Background: Pacemaker implantation (PMI) may predispose to venous thromboembolism (VTE) and obstruction (VO). This prospective study aimed at quantifying changes in venous calibers, and at determining the incidence of symptomatic and asymptomatic VTE/VO after PMI. Further goals included an assessment of the role of transesophageal echocardiography (TEE) in the diagnosis of lead-related central venous thrombi (CVT), and determination of predictors for VTE/VO. Methods: 150 (mean age 67; 61% male) consecutive patients with first PMI were enrolled and followed for 6 months. Contrast venography was performed at baseline and 6 months after PMI to measure venous diameters, and to detect stenosis, total occlusions and thrombi. TEE was conducted in 66 patients. Based on clinical suspicion, work-up for pulmonary embolism (PE) or acute deep vein thrombosis (DVT) were performed as needed. A total of 50 cases underwent longer-term (mean 2.4 years) follow-up venography. All cases with VTE/VO during the initial 6 months, and their matched controls, were selected for a case-control study focused on possible predictive role of laboratory and patient-related factors for the development of VTE/VO. Results: 10 (7 %) patients were found to have baseline venous abnormalities (e.g. 8 obstructions). Mean venous diameters diminished significantly during the first 6 months, but no further reduction occurred in late follow-up. New VO was discovered in 19 patients (14 %; 14 stenosis, 5 total occlusions; all asymptomatic). Small non-obstructive thrombi were found in 20/140 (14 %) 6-month venograms. TEE at 6 months disclosed CVT in 6 (9 %) patients. One (0.7 %) patient had acute symptomatic upper-extremity DVT, and PE was discovered in 5/150 (3.3 %) patients during the first 6 months with no further cases thereafter. At 6 months, the total number of cases with VTE/VO amounted to 47 (31.3 %). Additionally, the later 2-year venograms (n=50) disclosed 4 (8 %) total occlusions and 1 (2 %) stenosis. In the case-control study, no parameter was predictive of venous end-points as a single variable, but there appeared to be significant clustering of traditional VTE risk-factors among the cases. Laboratory parameters showed a definite acute hypercoagulative state induced by PMI, but its degree did not predict subsequent development of VTE/VO. Conclusions: This study shows that VTE/VO is relatively common after PMI with an overall incidence of at least 30 %. Although the majority of the lesions are asymptomatic and clinically benign, cases of PE were also encountered, and totally occluded veins may hamper future upgrading or replacement of pacing system. Venous complications seem difficult to prognosticate as firm predictors were not identified from a wide range of parameters analyzed in this study, although clustering of classic VTE risk factors may be a predisposing factor. Parameters related to implantation procedure or pacing systems and the severity of implantation-induced trauma did not emerge as predictors.

The precise nature of the reaction between nitric acid and sodium ortho-vanadate solutions has been studied by means of electrometric techniques involving potentiometric and conductometric titrations. The well defined inflections and breaks in the titration curves confirm the existence of the anions, pyro-V2O7(4-), meta-VO3- and poly-H2V10O28(4-) corresponding to the ratios of VO4(3-):H+ as 1:1, 1:2 and 1:2.6 in the neighborhood of pH 10.5, 7.4 and 3.6, respectively. The interaction of cerium(III) nitrate with sodium vanadate solutions, at specific pH levels 12.4, 10.5, 7.4 and 3.6 was also studied by potentiometric and conductometric titrations between the reactants. The end-points obtained from the sharp inflections in the titration curves provide definite evidence for the formation and precipitation of cerium ortho-Ce2O3.V2O5, pyro-2Ce2O3.3V2O5 and meta-Ce2O3.3V2O5 vanadates in the neighborhood of pH 7.4, 6.2 and 4.8, respectively. Analytical investigations on the precipitates formed confirm the results of the electrometric study.

In order to assess the effect of air pollution on pediatric respiratory morbidity, we carried out a time series study using daily levels of PM10, SO2, NO2, ozone, and CO and daily numbers of pediatric respiratory emergency room visits and hospital admissions at the Children's Institute of the University of São Paulo Medical School, from August 1996 to August 1997. In this period there were 43,635 hospital emergency room visits, 4534 of which were due to lower respiratory tract disease. The total number of hospital admissions was 6785, 1021 of which were due to lower respiratory tract infectious and/or obstructive diseases. The three health end-points under investigation were the daily number of emergency room visits due to lower respiratory tract diseases, hospital admissions due to pneumonia, and hospital admissions due to asthma or bronchiolitis. Generalized additive Poisson regression models were fitted, controlling for smooth functions of time, temperature and humidity, and an indicator of weekdays. NO2 was positively associated with all outcomes. Interquartile range increases (65.04 µg/m³) in NO2 moving averages were associated with an 18.4% increase (95% confidence interval, 95% CI = 12.5-24.3) in emergency room visits due to lower respiratory tract diseases (4-day moving average), a 17.6% increase (95% CI = 3.3-32.7) in hospital admissions due to pneumonia or bronchopneumonia (3-day moving average), and a 31.4% increase (95% CI = 7.2-55.7) in hospital admissions due to asthma or bronchiolitis (2-day moving average). The study showed that air pollution considerably affects children's respiratory morbidity, deserving attention from the health authorities.

