O corte ou desgaste dos dentes dos leitões recém nascidos é uma prática comum na suinocultura, com o objetivo de reduzir lesões cutâneas na face dos leitões e no aparelho mamário das matrizes. Para avaliação da presença de lesões dentárias, foram analisados após eutanásia 280 animais que apresentavam sinais clínicos de definhamento (refugagem) compatíveis com uma forma de infecção por circovírus (síndrome da refugagem multissistêmica). Entre esses, 58 leitões (21%) apresentaram ao menos um abscesso periapical (total de 70 abscessos), onde: 3os incisivos superiores, 3os incisivos inferiores, caninos superiores e caninos inferiores foram respectivamente responsáveis por 31%, 23%, 6%, e 33% dos abscessos. Outros dentes foram responsáveis por 7% do total de abscessos. A prevalência de abscessos predominantemente nos dentes 3os incisivos e caninos inferiores sugere que estes dentes sofreram maior área de corte/desgaste, refletindo em superior prevalência de abscessos periapicais. Dentre as bactérias classificadas, o Streptococcus sp. foi a mais isolada (21,48% dos isolados em aerobiose e 27,7% em anaerobiose). Houve um isolamento significativamente maior de bactérias corineformes em atmosfera anaeróbica, quando comparado com isolamentos em aerobiose. Houve preponderância de isolamentos de bactérias Gram positivas. Através de um experimento a campo em granja de produção de leitões, foram avaliados 7 tipos de manejos dentários, medindo a influência dos mesmos na variável ganho de peso na maternidade e creche. Nenhuma entre as técnicas avaliadas mostrou diferença estatisticamente significativa.

Acute infections maxillo-facial are of great importance, both for its high population, as the risk of complications. The aim of this study was to present a clinical case of extensive odontogenic abscess in a patient of 31 years of age attended the Service of Surgery and Maxillo-Facial, Faculty of Dentistry of Araçatuba - UNESP in Araçatuba, SP. Complications of infections odontogências although infrequent can lead the patient to death.

Introduction Sphingosine-1-phosphate receptor 1 (S1P1) is crucial for regulation of immunity and bone metabolism. This study aimed to investigate the expression of S1P1 in rat periapical lesions and its relationship with receptor activator of nuclear factor kappa B ligand (RANKL) and regulatory T (Treg) cells. Methods Periapical lesions were induced by pulp exposure in the first lower molars of 55 Wistar rats. Thirty rats were killed on days 0, 7, 14, 21, 28, and 35, and their mandibles were harvested for x-ray imaging, micro–computed tomography scanning, histologic observation, immunohistochemistry, enzyme histochemistry, and double immunofluorescence analysis. The remaining 25 rats were killed on days 0, 14, 21, 28, and 35, and mandibles were harvested for flow cytometry. Results The volume and area of the periapical lesions increased from day 0 to day 21 and then remained comparably stable after day 28. S1P1-positive cells were observed in the inflammatory periapical regions; the number of S1P1-positive cells peaked at day 14 and then decreased from day 21 to day 35. The distribution of S1P1-positive cells was positively correlated with the dynamics of RANKL-positive cells but was negatively correlated with that of Treg cells. Conclusions S1P1 expression was differentially correlated with RANKL and Treg cell infiltration in the periapical lesions and is therefore a contributing factor to the pathogenesis of such lesions.

