Duzentos e oitenta leitões, entre 40 e 70 dias de idade, afetados pela sindrome multissistêmica do definhamento (SMD) foram examinados após eutanásia para pesquisa de lesões dentárias. Pelo menos um abscesso periapical foi observado em 58 leitões (20,7%), dos quais 12 apresentaram abscessos múltiplos. Terceiros incisivos superiores, 3os incisivos inferiores, caninos superiores, caninos inferiores e outros dentes apresentaram respectivamente 22 (31,4%), 16 (22,9%), 4 (5,7%), 23 (32,9%) e 5 (7,1%) abscessos periapicais. A maior prevalência de abscessos observada nos dentes 3os incisivos e caninos inferiores está provavelmente associada com a maior área de corte ou desgaste que sofrem esses dentes. Entre as bactérias isoladas de amostras de 65 abscessos, Streptococcus sp. foi a mais prevalente e esteve presente em 21,48% e 27,7% dos isolados em aerobiose e anaerobiose, respectivamente. Na segunda posição em prevalência estiveram as bactérias corineformes, as quais foram mais freqüentemente isoladas em atmosfera anaeróbica do que aeróbica. Houve preponderância de isolamentos de bactérias Gram-positivas. Não foi possível determinar se as co-infecções bacterianas predispuseram às lesões características de SMD ou foram conseqüentes à imunossupressão causada pela infecção com PCV2.

INTRODUÇÃO: estudos recentes apontam a doença periodontal (DP) como fator de risco para doença renal crônica (DRC). O objetivo do presente estudo foi avaliar o grau de conhecimento, atitude e prática de médicos e enfermeiros que atuam na nefrologia relativos à DP. MÉTODOS: um questionário foi aplicado a médicos e a enfermeiros participantes do XXV Congresso Brasileiro de Nefrologia (2010) e IX Congresso Mineiro de Nefrologia (2011), abrangendo os seguintes aspectos da DP: epidemiologia, apresentação clínica, fatores predisponentes, repercussão sistêmica, a inclusão do exame da cavidade bucal no exame clínico dos pacientes com DRC e a frequência de encaminhamento para o dentista. RESULTADOS: a maioria dos médicos e enfermeiros responderam corretamente às perguntas que abordaram os conhecimentos gerais sobre a DP. À pergunta referente à inclusão do exame da cavidade bucal no exame físico do paciente, 42,2% dos médicos e 38% dos enfermeiros responderam não fazê-la (p > 0,05). Contudo, a maioria dos pacientes vistos por nefrologistas (59,4%) e enfermeiros (61,8%) são encaminhados ao dentista em menos de 30% das consultas (p > 0,05). CONCLUSÃO: A amostra de nefrologistas e de profissionais de enfermagem participantes do estudo demonstrou conhecimento autorrelatado sobre DP considerado bom, embora com prática clínica limitada, expressada pelo baixo percentual de encaminhamento para tratamento especializado da doença. Os achados sinalizam para a necessidade da instituição de treinamento teórico-prático em saúde bucal nos cursos de graduação (medicina e enfermagem) e pós-graduação (residência médica e multiprofissional).

O corte ou desgaste dos dentes dos leitões recém nascidos é uma prática comum na suinocultura, com o objetivo de reduzir lesões cutâneas na face dos leitões e no aparelho mamário das matrizes. Para avaliação da presença de lesões dentárias, foram analisados após eutanásia 280 animais que apresentavam sinais clínicos de definhamento (refugagem) compatíveis com uma forma de infecção por circovírus (síndrome da refugagem multissistêmica). Entre esses, 58 leitões (21%) apresentaram ao menos um abscesso periapical (total de 70 abscessos), onde: 3os incisivos superiores, 3os incisivos inferiores, caninos superiores e caninos inferiores foram respectivamente responsáveis por 31%, 23%, 6%, e 33% dos abscessos. Outros dentes foram responsáveis por 7% do total de abscessos. A prevalência de abscessos predominantemente nos dentes 3os incisivos e caninos inferiores sugere que estes dentes sofreram maior área de corte/desgaste, refletindo em superior prevalência de abscessos periapicais. Dentre as bactérias classificadas, o Streptococcus sp. foi a mais isolada (21,48% dos isolados em aerobiose e 27,7% em anaerobiose). Houve um isolamento significativamente maior de bactérias corineformes em atmosfera anaeróbica, quando comparado com isolamentos em aerobiose. Houve preponderância de isolamentos de bactérias Gram positivas. Através de um experimento a campo em granja de produção de leitões, foram avaliados 7 tipos de manejos dentários, medindo a influência dos mesmos na variável ganho de peso na maternidade e creche. Nenhuma entre as técnicas avaliadas mostrou diferença estatisticamente significativa.