Diabetic retinopathy, age-related macular degeneration and glaucoma are the leading causes of blindness worldwide. Automatic methods for diagnosis exist, but their performance is limited by the quality of the data. Spectral retinal images provide a significantly better representation of the colour information than common grayscale or red-green-blue retinal imaging, having the potential to improve the performance of automatic diagnosis methods. This work studies the image processing techniques required for composing spectral retinal images with accurate reflection spectra, including wavelength channel image registration, spectral and spatial calibration, illumination correction, and the estimation of depth information from image disparities. The composition of a spectral retinal image database of patients with diabetic retinopathy is described. The database includes gold standards for a number of pathologies and retinal structures, marked by two expert ophthalmologists. The diagnostic applications of the reflectance spectra are studied using supervised classifiers for lesion detection. In addition, inversion of a model of light transport is used to estimate histological parameters from the reflectance spectra. Experimental results suggest that the methods for composing, calibrating and postprocessing spectral images presented in this work can be used to improve the quality of the spectral data. The experiments on the direct and indirect use of the data show the diagnostic potential of spectral retinal data over standard retinal images. The use of spectral data could improve automatic and semi-automated diagnostics for the screening of retinal diseases, for the quantitative detection of retinal changes for follow-up, clinically relevant end-points for clinical studies and development of new therapeutic modalities.

Résumé Introduction L’amlodipine et l’atorvastatine offrent des avantages thérapeutiques au-delà de leur indication primaire, soit la réduction de la pression artérielle et des lipides sanguins, respectivement. L’amlodipine induit l’apoptose des cellules de muscle lisse vasculaire (CMLV) in vivo, contribuant à la régression de l'hypertrophie aortique chez le rat spontanément hypertendu (SHR). L'atorvastatine induit l’apoptose des CMLV in vitro, un effet proportionnel à la dose. Toutefois, cet effet reste à être démontré in vivo. Nous postulons que l’atorvastatine induira la régression de l’hypertrophie aortique via l’apoptose des CMLV chez le SHR, et que la combinaison de l’amlodipine et de l’atorvastatine aura un effet synergique sur la régression de l’hypertrophie aortique via l’apoptose des CMLV chez le SHR. Méthodologie L’amlodipine et l’atorvastatine ont été administrées à des SHR âgés de 11 semaines durant trois ou six semaines, individuellement ou en combinaison. Les points principaux à l'étude étaient le remodelage vasculaire et la pression artérielle. La fragmentation et le contenu en ADN, le stress oxydant, le taux de cholestérol et les niveaux de nitrates ont aussi été mesurés. Résultats Lorsque l’atorvastatine a été administrée seule, une diminution significative du stress oxydant et de la pression artérielle a été observée après trois et six semaines de traitement, respectivement. Par contre, aucune différence n’a pu être décelée quant au remodelage vasculaire. L'amlodipine a réduit la pression artérielle et l'hypertrophie aortique de façon dépendante de la dose. Une diminution significative de l'hyperplasie a été détectée après trois semaines de traitement avec la combinaison, et après six semaines avec une faible dose d'amlodipine. Conclusion Nos résultats ne supportent pas l'hypothèse que l'atorvastatine induit l'apoptose des CMLV in vivo. Par contre, lorsque combinée à l'amlodipine, elle pourrait ajouter un bénéfice supplémentaire au niveau de la réduction de l'hyperplasie aortique.

Introducion: Dado el incremento de los pacientes con edades por encima de los 80 años que son llevados a cirugía cada año en nuestra institución consideramos importante conocer el comportamiento en terminos de morbilidad y mortalidad temprana de los pacientes octogenarios sometidos a endarterectomia carotidea. Objetivo: Evaluar los resultados a 30 días en pacientes octogenarios con enfermedad carotidea severa sintomática y asintomática llevados a endarterectomía carotidea comparados con una cohorte historia de pacientes de menor edad operados desde 1995 en la fundación Cardioinfantil. Se registraron como variables dependientes mortalidad y evento cerebrovascular.

