INTRODUCTION: Among the sleep disorders reported by the American Academy of Sleep, the most common is obstructive sleep apnea-hypopnea syndrome (OSAHS), which is caused by difficulties in air passage and complete interruption of air flow in the airway. This syndrome is associated with increased morbidity and mortality in apneic individuals. OBJECTIVE: It was the objective of this paper to evaluate a removable mandibular advancement device as it provides a noninvasive, straightforward treatment readily accepted by patients. METHODS: In this study, 15 patients without temporomandibular disorders (TMD) and with excessive daytime sleepiness or snoring were evaluated. Data were collected by means of: Polysomnography before and after placement of an intraoral appliance, analysis of TMD signs and symptoms using a patient history questionnaire, muscle and TMJ palpation. RESULTS: After treatment, the statistical analysis (t-test, and the "before and after" test) showed a mean reduction of 77.6% (p=0.001) in the apnea-hypopnea index, an increase in lowest oxyhemoglobin saturation (p=0.05), decrease in desaturation (p=0.05), decrease in micro-awakenings or EEG arousals (p=0.05) and highly significant improvement in daytime sleepiness (p=0.005), measured by the Epworth Sleepiness Scale. No TMD appeared during the monitoring period. CONCLUSION: The oral device developed in this study was considered effective for mild to moderate OSAHS.

Introdução: A Acupunctura, enquanto área terapeutica mais popular na arte de cura na Medicina Tradicional Chinesa, tem sido amplamente utilizada para tratar variadíssimas patologias. A Acupunctura, de acordo com a nova terminologia da Organização Mundial da Saúde, é considerado um método de tratamento complementar. Nas últimas décadas têm surgido diversos estudos que se debruçam sobre a utilização da Acupunctura como meio anestésico, nomeadamente na Medicina Dentária. Objetivo: O presente trabalho visou rever os conceitos associados ao uso da Acupunctura na Medicina Dentária, enfatizando as suas vantagens e limitações e explorar as potencialidades da Acupunctura como meio anestésico em Medicina Dentária. Os objetivos deste trabalho consubstanciam-se em i) estabelecer uma comparação entre os estudos publicados, concretamente entre (1984-1999) e publicados recentemente (2000-2016) e ii) averiguar acerca da eficácia da Acupunctura como meio anestésico em Medicina Dentária. Conclusão: A Acupunctura enquanto técnica de intervenção na Medicina Dentária tem sido relevante devido ao grande número de trabalhos científicos publicados recentemente, que muito têm contribuído para a sua compreensão enquanto meio anestésico. Um grande número de profissionais já aderiu a esta prática, dado que esta apresenta resultados positivos no Tratamento de Distúrbios Temporomandibulares, Xerostomia, Nevralgia do Trigémeo, entre outras patologias odontológicas, e na diminuição do consumo de medicamentos pelos efeitos analgésicos e inflamatórios. O valor da Acupunctura como meio anestésico em Medicina Dentária deve ser tido em linha de conta, pois os estudos sugerem que esta pode ser uma alternativa valiosa, na medida em que representa uma terapia complementar satisfatória no tratamento da dor. Metodologia: A pesquisa bibliográfica foi efetuada nos motores de pesquisa da PubMed, B-On e ScienceDirect no período compreendido entre janeiro de 1984 e fevereiro de 2016 com um limite temporal de 32 anos. Dos 760 artigos encontrados foram selecionados 148 conforme os critérios de inclusão.