Objective. The objective of this study was to evaluate in vivo the revascularization and the apical and periapical repair after endodontic treatment using 2 techniques for root canal disinfection (apical negative pressure irrigation versus apical positive pressure irrigation plus triantibiotic intracanal dressing) in immature dogs` teeth with apical periodontitis. Study design. Two test groups of canals with experimentally induced apical periodontitis were evaluated according to the disinfection technique: Group 1, apical negative pressure irrigation (EndoVac system), and Group 2, apical positive pressure irrigation (conventional irrigation) plus triantibiotic intracanal dressing. In Group 3 (positive control), periapical lesions were induced, but no endodontic treatment was done. Group 4 (negative control) was composed of sound teeth. The animals were killed after 90 days and the maxillas and mandibles were subjected to histological processing. The sections were stained with hematoxylin and eosin and Mallory Trichrome and examined under light microscopy. A description of the apical and periapical features was done and scores were attributed to the following histopathological parameters: newly formed mineralized apical tissue, periapical inflammatory infiltrate, apical periodontal ligament thickness, dentin resorption, and bone tissue resorption. Intergroup comparisons were done by the Kruskal-Wallis and Dunn`s tests (alpha = 0.05). Results. Although statistically significant difference was found only for the inflammatory infiltrate (P < .05), Group 1 presented more exuberant mineralized formations, more structured apical and periapical connective tissue, and a more advanced repair process than Group 2. Conclusion. From the histological observations, sodium hypochlorite irrigation with the EndoVac system can be considered as a promising disinfection protocol in immature teeth with apical periodontitis, suggesting that the use of intracanal antibiotics might not be necessary. Objective. To evaluate the periapical repair after root canal treatment in the teeth of dogs using CT and conventional radiography and to compare these findings with the gold standard microscopic evaluation. Study design. The animals were divided into three groups according to endodontic treatment performed: Group 1, single-visit endodontic treatment in teeth without apical periodontitis; Group 2, single-visit endodontic treatment in teeth with apical periodontitis; and Group 3, endodontic treatment in teeth with apical periodontitis using calcium hydroxide as a root canal dressing. Group 4 consisted of teeth with apical periodontitis not submitted to root canal treatment and Group 5 consisted of healthy teeth without periapical disease. Radiographic, tomographic, and microscopic evaluations were performed by blind examiners. At 180 days experimental time, CT and radiographic measurements of periapical disease were compared with the gold standard microscopic measurement using intraclass correlation coefficient. Intergroup comparisons considering different methods of periapical lesions measurement or different clinical protocols of root canal treatment were performed by Kruskal Wallis test followed by Dunn. Integrity of lamina dura, presence of radiolucent areas, and presence of root resorption were analyzed by Fisher`s exact test. Results. There was discontinuity of the lamina dura and CPD in all teeth from Groups 2, 3, and 4 evaluated by tomography and radiography 45 days after CPD induction. Radiographically, 180 days after root canal treatment, there was no periapical lesion in teeth from Groups 1 and 3, different from groups 2 and 4 (p < .05). The highest reduction in the CPD size was observed on Group 3 (p < .05). According to the tomographic results, there was decrease of the size of the CPD on Group 3 but not on Groups 2 or 4. However, in all groups the periapical lesions presented larger mesio-distal extension if compared with radiography, both 45 days after CPD induction and 180 days after root canal treatment. At 180 days, CT measurements were closely related to microscopic results (ICC = 0.95) differently from radiographic evaluation (ICC = 0.86). Conclusion. CT Scan evaluation of periapical repair following root canal treatment provided similar information than that obtained by microscopic analysis, whereas radiographic evaluation underestimated the size do periapical lesion. Introduction: The inability to distinguish periapical cysts from granulomas before performing root canal treatment leads to uncertainty in treatment outcomes because cysts have lower healing rates. Searching for differential expression of molecules within cysts or granulomas could provide information with regard to the identity of the lesion or suggest mechanistic differences that may form the basis for future therapeutic intervention. Thus, we investigated whether granulomas and cysts exhibit differential expression of extracellular matrix (ECM) molecules. Methods: Human periapical granulomas, periapical cysts, and healthy periodontal ligament tissues were used to investigate the differential expression of ECM molecules by microarray analysis. Because matrix metalloproteinases (MMP) showed the highest differential expression in the microarray analysis, MMPs were further examined by in situ zymography and immunohistochemistry. Data were analyzed by using one-way analysis of variance followed by the Tu-key test. Results: We observed that cysts and granulomas differentially expressed several ECM molecules, especially those from the MMP family. Compared with cysts, granulomas exhibited higher MMP enzymatic activity in areas stained for MMP-9. These areas were composed of polymorphonuclear cells (PMNs) in contrast to cysts. Similarly, MMP-13 was expressed by a greater number of cells in granulomas compared with cysts. Conclusion: Our findings indicate that high enzymatic MIMP activity in PMNs together with MMP-9 and MMP-13 stained cells could be a molecular signature of granulomas unlike periapical cysts. Introduction: The aim of this study was to evaluate the accuracy of two imaging methods in diagnosing apical periodontitis (AP) using histopathological findings as a gold standard. Methods: The periapex of 83 treated or untreated roots of dogs` teeth was examined using periapical radiography (PR), cone-beam computed tomography (CBCT) scans, and histology. Sensitivity, specificity, predictive values, and accuracy of PR and CBCT diagnosis were calculated. Results: PR detected AP in 71% of roots, a CBCT scan detected AP in 84%, and AP was histologically diagnosed in 93% (p = 0.001). Overall, sensitivity was 0.77 and 0.91 for PR and CBCT, respectively. Specificity was 1 for both. Negative predictive value was 0.25 and 0.46 for PR and CBCT, respectively. Positive predictive value was 1 for both. Diagnostic accuracy (true positives + true negatives) was 0.78 and 0.92 for PR and CBCT (p = 0.028), respectively. Conclusion: A CBCT scan was more sensitive in detecting AP compared with PR, which was more likely to miss AP when it was still present. (J Endod 2009;35:1009-1012)

