This study aimed to characterize the nociceptive withdrawal reflex (NWR) and to define the nociceptive threshold in 25 healthy, non-medicated experimental sheep in standing posture. Electrical stimulation of the dorsal lateral digital nerves of the right thoracic and the pelvic limb was performed and surface-electromyography (EMG) from the deltoid (all animals) and the femoral biceps (18 animals) or the peroneus tertius muscles (7 animals) was recorded. The behavioural reaction following each stimulation was scored on a scale from 0 (no reaction) to 5 (strong whole body reaction). A train-of-five 1 ms constant-current pulse was used and current intensity was stepwise increased until NWR threshold intensity was reached. The NWR threshold intensity (It) was defined as the minimal stimulus intensity able to evoke a reflex with a minimal Root-Mean-Square amplitude (RMSA) of 20 μV, a minimal duration of 10 ms and a minimal reaction score of 1 (slight muscle contraction of the stimulated limb) within the time window of 20 to 130 ms post-stimulation. Based on this value, further stimulations were performed below (0.9It) and above threshold (1.5It and 2It). The stimulus-response curve was described. Data are reported as medians and interquartile ranges. At the deltoid muscle It was 4.4 mA (2.9–5.7) with an RMSA of 62 μV (30–102). At the biceps femoris muscle It was 7.0 mA (4.0–10.0) with an RMSA of 43 μV (34–50) and at the peroneus tertius muscle It was 3.4 mA (3.1–4.4) with an RMSA of 38 μV (32–46). Above threshold, RMSA was significantly increased at all muscles. Below threshold, RMSA was only significantly smaller than at It for the peroneus tertius muscle but not for the other muscles.

Chapter 1 gives an overview about Streptococcus pneumoniae, its role as a human pathogen and its virulence factors. Additionally, biofilm development and its relevance in clinics are introduced, and the innate immune response to pneumococcus as well as bacterial-viral interactions in the upper respiratory tract are also discussed. Chapter 2 emphasizes the three main topics of this thesis: the role of capsule and pneumolysin in the immune response in the respiratory tract, biofilm formation of S. pneumoniae serotypes and commensal streptococci in vitro, and host innate immune responses to RSV and S. pneumoniae during in vitro co-infections. Aims and hypotheses are provided here. Chapter 3 is divided into two parts: First, the release of the pro-inflammatory cytokines CXCL8 and IL-6 from the human pharyngeal epithelial cell line Detroit 562 and from human bronchial epithelial cells (iHBEC) is described in response to S. pneumoniae. Capsule was shown to suppress the release of both cytokines in both cell lines tested, but release was much less from iHBEC cells. During intranasal colonization of mice, suppression of CXCL8 release by the capsule was also observed in vivo, but the effect was only measured in the absence of pneumolysin. Long term, stable nasopharyngeal carriage in a mouse model resulted in the dissemination of nonencapsulated pneumococci into the lungs, whereas encapsulated strains remained in the nasopharynx. The S. pneumoniae capsule thus plays a role in modulation of the pro-inflammatory immune response in the respiratory tract. Second, results on immunological cells and immune regulation in a long term, stable nasopharyngeal carriage mouse model are presented. Mice were infected with encapsulated or nonencapsulated pneumococcal strains, and after 1, 3, 8 and 15 days, were sacrificed to evaluate the numbers of CD45+ cells, neutrophils, macrophages, FoxP3+ regulatory T-cells and CD3+ T-cells in the nasal mucosa as well as the amount of secreted IL-10 in the nasopharynx. Nasopharyngeal colonization which is effectively silent resulted in the stimulation of FoxP3+ regulatory T-cells and IL-10 release associated with immune homeostasis, whereas lung infiltration was required to increase the number of neutrophils and macrophages resulting in a stronger innate immune response in the nasal mucosa. Chapter 4 contains results of mono- and co-stimulation using RSV and pneumococci or pneumococcal virulence factors on the human bronchial epithelial cell line BEAS-2B. An increase in CXCL8 and IL-6 levels was measured for mixed stimulations of RSV and pneumococcus when encapsulated bacteria were used. Increasing pneumolysin concentrations resulted in enhanced CXCL8 levels. Priming of bronchial epithelial cells with RSV opens the door for more severe pneumococcal infections. Chapter 5 is composed of two parts: The first part describes initial biofilm formation of serotypes 6B and 7F in a static model in vitro. Biofilms of both serotypes contained SCVs, but