Background: Critically ill patients are at high risk for pressure ulcer (PrU) development due to their high acuity and the invasive nature of the multiple interventions and therapies they receive. With reported incidence rates of PrU development in the adult critical care population as high as 56%, the identification of patients at high risk of PrU development is essential. This paper will explore the association between PrU development and risk factors. It will also explore PrU development and the use of risk assessment scales for critically ill patients in adult intensive care units. Method: A literature search from 2000 to 2012 using the CINHAL, Cochrane Library, EBSCOHost, Medline (via EBSCOHost), PubMed, ProQuest and Google Scholar databases was conducted. Key words used were: pressure ulcer/s; pressure sore/s; decubitus ulcer/s; bed sore/s; critical care; intensive care; critical illness; prevalence; incidence; prevention; management; risk factor; risk assessment scale. Results: Nineteen articles were included in this review; eight studies addressing PrU risk factors, eight studies addressing risk assessment scales and three studies overlapping both. Results from the studies reviewed identified 28 intrinsic and extrinsic risk factors which may lead to PrU development. Development of a risk factor prediction model in this patient population, although beneficial, appears problematic due to many issues such as diverse diagnoses and subsequent patient needs. Additionally, several risk assessment instruments have been developed for early screening of patients at higher risk of developing PrU in the ICU. No existing risk assessment scales are valid for identification high risk critically ill patient,with the majority of scales potentially over-predicting patients at risk for PrU development. Conclusion: Research studies to inform the risk factors for potential pressure ulcer development are inconsistent. Additionally, there is no consistent or clear evidence which demonstrates any scale to better or more effective than another when used to identify the patients at risk for PrU development. Furthermore robust research is needed to identify the risk factors and develop valid scales for measuring the risk of PrU development in ICU.

A computerized handheld procedure is presented in this paper. It is intended as a database complementary tool, to enhance prospective risk analysis in the field of occupational health. The Pendragon forms software (version 3.2) has been used to implement acquisition procedures on Personal Digital Assistants (PDAs) and to transfer data to a computer in an MS-Access format. The data acquisition strategy proposed relies on the risk assessment method practiced at the Institute of Occupational Health Sciences (IST). It involves the use of a systematic hazard list and semi-quantitative risk assessment scales. A set of 7 modular forms has been developed to cover the basic need of field audits. Despite the minor drawbacks observed, the results obtained so far show that handhelds are adequate to support field risk assessment and follow-up activities. Further improvements must still be made in order to increase the tool effectiveness and field adequacy.

Actuarial instruments for assessing sex offender recidivism have limited utility for specific risk assessment questions, such as the risk posed to particular types of victim. In order to obtain variables that discriminate between offenders with different classes of victim, data were coded from 324 files of child sexual offenders from a community-based sexual offender treatment program. Offenders with single or multiple victims were compared, as were offenders who did or did not offend against victims of both genders, and offenders with only intra-familial or extra-familial victims versus offenders with victims in both relationship categories. Variables that discriminated single-victim and multiple-victim offenders were similar to those identified in actuarial risk assessment scales, with the exception of history of childhood sexual abuse. With the exception of physical abuse history, the same variables discriminated specific offender groups according to victim gender and victim relationship, although in different combinations. There was limited support for the notion of specific risk variables.

Resumen: La satisfacción laboral es considerada una actitud frente a las experiencias laborales. En los últimos diez años su estudio se ha desplazado desde una posición periférica hasta el lugar central que ocupa en nuestros días. En línea con este renovado interés, el propósito del presente trabajo es proporcionar una actualización bibliográfica sobre la satisfacción laboral. En primer lugar, se realizan algunas consideraciones conceptuales en torno al constructo. A continuación, se presenta una síntesis de los instrumentos para su medición desarrollados hasta la fecha. Seguidamente, se reseñan las evidencias empíricas más recientes en relación a sus antecedentes contextuales y disposicionales, así como sus implicancias tanto para los empleados como para las organizaciones. Finalmente, se concluye con algunas sugerencias para futuras investigaciones.

