L’insomnie, commune auprès de la population gériatrique, est typiquement traitée avec des benzodiazépines qui peuvent augmenter le risque des chutes. La thérapie cognitive-comportementale (TCC) est une intervention non-pharmacologique ayant une efficacité équivalente et aucun effet secondaire. Dans la présente thèse, le coût des benzodiazépines (BZD) sera comparé à celui de la TCC dans le traitement de l’insomnie auprès d’une population âgée, avec et sans considération du coût additionnel engendré par les chutes reliées à la prise des BZD. Un modèle d’arbre décisionnel a été conçu et appliqué selon la perspective du système de santé sur une période d’un an. Les probabilités de chutes, de visites à l’urgence, d’hospitalisation avec et sans fracture de la hanche, les données sur les coûts et sur les utilités ont été recueillies à partir d’une revue de la littérature. Des analyses sur le coût des conséquences, sur le coût-utilité et sur les économies potentielles ont été faites. Des analyses de sensibilité probabilistes et déterministes ont permis de prendre en considération les estimations des données. Le traitement par BZD coûte 30% fois moins cher que TCC si les coûts reliés aux chutes ne sont pas considérés (231$ CAN vs 335$ CAN/personne/année). Lorsque le coût relié aux chutes est pris en compte, la TCC s’avère être l’option la moins chère (177$ CAN d’économie absolue/ personne/année, 1,357$ CAN avec les BZD vs 1,180$ pour la TCC). La TCC a dominé l’utilisation des BZD avec une économie moyenne de 25, 743$ CAN par QALY à cause des chutes moins nombreuses observées avec la TCC. Les résultats des analyses d’économies d’argent suggèrent que si la TCC remplaçait le traitement par BZD, l’économie annuelle directe pour le traitement de l’insomnie serait de 441 millions de dollars CAN avec une économie cumulative de 112 billions de dollars canadiens sur une période de cinq ans. D’après le rapport sensibilité, le traitement par BZD coûte en moyenne 1,305$ CAN, écart type 598$ (étendue : 245-2,625)/personne/année alors qu’il en coûte moyenne 1,129$ CAN, écart type 514$ (étendue : 342-2,526)/personne/année avec la TCC. Les options actuelles de remboursement de traitements pharmacologiques au lieu des traitements non-pharmacologiques pour l’insomnie chez les personnes âgées ne permettent pas d’économie de coûts et ne sont pas recommandables éthiquement dans une perspective du système de santé.

Le vieillissement étant un enjeu démographique majeur, il est capital de mieux comprendre les changements qui surviennent durant cette période de la vie. Il est connu que certaines fonctions cognitives sont modifiées avec l’avancée en âge. D’ailleurs, 25 à 50 % des personnes âgées de 65 ans et plus rapportent avoir observé un déclin de leur cognition et de leur mémoire. Les travaux de cette thèse portent sur la caractérisation de la plainte cognitive chez des personnes âgées saines et chez des aînés ayant un trouble cognitif léger (TCL) ainsi que sur son évolution au fil de la progression vers la maladie d’Alzheimer. La première étude (Chapitre II) avait pour objectif d’identifier les différents domaines de plainte mnésique chez des personnes d’âge moyen et des individus plus âgés. Elle visait également à vérifier si les domaines de plainte étaient associés aux performances aux tests neuropsychologiques. L’effet sur la plainte de certaines caractéristiques personnelles (âge, sexe, niveau de scolarité et symptômes dépressifs) a aussi été examiné. Le Questionnaire d’auto-évaluation de la mémoire (QAM; Van der Linden, Wijns, Von Frenkell, Coyette, & Seron, 1989) et plusieurs tests neuropsychologiques ont été complétés par 115 adultes sains âgés de 45 à 87 ans. Une analyse en composantes principales réalisée sur l'ensemble des questions du QAM a permis d’identifier sept grands domaines de plainte. Des analyses subséquentes ont révélé que les plaintes les plus fréquemment rapportées par les participants sont associées à des situations où des facteurs internes et externes interfèrent avec la performance mnésique. Les analyses ont aussi montré que les plaintes relatives à des oublis dont les conséquences menacent l’autonomie et la sécurité témoigneraient de problèmes cognitifs et fonctionnels plus sévères. Enfin, nos résultats ont indiqué que les différents domaines de plainte reflètent globalement les problèmes cognitifs objectifs. Aucune association n’a été trouvée entre la plainte et la plupart des caractéristiques démographiques. La seconde étude (Chapitre III) avait pour but de caractériser la plainte cognitive dans le TCL ainsi que son évolution dans la progression de la démence. L’étude cherchait aussi à déterminer si les changements dans certains domaines de plainte étaient reliés au déclin de ii fonctions cognitives spécifiques chez les individus avec TCL qui ont progressé vers la démence (progresseurs). Des personnes avec TCL et des individus âgés sains ont été évalués annuellement pendant trois ans. Le QAM et le Multifactorial Memory Questionnaire (MMQ; Fort, Holl, Kaddour, & Gana, 2004) ont été utilisés pour mesurer leurs plaintes. Les résultats ont révélé que les progresseurs rapportaient davantage de difficultés associées à la mémorisation de contenus complexes (ex. : textes ou conversations), d’événements récents et d’informations sur leurs proches que les personnes âgées saines et ce, jusqu’à trois ans avec le diagnostic de démence. L’analyse des effets de groupe a indiqué que l’intensité des plaintes des progresseurs semble être demeurée stable durant le suivi. Il est donc possible qu’une proportion des progresseurs présentent une méconnaissance de leurs difficultés cognitives. Cependant, des analyses corrélationnelles ont montré que l’augmentation des plaintes reliées à trois domaines était associée à l’accroissement de certaines atteintes cognitives durant les trois ans. Ainsi, certaines plaintes pourraient permettre de mieux comprendre les difficultés cognitives qui sont vécues par la personne avec TCL. Les implications théoriques et cliniques de ces résultats seront discutées dans le dernier chapitre de la thèse (Chapitre IV).

