RESUMO Objetivo: Este estudo tem como objetivos comparar a literacia em saúde mental dos adolescentes e jovens portugueses, relativamente à depressão e ao abuso de álcool e analisar o padrão de respostas em termos de consistência e concordância para ambas as perturbações. Método: A amostra é constituída por 4938 adolescentes e jovens, 43,0% do sexo masculino e 56,7% do sexo feminino, com uma média de idades de 16,75 anos (desvio padrão = 1,62 anos), que frequentam as escolas do 3.º ciclo do ensino básico e do ensino secundário, pertencentes à Direção Regional de Educação do Centro. Para a colheita de dados foi utilizado o QuALiSMental (LSM). Resultados: Os resultados revelam um nível de LSM modesto na generalidade das componentes. Ainda que se encontrem diferenças estatisticamente significativas (p < 0,05) em 88,0% dos itens da LSM, o que indicia diferentes formas de encarar ambos os problemas de saúde mental, os resultados revelam consistência, em termos das componentes conhecimentos e competências para prestar a primeira ajuda e apoio aos outros e conhecimentos acerca do modo de prevenção das perturbações mentais. ABSTRACT Objectives: This study aims to compare mental health literacy of adolescents and young Portuguese in what concerns depression and alcohol abuse and analyzes the pattern of responses in terms of consistency and agreement for both disorders. Method: The sample consisted of 4938 adolescents and young people, 43.0% males and 56.7% females with a mean age of 16.75 years (standard deviation = 1,62 years), who attend schools of the 3rd cycle of basic education and secondary school, belonging to the Regional Education Directorate – Center. For data collection was used QuALiSMental (MHL). Results: The results showed a modest level of MHL in most components. Although there are statistically significant differences (p < 0.05) in 88.0% of the items of MHL, suggesting different ways of looking to both mental health problems, the results show consistency in terms of the components of knowledge and skills to providing first aid and support to others and knowledge about the prevention of mental disorders.

Identificar los casos de abuso de alcohol y desarrollar una estrategia de prevención mediante fortalecimiento de las habilidades sociales desde un enfoque cognitivo-conductual. Material y métodos: Se desarrolló un estudio con enfoque cuantitativo, diseño descriptivo y de intervención. Se incluyó a 232 estudiantes matriculados en el colegio Ezequiel Cárdenas Espinoza, de la ciudad de Azogues, que aceptaron participar mediante el asentimiento informado y que no presentaban enfermedades físicas o mentales. Se aplicó el test Self Reporting Questionnaire (SRQ) y los CAPs para evaluar los conocimientos, actitudes y las prácticas. Se aplicó un programa psicoterapéutico con enfoque cognitivo-conductual para fortalecer las habilidades sociales de los estudiantes seleccionados, en 16 sesiones con los subtemas específicos, mediante talleres participativos y motivacionales. Resultados: Los 232 estudiantes tuvieron la edad media de 14.9 ± 2.07 años. El 47% sexo masculino y el 53% sexo femenino; Según el SRQ, el 23.3% (n=54) de estudiantes consumía alcohol con patrón que sugiere consumo problemático, los varones consumen más que las mujeres en relación 3 a 1. Con la estrategia de prevención de enfoque cognitivo-conductual se obtuvo cambios estadísticamente significativos en conocimientos y actitudes (p < 0.05), no así en el cambio de prácticas (p > 0.05). Conclusión: La prevalencia de abuso de alcohol en los estudiantes del colegio Ezequiel Cárdenas es más alto que el reportado en otros estudios. La intervención con enfoque cognitivo-conductual mediante fortalecimiento de habilidades sociales produjo mejoría en conocimientos y actitudes

While the consumption of alcohol has been part of the collective psyche of Australians since colonisation, the overconsumption of alcohol has been and continues to be a significant problem for the Australian community. Currently motivational interviewing and cognitive behaviour therapy are seen as the two standard psychological interventions for alcohol abuse and dependence. While these two approaches have shown significant impact on reducing alcohol abuse and dependence, they are not without their limitations. As such there is a need to continue to explore the application of newer developments in psychotherapy to the treatment problematic drinking behaviours. In this chapter we propose that Metacognitive Therapy is one such psychotherapy that is likely to provide a promising new approach to the treatment of alcohol abuse and dependence. In this chapter we will first briefly outline the history and significance of problematic drinking behaviours in Australia. Following this, we will quickly summarise the literature regarding motivational interviewing and cognitive behaviour therapy. Next we will provide an outline of the theoretical framework of Metacognitive Therapy and then describe two brief case studies illustrating the application of Metacognitive Therapy to the treatment of alcohol abuse and dependence. From this discussion we propose that the combination of Motivational Interviewing and Metacognitive Therapy is a promising new approach that can provide great assistance for the treatment of alcohol abuse or dependence.

