The novel ss469415590 variant predicts virological response to therapy in patients with chronic hepatitis C virus type 1 infection.

BACKGROUND: A novel dinucleotide variant TT/∆G (ss469415590) has been associated with hepatitis C virus clearance. AIM: To assess the role of the ss469415590 variant, compared with the known IL28B polymorphisms (rs8099917, rs12979860 and rs12980275) for predicting virological response to therapy in chronic hepatitis C, and its association with the CXCL10 chemokine serum levels - a surrogate marker of interferon-stimulated genes activation. METHODS: Multivariate analysis of factors predicting rapid and sustained virological response in 280 consecutive, treatment-naïve, nondiabetic, Caucasian patients with chronic hepatitis C treated with peginterferon alpha and ribavirin. RESULTS: In hepatitis C virus genotype 1, the OR (95% CI) for rapid and sustained virological response for the wild-type ss469415590 TT was 9.88 (1.99-48.99) and 7.25 (1.91-27.51), respectively, similar to those found for rs12979860 CC [9.55 (1.93-47.37) and 6.30 (1.71-23.13)] and for rs12980275 AA [9.62 (1.94-47.77] and 7.83 (2.02-30.34)], but higher than for rs8099917 TT [4.8 (1.73-13.33) and 4.75 (2.05-10.98)]. In hepatitis C virus genotype 1, mean (SD) CXCL10 levels in patients with the TT/TT, TT/∆G and ∆G/∆G variants were, respectively, 355.1 (240.6), 434.4 (247.4) and 569.9 (333.3) (P = 0.04). In patients with genotypes 2 and 3 no significant association was found for TT/∆G with viral response. The predictive value of ss469415590 was stronger in patients with advanced fibrosis. CONCLUSIONS: The novel IL28B variants at marker ss469415590 predict response to IFN alpha in chronic hepatitis C patients, especially in those with advanced fibrosis. Their determination may be superior to that of known IL28B variants for patient management using IFN-based regimens.