only serotype 6B increased in SCV formation between 16 and 65h of incubation. SCV stability was tested by passaging clones in complex medium, where SCV production is not associated with advantages in growth. Serotype 6B lost the SCV phenotype indicating a fast adaptation to a changing nutritional environment. Limitations of our in vitro model are discussed. The second part is about initial biofilm formation of mixed culture growth of S. pneumoniae with commensal streptococci. Competition dominates this process. S. oralis and pneumococcus compete for nutrients, whereas mixed species growth of S. mitis or S. pseudopneumoniae with S. pneumoniae is mainly influenced by other factors. In Chapter 6 the findings of chapters 3, 4 and 5 are discussed and an outlook for further studies is provided. BACKGROUND Leptospirosis is caused by pathogenic spirochetes of the genus Leptospira. The bacteria enter the human body via abraded skin or mucous membranes and may disseminate throughout. In general the clinical picture is mild but some patients develop rapidly progressive, severe disease with a high case fatality rate. Not much is known about the innate immune response to leptospires during haematogenous dissemination. Previous work showed that a human THP-1 cell line recognized heat-killed leptospires and leptospiral LPS through TLR2 instead of TLR4. The LPS of virulent leptospires displayed a lower potency to trigger TNF production by THP-1 cells compared to LPS of non-virulent leptospires. METHODOLOGY/PRINCIPAL FINDINGS We investigated the host response and killing of virulent and non-virulent Leptospira of different serovars by human THP-1 cells, human PBMC's and human whole blood. Virulence of each leptospiral strain was tested in a well accepted standard guinea pig model. Virulent leptospires displayed complement resistance in human serum and whole blood while in-vitro attenuated non-virulent leptospires were rapidly killed in a complement dependent manner. In vitro stimulation of THP-1 and PBMC's with heat-killed and living leptospires showed differential serovar and cell type dependence of cytokine induction. However, at low, physiological, leptospiral dose, living virulent complement resistant strains were consistently more potent in whole blood stimulations than the corresponding non-virulent complement sensitive strains. At higher dose living virulent and non-virulent leptospires were equipotent in whole blood. Inhibition of different TLRs indicated that both TLR2 and TLR4 as well as TLR5 play a role in the whole blood cytokine response to living leptospires. CONCLUSIONS/SIGNIFICANCE Thus, in a minimally altered system as human whole blood, highly virulent Leptospira are potent inducers of the cytokine response.

El presente proyecto pretende ser una herramienta para la enseñanza de la lectoescritura (enseñar a leer y a escribir) para niños con discapacidad, haciendo para ello uso de una aplicación que se ejecuta en una tablet con Sistema Operativo (S.O.) Android. Existe un vacío en el mundo de las aplicaciones para tabletas en este campo en el que se intentará poner un grano de arena para, al menos, tener una aplicación que sirva de toma de contacto a los interesados en este campo. Para establecer las funcionalidades más adecuadas al propósito de la herramienta, se ha consultado a profesionales de la logopedia de un colegio de educación especial, con cuya colaboración se ha dado forma a la estructura de la misma. La implementación de la aplicación se ha llevado a cabo con programación en entorno Java para Android. Se han incluido diferentes recursos como imágenes, pictogramas y locuciones tanto elementos con licencia libre, como elementos propios generados ‘ex profeso’ para dar la forma final a la herramienta. Podemos decir que en general esta aplicación puede ser usada para enseña a leer y escribir a cualquier niño, pero se ha dotado de unas ciertas características que la confieren una orientación especial hacia niños con necesidades educativas especiales. Para ello se ha cuidado mucho la estética, para que ésta sea lo más simple y suave posible, para hacer especial hincapié en la atención de los niños y evitar su distracción con elementos visuales innecesarios. Se ha dotado de estímulos visuales y sonoros para fomentar su interés (aplausos en caso de acierto, colores para diferenciar aciertos y errores, etc.). Se han utilizado los tamaños de letra más grandes posibles (para las discapacidades visuales), etc. El mercado cuenta con una ingente cantidad de dispositivos Android, con características muy dispares, de tamaño de pantalla, resolución y versiones del S.O. entre otras. La aplicación se ha desarrollado tratando de dar cobertura al