BACKGROUND: Patients with chronic hepatitis C virus (HCV) infection have high rates of alcohol consumption, which is associated with progression of fibrosis and lower response rates to HCV treatment. AIMS: This prospective cohort study examined the feasibility of a 24-week integrated alcohol and medical treatment to HCV-infected patients. METHODS: Patients were recruited from a hepatology clinic if they had an Alcohol Use Disorders Identification Test score >4 for women and >8 for men, suggesting hazardous alcohol consumption. The integrated model included patients receiving medical care and alcohol treatment within the same clinic. Alcohol treatment consisted of 6 months of group and individual therapy from an addictions specialist and consultation from a study team psychiatrist as needed. RESULTS: Sixty patients were initially enrolled, and 53 patients participated in treatment. The primary endpoint was the Addiction Severity Index (ASI) alcohol composite scores, which significantly decreased by 0.105 (41.7% reduction) between 0 and 3 months (P < 0.01) and by 0.128 (50.6% reduction) between 0 and 6 months (P < 0.01) after adjusting for covariates. Alcohol abstinence was reported by 40% of patients at 3 months and 44% at 6 months. Patients who did not become alcohol abstinent had reductions in their ASI alcohol composite scores from 0.298 at baseline to 0.219 (26.8% reduction) at 6 months (P = 0.08). CONCLUSION: This study demonstrated that an integrated model of alcohol treatment and medical care could be successfully implemented in a hepatology clinic with significant favorable impact on alcohol use and abstinence among patients with chronic HCV.

Ante los datos del consumo de alcohol entre los jóvenes, es necesario poner en marcha un plan de prevención junto con la aplicación de la legislación vigente. Desde la Fundación Alcohol y Sociedad se proponen diferentes programas y se ofrecen unas pautas que deberían seguirse en cualquier programa preventivo. Se señala la importancia de hacer evaluaciones de los programas realizados, así como de analizar de que manera las actividades preventivas motivan un cambio en los jóvenes.

Background. This study examined whether alcohol abuse patients are characterized either by enhanced schematic processing of alcohol related cues or by an attentional bias towards the processing of alcohol cues. Method. Abstinent alcohol abusers (N = 25) and non-clinical control participants (N = 24) performed a dual task paradigm in which they had to make an odd/even decision to a centrally presented number while performing a peripherally presented lexical decision task. Stimuli on the lexical decision task comprised alcohol words, neutral words and non-words. In addition, participants completed an incidental recall task for the words presented in the lexical decision task. Results. It was found that, in the presence of alcohol related words, the performance of patients on the odd/even decision task was poorer than in the presence of other stimului. In addition, patients displayed slower lexical decision times for alcohol related words. Both groups displayed better recall for alcohol words than for other stimuli. Conclusions. These results are interpreted as supporting neither model of drug cravings. Rather, it is proposed that, in the presence of alcohol stimuli, alcohol abuse patients display a breakdown in the ability to focus attention.

Increased serum bile salt levels have been associated to a single-nucleotide polymorphism in the bile salt export pump (BSEP; ABCB11) in several acquired cholestatic liver diseases but there is little evidence in alcoholic liver disease (ALD). Furthermore, a crosstalk between vitamin D and bile acid synthesis has recently been discovered. Whether this crosstalk has an influence on the course of ALD is unclear to date. Our aim was to analyse the role of genetic polymorphisms in BSEP and the vitamin D receptor gene (NR1I1) on the emergence of cirrhosis in patients with ALD. Therefore, 511 alcoholic patients (131 with cirrhosis and 380 without cirrhosis) underwent ABCB11 genotyping (rs2287622). Of these, 321 (131 with cirrhosis and 190 without cirrhosis) were also tested for NR1I1 polymorphisms (bat-haplotype: BsmI rs1544410, ApaI rs7975232 and TaqI rs731236). Frequencies of ABCB11 and NR1I1 genotypes and haplotypes were compared between alcoholic patients with and without cirrhosis and correlated to serum bile salt, bilirubin and aspartate aminotransferase levels in those with cirrhosis. Frequencies of ABCB11 and NR1I1 genotypes and haplotypes did not differ between the two subgroups and no significant association between genotypes/haplotypes and liver function tests could be determined for neither polymorphism. We conclude that ABCB11 and NR1I1 polymorphisms are obviously not associated with development of cirrhosis in patients with ALD.