Kuopion ja Siilinjärven taksien yhteisliikenteen vaikutusarviointi

Kuopion ja Siilinjärven taksilupien asemapaikat yhtenäistettiin osittain ensimmäisen kerran jo vuonna 1991. Yhtenäistämispäätös on ollut usean otteeseen hallintotuomioistuinten käsiteltävänä ja viimein marraskuussa 2014 korkeimman hallinto-oikeuden päätöksellä Kuopion ja Siilinjärven taksien täydellinen yhteisliikenne vahvistettiin. Kuopio ja Siilinjärvi muodostavat yhteisen työssäkäynti- ja talousalueen, jossa taksien kysyntä ja keskimääräinen liikevaihto on selvästi suurempi kuin Pohjois-Savon taksien keskimääräinen liikevaihto. Vuosina 2011 ja 2012 Siilinjärven taksien keskimääräinen liikevaihto oli noin 7–8 % pienempi kuin Kuopion taksien keskimääräinen liikevaihto, mutta vuosina 2013 ja 2014 Siilinjärven taksien keskimääräinen liikevaihto oli 2–3 % suurempi kuin Kuopion taksien keskimääräinen liikevaihto. On vaikea arvioida johtuuko Siilinjärven taksien hieman parempi keskimääräisen liikevaihdon kehitys yhteisliikenteestä vai jostakin muusta syystä. Taksinkuljettajille suunnatun kyselyn mukaan Kuopion takseista joka kolmas ottaa vastaan välitetyn kyydin Siilinjärven alueella harvoin tai ei ota sitä vastaan. Vastaavasti Siilinjärven taksit ottavat vastaan välitetyn kyydin aina tai melkein aina koko yhteisliikennealueella. Yleisesti taksien kysyntä on vähentynyt sekä Kuopion, Vuorelan ja Siilinjärven alueella että koko Suomessa viime vuosina. Yhteisliikennealueella taksipalvelut toimivat asiakkaan kannalta kokonaisuutena hyvin. Taksin saatavuus Kuopion keskusta-alueella on vähintään hyvä ja muualla alueella muutamia poikkeuksia lukuun ottamatta riittävää. Myös odotusaika asiakkaalle on pääsääntöisesti lyhyt. Taksinkuljettajien asiakaspalvelu ja paikallistuntemus ovat vähintään hyvällä tasolla. Saman taksimatkan hinta saattaa kuitenkin joissakin tapauksissa vaihdella yhteisliikennealueella, mikä on asiakkaalle hämmentävää. Kuopion ja Siilinjärven taksinkuljettajien välinen työilmapiiri on kyselyn mukaan hieman heikentynyt yhteisliikenteen myötä. Taksiyrittäjien keskuudessa koetaan, etteivät kaikki toimi samojen pelisääntöjen mukaan. Yhteisliikennepäätös on voimassa ja yhteisten toimintatapojen ottaminen käyttöön on asiakaspalvelun kannalta välttämätöntä. Toimintatapoja kehitetään yhteistyössä taksialan ja Pohjois-Savon ELY-keskuksen kanssa. Pohjois-Savon ELY-keskus tulee parantamaan tiedottamista sekä taksien asiakkaiden että taksiyrittäjien suuntaan ja tulee myös seuraamaan yhteisliikenteen toimivuutta säännöllisesti. Savon Taksidata Oy tekee määräajoin taksiautoilijakyselyn, jonka avulla seurataan taksiautoilijoiden kokemuksia muiden muassa yhteisliikenteestä. Lisäksi Savon Taksidata Oy on tekemässä syksyllä 2015 asiakaskyselyä, joka huomioidaan seurannassa. Työpajan ehdotusten pohjalta hinnoittelua voidaan yhdenmukaistaa ja taksin saatavuutta parantaa esimerkiksi yhdistämällä Kuopion ja Vuorelan perusmaksun soveltamisalueet tai lyhentämällä yhteisliikennealueen hakuketjuja niin, että kyytitilaukselle haetaan sopivinta autoa pääsääntöisesti samalta perusmaksun soveltamisalueelta, jolta kyytitilaus on. Jos autoa ei löydy, tilaus jää hakuun ja se tulee näkyviin avoimena tilauksena. Kun kuljettaja ottaa kyytitilauksen vapaaehtoisesti avoimista tilauksista, hän sitoutuu käyttämään tilausta lähimpänä olevan perusmaksun soveltamisalueen taksoitusta. Edellä kuvattu menettely ratkaissee saatavuuden ohella myös taksimatkan hinnoitteluun liittyviä ongelmia.

Dinámicas contemporáneas de las relaciones norte-sur : una aproximación crítica al acaparamiento mundial de tierras en Mozambique. Periodo 2007-2011

El objetivo de la presente monografía es analizar desde una postura crítica el fenómeno del acaparamiento de tierras en Mozambique durante el periodo 2007-2011. El resultado de la presente investigación permite afirmar que el acaparamiento de tierras por parte de países con mayor crecimiento económico en Mozambique durante dicho periodo se relaciona con las dinámicas contemporáneas de las relaciones Norte-Sur en dos sentidos. En primer lugar, es consecuencia de la posición estructural de Mozambique como Estado periférico en la economía-mundo capitalista. Y en segundo lugar, contribuye a exacerbar la polarización imperante en las relaciones actuales entre el Norte y el Sur, a la vez que complejiza las relaciones de explotación y dependencia al interior del Sur mismo.

