Attending potentially dangerous and traumatic incidents is inherent in the role of emergency workers, yet there is a paucity of literature aimed at examining variables that impact on the outcomes of such exposure. Coping has been implicated in adjusting to trauma in other contexts, and this study explored the effectiveness of coping strategies in relation to positive and negative posttrauma outcomes in the emergency services environment. One hundred twenty-five paramedics completed a survey battery including the Posttraumatic Growth Inventory (PTGI; Tedeschi & Calhoun, 1996), the Impact of Events Scale–Revised (IES-R; Weiss & Marmar, 1997), and the Revised-COPE (Zuckerman & Gagne, 2003). Results from the regression analysis demonstrated that specific coping strategies were differentially associated with positive and negative posttrauma outcomes. The research contributes to a more comprehensive understanding regarding the effectiveness of coping strategies employed by paramedics in managing trauma, with implications for their psychological well-being as well as the training and support services available.

Most emergency service organisations have some form of staff support program that share general aims of promoting and maintaining the mental health of their workforce. Yet few of these services have been subject to evaluation and fewer still have commissioned external professional researchers to scrutinise their programs. The Queensland Ambulance (QAS) Service provides a comprehensive and multifaceted program that is both proactive and reactive in design and with the support of the Commissioner, was the subject of a rigorous evaluation throughout 2013. In this paper the program services are briefly outlined and the considered approach to the evaluation is presented within the context of existing scientific literature. Using focus groups, information regarding the uptake of the program’s various ‘arms’, and survey data, results suggest the program is widely used and that staff are very satisfied with the services provided. Further, analysis of established psychometric measures demonstrated organisational and interpersonal factors that are important in the promotion of mental health and in warding off the deleterious impacts that frontline emergency service staff can endure. Data presented in this paper indicate how best to ensure a professional quality of life for ambulance personnel, how to promote resilience to the sometimes extremely challenging aspects of the work role, and ways in which difficulties such as depression may be minimised.

Introduction: Emergency prehospital medical care providers are frontline health workers during emergencies. However, little is known about their attitudes, perceptions, and likely behaviors during emergency conditions. Understanding these attitudes and behaviors is crucial to mitigating the psychological and operational effects of biohazard events such as pandemic influenza, and will support the business continuity of essential prehospital services. ----- ----- Problem: This study was designed to investigate the association between knowledge and attitudes regarding avian influenza on likely behavioral responses of Australian emergency prehospital medical care providers in pandemic conditions. ----- ----- Methods: Using a reply-paid postal questionnaire, the knowledge and attitudes of a national, stratified, random sample of the Australian emergency prehospital medical care workforce in relation to pandemic influenza were investigated. In addition to knowledge and attitudes, there were five measures of anticipated behavior during pandemic conditions: (1) preparedness to wear personal protective equipment (PPE); (2) preparedness to change role; (3) willingness to work; and likely refusal to work with colleagues who were exposed to (4) known and (5) suspected influenza. Multiple logistic regression models were constructed to determine the independent predictors of each of the anticipated behaviors, while controlling for other relevant variables. ----- ----- Results: Almost half (43%) of the 725 emergency prehospital medical care personnel who responded to the survey indicated that they would be unwilling to work during pandemic conditions; one-quarter indicated that they would not be prepared to work in PPE; and one-third would refuse to work with a colleague exposed to a known case of pandemic human influenza. Willingness to work during a pandemic (OR = 1.41; 95% CI = 1.0–1.9), and willingness to change roles (OR = 1.44; 95% CI = 1.04–2.0) significantly increased with adequate knowledge about infectious agents generally. Generally, refusal to work with exposed (OR = 0.48; 95% CI = 0.3–0.7) or potentially exposed (OR = 0.43; 95% CI = 0.3–0.6) colleagues significantly decreased with adequate knowledge about infectious agents. Confidence in the employer’s capacity to respond appropriately to a pandemic significantly increased employee willingness to work (OR = 2.83; 95% CI = 1.9–4.1); willingness to change roles during a pandemic (OR = 1.52; 95% CI = 1.1–2.1); preparedness to wear PPE (OR = 1.68; 95% CI = 1.1–2.5); and significantly decreased the likelihood of refusing to work with colleagues exposed to (suspected) influenza (OR = 0.59; 95% CI = 0.4–0.9). ----- ----- Conclusions:These findings indicate that education and training alone will not adequately prepare the emergency prehospital medical workforce for a pandemic. It is crucial to address the concerns of ambulance personnel and the perceived concerns of their relationship with partners in order to maintain an effective prehospital emergency medical care service during pandemic conditions.