mayor porcentaje de ellos posible. El requisito mínimo de tamaño de pantalla sería de siete pulgadas. Esta herramienta no tiene demasiado sentido en dispositivos con pantallas menores por las características intrínsecas de la misma. No obstante se ha trabajado también en la configuración para dispositivos pequeños, como “smartphones”, no por su valor como herramienta para la enseñanza de la lectoescritura (aunque en algunos casos podría ser viable) sino más bien con fines de prueba y entrenamiento para profesores, padres o tutores que realizarán la labor docente con dispositivos tablet. Otro de los requisitos, como se ha mencionado, para poder ejecutar la aplicación sería la versión mínima de S.O., por debajo de la cual (versiones muy obsoletas) la aplicación sería inviable. Sirva este proyecto pues para cubrir, mediante el uso de la tecnología, un aspecto de la enseñanza con grandes oportunidades de mejora. ----------------------- This Project is aimed to be a tool for teaching reading and writing skills to handicapped children with an Android application. Speech therapy professionals from a special needs school have been asked for the most suitable functions to be included in this tool. The structure of this tool has been made with the cooperation of these professionals. The implementation of the application has been performed through Java coding for Android. Different resources have been included such as pictures, pictograms and sounds, including free licenses resources and self-developed resources. In general, it can be said that this application can be used to teach learning and writing skills to any given kid, however it has been provided of certain features that makes it ideal for children with special educational needs. It has been strongly taken into account the whole aesthetic to be as simple and soft as possible, in order to get attention of children, excluding any visual disturbing elements. It has been provided with sound and visual stimulations, to attract their interest (applauses in cases of correct answers, different colours to differentiate right or wrong answers), etc. There are many different types of Android devices, with very heterogeneous features regarding their screen size, resolution and O.S. version, etc., available today. The application has been developed trying to cover most of them. Minimum screen resolution is seven inches. This tool doesn’t seem to be very useful for smaller screens, for its inner features. Nevertheless, it has been developed for smaller devices as well, like smartphones, not intended to be a tool for teaching reading and writing skills (even it could be possible in some cases), but in a test and training context for teachers, parents or guardians who do the teaching work with tablet devices. Another requirement, as stated before, in order to be able to run the application, it would be the minimum O.S. version, below that (very obsolete versions) the application would become impracticable. Hope this project to be used to fulfill, by means of technology, one area of teaching with great improvement opportunities.

Neuronal apoptosis was observed in the rat dentate gyrus in two experimental models of human limbic epilepsy. Five hours after one hippocampal kindling stimulation, a marked increase of in situ terminal deoxynucleotidyltransferase-mediated dUTP nick-end labeling (TUNEL) of fragmented DNA was observed in nuclei located within and on the hilar border of the granule cell layer and in the polymorphic region. Forty kindling stimulations with 5-min interval produced higher numbers of labeled nuclei compared with one stimulation. The increase of TUNEL-positive nuclei was prevented by the protein synthesis inhibitor cycloheximide but not affected by the N-methyl-d-aspartate receptor antagonist MK-801. Kainic acid-induced seizures lead to a pattern of labeling in the hippocampal formation identical to that evoked by kindling. A large proportion of cells displaying TUNEL-positive nuclei was double-labeled by the neuron-specific antigen NeuN, demonstrating the neuronal identity of apoptotic cells. Either 1 or 40 kindling stimulations also gave rise to a marked increase of the number of cells double-labeled with the mitotic marker bromodeoxyuridine and NeuN in the subgranular zone and on the hilar border of the dentate granule cell layer. The present data show that single and intermittent, brief seizures induce both apoptotic death and proliferation of dentate gyrus neurons. We hypothesize that these processes, occurring early during epileptogenesis, are primary events in the development of hippocampal pathology in animals and possibly also in patients suffering from temporal lobe epilepsy.