Acute infections maxillo-facial are of great importance, both for its high population, as the risk of complications. The aim of this study was to present a clinical case of extensive odontogenic abscess in a patient of 31 years of age attended the Service of Surgery and Maxillo-Facial, Faculty of Dentistry of Araçatuba - UNESP in Araçatuba, SP. Complications of infections odontogências although infrequent can lead the patient to death.

Este objeto começa destacando que a abordagem da dor endodôntica é de grande relevância na prática da Atenção Básica e lembra que a situação é preocupante uma vez que, caso o usuário não receba o devido tratamento, a doença poderá evoluir a ponto de gerar lesões mais complexas, que produzam inflamações pulpares. Enfoca a prevenção, indicando que as dores pulpares podem ser causadas por agentes biológicos, agentes físicos e agentes químicos, explicando cada um deles. Conceitua a dor endodôntica como aquela que se origina na polpa dentária, em decorrência de cárie ou de trauma dental, que pode acometer os tecidos periodontais apicais. Especifica como são classificadas as situações de dor endodôntica e define cada caso, abordando detalhes sobre as alterações pulpares (pulpagia hiper-reativa, pulpite sintomática, pulpite assintomática) e as alterações periapicais (pericementite apical, abscesso periapical agudo, abscesso periapical crônico agudizado – abscesso Fênix). Em seguida, orienta sobre a importância da avaliação diagnóstica correta do estado pulpar e periapical, salientando que a interpretação de testes deve estar aliada às informações obtidas na anamnese, pois são fatores que indicam ao profissional o estado de saúde pulpar e periapical do usuário, e apontam, assim, as possibilidades de tratamento a serem executadas, detalhando cada classificação. Ressalta a importância do método sistemático de eliminação, conhecido como diagnóstico diferencial, que conduz o dentista a um diagnóstico definitivo, apresentando um quadro sobre diagnóstico diferencial das alterações pulpares e outro sobre alterações periapicais. Indica que, na abordagem inicial, o principal objetivo é a eliminação do agente agressor e a medicação inicial para posterior atendimento sequencial, e orienta sobre as medidas a serem tomadas para cada um dos casos. Em relação ao atendimento sequencial, aborda que nem sempre será possível resolver a situação com somente um atendimento ao usuário, e repassa orientações sobre o tema tanto para alterações pulpares quanto para alterações periapicais. Finaliza observando que o encaminhamento do usuário para o Centro de Especialidades Odontológicas (CEO) será necessário sempre que o tratamento definitivo for a endodontia e que, em todos os casos de encaminhamentos, há necessidade de que o cirurgião-dentista da Atenção Básica faça o acompanhamento do usuário enquanto este aguarda atendimento no CEO.

This study investigated the presence of the Treponema species in longstanding endodontic retreatment-resistant lesions of teeth with apical periodontitis, the association of this species with clinical/radiographic features, and the association among the different target species. Microbial samples of apical lesions were collected from twenty-five adult patients referred to endodontic surgery after unsuccessful root canal retreatment. Nested-PCR and conventional PCR were used for Treponema detection. Twenty-three periradicular tissue samples showed detectable levels of bacterial DNA. Treponema species were detected in 28% (7/25) of the cases. The most frequently detected species were T. socranskii (6/25), followed by T. maltophilum (3/25), T. amylovorum (3/25), T. lecithinolyticum (3/25), T. denticola (3/25), T. pectinovorum (2/25) and T. medium (2/25). T. vicentii was not detected in any sample. Positive statistical association was found between T. socranskii and T. denticola, and between T. maltophilum and T. lecithinolyticum . No association was detected between the presence of any target microorganism and the clinical or radiographic features. Treponema spp. are present, in a low percentage, in longstanding apical lesions from teeth with endodontic retreatment failure.

