The main structural and geomorphological features along the Amazon River are closely associated with Mesozoic and Cenozoic tectonic events. The Mesozoic tectonic setting is characterised by the Amazonas and Marajó Basins, two distinct extensional segments. The Amazonas Basin is formed by NNE-SSW normal faults, which control the emplacement of dolerite dykes and deposition of the sedimentary pile. In the more intense tectonic phase (mid-Late Cretaceous), the depocentres were filled with fluvial sequences associated with axial drainage systems, which diverge from the Lower Tapajós Arch. During the next subsidence phase, probably in the Early Tertiary, and under low rate extension, much of the drainage systems reversed, directing the paleo-Amazon River to flow eastwards. The Marajó Basin encompasses NW-SE normal faults and NE-SW strike-slip faults, with the latter running almost parallel to the extensional axes. The normal faults controlled the deposition of thick rift and post-rift sequences and the emplacement of dolerite dykes. During the evolution of the basin, the shoulder (Gurupá Arch) became distinct, having been modelled by drainage systems strongly controlled by the trend of the strike-slip faults. The Arari Lineament, which marks the northwest boundary of the Marajó Basin, has been working as a linkage corridor between the paleo and modern Amazon River with the Atlantic Ocean. The neotectonic evolution since the Miocene comprises two sets of structural and geomorphological features. The older set (Miocene-Pliocene) encompasses two NE-trending transpressive domains and one NW-trending transtensive domain, which are linked to E-W and NE-SW right-lateral strike-slip systems. The transpressive domains display aligned hills controlled by reverse faults and folds, and are separated by large plains associated with pull-apart basins along clockwise strike-slip systems (e.g. Tupinambarana Lineament). Many changes were introduced in the landscape by the transpressive and transtensive structures, such as the blockage of major rivers, which evolved to river-lakes, transgression of the sea over a large area in the Marajó region, and uplift of long and narrow blocks that are oblique to the trend of the main channel. The younger set (Pliocene-Holocene) refers to two triple-arm systems of rift/rift/strike-slip and strike-slip/strike-slip/rift types, and two large transtensive segments, which have controlled the orientation of the modern drainage patterns. © 2001 Elsevier Science Ltd. Background: Fondaparinux is considered an agent with a well-established safety and efficacy profile in the treatment of non-ST segment elevation acute coronary syndromes, but when used alone, is associated to a higher incidence of thrombotic complications during invasive coronary procedures, requiring the supplementation of an anti-IIa agent. This study aimed to evaluate the efficacy and safety of percutaneous coronary intervention (PCI) in patients with non-ST segment elevation acute coronary syndromes previously treated with fondaparinux. Methods: Prospective, controlled registry enrolling 127 consecutive patients submitted to an early invasive stratification during treatment with fondaparinux, with supplementation of intravenous unfractionated heparin at a dose of 85 U/kg at the time of PCI. Results: The rate of the composite primary endpoint including death, acute myocardial infarction, stroke, stent thrombosis or emergency myocardial revascularization was 3.2%. The cumulative incidence of major bleeding and vascular complications was 3.2%. There were no cases of guidecatheter thrombosis or abrupt vessel closure. Conclusions: PCI in patients with acute coronary syndromes receiving fondaparinux is associated with a low rate of major adverse cardiovascular ischemic events and severe hemorrhagic complications. Supplementation of unfractionated heparin during the invasive procedures eliminates the risk of catheter-related thrombosis.

Background : Deep venous thrombosis (DVP) is a frequent disease. Prophylaxis is the best means to reduce its incidence, for lowering morbidity and mortality rates and treatment costs caused by its complications. Objective : To evaluate the knowledge and use of any kind of DVT prophylaxis by Brazilian Oral and Maxillofacial surgeons. Materials and Methods : A questionnaire was sent to all Oral and Maxillofacial surgeons associated to the Brazilian College of Oral and Maxillofacial Surgeons that have a valid e-mail address. The data retrieved was evaluated and tabulated. Results : Of the 1100 questionnaires sent, only 4% were retrieved. The 42 retrieved were included in the study. Twenty six of the surgeons do not use any kind of deep venous thrombosis (DVT) prophylaxis, 11 use mechanical means as elastic compressive stockings or pneumatic compressive devices for prophylaxis, and 5 uses low-molecular weight heparins (LMWH) as the choice for prophylaxis. Conclusion : The data collected, despite the low rate of participation (4%) by the surgeons, shows that this subject still does not receive proper attention. Whereas other medical specialties make routine use of prophylactic means maybe the maxillofacial surgeons lack concern on that matter.

The bovine bone and sintetic hydroxyapatite (HA) bioceramics are reference materials to employment as a bone substitute, however, their slow rate of degradation and its low rate of bioactivity index (Ib) are presented as limiting factors for application as bone graft. In contrast, the bioglass is a resorbable and osteoinductive material. the present work objective the development of composites of dispersed bovine bone or sintetic HA in silicate-phosphate bioglass, seeking to obtain a biomaterial with properties suitable for application as bone grafts. The composites were prepared by mixing between the powder components followed by sintering for 1h. Were used HA and bioglass (45S5) with particle size <240μm. The tested proportions of HA/45S5 were 20/80, 30/70 and 40/60 (wt%). The composites characterization was made employing scanning electron microscopy, Infra-Red Spectrometry and hydrolytic resistance test. The test results indicate the potential use of the materials developed for applications such as bone graft. © (2012) Trans Tech Publications, Switzerland.

Background: In the absence of autologous bone for harvesting, fresh-frozen bone allografts turned into an alternative for bone reconstruction procedures. Purpose: The purpose of this study was to make a histological analysis of fresh-frozen onlay bone allografts (ALs), compared with autografts, in patients who needed maxillary reconstruction prior to dental implants placement. Materials and Methods: Twelve patients with bone deficiencies (width inferior to 4mm) in the sites where the implants were planned were enrolled in the study. From these, six were elected to be treated with autogenous (AT) bone grafts and six with fresh-frozen bone AL. This last group included the patients who had absence of a convenient amount of bone in donor sites. Each patient received from one to six graft blocks, totalling to 12 ATs and 17 ALs. Seven months after grafting procedures, biopsies of the grafts were made using 2-mm internal diameter trephine burs, and processed for histological analysis. One biopsy was retrieved from each patient. Results: Clinically, all grafts were found to be firm in consistency and well-incorporated to the receptor bed. Histological analysis showed a large amount of necrotic bone surrounded by few spots of new-formed bone in the AL group, suggesting low rate of graft remodeling. In the AT group, an advanced stage of bone remodeling was seen. Conclusions: Human fresh-frozen bone block AL showed clinical compatibility for grafting procedures, although associated to slow remodeling process. Further studies are needed to define, at long term, the remodeling process chronology the clinical longitudinal results for fresh-frozen bone AL. Copyright © 2013 Wiley Periodicals, Inc.