The purpose of this study was to compare the favorable outcome of root canal treatment determined by periapical radiographs (PRs) and cone beam computed tomography (CBCT) scans. Ninety-six roots of dogs` teeth were used to form four groups (n = 24). In group 1, root canal treatments were performed in healthy teeth. Root canals in groups 2 through 4 were infected until apical periodontitis (AP) was radiographically confirmed. Roots with AP were treated by one-visit therapy in group 2, by two-visit therapy in group 3, and left untreated in group 4. The radiolucent area in the PRs and the volume of CBCT-scanned periapical lesions were measured before and 6 months after the treatment. In groups 1, 2, and 3, a favorable outcome (lesions absent or reduced) was shown in 57 (79%) roots using PRs but only in 25 (35%) roots using CBCT scans (p = 0.0001). Unfavorable outcomes occurred more frequently after one-visit therapy than two-visit therapy when determined by CBCT scans (p = 0.023). (J Endod 2009; 35:723-726)

The objective of the study was to evaluate the biocompatibility of RoekoSeal sealer Roeko (Dental Products, Langenau, Germany) with the periapical tissues of dogs and compare it with AH Plus sealer (Dentsply/De Trey, Konstanz, Germany). The pulps of 32 root canals were removed, the apical cementum layer perforated, the biomechanical preparation performed, and the root canals filled by lateral condensation technique. Ninety days after the surgery, the animals were euthanized, the bone with teeth removed, and the samples prepared for histopathological analysis. In group 1 (RoekoSeal Automix), deposition of mineralized tissue was observed, with complete newly mineralized apical formed tissue in 43.8% and partial sealing in 56.2%. In group 2 (AH Plus), in 12.5% there was complete newly mineralized apical formed tissue, in 75% the sealing was partial, and in 12.5% there was no sealing (p < 0.05). There were no differences between the groups in relation to the inflammatory infiltrate; thickness of the periodontal ligament; and the resorption of dentin, cementum or bone (p > 0.05).