The temporally encoded information obtained by vibrissal touch could be decoded “passively,” involving only input-driven elements, or “actively,” utilizing intrinsically driven oscillators. A previous study suggested that the trigeminal somatosensory system of rats does not obey the bottom-up order of activation predicted by passive decoding. Thus, we have tested whether this system obeys the predictions of active decoding. We have studied cortical single units in the somatosensory cortices of anesthetized rats and guinea pigs and found that about a quarter of them exhibit clear spontaneous oscillations, many of them around whisking frequencies (≈10 Hz). The frequencies of these oscillations could be controlled locally by glutamate. These oscillations could be forced to track the frequency of induced rhythmic whisker movements at a stable, frequency-dependent, phase difference. During these stimulations, the response intensities of multiunits at the thalamic recipient layers of the cortex decreased, and their latencies increased, with increasing input frequency. These observations are consistent with thalamocortical loops implementing phase-locked loops, circuits that are most efficient in decoding temporally encoded information like that obtained by active vibrissal touch. According to this model, and consistent with our results, populations of thalamic “relay” neurons function as phase “comparators” that compare cortical timing expectations with the actual input timing and represent the difference by their population output rate.

Subcortical nuclei in the thalamus, which play an important role in many functions of the human brain, provide challenging targets for functional mapping with neuroimaging techniques because of their small sizes and deep locations. In this study, we explore the capability of high-resolution functional magnetic resonance imaging at 4 Tesla for mapping the retinotopic organization in the lateral geniculate nucleus (LGN). Our results show that the hemifield visual stimulation only activates LGN in the contralateral hemisphere, and the lower-field and upper-field visual stimulations activate the superior and inferior portion of LGN, respectively. These results reveal a similar retinotopic organization between the human and nonhuman primate LGN and between LGN and the primary visual cortex. We conclude that high-resolution functional magnetic resonance imaging is capable of functional mapping of suborganizations in small nuclei together with cortical activation. This will have an impact for studying the thalamocortical networks in the human brain.

GABAergic (GABA = γ-aminobutyric acid) neurons from different brain regions contain high levels of parvalbumin, both in their soma and in their neurites. Parvalbumin is a slow Ca2+ buffer that may affect the amplitude and time course of intracellular Ca2+ transients in terminals after an action potential, and hence may regulate short-term synaptic plasticity. To test this possibility, we have applied paired-pulse stimulations (with 30- to 300-ms intervals) at GABAergic synapses between interneurons and Purkinje cells, both in wild-type (PV+/+) mice and in parvalbumin knockout (PV−/−) mice. We observed paired-pulse depression in PV+/+ mice, but paired-pulse facilitation in PV−/− mice. In paired recordings of connected interneuron-Purkinje cells, dialysis of the presynaptic interneuron with the slow Ca2+ buffer EGTA (1 mM) rescues paired-pulse depression in PV−/− mice. These data show that parvalbumin potently modulates short-term synaptic plasticity.

Membrane depolarization of NG108 cells gives rapid (< 5 min) activation of Ca2+/calmodulin-dependent protein kinase IV (CaM-KIV), as well as activation of c-Jun N-terminal kinase (JNK). To investigate whether the Ca2+-dependent activation of mitogen-activated protein kinases (ERK, JNK, and p38) might be mediated by the CaM kinase cascade, we have transfected PC12 cells, which lack CaM-KIV, with constitutively active mutants of CaM kinase kinase and/or CaM-KIV (CaM-KKc and CaM-KIVc, respectively). In the absence of depolarization, CaM-KKc transfection had no effect on Elk-dependent transcription of a luciferase reporter gene, whereas CaM-KIVc alone or in combination with CaM-KKc gave 7- to 10-fold and 60- to 80-fold stimulations, respectively, which were blocked by mitogen-activated protein (MAP) kinase phosphatase cotransfection. When epitope-tagged constructs of MAP kinases were co-transfected with CaM-KKc plus CaM-KIVc, the immunoprecipitated MAP kinases were activated 2-fold (ERK-2) and 7- to 10-fold (JNK-1 and p38). The JNK and p38 pathways were further investigated using specific c-Jun or ATF2-dependent transcriptional assays. We found that c-Jun/ATF2-dependent transcriptions were enhanced 7- to 10-fold by CaM-KIVc and 20- to 30-fold by CaM-KKc plus CaM-KIVc. In the case of the Jun-dependent transcription, this effect was not due to direct phosphorylation of c-Jun by activated CaM-KIV, since transcription was blocked by a dominant-negative JNK and by two MAP kinase phosphatases. Mutation of the phosphorylation site (Thr196) in CaM-KIV, which mediates its activation by CaM-KIV kinase, prevented activation of Elk-1, c-Jun, and ATF2 by the CaM kinase cascade. These results establish a new Ca2+-dependent mechanism for regulating MAP kinase pathways and resultant transcription.

