits of biosimilars. However, benefits offered in the scenarios do not compensate for the change from the originator to the biosimilar treatment of patients already treated with biologicals.

Despite the realisation of the potential implications from biosimilars is relatively recent, much has already been written about raising the awareness of differences between biosimilars and originating/ reference listed (innovator) pharmaceuticals. The European Medicines Agency has led the global charge in regulating biosimilars. Regardless of sufficient similarities across international regulations, differences do exist across jurisdictions. The consideration of regulating biosimilars demands a congruent approach across all stages: pre-registration (Australian copyright protection, patent, international obligations), registration (confidential information, international regulators, safety and efficacy), post-registration (Pharmaceutical Benefit Scheme, prescriber and dispenser awareness). Our National Medicines Policy could provide the necessary congruent framework and function for national and international regulation of biosimilars. The Policy concedes that pharmaceuticals will be affected by financial policies and trade considerations, international treaty obligations, industrial policies, education policies and the need for public-private partnerships.

The biosimilars market is potentially the single fastest growing pharmaceutical sector with an estimated worth of US$67bn in global sales by 2020. This market generally refers to larger molecule, biological, protein-based pharmaceuticals which have lost its patent. This has stimulated the emergence of non-conventional pharmaceutical investors such as Fujifilm and Samsung as well as host countries such as Brazil, Mexico, China, India, South Korea, Turkey and Russia, which view biosimilars as a key macroeconomic driver of growth. Internationally, the European Medicines Agency has led the regulation of the quality, safety and efficacy of biosimilars; however, many countries have developed their own biosimilar regulatory frameworks. Despite the similarity of these with European guidelines, differences do exist across jurisdictions and have implications for cross-jurisdictional registration and regulation. The consideration of biosimilar regulation, however, demands attention beyond quality, safety and efficacy. The potential implications of extended patent protection, international trade and globalisation require a congruent policy approach to their regulation. Notwithstanding the fact that Australia is a relatively small pharmaceutical market and that there are only 14 biosimilar products currently approved for use, Australia’s geographical proximity to pharm-emerging countries and its trade relation with the major pharmaceutical markets have positioned Australia in a unique position to influence international development and regulation of biosimilars. Australia’s National Medicines Policy (2000) potentially provides the foundation for a partnership approach to biosimilar regulation, minimise duplication of regulatory efforts while at the same time fostering a viable pharmaceutical industry.

Australia currently has a small generic and biosimilar industry despite having a good track record in biomedical research and a sound reputation in producing high quality but small volume biological pharmaceuticals. In recent times, Australia has made incremental changes to its regulation of biosimilars – in patent registration, in the use of commercial confidential information, and in remuneration. These improvements, together with Australia’s geographical proximity and strong trade relationship with the Asian biocluster have positioned Australia to take advantage of potential public cost savings from the increased use of biosimilars.

Malgré le progrès technologique et nos connaissances pharmaceutiques et médicales croissantes, le développement du médicament demeure un processus difficile, dispendieux, long et très risqué. Ce processus mérite d'être amélioré pour faciliter le développement de nouveaux traitements. À cette fin, cette thèse vise à démontrer l’utilité de principes avancés et d’outils élaborés en pharmacocinétique (PK), actuels et nouveaux. Ces outils serviront à répondre efficacement à des questions importantes lors du développement d’un médicament, sauvant ainsi du temps et des coûts. Le premier volet de la thèse porte sur l’utilisation de la modélisation et des simulations et la création d’un nouveau modèle afin d’établir la bioéquivalence entre deux formulations de complexe de gluconate ferrique de sodium en solution de sucrose pour injection. Comparé aux méthodes courantes, cette nouvelle approche proposée se libère de plusieurs présuppositions, et requiert moins de données. Cette technique bénéficie d’une robustesse scientifique tout en étant associée à des économies de temps et de coûts. Donc, même si développé pour produits génériques, elle pourra également s’avérer utile dans le développement de molécules innovatrices et « biosimilaires ». Le deuxième volet décrit l’emploi de la modélisation pour mieux comprendre et quantifier les facteurs influençant la PK et la pharmacodynamie (PD) d’une nouvelle protéine thérapeutique, la pegloticase. L’analyse a démontré qu’aucun ajustement posologique n’était nécessaire et ces résultats sont inclus dans la monographie officielle du produit. Grâce à la modélisation, on pouvait répondre à des questions importantes concernant le dosage d’un médicament sans passer par des nouvelles études ni d'évaluations supplémentaires sur les patients. Donc, l’utilisation de cet outil a permis de réduire les dépenses sans prolonger le processus de développement. Le modèle développé dans le cadre de cette analyse pourrait servir à mieux comprendre d’autres protéines thérapeutiques, incluant leurs propriétés immunogènes. Le dernier volet démontre l’utilité de la modélisation et des simulations dans le choix des régimes posologiques d’un antibiotique (TP-434) pour une étude de Phase 2. Des données provenant d’études de Phase 1 ont été modélisées au fur et à mesure qu’elles devenaient disponibles, afin de construire un modèle décrivant le profil pharmacocinétique du TP-434. Ce processus de modélisation exemplifiait les cycles exploratoires et confirmatoires décrits par Sheiner. Ainsi, en se basant sur des relations PK/PD d’un antibiotique de classe identique, des simulations ont été effectuées avec le modèle PK final, afin de proposer de nouveaux régimes posologiques susceptibles d’être efficace chez les patients avant même d'effectuer des études. Cette démarche rationnelle a mené à l’utilisation de régimes posologiques avec une possibilité accrue d’efficacité, sans le dosage inutile des patients. Ainsi, on s’est dispensé d’études ou de cohortes supplémentaires coûteuses qui auraient prolongé le processus de développement. Enfin, cette analyse est la première à démontrer l’application de ces techniques dans le choix des doses d’antibiotique pour une étude de Phase 2. En conclusion, cette recherche démontre que des outils de PK avancés comme la modélisation et les simulations ainsi que le développement de nouveaux modèles peuvent répondre efficacement et souvent de manière plus robuste à des questions essentielles lors du processus de développement du médicament, tout en réduisant les coûts et en épargnant du temps.