If a product is being designed to be genuinely inclusive, then the designers need to be able to assess the level of exclusion of the product that they are working on and to identify possible areas of improvement. To be of practical use, the assessments need to be quick, consistent and repeatable. The aim of this workshop is to invite attendees to participate in the evaluation of a number of everyday objects using an assessment technique being considered by the workshop organisers. The objectives of the workshop include evaluating the effectiveness of the assessment method, evaluating the accessibility of the products being assessed and to suggest revisions to the assessment scales being used. The assessment technique is to be based on the ONS capability measures [1]. This source recognises fourteen capability scales of which seven are particularly pertinent to product evaluation, namely: motion, dexterity, reach and stretch, vision, hearing, communication, and intellectual functioning. Each of these scales ranges from 0 (fully able) through 1 (minimal impairment) to 10 (severe impairment). The attendees will be asked to rate the products on these scales. Clearly the assessed accessibility of the product depends on the assumptions made about the context of use. The attendees will be asked to clearly note the assumptions that they are making about the context in which the product is being assessed. For instance, with a hot water bottle, assumptions have to be made about the availability of hot water and these can affect the overall accessibility rating. The workshop organisers will not specify the context of use as the aim is to identify how assessors would use the assessment method in the real world. The objects being assessed will include items such as remote controls, pill bottles, food packaging, hot water bottles and mobile telephones. the attendees will be encouraged to assess two or more products in detail. Helpers will be on hand to assist and observe the assessments. The assessments will be collated and compared and feedback about the assessment method sought from the attendees. Drawing on a preliminary review of the assessment results, initial conclusions will be presented at the end of the workshop. More detailed analyses will be made available in subsequent proceedings. It is intended that the workshop will provide workshop attendees with an opportunity to perform hands-on assessment of a number everyday products and identify features which are inclusive and those that are not. It is also intended to encourage an appreciation of the capabilities to be considered when evaluating accessibility.

Introducción: En la actualidad se están implementando nuevas técnicas, para el tratamiento de líneas de expresión facial. El Plasma Rico en Plaquetas (PRP) utiliza factores de crecimiento humano autólogos con fines médicos estéticos. El objetivo de este trabajo fue evaluar el tratamiento con plasma rico en plaquetas en el manejo del rejuvenecimiento periocular. Materiales y métodos: Estudio descriptivo retrospectivo en una cohorte de 27 pacientes entre 30 a 70 años de ambos sexos,tratados con PRP sin tratamientos médicos estéticos previos . Se compararon fotografías del sistema VISIA®, previo y posterior el PRP, para determinar los cambios del área periocular. Con análisis comparativo de medias utilizando pruebas t de student. Resultados: De 27 historias clínicas revisadas 96,3% eran mujeres, la edad promedio fue de 52.67 años. Se observaron cambios clínicos satisfactorios en el manejo del foto envejecimiento periocular con mejoría estadísticamente significativa entre el promedio inicial y el post tratamiento en arrugas, textura y porfirinas (p: 0.000). No se observaron diferencias estadísticamente significativas entre los grupos de edad (p = 0,62). Los pacientes analizados posterior al tratamiento se encuentran en mejor estado que el 63,78% de la población de su mismo sexo, edad y fototipo de piel. Los eventos adversos fueron disminuyendo en su frecuencia en cada una de las sesiones siguientes. Discusión: El PRP proporciona una mejoría global en los parámetros de envejecimiento periocular lo cual se correlaciono con los estudios previos in vitro. Conclusiones: El PRP es seguro y eficaz en contorno de ojos.

The International Citicoline Trial in acUte Stroke is a sequential phase III study of the use of the drug citicoline in the treatment of acute ischaemic stroke, which was initiated in 2006 in 56 treatment centres. The primary objective of the trial is to demonstrate improved recovery of patients randomized to citicoline relative to those randomized to placebo after 12 weeks of follow-up. The primary analysis will take the form of a global test combining the dichotomized results of assessments on three well-established scales: the Barthel Index, the modified Rankin scale and the National Institutes of Health Stroke Scale. This approach was previously used in the analysis of the influential National Institute of Neurological Disorders and Stroke trial of recombinant tissue plasminogen activator in stroke. The purpose of this paper is to describe how this trial was designed, and in particular how the simultaneous objectives of taking into account three assessment scales, performing a series of interim analyses and conducting treatment allocation and adjusting the analyses to account for prognostic factors, including more than 50 treatment centres, were addressed. With an increasingly aged population, many patients will present with cancer in their 80s and 90s. Although some may be very fit, frail individuals will require the input of geriatricians to aid in the assessment of co-existing morbidity, in an attempt to assess those most likely to benefit from active treatment of their cancer, and those in whom the ‘giants of geriatric medicine’ require special consideration before undergoing definitive cancer therapy. The role of the geriatrician in assessment and management of such patients, together with communication and end of life care, may be more important in ensuring a good quality of life, than the cancer therapy itself. Whilst numbers of geriatricians will not be adequate to care for all elderly patients with cancer, a variety of assessment scales will help target financial and manpower resources to those most at risk.