Purentaelimistön toimintahäiriöt (temporomandibular disorders,TMD) ovat kasvojen alueen yleisin kiputila. Sillä tarkoitetaan puremalihasten ja leukanivelten sekä niitä ympäröivien kudosten toimintahäiriöitä ja kiputiloja. TMD:hen liittyy usein samanaikaisia monien eri järjestelmien häiriöitä ja muita kipu- ja terveysongelmia. Purentaelimistön toimintahäiriöt voivat olla luonteeltaan ohimeneviä, mutta pieni osa potilaista kärsii kroonisesta kivusta ja hankalahoitoisesta tilasta. TMD-potilaiden diagnostiikan selkeyttämiseksi on kehitetty diagnostinen järjestelmä, Research Diagnostic Criteria for TMD (RDC/TMD), jonka avulla voidaan selvittää potilaan somaattinen diagnoosi ja psykososiaalinen status. Lisäksi RDC/TMD:tä hyödyntäen voidaan potilaan toimintakyvyn arvioimiseksi laskea haitta-aste, jota voidaan käyttää apuna hoitojen räätälöinnissä. Tutkielman tarkoituksena oli tarkastella Tyksin suu- ja leukasairauksien klinikalle hoitoon lähetettyä potilasmateriaalia haitta-aste luokituksen valossa ja selvittää miten luokittelu ohjasi hoitojen räätälöintiä luokittelun käyttöönoton yhteydessä, ja millaisia tuloksia hoidoilla saavutettiin. Tutkielmassa perehdyttiin potilasaineiston määrään, hoidon vastuuhenkilöihin, potilaiden haitta-astejakaumaan, hoitojen räätälöintiin sekä saavutettuihin hoitotuloksiin. Tutkielman otannassa oli 39 potilasta, jotka oli lähetetty Tyksin suu- ja leukasairauksien poliklinikalle kasvojen alueen kipujen vuoksi 8/2013−4/2014 välisenä aikana. Potilaat täyttivät RDC/TMD:hen pohjautuvan kipukyselykaavakkeen, jonka perusteella hoidot räätälöitiin haitta-asteryhmittäin: ei haittaava TMD, lievästi haittaava TMD ja runsaasti haittaava TMD. Lisäksi potilaille lähetettiin seurantaa varten uusi kipukyselykaavake 2/2016. Alkutilanteessa ei haittaavaa TMD-kipua koki 35 %, lievästi haittaavaa 21 % ja runsaasti haittaavaa TMD-kipua 44 % potilaista. Runsaasti haittavasta TMD-kivusta kärsivät olivat psyykkisesti kuormittuneempia ja heillä oli enemmän muita kipuja kuin muilla potilailla. Tulokset olivat samankaltaisia kuin aiemmin kuvatulla erikoissairaanhoidon potilasmateriaalilla. Seuranta-ajan jälkeen runsaasti haittaavasta TMD:stä kärsivien potilaiden määrä oli vähentynyt merkittävästi, ja osalla potilaista kasvojen alueen kivut olivat kokonaan hävinneet. Terveyspalveluiden käyttö kaikissa potilasryhmissä oli vähentynyt merkittävästi.

Temporomandibular disorder (TMD) is characterized by a combination of symptoms affecting the temporomandibular joint and/or chewing muscles. The two most common clinical TMD symptoms are pain and dysfunction. Pain is usually caused by dysfunction, and emergency therapy has focused on controlling it. Recent investigations into TMD have led to the recommendation of antidepressants as a supporting treatment against constant neuralgic pain. The aim of this double-blind study was to verify the efficiency of antidepressants (amitriptyline) as a support in the treatment of chronic TMD pain. Twelve female volunteers presenting chronic TMD pain were divided into two groups and treated for 14 days: Group 1 with 25 mg/day of amitriptyline and Group 2 with a placebo. The intensity of pain and discomfort was evaluated daily, using a visual analog scale (VAS), over a period of seven days preceding the treatment (baseline), during the 14-day treatment, and for seven days after the treatment. The results revealed a significant reduction of pain and discomfort in Group 1 (75%) compared to Group 2 (28%) during the three weeks beginning at baseline (p< 0.01). Amitriptyline proved to be an efficient alternative treatment for chronic pain in TMD patients. Copyright © 2003 by CHROMA, Inc.