Previous research has considered entrepreneurship as a way out of poverty and as a chance to foster economic growth. Moreover, specifically start-ups headed by women have played an important role in the economic development and it has been argued that gender-related issues, amongst others, play a significant role for the performance of a country or region. Against this background, this qualitative study explores desires, reluctances and constraints toward entrepreneurial activities of a comparably homogenous group of potential (poor) entrepreneurs in an emerging economy—cleaning ladies in Istanbul. We focus on this particular context as still rather little is known on reasons why women do not start a business (in Turkey). We believe exploring the reasons why certain individuals choose not to become entrepreneurs is at least as telling as investigating why they do so. We draw upon the social dimensions of entrepreneurship by Shapero and Sokol (1982) alongside Institutional Theory and posit that normative and cognitive forces may shape individual decisions on entrepreneurship. We identified two basic clusters of women and discuss possible hindrance factors undermining entrepreneurial desires and limitations for entrepreneurship as well as possible avenues for policy makers (and MNCs) to foster entrepreneurship in the given community.

This paper proposes to promote autonomy in digital ecosystems so that it provides agents with information to improve the behavior of the digital ecosystem in terms of stability. This work proposes that, in digital ecosystems, autonomous agents can provide fundamental services and information. The final goal is to run the ecosystem, generate novel conditions and let agents exploit them. A set of evaluation measures must be defined as well. We want to provide an outline of some global indicators, such as heterogeneity and diversity, and establish relationships between agent behavior and these global indicators to fully understand interactions between agents, and to understand the dependence and autonomy relations that emerge between the interacting agents. Individual variations, interaction dependencies, and environmental factors are determinants of autonomy that would be considered. The paper concludes with a discussion of situations when autonomy is a milestone