To present a case report of a metastasis from cervical cancer to the maxilla, which was misdiagnosed as periapical disease and to caution clinicians that metastases could have a disguised clinical presentation that must be taken into account in the differential diagnosis of periapical disease in oncologic patients. Although metastatic tumours of the jaws are uncommon, they may mimic benign inflammatory processes and reactive lesions. The ability of metastatic lesions to mimic periapical disease is discussed and a brief review of the literature is presented, emphasizing the importance of correct diagnosis to prevent delay in diagnosing cancer. Attention should therefore be given to the patient's medical history, especially of those with a previous history of cancer, and all dental practitioners should be aware of the possibility of metastases that may be confused with periapical disease. Finally, endodontists are well placed to recognize malignant and metastatic oral lesions during the initial clinical stages, given that their treatments are usually based on frequent dental appointments and long-term follow-ups.

The aim of this study was to evaluate periapical repair after root canal filling with different endodontic sealers. Sixty-four root canals from dog´s teeth were filled, divided into 4 groups (n=16). Root canals were instrumented with K-type files and irrigated with 1% sodium hypochlorite solution. Root canals were filled in the same session by active lateral condensation of the cones and sealers: Intrafill, AH Plus, Roeko Seal and Resilon/Epiphany System. After 90 days, the animals were euthanized and the tissues to be evaluated were processed and stained with hematoxylin and eosin. For histopathological analysis, the following parameters were evaluated: inflammatory process, mineralized tissue resorption, and apical mineralized tissue deposition. Histopathological analysis demonstrated that Intrafill had less favorable results in terms of apical and periapical repair, compared to the other sealers (p<0.05). AH Plus, Roeko Seal, and Epiphany sealers had similar and satisfactory results (p>0.05). In conclusion, AH Plus and the materials Roeko Seal and Epiphany are good options for clinical use in Endodontics.

The aim of this study was to quantify radiographically the periapical bone resorption in dogs' teeth contaminated with bacterial endotoxin (LPS), associated or not with calcium hydroxide. After pulp tissue removal, 60 premolars were randomly assigned to 4 groups and were either filled with LPS (group 1), filled with LPS plus calcium hydroxide (group 2) or filled with saline (group 3) for a period of 30 days. In group 4, periapical lesion formation was induced with no canal treatment. Standardized radiographs were taken at the beginning of the treatment and after 30 days and the Image J Program was used for measurement of periapical lesion size. Periapical lesions were observed in groups 1 (average of 8.44 mm2) and 4 (average of 3.02 mm2). The lamina dura was intact and there were no areas of periapical bone resorption in groups 2 and 3. It may be concluded that calcium hydroxide was effective in inactivating LPS, as demonstrated by the absence of apical periodontitis in the roots that were filled with bacterial endotoxin plus calcium hydroxide.

This study evaluated histopathologically different methods of experimental induction of periapical periodontitis. The radiographic and microbiological evaluations have been performed in a previous investigation. Fifty-seven root canals from dogs' teeth were assigned to 4 groups. In GI (n=14) and GII (n=14), the root canals were exposed to oral environment for 180 days; in GIII (n=14) and GIV (n=15) the root canals were exposed for 7 days and then the access cavities were restored and remained sealed for 53 days. The root apices of GI and GIII were perforated, whilst those of GII and GIV remained intact. After induction of periapical periodontitis, the dogs were euthanized. Serial sections were obtained and stained with hematoxylin and eosin. Data of the histopathological evaluation were submitted to Kruskal-Wallis and Dunn's tests at 5% significance level. The inflammatory periapical reaction and resorption of mineralized tissues were less intense in GII than in the other groups (p<0.05). There was no histopathological difference among the experimentally induced periapical lesions in the teeth with coronal sealing. On the other hand, when coronal sealing was not performed, greater intensity of induced periapical periodontitis was observed in the teeth with apical perforation.

The aim of this study was to evaluate the microbial distribution in the root canal system after periapical lesion induction in dogs' teeth using different methods. Fifty-two root canals were assigned to 4 groups (n=13). Groups I and II: root canals were exposed to the oral cavity for 180 days; groups III and IV: root canals were exposed for 7 days and then the coronal openings were sealed for 53 days. The root apices of groups I and III were perforated, while those of groups II and IV remained intact. After the experimental periods, the animals were euthanized and the anatomic pieces containing the roots were processed and stained with the Brown & Brenn method to assess the presence and distribution of microorganisms. The incidence of microorganisms at different sites of the roots and periapical lesions was analyzed statistically by the chi-square test at 5% significance level. All groups presented microorganisms in the entire root canal system. A larger number of microorganisms was observed on the root canal walls, apical delta and dentinal tubules (p<0.05), followed by cementum and cemental resorption areas. In spite of the different periods of exposure to the oral environment, the methods used for induction of periapical periodontitis yielded similar distribution of microorganisms in the root canal system.