Even nowadays there is in Brasil a large number of edentulous and a precarious financial condition of most of the population. In addition, World Health Organization aims for oral health, which consists on the maintenance of a natural dentition, functional and aesthetic composed of at least 20 teeth, without need of prosthetic intervention throughout life. From this and considering the lack of researches about the permanence of edentulous spaces in the oral cavity, and also avoiding overtreatment, this research has been proposed. Thus, the aim was to evaluate the effect of different lengths of the shortened lower dental arch in the presence or absence of a removable partial denture (RPD) on masticatory function, quality of life and occurrence of temporomandibular dysfunction. To achieve this goal, we compared the masticatory efficiency (colorimetric test), the oral comfort through the analysis of the impact of oral health in quality of life (OHIP-14), the presence of temporomandibular dysfunction (RDC/TMD) and the general quality of life (WHOQOL) of patients with shortened dental arches (SDA) (n=60), which is an arch with a reduction of teeth starting posteriorly, and patients with complete dental arch (Complete DA) (n = 34). The group of patients whit SDA was divided among PPR wears (PPD + SDA) (n = 17) and non-wears (n = 43). The population of this study consisted on patients who received or looked for treatment at the clinics of the Department of Dentistry of UFRN, from clinical analysis and records. The sample was chosen by convenience. For statistical analysis, it was a database in SPSS 17.0, followed by descriptive analysis with frequencies, absolute values, tests of central tendency and variability. The statistical tests used were chi-squared and analysis of variance as well as Tukey s post test, when applicable, all with a 95% confidence level. The results shown a prevalence of TMD of 47,1% among patients using PRP and 69,8% among those who didn t, but this result wasn t statistically significant. The mean of the results of masticatory efficiency, WHOQOL and OHIP didn t show association to the presence or absence of PPR and to the lower number of occlusal units of the patients (0, 1, 2 or more occlusal). The association only occurred among the different groups of SDA and the patients with complete dental arch. Taking into account the results, it could be observed that studied patients with low posterior support using lower PRP didn t have better masticatory efficiency, general quality of life, less impacts of their oral conditions in quality of life or not even less temporomandibular dysfunction or better masticatory efficiency when compared to those who didn t use the prosthesis

The present study compared the duration of the electromyographic silent period (SP) of the masseter muscles elicited by chin-tapping in normal asymptomatic adults (N = 39) and in a group with symptoms of temporomandibular joint dysfunction (TMJD) (N = 31).2. EMG activity was recorded from right (RM) and left (LM) masseter muscles using bipolar surface electrodes coupled to a DISA 1500 EMG-System. During maximal clenching, ten taps were applied downwards to the chin with a reflex hammer.3. The mean SP durations obtained for the normal group were 25.45 +/- 4.20 ms (RM) and 25.33 +/- 4.18 ms (LM), whereas the TMJD group presented significantly greater values (P < 0.01, Student t-test) of 41.89 +/- 12.94 ms (RM) and 42.40 +/- 12.99 ms (LM). The upper limits of normality calculated for RM and LM were 32.36 ms and 31.21 ms, respectively. Eighty-four percent of patients with TMJD showed SP durations above these limits.4. The results indicate that the measurement of masseteric SP duration may be used as an objective diagnostic method of TMJ disorders, provided that borderline values are interpreted with caution along with clinical impressions.

In order to evaluate the presence of TMD (temporomandibular disorder), dissatisfaction related to the use of removable partial dentures (RPD) and the effect of the treatment on temporomandibular joint noises, 13 female patients with Kennedy class I and II mandibular arch were selected. Another 13 young, asymptomatic, dentate patients, also female, were used as reference. After four years, 38.4% were found to have a moderate or severe degree of TMD. Over the four years, the degree of TMD increased in 46.15% of the patients, was not affected in 20.07%, while in 15.38% it decreased or the patients remained free from signs and symptoms. About 30% of the patients at the second year and 46% after the fourth year, had complaints regarding retention and stability. It was concluded that there is no relationship between TMD and the condition of partially edentulous Kennedy class I and II, but patient dissatisfaction increased after the second year and temporomandibular joint noise was reduced significantly with the replacement of the teeth.