Small, single-module proteins that fold in a single cooperative step may be paradigms for understanding early events in protein-folding pathways generally. Recent experimental studies of the 64-residue chymotrypsin inhibitor 2 (CI2) support a nucleation mechanism for folding, as do some computer stimulations. CI2 has a nucleation site that develops only in the transition state for folding. The nucleus is composed of a set of adjacent residues (an alpha-helix), stabilized by long-range interactions that are formed as the rest of the protein collapses around it. A simple analysis of the optimization of the rate of protein folding predicts that rates are highest when the denatured state has little residual structure under physiological conditions and no intermediates accumulate. This implies that any potential nucleation site that is composed mainly of adjacent residues should be just weakly populated in the denatured state and become structured only in a high-energy intermediate or transition state when it is stabilized by interactions elsewhere in the protein. Hierarchical mechanisms of folding in which stable elements of structure accrete are unfavorable. The nucleation-condensation mechanism of CI2 fulfills the criteria for fast folding. On the other hand, stable intermediates do form in the folding of more complex proteins, and this may be an unavoidable consequence of increasing size and nucleation at more than one site.

The aim of this thesis was to validate the use of infrared thermography (IRT) to non-invasively measure emotional reactions to different situations in pet dogs (Canis familiaris). A preliminary test, aimed to evaluate the correlation between eye-temperature and rectal temperature in dog, was performed. Then, in three different situations, negative (veterinary visit), positive (palatable food rewards), and mildly stressing followed by mildly positive (separation from and reunion with the owner), variations in heat emitted from lacrimal caruncle (referred to as eye temperature) were measured with an infrared thermographic camera. In addition, heart rate and heart rate variability parameters were collected using a non-invasive heart rate monitor designed for human use and validated on dogs. All experiments were video recorded to allow behavioral coding. During the negative situation dogs’ level of activity and stress related behaviors varied across compared to the baseline and dogs showed an increase in eye temperature despite having a significant decrease in the level of activity. The positive situation was characterized by a peak in eye temperature and mean HR and dogs engaged in behaviors indicating a positive arousal, focusing on food treats and tail wagging but there were not variations in HRV during stimulation but only an increment in SDNN immediately after the stimulus. In the separation from and reunion with the owner dogs’ eye temperature and mean HR did not vary neither in the stressful nor in the positive situations, RMSSD increased after the positive episode, SDNN dropped during the two stimulations and it increased after the stimulations. During the separation from the owner dogs were mainly directed to the door or to the experimenter while during the reunion with the owner dogs were focused mainly on the owner and on the environment, exhibiting safe base effect. A different approach was used to assess the welfare of shelter dogs. Dogs were implanted with a telemeter and after implantation dogs were housed in sequence in four different situations lasting 1 week: alone, alone with toys and a stretch cot for sleeping, with an unknown, spayed, female, and alone with a daily 2-hours interaction with an experimenter. Two different approaches were tried: partially random extracted fragments from every week, behaviors from 8 a.m. to 4 p.m. were continuous during baseline and the female situation. Results showed different reactions by dogs to the different situations and interestingly not all enrichments were enjoyed by the dogs improving their welfare. Overall results suggest that IRT may represent a useful tool to investigate emotional reactions in dogs. Nevertheless, further research is needed to establish the specificity and sensivity of IRT in this context and to assess how different dogs’ characteristics, breed, previous experience and the valence and arousal elicited by the stimulus could influence the magnitude and type of the response. The role of HRV in understanding emotional valence and the one of telemeters in understanding long-term effects on sheltered dogs’ welfare is also discussed.