A retrospective survey was designed to identify diagnostic subgroups and clinical factors associated with odontogenic pain and discomfort in dental urgency patients. A consecutive sample of 1,765 patients seeking treatment for dental pain at the Urgency Service of the Dental School of the Federal University of Goiás, Brazil, was selected. Inclusion criteria were pulpal or periapical pain that occurred before dental treatment (minimum 6 months after the last dental appointment), and the exclusion criteria were teeth with odontogenic developmental anomalies and missing information or incomplete records. Clinical and radiographic examinations were performed to assess clinical presentation of pain complaints including origin, duration, frequency and location of pain, palpation, percussion and vitality tests, radiographic features, endodontic diagnosis and characteristics of teeth. Chi-square test and multiple logistic regression were used to analyze association between pulpal and periapical pain and independent variables. The most frequent endodontic diagnosis of pulpal pain were symptomatic pulpitis (28.3%) and hyperreactive pulpalgia (14.4%), and the most frequent periapical pain was symptomatic apical periodontitis of infectious origin (26.4%). Regression analysis revealed that closed pulp chamber and caries were highly associated with pulpal pain and, conversely, open pulp chamber was associated with periapical pain (p<0.001). Endodontic diagnosis and local factors associated with pulpal and periapical pain suggest that the important clinical factor of pulpal pain was closed pulp chamber and caries, and of periapical pain was open pulp chamber.

This study examines the role of Th1 (interferon-gamma [IFN-gamma]) and Th2 (interleukin-4 [IL-4] and IL-10) cytokines, an intercellular adhesion molecule (ICAM-1), and a chemokine receptor (CCR5) in the pathogenesis of periapical lesions at different stages of development in knockout mice. For lesion induction, the first molar was opened and inoculated with 4 bacterial strains and left open to the oral environment. After 21 and 42 days, the IFN-gamma, IL-10, ICAM-1, and CCR5 knockout animals presented periapical lesions larger than those of wild-type animals. There was no statistically significant difference between periapical lesions induced in IL-4 knockout and wild-type animals during the periods evaluated. Our findings suggest an important role for IFN-gamma, IL-10, ICAM-1, and CCR5 in the pathogenesis of experimentally induced pulp infection and bone destruction as endogenous suppressor of periapical lesion development, whereas IL-4 appears to present a nonsignificant effect on periapical lesion modulation.

Objective. The aim of this study was to determine the influence of periapical radiographs, cone beam computed tomography (CBCT) sections, and cone beam volumetric data on the determination of periapical bone destruction in endodontically treated distal root canals of premolar canine teeth. Nontreated mesial roots were used as controls. Study design. Enterococcus faecalis strain (ATCC 29212) was inoculated into 30 root canals of 2 mongrel dogs to induce apical periodontitis. After 60 days, the root canals of the distal roots of the 11 mandibular and 4 maxillary premolars were endodontically treated (n = 15). The mesial root canals were used as controls (no treatment). The bone destruction was evaluated after 6 months by 5 evaluators using periapical radiographs and by CBCT (coronal and sagittal sections). After the experimental period, the area of the lesions in periapical radiographs and CBCT sections were measured in mm(2) using the ImageTool software. A single evaluator measured the volumetric data using the OsiriX software. The comparison between the diagnosis methods in treated root canals and controls was performed using parametric and nonparametric criteria. The Pearson correlation coefficient was computed between radiographic values and CBCT volumetric data in treated root canals and controls. Results. The results showed the presence of chronic apical periodontitis in every inoculated tooth. After 6 months, periapical radiographs, coronal CBCT sections, and volumetric data showed lower bone destruction in endodontically treated teeth in comparison with the control group (P < .05). The 5 evaluators found no differences between the apical periodontitis area of treated teeth and controls when CBCT sagittal sections were used (P > .05). No correlation was found between x-ray and CBCT volumetric values in treated root canals. Conclusions. Although selected CBCT sagittal sections showed similar values of bone destruction in endodontically and nontreated root canals, volumetric CBCT data showed that periapical lesions of endodontically treated root canals had half of the volume of periapical lesions in nontreated root canals. No relationship could be found between the periapical values of bone destruction and volumetric data found in CBCT of treated rood canals.
