Vertical profiles of light scattering at a right angle and turbidity profiles in seawater indicating suspended matter concentration in the near-bottom nepheloid layer (NNL) were measured simultaneously with temperature, salinity, and density profiles at the continental slope off the northwestern Africa. About 100 stations 5' apart in latitude and longitude were carried out over an ocean area of 6100 sq. km. Special features of the NNL variability in the area were analyzed. It was found that some structural parameters of the NNL (maximum transparency depth, that is the upper boundary of NNL; NNL thickness; maximum and total turbidity) correlate with ocean depth. On the average, thickness of the NNL in the area is 20-40% of the ocean depth. At most stations the NNL is fairly strong. In the shelf region NNL turbidity was influenced by the intensive near-shore upwelling. Formation of ''high-energy near-bottom layers'' in the shelf region resulted from passing of a mesoscale cyclonic eddy that caused redistribution of measured quantities within the entire water column.

Viruses such as human immunodeficiency virus (HIV) require cellular activation for expression. Cellular activation in lymphoid cells is associated with augmented accumulation of certain phosphatidic acid (PA) species derived from the hydrolysis of glycan phosphatidylinositol (GPI). This suggests that activation of a phospholipid pathway may play a role in initiation of viral replication. To test this hypothesis, we examined the effect of tat gene expression on the production of cellular PA species, as the Tat protein is essential for HIV expression and has been implicated in activating the expression of multiple host cellular genes. Expression of tat increased the expression of PA. We then tested whether synthetic inhibitors of PA metabolism would inhibit activation of the HIV long terminal repeat by Tat and tumor necrosis factor alpha (TNF-alpha). CT-2576 suppressed both PA generation induced by Tat and HIV long terminal repeat-directed gene expression in response to Tat or TNF-alpha at a posttranscriptional step. CT-2576 also inhibited constitutive as well as TNF-alpha- and interleukin 6-induced expression of HIV p24 antigen in chronically infected U1 cells and in peripheral blood lymphocytes acutely infected with a clinical isolate of HIV. Pharmacological inhibition of synthesis of selected PA species may therefore provide a therapeutic approach to suppression of HIV replication.

Objectives: To describe current practice for the discontinuation of continuous renal replacement therapy in a multinational setting and to identify variables associated with successful discontinuation. The approach to discontinue continuous renal replacement therapy may affect patient outcomes. However, there is lack of information on how and under what conditions continuous renal replacement therapy is discontinued. Design: Post hoc analysis of a prospective observational study. Setting. Fifty-four intensive care units in 23 countries. Patients: Five hundred twenty-nine patients (52.6%) who survived initial therapy among 1006 patients treated with continuous renal replacement therapy. Interventions: None. Measurements and Main Results., Three hundred thirteen patients were removed successfully from continuous renal replacement therapy and did not require any renal replacement therapy for at least 7 days and were classified as the ""success"" group and the rest (216 patients) were classified as the ""repeat-RRT"" (renal replacement therapy) group. Patients in the ""success"" group had lower hospital mortality (28.5% vs. 42.7%, p < .0001) compared with patients in the ""repeat-RRT"" group. They also had lower creatinine and urea concentrations and a higher urine output at the time of stopping continuous renal replacement therapy. Multivariate logistic regression analysis for successful discontinuation of continuous renal replacement therapy identified urine output (during the 24 hrs before stopping continuous renal replacement therapy: odds ratio, 1.078 per 100 mL/day increase) and creatinine (odds ratio, 0.996 per mu mol/L increase) as significant predictors of successful cessation. The area under the receiver operating characteristic curve to predict successful discontinuation of continuous renal replacement therapy was 0.808 for urine output and 0.635 for creatinine. The predictive ability of urine output was negatively affected by the use of diuretics (area under the receiver operating characteristic curve, 0.671 with diuretics and 0.845 without diuretics). Conclusions. We report on the current practice of discontinuing continuous renal replacement therapy in a multinational setting. Urine output at the time of initial cessation (if continuous renal replacement therapy was the most important predictor of successful discontinuation, especially if occurring without the administration of diuretics. Purpose: Most groups have reported disappointing results with autoaugmentation or detrusor myectomy for low capacity/compliance neuropathic bladders. Failure may be due to an ischemic diverticulum or mucosal shrinkage. We investigated whether a Silimed (R) silicone balloon placed in the bladder after autoaugmentation could prevent these problems, improving surgical results. Materials and Methods: We compared the results of standard bladder autoaugmentation in 12 children (group 1) with those in 10 (group 2) who underwent the same surgery using a bladder conformer. The conformer was a silicone balloon filled with saline that remained in the bladder for 2 weeks. All patients had a neuropathic bladder with poor capacity and compliance, resulting in urinary leakage between catheterizations. Preoperative and postoperative evaluation included a voiding diary, ultrasound, voiding cystourethrogram and urodynamics. Results: In group 1 only 1 patient became dry, 4 had little improvement in continence, 4 remained unchanged and 3 became worse. In group 2, 6 patients (60%) become continent without medication, 2 (20%) become continent with oxybutynin and 2 remained unchanged. Bladder capacity and compliance did not change significantly in group 1. However, in group 2 capacity changed from a mean of 140 to 240 ml and mean +/- SD compliance increased from 15.6 +/- 16.8 to 34.3 +/- 22.8 ml/cm H(2)O (p = 0.02). Conclusions: The inflatable balloon improved our long-term results of bladder auto-augmentation. A larger series may be necessary to confirm procedure efficacy and safety.