Introducción: La displasia renal multiquistica es una variante de displasia renal, anomalía congénita frecuente del tracto urinario, con una prevalencia del 2.9 - 50 % de insuficiencia renal crónica; frecuentemente asociada a otras malformaciones urinarias, sin datos en bogotá sobre prevalencia y comportamiento clínico. Objetivo: Evaluar la prevalencia de insuficiencia renal crónica en niños con displasia renal multiquistica valorados en consulta de nefrología en Fundación Cardioinfantil, instituto de cardiología de Bogotá. Metodología: Estudio de corte transversal, en niños con displasia renal multiquistica, confirmado por ecografía, valorados en consulta de Nefrología Pediátrica en los últimos diez años. Se realizó un análisis descriptivo de las variables, cálculos de prevalencia de Insuficiencia renal crónica. Resultados: Se revisó información de 70 pacientes, encontrando una prevalencia de IRC de 22.85% (IC 95 % 13.0 %-35.1 %); mayor frecuencia mujeres 12.85 %; 14.28% con otras malformaciones renales; 5.71 % en involución parcial, 4.28% en pacientes con involución completa del tamaño del riñón displásico. Se encontró 31.4% proteinuria; 22.8 % hiperfiltración; 4.28% hipertrofia compensadora del riñón sano; 24.2% involución parcial, 31.4 % involución completa del tamaño renal; frecuencia de HTA de 7,1% (IC95% 1%-9%). El 87.14% tuvo diagnóstico prenatal (IC 95% 81.0%-96.0%). Discusión: La prevalencia se encuentra dentro de los rangos de la literatura mundial, mayor a la colombiana y suramericana, predominando en pacientes con otras malformaciones renales asociadas, con mayor prevalencia de hipertensión arterial, que requiere estudios multicéntricos para determinar causalidad o presencia de otros factores.

Objectives The administration of unfractionated heparin (UFH) prior to carotid clamping during carotid endarterectomy (CEA) transiently increases the platelet aggregation response to arachidonic acid (AA) despite the use of aspirin. We hypothesized that this phenomenon might be reduced by using low molecular weight heparin (LMWH) resulting in fewer emboli in the early post-operative period. Methods 183 aspirinated patients undergoing CEA were randomised to 5000 IU UFH (n = 91) or 2500 IU LMWH (dalteparin, n = 92) prior to carotid clamping. End-points were: transcranial Doppler (TCD) measurement of embolisation, effect on bleeding and platelet aggregation to AA and adenosine 5′-diphosphate (ADP). Results Patients randomised to UFH had twice the odds of experiencing a higher number of emboli in the first 3 h after CEA, than those randomised to LMWH (p = 0.04). This was not associated with increased bleeding (mean time from flow restoration to operation end: 23 min (UFH) vs. 24 min (LMWH), p = 0.18). Platelet aggregation to AA increased significantly following heparinisation, but was unaffected by heparin type (p = 0.90). The platelets of patients randomised to LMWH exhibited significantly lower aggregation to ADP compared to UFH (p < 0.0001). Conclusions Intravenous LMWH is associated with a significant reduction in post-operative embolisation without increased bleeding. The higher rate of embolisation seen with UFH may be mediated by increased platelet aggregation to ADP, rather than to AA.