Syftet med denna litteraturstudie var att beskriva vilka omvårdnadsåtgärder som kan förebygga uppkomsten av trycksår samt vilken förmåga att identifiera patienter som riskerar att utveckla trycksår sjuksköterskan har med de bedömningsinstrument som finns. De vetenskapliga artiklarna (n = 20) som ingick i studien söktes via Högskolan Dalarnas databaser Blackwell Synergy, PubMed och ELIN@Dalarna. Sökorden som användes i olika kombinationer var pressure sore, pressure ulcers, nursing, routines, treatment, knowledge, methods, management, measures, risk assessment, scales, risk patients och prevention/preventing. Inklusionskriterierna var att de skulle vara empiriska, skrivna på svenska eller engelska samt vara publicerade efter 1997. Resultatet visade att det finns många olika förebyggande åtgärder som vidtas. I preventivt syfte var evidensbaserade bedömningsinstrument ett bra och användbart verktyg. Även tryckreducerande underlag som luft- eller trögskumsmadrasser, fårskinn eller vattenfyllda handskar användes som trycksårspreventiva åtgärder. Lägesändringar med hjälp av vändscheman samt tidig mobilisering var också förebyggande åtgärder som vidtogs. Patienternas näringsstatus har stor betydelse i det preventiva arbetet. Resultat visade att det förekommer näringstillägg som en förebyggande strategi. Kontinuerlig inspektion av hud samt noggrann hygien var även något som utfördes i preventivt syfte. Det finns en mängd olika bedömningsinstrument och resultatet av litteraturstudien visar att resultaten vad gällde skalornas förmåga att förutse trycksår varierade mellan olika studier. Uppsatsförfattarna anser att trycksårsprevention är viktigt då trycksår orsakar lidande för patienten och är kostsamma för samhället. Preventiva rutinmässiga åtgärder borde utföras inom de flesta patientgrupper och då är bedömningsinstrumenten en viktig del i det arbetet.

Background and aims: The population of older people in our society is increasing. Agerelated changes in the skin results in a diminished perception of pain and pressure and a decreased microcirculation in the skin affects its ability to adapt to injury. Occurrence of pressure sore on geriatrikal clinics are 5-10%, witch means that between five and ten thousand patients gets daily treat for pressure sores. When the patient gets a pressure sore the need for help increases. A common apprehension is that if the patient’s affects with pressure sores it’s because of deficiency in care. According to the law, all nursing interventions should be performed according to scientific and evidence and the nurse’s assistants are responsible for how they perform. The aim of this study was to examine how much knowledge the nurses assistants in community care services has about preventing, predicting and locate riskfactors for pressure sores and if they get the right education. Methods: A questionnaire based on 20 questions was maid and used for this purpose. Out of 99 persons the questionnaires was answered bye 65 nurses assistants working in community care service in a small town in Sweden. Results: The results shown that the nurses assistants don’t use risk assessment scales in attempt to identify patients vulnerable to pressure sores and they are not well associated with the riskfactors. The study even shows that they have little knowledge in how to prevent pressure sores from appearing. The nursing model are some times out of date and the nurses assistants personal view attends to decide witch care they will perform instead of scientific and evidenced based nursing.

Objective: To briefly review the clinical and biological distinctions between unipolar and bipolar depression critiquing in particular currently available depression rating scales and discuss the need for a new observer-rated scale tailored to bipolar depression.

Method: Relevant literature pertaining to the symptomatic differences between bipolar disorder and unipolar disorder as well as their measurement using existing assessment scales was identified by computerized searches and reviews of scientific journals known to the authors.

Results
: Bipolar depression is distinct from unipolar depression in terms of phenomenology and clinical characteristics. These distinguishing features can be used to identify bipolarity in patients that present with recurrent depressive episodes. This is important because current self-report and observer-rated scales are optimized for unipolar depression, and hence limited in their ability to accurately assess bipolar depression.

Conclusion
: The development of a specific bipolar depression rating scale will improve the assessment of bipolar depression in both research and clinical settings and assist the development of better treatments and interventions.

To promote efficient clinical practice, interest has been growing in brief assessment scales to replace full-scale versions in some circumstances. In nonclinical populations, the Wechsler Abbreviated Scale of Intelligence (WASI) has substituted for the Wechsler Intelligence Scale for Children—Third Edition (WISC-III). Agreement between these scales remains untested in clinical populations. Twenty-five children, aged 6 to 15 years old were assessed. A correlational and within-participant design was used. These scales were significantly correlated. Despite a fourth edition of the WISC now available, WASI administration alone would at times appear to be a quick and valid estimate of IQ. Replication with the WISC-IV seems necessary to clarify the verbal/ performance distance criterion discrepancy and also the meaning of some variability between the scale and subtests.
