OBJECTIVE: To analyze surgical and pathological parameters and outcome and prognostic factors of patients with nonsmall cell lung cancer (NSCLC) who were admitted to a single institution, as well as to correlate these findings to the current staging system. METHOD: Seven hundred and thirty seven patients were diagnosed with NSCLC and admitted to Hospital do Cancer A. C. Camargo from 1990 to 2000. All patients were included in a continuous prospective database, and their data was analyzed. Following staging, a multidisciplinary team decision on adequate management was established. Variables included in this analysis were age, gender, histology, Karnofsky index, weight loss, clinical stage, surgical stage, chemotherapy, radiotherapy, and survival rates. RESULTS: 75.5% of patients were males. The distribution of histologic type was squamous cell carcinoma 51.8%, adenocarcinoma 43.1%, and undifferentiated large cell carcinoma 5.1%. Most patients (73%) presented significant weight loss and a Karnofsky index of 80%. Clinical staging was IA 3.8%, IB 9.2%, IIA 1.4%, IIB 8.1%, IIIA 20.9%, IIIB 22.4%, IV 30.9%. Complete tumor resection was performed in 24.6% of all patients. Surgical stage distribution was IA 25.3%, IB 1.4%, IIB 17.1%, IIIA 16.1%, IIIB 20.3%, IV 11.5%. Chemotherapy and radiotherapy were considered therapeutic options in 43% and 72%, respectively. The overall 5-year survival rate of nonsmall cell lung cancer patients in our study was 28%. Median survival was 18.9 months. CONCLUSIONS: Patients with NSCLC who were admitted to our institution presented with histopathologic and clinical characteristics that were similar to previously published series in cancer hospitals. The best prognosis was associated with complete tumor resection with lymph node dissection, which is only achievable in earlier clinical stages.

Purpose: Retinoblastoma is a malignant tumor that usually develops in early childhood. During retinoblastoma spreading, RB1 gene inactivation is followed by additional genomic modifications which progressively lead to resistance of tumor cells to death. Drugs that act at downstream levels of death signaling pathways should therefore be interesting in killing retinoblastoma cells. ABT-737, a BH3 mimetic molecule effective at the mitochondrial level, has been shown to induce apoptosis in different human tumoral cell lines as well as in primary patient-derived cells, and in a mouse xenograph model. Methods: In this report, we analyzed the pro-death effect of ABT-737 on two human retinoblastoma cell lines, Y79 and WERI-Rb, as well as on the mouse photoreceptor cell line 661W. Results: We observed that ABT-737 was very effective as a single agent in inducing human WERI-Rb cells apoptosis without affecting the mouse 661W photoreceptor cells. However human Y79 cells were resistant to ABT-737, as a probable consequence of the absence of Bax. The high sensitivity of WERI-Rb to ABT-737 can be increased by downregulating Mcl-1 using the proteasome inhibitor MG-132. Preliminary analysis in primary mouse retinoblastoma tumoral cell lines predicts high sensitivity to ABT-737. Conclusion: Our data suggest that ABT-737 or related compounds could be a highly effective drug in the treatment of some retinoblastomas.

Introduction.- La radiochirurgie Gamma Knife (GKS) est une approche maintenant courante du traitement de la névralgie faciale (NF). Nous proposons les résultats d'une évaluation prospective a très long terme. Patients et méthode.- Entre décembre 1992 et novembre 2010, 737 patients présentant une NF typique (selon HIS 2003) ont été prospectivement sélectionnés, opérés par radiochirurgie et suivi cliniquement. Un isocentre unique de 4 mm avec une dose au maximum en moyenne de 85 Gy (70-90) été positionné sur la portion cisternale rétrogassérienne du trijumeau (IRM et CTscan stéréotaxiques). Les patients souffrant d'une SEP (n = 45) et ceux présentant un mégadolichotronc basilaire (MTB) sont exclus du fait d'une plus forte incidence des échecs dans ces deux sous-groupes (n = 45 et 29) (p = 0,0001). Ceux traités à plusieurs reprises (n = 14) par radiochirurgie le furent en raison d'un risque d'hypoesthésie plus élevé (p = 0,0193). Résultats ou cas rapporté.- Sont présentés les résultats des 497 patients avec un suivi de plus d'un an. Leur suivi médian est de 43 mois (12-174,41) ; et l'âge médian de 79 ans (28-93 ans). La douleur a disparu chez 91,75 % des patients et ce en moyenne en dix jours (1-459 jours). Une hypoesthésie est apparue chez 14,4 % des patients dans un délai médian de 12 mois (1-65). Le taux d'hypoesthésie gênante et très gênante est de 4,42 % et 2,21 % (BNI III et IV). Une récurrence survint chez 34,42 % des patients dans un délai médian de 24 mois (0,6-150). À 10 ans, 67,8 % des patients étaient libres de crise sans nouvelle chirurgie et 45,3 % sans traitement médicamenteux. Conclusion.- Cette étude prospective, unique par sa taille et la longueur de son suivi, démontre qu'en utilisant une cible rétrogassérienne, la majorité des patients présentent un soulagement durable après la GKS. L'hypoesthésie, seule complication rapportée, reste limitée à un petit pourcentage de patients et n'est pas nécessaire à une bonne efficacité de la GKS. Cette série rend légitime le recours à la GKS de première intention.