INTRODUÇÃO: Líquen plano (LP) é uma doença mucocutânea de natureza inflamatória crônica de etiologia ainda desconhecida. A estimulação da imunidade inata via os receptores Toll-like (TLRs) podem influenciar as células dendríticas e direcionar a resposta de células T CD4+ e CD8+ efetoras, assim como também favorecer o estado inflamatório do LP. OBJETIVOS: Avaliar o perfil fenotípico de células dendríticas mielóides (mDCs) e plasmocitóides (pDCs) e de linfócitos T CD4+ e CD8+ após estímulo com agonistas de TLRs no sangue periférico de pacientes com LP. Além disto, avaliar a frequência, perfil de maturação e os subtipos de células T CD4+ e TCD8+ reguladores. MÉTODOS: Foram selecionados 18 pacientes com LP (15 mulheres, 3 homens), com 41,57 ± 4,73 anos de idade e um grupo controle com 22 indivíduos sadios (18 mulheres, 4 homens), com 43,92 ± 7,83 anos de idade. As células mononucleares (CMNs) de sangue periférico foram avaliadas por citometria de fluxo quanto à: 1) Produção de TNF-? em mDCs e de IFN-? em pDCs em CMNs ativadas por agonistas de TLR 4, 7, 7/8 e 9; 2) Análise de células T CD4+ e CD8+ monofuncionais e polifuncionais após estímulo com agonistas de TLR 4, 7/8, 9 e enterotoxina B de Staphylococcus aureus (SEB); 3) Avaliação de células Th17 e Th22/Tc22 em CMNs após estímulo com SEB; 4) Frequência, perfil de maturação e subtipos de células T CD4+ e CD8+ reguladoras. RESULTADOS: 1) Nos pacientes com LP foi demonstrado um aumento na frequência de mDCs TNF-alfa+ após estímulo com agonistas de TLR4/LPS e TLR7-8/CL097, mas com imiquimode/TLR7 houve diminuição da expressão de CD83. Já nas pDCs do grupo LP, o imiquimode foi capaz de diminuir a expressão de CD80 e o CpG/TLR9 diminuiu a expressão de CD83 no LP. 2) As células T CD4+ secretoras de IL-10 mostraram aumento da frequência nos níveis basais, que diminuiu após estímulo com LPS e SEB. Em contraste, a produção de IFN-y aumentou em resposta ao LPS enquanto diminuiu para CpG. As células T CD4+ polifuncionais, secretoras de 5 citocinas simultâneas (CD4+IL-17+IL-22+TNF+IL-10+IFN-y+) diminuíram no LP após estímulo com CL097 e CpG. Entretanto, na ausência de IL-10, houve aumento da frequência de células CD4+IL-17+IL-22+TNF+IFN-y+ em resposta ao LPS. Um aumento na polifuncionalidade foi observado em células TCD4+ que expressam CD38, marcador de ativação crônica e na ausência de IL-10. Similarmente, às TCD4+, uma diminuição de células T CD8+ IFN-y+ e TNF+ foram detectadas após estímulo com CpG. 3) As células Th22/Tc22 nos níveis basais e após estímulo com SEB se mostraram aumentadas. As células Th17 não mostraram diferenças entre os grupos. 4) A frequência das células T CD4+ e CD8+ reg totais (CD25+Foxp3+CD127low/-) está elevada no LP. Quanto aos perfis de maturação, há aumento na frequência de células TCD4+ de memória efetora enquanto que para as células T CD8+ há predomínio das células de memória central. Quanto aos subtipos, há aumento nas células T CD4+ regs periféricas (pT reg). CONCLUSÕES: O estado de ativação das mDCs após ativação das vias de TLRs 4 e 7/8 pode influenciar na geração de resposta T efetoras no LP. O perfil de resposta monofuncional e polifuncional aos estímulos TLRs reflete a ativação destas células no sangue periférico. Além disso, o aumento de Th22/Tc22 e das células T regs indicam uma relação entre regulação e células efetoras no sangue periférico evidenciando que existem alterações extracutâneas no LP

A estimulação elétrica neuromuscular (EENM) é uma recente técnica terapêutica no tratamento das disfagias orofaríngeas. Poucos estudos utilizaram a EENM em casos oncológicos, havendo muitas dúvidas sobre o método de aplicação e os resultados de diferentes condições de estimulação nessa população. Este trabalho teve por objetivo verificar o efeito imediato da EENM sensorial e motora, nas fases oral e faríngea da deglutição, em pacientes após tratamento do câncer de cabeça e pescoço. Para isso foi realizado um estudo transversal intervencional que incluiu 11 pacientes adultos e idosos (mediana de 59 anos) acometidos por câncer de cabeça e pescoço. Todos os indivíduos foram submetidos ao exame de videofluoroscopia da deglutição, no qual, de modo randomizado, foram solicitadas deglutições de 5 ml de alimentos nas