A Perturbação de Hiperatividade e Défice de Atenção (PHDA) é um distúrbio que, segundo a literatura, afeta entre 3-7% das crianças em idade escolar e que consiste numa tríade de desatenção, hiperatividade e impulsividade. Este distúrbio é prevalente e para os atingidos pode ter consequências gravosas sendo o insucesso escolar um dos primeiros obstáculos. Muitas vezes entendidas pela classe docente como falta de educação e preguiça, estas características não recebem a atenção devida. Baseado num aluno do 8º ano com diagnóstico de PHDA do tipo misto desde os 5 anos de idade, este projeto destinou-se a estudar sumariamente a PHDA e a encontrar, aplicar e avaliar a eficácia de um conjunto de estratégias de aprendizagem que, numa perspetiva inclusiva, permitissem quer ao aluno quer ao grupo turma desenvolver os seus potenciais de aprendizagem. O aluno não se encontra abrangido pelo Decreto Lei 3/2008 mas apresentou insucesso escolar no 7º ano que quase comprometeu a sua progressão. Integra uma turma de dezasseis alunos que inclui três alunos abrangidos pelo referido decreto sendo um de currículo específico individual. Este projeto desenvolveu-se no primeiro período do ano letivo 2012-2013 e possui duas vertentes: a tutoria individual - num tempo semanal – assumido na qualidade de diretora de turma, e o espaço de sala de aula curricular de Físico-Química – em três tempos semanais – da qual a autora do presente trabalho é docente. O projeto de tutoria, sugerido pelo psicólogo que acompanha o aluno, destinou-se à implementação de um Programa de Promoção da Função Cognitiva e de Métodos e Hábitos de Estudo incidindo nos seguintes domínios: componente motivacional, componente comportamental, componente cognitiva, autoconhecimento e autoavaliação, e planeamento de uma metodologia de estudo mais eficaz. Em sala de aula o enfoque esteve na componente motivacional para o estudo da disciplina com o intento de otimizar as funções executivas de Ativação, Foco e Esforço mencionadas por Brown (2009), e na atribuição de significado aos conteúdos estudados para trabalhar a Memória de curto e longo prazo. Para a abordagem dos vários temas utilizaram-se recursos variados procurando respeitar os vários estilos de aprendizagem mas o recurso a objetos e atividades experimentais foram os que se revelaram mais eficazes. No final desta intervenção, a avaliação do período em estudo revelou um aluno mais motivado e participativo na generalidade das disciplinas e especialmente em Físico-Química onde obteve classificações acima da média da turma. The degree to which perceived controllability alters the way a stressor is experienced varies greatly among individuals. We used functional magnetic resonance imaging to examine the neural activation associated with individual differences in the impact of perceived controllability on self-reported pain perception. Subjects with greater activation in response to uncontrollable (UC) rather than controllable (C) pain in the pregenual anterior cingulate cortex (pACC), periaqueductal gray (PAG), and posterior insula/SII reported higher levels of pain during the UC versus C conditions. Conversely, subjects with greater activation in the ventral lateral prefrontal cortex (VLPFC) in anticipation of pain in the UC versus C conditions reported less pain in response to UC versus C pain. Activation in the VLPFC was significantly correlated with the acceptance and denial subscales of the COPE inventory [Carver, C. S., Scheier, M. F., & Weintraub, J. K. Assessing coping strategies: A theoretically based approach. Journal of Personality and Social Psychology, 56, 267–283, 1989], supporting the interpretation that this anticipatory activation was associated with an attempt to cope with the emotional impact of uncontrollable pain. A regression model containing the two prefrontal clusters (VLPFC and pACC) predicted 64% of the variance in pain rating difference, with activation in the two additional regions (PAG and insula/SII) predicting almost no additional variance. In addition to supporting the conclusion that the impact of perceived controllability on pain perception varies highly between individuals, these findings suggest that these effects are primarily top-down, driven by processes in regions of the prefrontal cortex previously associated with cognitive modulation of pain and emotion regulation.

Using functional magnetic resonance imaging, we examined whether individual differences in amygdala activation in response to negative relative to neutral information are related to differences in the speed with which such information is evaluated, the extent to which such differences are associated with medial prefrontal cortex function, and their relationship with measures of trait anxiety and psychological well-being (PWB). Results indicated that faster judgments of negative relative to neutral information were associated with increased left and right amygdala activation. In the prefrontal cortex, faster judgment time was associated with relative decreased activation in a cluster in the ventral anterior cingulate cortex (ACC, BA 24). Furthermore, people who were slower to evaluate negative versus neutral information reported higher PWB. Importantly, higher PWB was strongly associated with increased activation in the ventral ACC for negative relative to neutral information. Individual differences in trait anxiety did not predict variation in judgment time or in amygdala or ventral ACC activity. These findings suggest that people high in PWB effectively recruit the ventral ACC when confronted with potentially aversive stimuli, manifest reduced activity in subcortical regions such as the amygdala, and appraise such information as less salient as reflected in slower evaluative speed.

Fifty-nine healthy infants were filmed with their mothers and with a researcher at two, four, six and nine months in face-to-face play, and in toy-play at six and nine months. During toy-play at both ages, two indices of joint attention (JA)—infant bids for attention, and percent of time in shared attention—were assessed, along with other behavioural measures. Global ratings were made at all four ages of infants’ and mothers’ interactive style. The mothers varied in psychiatric history (e.g., half had experienced postpartum depression) and socioeconomic status, so their interactive styles were diverse. Variation in nine-month infant JA — with mother and with researcher — was predicted by variation in maternal behaviour and global ratings at six months, but not at two or four months. Concurrent adult behaviour also influenced nine-month JA, independent of infant ratings. Six-month maternal behaviours that positively predicted later JA (some of which remained important at nine months) included teaching, conjoint action on a toy, and global sensitivity. Other behaviours (e.g., entertaining) negatively predicted later JA. Findings are discussed in terms of social-learning and neurobiological accounts of JA emergence.