La vivienda en la perspectiva de seguridad

El presente trabajo busca encontrar posibles conexiones entre la vivienda y el análisis de seguridad. Los avances en la teoría de la seguridad han permitido la inclusión de nuevos aspectos señalados como posibles amenazas que ameritan ser considerados dentro de la óptica de seguridad. En esta perspectiva, el tema de seguridad humana cobra fuerza y permite un análisis multidimensional incluyendo aspectos como la seguridad económica y la seguridad ambiental. En este nuevo marco, la vivienda puede encontrar un espacio para complementar el análisis de inseguridad económica, asociado a la pobreza y calidad de vida y al análisis de inseguridad ambiental vinculada a los desastres naturales y el entorno en que se ubican las viviendas. Al final se concluye que, si bien el análisis puede hacerse, el paso real hacia la securitización de estos temas exige un compromiso enorme del Estado para enfrentar el tema al nivel de una amenaza.

The validity of critical speed determined from track cycling for identification of the maximal lactate steady state

The objective of this study was to determine the critical speed (CS) for track cycling and to assess whether a lactate steady state occurs at this speed. Fourteen competitive cyclists performed the following tests on an official cycling track (333.3 m): 1) incremental test for determination of the intensity corresponding to 4 mM of blood lactate (onset of blood lactate accumulation, OBLA) and maximal oxygen uptake (VO(2)max); 2) CS: 3 maximal bouts for distances of 2, 4 and 6 km executed in random order and with a period of recovery of 40 to 50 min between bouts. CS was determined for each subject from the linear regression between the distance and the time taking to cycle it; 3) Endurance test in which subjects were instructed to pedal at 100% of their individually determined CS for 30 min. At the 10(th) and 30(th) min (or upon exhaustion), 25 mul of blood were collected from ear lobe for later analysis of blood lactate [Lac]b. An increase less than or equal to1 mM between 10 and 30 min of exercise was considered as the criterion for the occurrence of the lactate steady state. CS (49.6 +/- 8.6 ml.kg(-1).min(-1); 36.9 +/- 2.7 km.h(-1)) was significantly higher than OBLA (43.7 8.0 ml.kg(-1).min(-1); 35.24 +/- 2.6 km.h(-1)) although the two parameters were highly correlated (r=0.97). During the endurance test, only 8 of the 14 subjects completed the 30 min period at CS. of these 8 subjects, only 2 presented a lactate steady state. Time to exhaustion at CS was 20.3 +/- 1.6 min for the remaining 6 subjects. The 12 subjects who did not reach a lactate steady state presented mean [Lac]b values of 7.4 +/- 1.3 mM at 10 min and of 9.4 +/- 1.9 mM at the end of the test (exhaustion), characterizing an exercise intensity of high lactacidemia. on the basis of the present results, we can conclude that CS determined by a track cycling test seems to overestimate the intensity of the maximal lactate steady state for most subjects.

Evidence for keratinocyte stem cells in vitro: Long term engraftment and persistence of transgene expression from retrovirus-transduced keratinocytes

Epidermis is renewed by a population of stem cells that have been defined in vivo by slow turnover, label retention, position in the epidermis, and enrichment in β1 integrin, and in vitro by clonogenic growth, prolonged serial passage, and rapid adherence to extracellular matrix. The goal of this study is to determine whether clonogenic cells with long-term growth potential in vitro persist in vivo and give rise to a fully differentiated epidermis. Human keratinocytes were genetically labeled in culture by transduction with a retrovirus encoding the lacZ gene and grafted to athymic mice. Analysis of the cultures before grafting showed that 21.1–27.8% of clonogenic cells with the capacity for >30 generations were successfully transduced. In vivo, β-galactosidase (β-gal) positive cells participated in the formation of a fully differentiated epithelium and were detected throughout the 40-week postgraft period, initially as loosely scattered clusters and later as distinct vertical columns. Viable cells recovered from excised grafts were seeded at clonal densities and 23.3–33.3% of the colonies thus formed were β-gal positive. In addition, no evidence of transgene inactivation was obtained: all keratinocyte colonies recovered from grafted tissue that were β-gal negative also lacked the lacZ transgene. These results show that cells with long-term growth properties in vitro do indeed persist in vivo and form a fully differentiated epidermis, thereby exhibiting the properties of stem cells.