Liver transplantation is nowadays the only effective answer to adjourn the outcome of functional limitations associated with familial amyloidotic polyneuropathy (FAP), a neurodegenerative disease characterized by sensory and motor polyneuropathies. Nevertheless, there is a detrimental impact associated with the after-surgery period on the fragile physical condition of these patients. Exercise training has been proven to be effective on reconditioning patients after transplantation. However, the effects of exercise training in liver transplanted FAP patients have not been scrutinized yet.

Com o desenvolvimento económico a gestão de recursos é um desafio que a sociedade moderna enfrenta. Um desenvolvimento sustentável, que procure satisfazer as necessidades da geração actual e futuras, é fundamental. É pois necessário encontrar novas fontes de energia e reduzir/valorizar resíduos. O objectivo deste trabalho prende-se com a valorização de um resíduo de ácidos gordos livres como fonte alternativa de energia. Estes ácidos gordos são obtidos na purificação primária da glicerina proveniente da produção de biodiesel por via alcalina, cedida pela empresa SOCIPOLE SA. Inicialmente separaram-se os ácidos gordos livres e ésteres presentes na glicerina, e caracterizou-se o produto obtido. Efectuaram-se dois ensaios, obtendo-se 42,9% e 53,8% em massa de ácidos gordos livres e 41,7% e 34,0% de ésteres no 1º e 2º ensaio respectivamente. Além dos ésteres e ácidos gordos livres, no 1º produto existem 15,4% de matéria não esterificável e no 2º 12,2%. Na primeira parte do trabalho, estudou-se a possibilidade de utilização directa deste produto como combustível. Determinou-se o PCI, obtendo-se 29,8 e 29,4 MJ/kg para o 1º e 2º ensaio respectivamente. Estes valores são superiores aos que se encontram no despacho 17313/2008 de 26 de Junho de 2008, para biogasolina e biodiesel (27,2 MJ/kg), significando que os ácidos gordos livres separados têm forte potencial para serem usados como combustível. Efectuou-se também um estudo de corrosão de aço do corpo e tubos de uma caldeira através de ensaios de polarização, usando um Potenciostato/Galvanostato, e por variação da massa de amostras mergulhadas nos ácidos gordos livres a 105ºC e à temperatura ambiente. Nas condições estudadas, os ácidos gordos não promoveram corrosão nas amostras. Por fim, fez-se a combustão de uma amostra dos ácidos gordos livres num bico de Meeker e verificou-se a formação de depósitos equivalentes a 1,93% da massa inicial. Na segunda parte estudou-se a possibilidade de produzir biodiesel por esterificação, dos ácidos gordos livres. Fez-se um estudo prévio para verificar qual a melhor razão molar de amostra/metanol a utilizar, optando-se por 1:6. Após cinco etapas de esterificação, obteve-se um produto final com 85,6% de ésteres. Não foram então atingidos os 96,5% exigidos pela norma europeia EN 14214, devido à presença de matéria não esterificável na matéria-prima, que não foi possível separar totalmente do produto durante o processo (esterificação, lavagem e secagem). Pela análise de todos os parâmetros, o produto obtido não pode ser considerado biodiesel, não sendo possível ser comercializado como tal. A análise dos resultados aponta para a opção da valorização do resíduo de ácidos gordos na queima directa em geradores e caldeiras que deverá no entanto, ser precedida de um estudo mais detalhado que inclua a análise da constituição da matéria não esterificável, para garantir uma combustão segura.

Detection of Onchocerca volvulus in Simulium populations is of primary importance in the assessment of the effectiveness of onchocerciasis control programs. In Brazil, the main focus of onchocerciasis is in the Amazon region, in a Yanomami reserve. The main onchocerciasis control strategy in Brazil is the semi-annually mass distribution of the microfilaricide ivermectin. In accordance with the control strategy for the disease, polymerase chain reaction (PCR) was applied in pools of simuliids from the area to detect the helminth infection in the vectors, as recommended by the Onchocerciasis Elimination Program for the Americas and the World Health Organization. Systematic sampling was performed monthly from September 1998 to October 1999, and a total of 4942 blackflies were collected from two sites (2576 from Balawaú and 2366 from Toototobi). The molecular methodology was found to be highly sensitive and specific for the detection of infected and/or infective blackflies in pools of 50 blackflies. The results from the material collected under field conditions showed that after the sixth cycle of distribution of ivermectin, the prevalence of infected blackflies with O. volvulus had decreased from 8.6 to 0.3% in Balawaú and from 4 to 0.1% in Toototobi.