Ankylosis of the temporomandibular joint in children is one the most difficult and complex conditions managed by oral and maxillofacial surgeons, and often leads to some facial deformity. Distraction osteogenesis of the mandible provides an excellent treatment for mandibular airway obstruction in children who do not respond to conservative measures, and allows for early removal of the tracheostomy. We report the case of a 1-year-old boy with severe micrognathia and temporomandibular ankylosis who was dependent on a tracheostomy; he was treated with piezosurgery and mandibular advancement by distraction osteogenesis. © 2010 The British Association of Oral and Maxillofacial Surgeons. Anamnesis, clinical examinations and temporomandibular joint transcraneal radiographs for 22 adults with cleft lip and palate were carried out in order to evaluate the occlusion and correlate it with radographic findings. The conclusions were: 72.8% of the patients have at least one sign or symptom of craniomandibular disorders (CMD); although the occlusal conditions were severely altered, most of the signs and symptoms were classified as mild; the greater frequency of the signs and symptoms occurred among women; in the radiographic evaluation, all of the assymptomatic patients had both condyles with normal contour and all of the patients with altered contour had at least one sign or symptom; the bilateral centered position of the condyles in the fossa e did not warrant the absence of signs and symptoms; some patients with bilateral condyles positioned posteriorly or caudally or even assimetrically, did not present signs and symptoms of dysfunction; the radiographic findings should be correlated with clinical findings; and a great number of patients were not observed with clinical board of C:MD caused by the occlusion. This study gathered some of the most relevant researches already undertaken regarding the Temporomandibular Joint, presenting descriptions of its anatomy and dynamics, and relating them to the main aspects of joint dysfunctions. Literature recognizes that the Temporomandibular Joint, responsible for lower jaw movements, comprehends the skull base and the jaw bone. Its anatomy and dynamics are of great importance in order to understand the functions of Stomatognathic System as well as to study Temporomandibular Joint Dysfunction. The elements that compose this synovial joint and the bilateral characteristic confer eminent importance to the ATM before the skeleton head, the ability to perform broad movements and high complexity in its operation. It is accepted that the overload of one anatomical structure from the TMJ or other components of the Stomatognathic System leads to Temporomandibular Joint Dysfunctions. These disorders have different etiologies, ranging from malocclusion to emotional stress, and various forms of treatment, which are related in this study

Purpose: The aim of this study was to test the association between quality of sleep and stress in individuals with temporomandibular disorder (TMD). Methods: The study sample consisted of 354 adult subjects (males and females) from the municipality of Piacatu, São Paulo state, in the Southeast region of Brazil. Data were collected using the Fonseca’s Questionnaire to record the level of TMD, the Pittsburgh Sleep Quality Index (PSQI) to assess quality of sleep and the Social Readjustment Rating Scale (SRRS) to record stress level. The data were analyzed by the software Epi Info 2000 version 3.2 using a chi-square test at the 0.05 level of significance. Results: One hundred and eighty (50.8%) subjects had some level of TMD. The statistical analysis showed a significant relationship between the three stress scores and the presence or absence of sleep disorders, considering an overall PSQI score > 5 as an indicator of a subject with sleep problems (P<0.01). Conclusion: Both quality of sleep and stress levels were associated with TMD in this sample.

Objective: The objective of this study was to determine the frequency of signs and symptoms of temporomandibular disorder (TMD) in fibromyalgic patients. Methods: Sixty subjects of both sexes (mean age, 49.2 +/- 13.8 years) with fibromyalgia (FM) diagnosis were included in this study. All patients were examined by a calibrated investigator to identify the presence of TMD using the Research Diagnostic Criteria for TMD. Results: The most common signs (A) and symptoms (B) reported by FM patients were (A) pain in the masticatory muscles (masseter, 80%; posterior digastric, 76.7%), pain in the temporomandibular joint (83.3%), and 33.3% and 28.3%, respectively, presented joint sounds when opening and closing the mouth; (B) headache (97%) and facial pain (81.7%). In regard to the classic triad for the diagnosis of the TMD, it was found that 35% of the FM patients presented, at the same time, pain, joint sounds, and alteration of the mandibular movements. It was verified that myofascial pain without limitation of mouth opening was the most prevalent diagnosis (47%) for the RDC subgroup I. For the subgroup II, the disk displacement with reduction was the most prevalent diagnosis (21.6%). For the subgroup III, 36.7% of the subjects presented osteoarthritis. Conclusions: Thus, there is a high prevalence of signs and symptoms of TMD in FM patients, indicating the need for an integrated diagnosis and treatment of these patients, which suggest that the FM could be a medium- or long-term risk factor for the development of TMD.