consistências líquida, mel e pudim em três condições distintas: sem estimulação, com EENM sensorial, com EENM motora. Foi classificado o grau da disfunção da deglutição por meio da escala DOSS (Dysphagia Outcome and Severity Scale), a presença de estase de alimentos (escala de Eisenhuber), de penetração laríngea, aspiração laringotraqueal (Penetration and Aspiration Scale - PAS), além da medida do tempo de trânsito oral e faríngeo (em segundos). Para a comparação dos resultados, considerando os três estímulos aplicados, na escala de resíduos, na escala de penetração aspiração, na escala DOSS e no tempo de trânsito oral e faríngeo foi aplicado o teste de Friedman ou a análise de variância para medidas repetidas (de acordo com a distribuição dos dados). Para todos os testes foi adotado nível de significância de 5%. Os resultados demonstraram que houve melhora com a estimulação sensorial e motora na escala DOSS e na escala PAS para um paciente tratado de câncer de boca e outro de laringe e piora, em ambas as escalas, para dois pacientes (câncer de boca), sendo um para a estimulação motora e outro na sensorial. A aplicação da escala de Eisenhuber permitiu verificar que a EENM, tanto em nível sensorial como motor, modificou de forma variável a presença de resíduos para os casos de câncer de boca, enquanto para o paciente com câncer de laringe houve redução de resíduos em valécula/raiz da língua para a estimulação sensorial e motora, além de aumento de resíduos em parede posterior da faringe com o estímulo motor. Além disso, não foi encontrada diferença estatisticamente significante para o tempo de trânsito oral e faríngeo nas diferentes estimulações para todas as consistências testadas (p>0,05). Diante desses achados, concluiu-se que a EENM, em nível sensorial e motor, apresentou variável impacto imediato nas fases oral e faríngea da deglutição, podendo melhorar a função de deglutição de pacientes com significante disfagia após o tratamento para o câncer de cabeça e pescoço, no que se diz respeito ao grau da disfagia e à presença de penetração e aspiração.

But: La perte unilatérale du cortex visuel postérieur engendre une cécité corticale controlatérale à la lésion, qu’on appelle hémianopsie homonyme (HH). Celle-ci est notamment accompagnée de problèmes d’exploration visuelle dans l’hémichamp aveugle dus à des stratégies oculaires déficitaires, qui ont été la cible des thérapies de compensation. Or, cette perte de vision peut s’accompagner d’une perception visuelle inconsciente, appelée blindsight. Notre hypothèse propose que le blindsight soit médié par la voie rétino-colliculaire extrastriée, recrutant le colliculus supérieur (CS), une structure multisensorielle. Notre programme a pour objectif d’évaluer l’impact d’un entraînement multisensoriel (audiovisuel) sur la performance visuelle inconsciente des personnes hémianopsiques et les stratégies oculaires. Nous essayons, ainsi, de démontrer l’implication du CS dans le phénomène de blindsight et la pertinence de la technique de compensation multisensorielle comme thérapie de réadaptation. Méthode: Notre participante, ML, atteinte d’une HH droite a effectué un entraînement d’intégration audiovisuel pour une période de 10 jours. Nous avons évalué la performance visuelle en localisation et en détection ainsi que les stratégies oculaires selon trois comparaisons principales : (1) entre l’hémichamp normal et l’hémichamp aveugle; (2) entre la condition visuelle et les conditions audiovisuelles; (3) entre les sessions de pré-entraînement, post-entraînement et 3 mois post-entraînement. Résultats: Nous avons démontré que (1) les caractéristiques des saccades et des fixations sont déficitaires dans l’hémichamp aveugle; (2) les stratégies saccadiques diffèrent selon les excentricités et les conditions de stimulations; (3) une adaptation saccadique à long terme est possible dans l’hémichamp aveugle si l’on considère le bon cadre de référence; (4) l’amélioration des mouvements oculaires est liée au blindsight. Conclusion(s): L’entraînement multisensoriel conduit à une amélioration de la performance visuelle pour des cibles non perçues, tant en localisation qu’en détection, ce qui est possiblement induit par le développement de la performance oculomotrice.