Los productos bioterapéuticos han revolucionado la terapéutica de enfermedades como la artritis reumatoide (AR). Los altos costos y el vencimiento de las patentes de productos bioterapéuticos de referencia (innovadores) han promovido el interés en productos bioterapéuticos similares (PBS), también conocidos como biosimilares. METODOLOGÍA: revisión sistemática con el objetivo de presentar el panorama actual (a Octubre del 2012) de los PBS en Latinoamérica (LA) en el contexto de AR. Consulta de: bases de datos internacionales y latinoamericanas, sitios web gubernamentales, registros de ensayos clínicos, noticias y páginas web relevantes. Se siguieron las guías PRISMA. RESULTADOS: Bases de datos: Selección de 30 artículos según criterios de inclusión. Selección de 17 artículos según revisión detallada. Búsqueda manual: selección de 45 referencias. Consolidado de 62 documentos. Regulaciones vigentes sobre PBS: México, Cuba, Guatemala, Costa Rica, Panamá, Venezuela, Perú, Brasil, Chile y Argentina. En Colombia se encontró una propuesta en desarrollo. PBS aprobados: Colombia (Etanar®, PBS de Etanercept), México (Kikuzubam®, PBS de Rituximab), Perú, Chile y Bolivia (En estos últimos 3 países, Reditux®, PBS de Rituximab). DISCUSIÓN: La situación regulatoria sobre PBS en LA ha mejorado en los últimos 6 años, sin embargo las regulaciones deben buscar mayor claridad. Deben desarrollarse esfuerzos para establecer regulaciones en los países de LA que no las tienen. Países como México, Cuba y Brasil tienen una industria biotecnológica creciente. La aprobación de PBS en LA ha generado controversia y opiniones divididas entre los diferentes actores. El ingreso de PBS es beneficioso pero debe seguir regulaciones claras que garanticen eficacia y seguridad.

In Brazil, the registration of new drugs is carried out only when the regulatory agency (Anvisa, acronym in Portuguese) is fully satisfied with the evidence of their quality, efficacy and safety, presented by a pharmaceutical industry that strive for this registration. With the patent expiration, pharmaceutical companies are attracted to produce biological medicines called biosimilar or biogenerics or simply generics, whose approval may result in reduced treatment costs. But it is necessary that the biosimilar be, at least, equally efective and safe and without contaminants in relation to the original. Recent consensus guidelines aim to establish criteria for efcacy and safety of these medicines. Preclinical studies in vitro and in vivo, the origin of raw materials and clinical studies phase I, II and III are recommended for biosimilar medicine registration in the international market. Low molecular weight heparins are found in this situation. In this review we specifcally addressed this type of medicine, which could serve as a benchmark for other biosimilar medicines.

A protocol to produce large amounts of bioactive homogeneous human interferon β1 expressed in Escherichia coli was developed. Human interferon β1 ser17 gene was constructed, cloned and subcloned, and the recombinant protein expressed in E. coli cells. Solubilization of recombinant human interferon β1 ser17 (rhIFN-β1 ser17) was accomplished by employing a brief shift to high alkaline pH in the presence of non-ionic detergent. The recombinant protein was purifi ed by three chromatographic steps. N-terminal amino acid sequencing and mass spectrometry analysis provided experimental evidence for the identity of the recombinant protein. Reverse phase liquid chromatography demonstrated that the content of deamidates and sulphoxides was similar to a commercial standard. Size exclusion chromatography demonstrated the absence of high molecular mass aggregates and dimers. The protocol represents an effi cient and high-yield method to obtain bioactive homogeneous monomeric rhIFN-β1 ser17 protein. It may thus represent an important step towards scaling up for rhIFN-β1 ser17 large-scale production. © 2010 Villela AD, et al.

Pharmacovigilance is responsible for monitoring the safety of medicines in normal clinical use andduring clinical trials. Legal requirements for pharmacovigilance in some Latin American countries (Argentina, Brazil, Chile, Paraguay and Uruguay) were reviewed. Disparities in the legal framework among the countries are observed being those for marketing authorization holders one of the most evident. Theactive rol of the universities and drug information centers for/of pharmacovilance seems to be a positivecommon point. Legal requirements regarding pharmacovigilance of biosimilar medicines, is still a pointto be developed.

The human granulocyte colony stimulating factor (hG-CSF) plays an important role in hematopoietic cell proliferation/differentiation and has been widely used as a therapeutic agent for treating neutropenias. Nartograstim is a commercial G-CSF that presents amino acid changes in specific positions when compared to the wildtype form, which potentially increase its activity and stability. The aim of this work was to develop an expression system in Escherichia coli that leads to the production of large amounts of a recombinant hG-CSF (rhG-CSF) biosimilar to Nartograstim. The nucleotide sequence of hg-csf was codon-optimized for expression in E. coli. As a result, high yields of the recombinant protein were obtained with adequate purity, structural integrity and biological activity. This protein has also been successfully used for the production of specific polyclonal antibodies in mice, which could be used in the control of the expression and purification in an industrial production process of this recombinant protein. These results will allow the planning of large-scale production of this mutant version of hG-CSF (Nartograstim), as a potential new biosimilar in the market.