Background: Endothelial dysfunction may be related to adverse effects of some dietary fatty acids (FAs). Although in vitro studies have failed to show consistent findings, this may reflect the diverse experimental protocols employed and the limited range of FAs and end points studied. Aims: To investigate the effect of dietary FA type (saturated, monounsaturated, n-6 and n-3 polyunsaturated fatty acids), concentration, incubation time and cell stimulation state, on a broad spectrum of endothelial inflammatory gene expression. Methods: Using human umbilical vein endothelial cells, with and without stimulation (+/- 10 ng/ml TNF alpha), the effects of arachidonic (AA), docosahexaenoic (DHA), eicosapentaenoic (EPA), linoleic (LA), oleic (OA) and palmitic acids (PA) (10, 25 and 100 mu M), on the expression of genes encoding a number of inflammatory proteins and transcription factors were assessed by quantitative real time RT-PCR. Results: Individual FAs differentially affect endothelial inflammatory gene expression in a gene-specific manner. EPA, LA and OA significantly up-regulated MCP-1 gene expression compared to AA (p = 0.001, 0.013, 0.008, respectively) and DHA (p < 0.0005, = 0.004, 0.002, respectively). Furthermore, cell stimulation state and FA incubation time significantly influenced reported FA effects on gene expression. Conclusions: The comparative effects of saturated, monounsaturated, n-6 and n-3 polyunsaturated FAs on endothelial gene expression depend on the specific FA investigated, its length of incubation, cell stimulation state and the gene investigated. PURPOSE: Most studies on probiotics utilise single strains, sometimes incorporated into yoghurts. There are fewer studies on efficacy of mixtures of probiotic strains. This review examines the evidence that (a) probiotic mixtures are beneficial for a range of health-related outcomes and (b) mixtures are more or less effective than their component strains administered separately. RESULTS: Mixtures of probiotics had beneficial effects on the end points including irritable bowel syndrome and gut function, diarrhoea, atopic disease, immune function and respiratory tract infections, gut microbiota modulation, inflammatory bowel disease and treatment of Helicobacter pylori infection. However, only 16 studies compared the effect of a mixture with that of its component strains separately, although in 12 cases (75%), the mixture was more effective. CONCLUSION: Probiotic mixtures appear to be effective against a wide range of end points. Based on a limited number of studies, multi-strain probiotics appear to show greater efficacy than single strains, including strains that are components of the mixtures themselves. However, whether this is due to synergistic interactions between strains or a consequence of the higher probiotic dose used in some studies is at present unclear.

Classical hypothesis testing focuses on testing whether treatments have differential effects on outcome. However, sometimes clinicians may be more interested in determining whether treatments are equivalent or whether one has noninferior outcomes. We review the hypotheses for these noninferiority and equivalence research questions, consider power and sample size issues, and discuss how to perform such a test for both binary and survival outcomes. The methods are illustrated on 2 recent studies in hematopoietic cell transplantation.

Oxidative DNA damage plays a role in disease development and the aging process. A prominent participant in orchestrating the repair of oxidative DNA damage, particularly single-strand breaks, is the scaffold protein XRCC1. A series of chronological and biological aging parameters in XRCC1 heterozygous (HZ) mice were examined. HZ and wild-type (WT) C57BL/6 mice exhibit a similar median lifespan of similar to 26 months and a nearly identical maximal life expectancy of similar to 37 months. However, a number of HZ animals (7 of 92) showed a propensity for abdominal organ rupture, which may stem from developmental abnormalities given the prominent role of XRCC1 in endoderm and mesoderm formation. For other end-points evaluated-weight, fat composition, blood chemistries, condition of major organs, tissues and relevant cell types, behavior, brain volume and function, and chromosome and telomere integrity-HZ mice exhibited by-and-large a normal phenotype. Treatment of animals with the alkylating agent azoxymethane resulted in both liver toxicity and an increased incidence of precancerous lesions in the colon of HZ mice. Our study indicates that XRCC1 haploinsufficiency in mammals has little effect on chronological longevity and many key biological markers of aging in the absence of environmental challenges, but may adversely affect normal animal development or increase disease susceptibility to a relevant genotoxic exposure.

Objetivou-se com este estudo comparar a seleção com base no ganho médio diário de peso na pré-desmama (GMD) e no número de dias para ganhar 160 kg nessa fase (D160), com e sem correção para efeitos de grupos de contemporâneos (GC), em bovinos da raça Guzerá. Utilizou-se o banco de dados de desenvolvimento ponderal da Associação Brasileira de Criadores de Zebu (ABCZ) para esta raça. A obtenção dos parâmetros e valores genéticos foi feita pelo método da máxima verossimilhança restrita utilizando-se modelo unicaracterístico com equações de modelos mistos. O modelo utilizado foi composto pelo efeito fixo de grupo genético e pelos efeitos aleatórios, genético aditivo direto e de ambiente permanente, além do erro residual. A média para D160 foi de 270,5 dias e para GMD, 642,3 g. As correlações de Spearman entre ganho médio diário e a precocidade em dias para ganhar 160 kg na pré-desmama (PD160), e GMD e PD160c (PD160 corrigido para o efeito de grupo de contemporâneo), foram iguais a 0,91 e 0,94, respectivamente. A seleção para PD160 favorece touros que produzem progênie com desempenho superior e menos variável e a padronização deste critério com base no grupo de contemporâneos melhorou sua eficiência. A classificação dos touros varia de acordo com o critério de seleção utilizado, GMD ou PD160, principalmente nos extremos, onde ocorrem seleção e descarte de reprodutores.
