Dysphagia is a common and problematic symptom characterised by varying degrees of difficulty swallowing food, fluids and medicines of differing consistencies. International primary care based studies have identified that between 1 in 4 and 1 in 5 patients have some form of dysphagia, it can affect medicines taking behaviour and healthcare professionals are largely unaware of this1,2. Similar research has not been undertaken in the UK. Adherence related pharmacy based services in the UK provide an opportunity for community pharmacists to identify the problem and facilitate better medicines use. The aim of this pilot study was to estimate the level of patient reported dysphagia in older persons using community pharmacies in the UK, describe how it affects their medicine taking behaviour and identify whether advanced pharmacy services are related to improved awareness of this.

Políticas públicas são estruturadas com a finalidade de ser uma resposta dada pelo poder público para as diversas demandas, problemas e tensões geradas na sociedade. Devem ter magnitude e relevância social, bem como possuir poder de barganha suficiente para fazer parte da agenda de prioridades de um determinado órgão fomentador de políticas. Desta forma, uma política é constituída pelo seu propósito, diretrizes e definição de responsabilidades das esferas de Governo e dos órgãos envolvidos. Assim, a política de medicamentos brasileira, inserida na Política de Saúde, constitui um dos elementos fundamentais para a implementação de ações capazes de promover melhoria nas condições de saúde. Preconiza a garantia da disponibilidade, do acesso e do uso racional de medicamentos por todos os setores da população, conforme seu perfil de morbimortalidade. Nessa perspectiva, o presente trabalho pretendeu fazer uma análise da Política Nacional de Medicamentos (PNM) para compreender os dados encontrados. Com base na abordagem qualitativa, levando em consideração o que explicita o documento fundador da PNM, além de uma revisão da literatura foram feitos o mapeamento e a análise dos referidos dados, gerando categorias (contexto, conteúdo e processos envolvidos). Este estudo permitiu concluir que a PNM não abrange muitos dos problemas relacionados ao uso do medicamento, como também não conseguiu ferramentas suficientes para dar todas as respostas governamentais necessárias para muitos dos problemas por ela levantados ou até mesmo daqueles existentes e que não foram por ela contemplados. Os governos, tanto o que a formulou quanto os que o sucederam, avançaram em suas diretrizes ou continuam envidando esforços para tal, no sentido de contribuir para a efetivação do direito à assistência terapêutica integral.

Nowadays, computing is migrating from traditional high performance and distributed computing to pervasive and utility computing based on heterogeneous networks and clients. The current trend suggests that future IT services will rely on distributed resources and on fast communication of heterogeneous contents. The success of this new range of services is directly linked to the effectiveness of the infrastructure in delivering them. The communication infrastructure will be the aggregation of different technologies even though the current trend suggests the emergence of single IP based transport service. Optical networking is a key technology to answer the increasing requests for dynamic bandwidth allocation and configure multiple topologies over the same physical layer infrastructure, optical networks today are still “far” from accessible from directly configure and offer network services and need to be enriched with more “user oriented” functionalities. However, current Control Plane architectures only facilitate efficient end-to-end connectivity provisioning and certainly cannot meet future network service requirements, e.g. the coordinated control of resources. The overall objective of this work is to provide the network with the improved usability and accessibility of the services provided by the Optical Network. More precisely, the definition of a service-oriented architecture is the enable technology to allow user applications to gain benefit of advanced services over an underlying dynamic optical layer. The definition of a service oriented networking architecture based on advanced optical network technologies facilitates users and applications access to abstracted levels of information regarding offered advanced network services. This thesis faces the problem to define a Service Oriented Architecture and its relevant building blocks, protocols and languages. In particular, this work has been focused on the use of the SIP protocol as a inter-layers signalling protocol which defines the Session Plane in conjunction with the Network Resource Description language. On the other hand, an advantage optical network must accommodate high data bandwidth with different granularities. Currently, two main technologies are emerging promoting the development of the future optical transport network, Optical Burst and Packet Switching. Both technologies respectively promise to provide all optical burst or packet switching instead of the current circuit switching. However, the electronic domain is still present in the scheduler forwarding and routing decision. Because of the high optics transmission frequency the burst or packet scheduler faces a difficult challenge, consequentially, high performance and time focused design of both memory and forwarding logic is need. This open issue has been faced in this thesis proposing an high efficiently implementation of burst and packet scheduler. The main novelty of the proposed implementation is that the scheduling problem has turned into simple calculation of a min/max function and the function complexity is almost independent of on the traffic conditions.

Lately, videoconference applications have experienced an evolution towards the World Wide Web. New technologies have given browsers real-time communications capabilities. In this context, WebRTC aims to provide this functionality by following and defining standards. Being a new effort, WebRTC still lacks advanced videoconferencing services such as session recording, media mixing and adjusting to varying network conditions. This paper analyzes these challenges and proposes an architecture based on a traditional communications entity, the Multipoint Control Unit or MCU as a solution.

Rural and remote community pharmacies service large areas of rural Queensland, and because of the distances involved often do not meet the patients for whom they provide medication. Telepharmacy would improve the quality of pharmaceutical services provided in rural and remote areas, by allowing community pharmacists to have realtime contact with dispensing doctors, aboriginal health workers and patients via a video-phone. We used commercial (analogue) videophones to connect community pharmacists to dispensing doctors, patients in depot pharmacies (i.e. those with no pharmacist) and aboriginal health workers. However, various problems occurred and only 10 video-phone interactions were recorded during the six-month project. In all of the recorded interactions, the video-phone was actually used as a conventional telephone because a video-connection could not be established at the time of the call. (This may have been due to telephone network problems in the rural areas.) Despite these problems, all project participants showed great enthusiasm for the potential benefits of such a service.

Session Initiation Protocol (SIP) is developed to provide advanced voice services over IP networks. SIP unites telephony and data world, permitting telephone calls to be transmitted over Intranets and Internet. Increase in network performance and new mechanisms for guaranteed quality of service encourage this consolidation to provide toll cost savings. Security comes up as one of the most important issues when voice communication and critical voice applications are considered. Not only the security methods provided by traditional telephony systems, but also additional methods are required to overcome security risks introduced by the public IP networks. SIP considers security problems of such a consolidation and provides a security framework. There are several security methods defined within SIP specifications and extensions. But, suggested methods can not solve all the security problems of SIP systems with various system requirements. In this thesis, a Kerberos based solution is proposed for SIP security problems, including SIP authentication and privacy. The proposed solution tries to establish flexible and scalable SIP system that will provide desired level of security for voice communications and critical telephony applications.