- Objective Ambulance personnel provide emergency medical services to the community, often attending to highly challenging and traumatic scenes in complex and chaotic circumstances. Currently the assessment of predictors of psychological well-being remains limited. The current study investigated whether workplace belongingness was significant in predicting psychological distress as well as the presence of resilience in ambulance personnel whilst controlling for more routinely examined factors. - Method Australian ambulance officers (N = 740) completed a survey battery including the Kessler 10 (Kessler & Mroczek, 1994), Brief Resilience Scale (Smith et al., 2008) and Psychological Sense of Organisational Membership (Cockshaw & Shochet, 2010) scale. - Results Controlling for more commonly examined factors such as severity of trauma exposure and length of service, hierarchical multiple regression analyses demonstrated that workplace belongingness was significantly associated with reduced distress levels and enhanced resilience levels. - Conclusions Results suggest that strategies to enhance a sense of workplace belongingness in emergency service organisations could promote the well-being of emergency workers despite routine exposure to potentially traumatic events.

Establecer la prevalencia de síntomas osteomusculares en el personal operativo de la empresa Emermédica S.A. durante los últimos 3 meses. Se realizó un estudio de corte trasversal en la empresa Emermédica S.A. de Bogotá en el año 2012, tomando una muestra de 75 trabajadores entre médicos, enfermeros y conductores del área operativa, a los cuales se les aplicó un instrumento de morbilidad sentida basada en el Cuestionario Nórdico y la Encuesta Europea, en búsqueda de síntomas osteomusculares asociados a la actividad desempeñada y a factores como edad, género, horas de trabajo por turno, tipo de vehículo, actividad deportiva, equipos utilizados y posturas adoptadas durante el desarrollo de sus actividades. Para el análisis de la información se creó una base de datos a partir de los cuestionarios contestados válidos y se utilizó el programa estadístico SPSS.

Antecedentes: Los factores de Riesgo psicosocial según la resolución 2646 de 2008 “comprenden los aspectos intralaborales, extralaborales o externos a la organización y las condiciones individuales o características intrínsecas al trabajador, los cuales en una interrelación dinámica, mediante percepciones y experiencias, influirían en la salud y el desempeño de las personas”. El objetivo del estudio es establecer la prevalencia de factores de riesgo psicosocial y la asociación existente en la población trabajadora del área administrativa y asistencial, en una empresa del sector salud nivel II, Yopal, 2014 Materiales y método: Se diseñó un estudio de corte transversal para establecer la prevalencia de los factores de riesgo psicosocial en una población de 92 trabajadores del área administrativa (50) y asistencial (42) en una empresa del sector salud nivel II en Yopal, por medio de la batería de Riesgo Psicosocial del Ministerio de la Protección Social. Resultados: Se realizó un análisis multivariado, se puede afirmar con un nivel de confianza del 95% que en la población de estudio, la prevalencia de riesgo intralaboral en la población administrativa está entre el 9,5% y 34,5% y en los trabajadores del área asistencial entre el 9,7% y el 37,9%. El riesgo extralaboral en el área administrativa está entre el 4,8% y el 27,2%, en el área asistencial la prevalencia está entre el 1,5% y el 19,5%. La prevalencia de Riesgo de Estrés se encuentra en la población administrativa entre el 29,2% y el 58,8% y en el área asistencial entre el 17,9% y el 48,8%. Se encontró una asociación (p = 0,007) entre estado civil y riesgo intralaboral en la población asistencial, entre tipo de vivienda y nivel de estres (p= 0,003) en la población administrativa, y entre número de personas a cargo económicamente y riesgo extralaboral (p=0,004) en el área asistencial. Discusión: La prevalencia de riesgo psicosocial es evidente en las dos áreas de trabajo tanto administrativos como asistenciales, sin embargo se encuentra una prevalencia mayor de riesgo en el área asistencial en la dimensión exigencias del rol dado a un incremento en la responsabilidad y tiempo extra que se debe realizar para el desarrollo de tareas y su cumplimiento ante el rol desempeñado; los resultados permiten implementar programas de vigilancia epidemiológica que permitan mitigar y controlar los hallazgos en este estudio.

Syfte: Syftet med denna litteraturöversikt var att beskriva vad som kännetecknar ett gott bemötande inom den prehospitala akutsjukvården och vilken betydelse bemötandet har för vården. Metod: Studien genomfördes som en litteraturöversikt och artiklar söktes via databaserna CINAHL och PubMed. De elva inkluderade artiklarna resulterade i kategorier och subkategorier som beskriver vad som är utmärkande för ett gott bemötande inom den prehospitala sjukvården och vilken betydelse det har för vården. Resultat: Resultatet visade att ambulanspersonalen ska anpassa kommunikationen efter patientens förutsättningar och de ska ha ett förhållningssätt som baseras på respekt, ödmjukhet och empati. En förutsättning för en god omvårdnad är att en omvårdnadsrelation, baserad på tillit och förtroende, upprättas. Omvårdnadsrelationen synliggör patientens sårbarhet, vilket ställer krav på ambulanspersonalen att visa respekt för patientens integritet. Slutsats: Patienten har inte bara fysiska utan även psykiska och sociala behov. Att ambulanspersonalen har en holistisk människosyn gör att patienten blir bemött som en unik individ och därmed får en bättre prognos medan brister i den prehospitala vårdkedjan kan äventyra patientsäkerheten.