Introduction: Periapical lesions are chronic inflammatory disorders of periradicular tissues caused by etiologic agents of endodontic origin. The inflammatory chemokines are thought to be involved in the latter observed osteolysis. With a murine model of experimental periapical lesion, the objective of this study was to evaluate the role of the chemokine receptor CCR2 in the lesion progression, osteoclast differentiation and activation, and expression of inflammatory osteolysis-related mediators. Methods: For lesion induction, right mandibular first molars were opened surgically with a (1)/(4) carbine bur, and 4 bacterial strains were inoculated in the exposed dental pulp; left mandibular first molars were used as controls. Animals were killed at 3, 7, 14, and 21 days after surgeries to evaluate the kinetics of lesion development. Results: CCR2 KO mice showed wider lesions than WT mice. CCR2 KO mice also expressed higher levels of the osteoclastogenic and osteolytic factors, receptor activator of nuclear factor kappa B ligand (RANKL) and cathepsin K, of the proinflammatory cytokine tumor necrosis factor alpha, and of the neutrophil migration related chemokine, KC. Conclusions: These results suggest that CCR2 is important in host protection to periapical osteolysis. (J Endod 2010;36:244-250)

A lesão inflamatória periapical é uma patologia bastante frequente, sendo na maioria dos casos, consequência da cárie dental. O objetivo deste trabalho foi quantificar as populações linfocitárias CD8+ e CD20+ em lesões inflamatórias periapicais crônicas. Foram utilizadas 90 lesões inflamatórias periapicais. Através da técnica de imunohistoquímica pelo método da estreptoavidina-biotina, utilizou-se os marcadores CD8 e CD20 para identificação dos linfócitos T citotóxicos/supressores e linfócitos B, respectivamente. A contagem das células foi feita em 3 campos microscópicos da lâmina, mantendo-se um aumento de 400 vezes. A média da contagem das células CD8+ foi de 7,72 células para os cistos inflamatórios, enquanto que nos grupos cisto abscedado e abscesso crônico foi 11,25 e 11,62, respectivamente, com diferença estatisticamente significante. A média da contagem das células CD20+ foi 12,19; 11,06 e 12,91 células nos cistos abscedados, cistos inflamatórios e abscessos crônicos, respectivamente, sem diferença estatisticamente significante. As lesões inflamatórias periapicais supuradas apresentaram número maior de linfócitos CD8+ do que as lesões não supuradas e; a presença de supuração e proliferação epitelial nas lesões não interferiu na quantidade de linfócitos CD20+ presentes.

O abscesso de pulmão continua sendo hoje, em plena era dos antibióticos, um importante problema médico. O presente trabalho se propõe mostrar aspectos diagnósticos e terapêuticos da doença em uma série de pacientes coletada nos últimos 34 anos em um hospital universitário especializado em doenças pulmonares. No período de 1968 a 2002 foram reunidos e estudados 241 casos de abscesso pulmonar de aspiração - 199 em homens e 42 em mulheres, com média de idade de 41,3 anos. Em 69,0% dos pacientes esteve presente a ingestão de álcool e em 64,0% o hábito tabágico. Tosse, expectoração, febre e comprometimento do estado geral foram os achados clínicos mais freqüentes, encontrados na quase totalidade dos casos; 62,5% tinham dor torácica e 30,0% hipocratismo digital. Verificaram-se dentes em mau estado de conservação em 81,7 % dos pacientes, episódio de perda de consciência em 78,0% e presença de odor fétido em 66,0%. Em 85,5% das vezes as lesões localizaram-se em segmento posterior de lobo superior ou segmento superior de lobo inferior, 97,1% delas unilaterais, ocorrendo com igual freqüência tanto no pulmão direito como no esquerdo. A maioria das lesões (66,0%) mediram entre 4,0 e 8,0 cm de diâmetro. Em 25 pacientes (10,4%) houve a associação de empiema pleural. Flora mista, indicativa da presença de germes anaeróbios, foi identificada em secreções broncopulmonares ou pleurais em 172 pacientes (71,4 %). Estreptococos e Gram negativos aeróbios foram também algumas vezes encontrados. Todos os pacientes foram inicialmente tratados com antibióticos (penicilina em 78,0% das vezes) e submetidos a sessões de drenagem postural. Em 51 (21,2%) acabou sendo necessário algum procedimento cirúrgico (24 drenagens de empiema, 21 ressecções pulmonares e 6 pneumostomias). Cura foi obtida em 231 pacientes (95,8%) e 10 (4,2%) foram ao óbito, estes em geral com grandes abscessos, três dos quais também com empiema.