Background: The objective was to evaluate the efficacy and tolerability of donepezil (5 and 10 mg/day) compared with placebo in alleviating manifestations of mild to moderate Alzheimer's disease (AD). Method: A systematic review of individual patient data from Phase II and III double-blind, randomised, placebo-controlled studies of up to 24 weeks and completed by 20 December 1999. The main outcome measures were the ADAS-cog, the CIBIC-plus, and reports of adverse events. Results: A total of 2376 patients from ten trials were randomised to either donepezil 5 mg/day (n = 821), 10 mg/day (n = 662) or placebo (n = 893). Cognitive performance was better in patients receiving donepezil than in patients receiving placebo. At 12 weeks the differences in ADAS-cog scores were 5 mg/day-placebo: - 2.1 [95% confidence interval (CI), - 2.6 to - 1.6; p < 0.001], 10 mg/day-placebo: - 2.5 ( - 3.1 to - 2.0; p < 0.001). The corresponding results at 24 weeks were - 2.0 ( - 2.7 to - 1.3; p < 0.001) and - 3.1 ( - 3.9 to - 2.4; p < 0.001). The difference between the 5 and 10 mg/day doses was significant at 24 weeks (p = 0.005). The odds ratios (OR) of improvement on the CIBIC-plus at 12 weeks were: 5 mg/day-placebo 1.8 (1.5 to 2.1; p < 0.001), 10 mg/day-placebo 1.9 (1.5 to 2.4; p < 0.001). The corresponding values at 24 weeks were 1.9 (1.5 to 2.4; p = 0.001) and 2.1 (1.6 to 2.8; p < 0.001). Donepezil was well tolerated; adverse events were cholinergic in nature and generally of mild severity and brief in duration. Conclusion: Donepezil (5 and 10 mg/day) provides meaningful benefits in alleviating deficits in cognitive and clinician-rated global function in AD patients relative to placebo. Increased improvements in cognition were indicated for the higher dose. Copyright © 2004 John Wiley & Sons, Ltd.

OBJECTIVES: To evaluate the evidence for strategies to prevent falls or fractures in residents in care homes and hospital inpatients and to investigate the effect of dementia and cognitive impairment. DESIGN: Systematic review and meta-analyses of studies grouped by intervention and setting (hospital or care home). Meta-regression to investigate the effects of dementia and of study quality and design. DATA SOURCES: Medline, CINAHL, Embase, PsychInfo, Cochrane Database, Clinical Trials Register, and hand searching of references from reviews and guidelines to January 2005. RESULTS: 1207 references were identified, including 115 systematic reviews, expert reviews, or guidelines. Of the 92 full papers inspected, 43 were included. Meta-analysis for multifaceted interventions in hospital (13 studies) showed a rate ratio of 0.82 (95% confidence interval 0.68 to 0.997) for falls but no significant effect on the number of fallers or fractures. For hip protectors in care homes (11 studies) the rate ratio for hip fractures was 0.67 (0.46 to 0.98), but there was no significant effect on falls and not enough studies on fallers. For all other interventions (multifaceted interventions in care homes; removal of physical restraints in either setting; fall alarm devices in either setting; exercise in care homes; calcium/vitamin D in care homes; changes in the physical environment in either setting; medication review in hospital) meta-analysis was either unsuitable because of insufficient studies or showed no significant effect on falls, fallers, or fractures, despite strongly positive results in some individual studies. Meta-regression showed no significant association between effect size and prevalence of dementia or cognitive impairment. CONCLUSION: There is some evidence that multifaceted interventions in hospital reduce the number of falls and that use of hip protectors in care homes prevents hip fractures. There is insufficient evidence, however, for the effectiveness of other single interventions in hospitals or care homes or multifaceted interventions in care homes.

Some commonly experienced signs and symptoms occur during abstinence from tobacco, but specific signs and symptoms and their intensity vary greatly from individual to individual. The aim of this study was to re-examine psychological and psychomotor symptoms in smokers in the general population, and to explore the individual variation in these. Quitting smokers (n = 123) reported their experiences pre- and post-cessation, on a questionnaire developed for the study. Analysis of variance and frequency analysis showed significant decreases between pre- and post-cessation on positive experiences (F = 9.81, p < 0.0001) but no significant change on negative experiences, suggesting a loss of pleasure rather than increased negative affect upon quitting. The variance of the pre- to post-cessation difference score suggested wide variation in the reporting of withdrawal symptoms. These results lead us to consider the implications for treatment, using cognitive therapies and moderating the significant emphasis that is at present put on withdrawal.

The feature model of immediate memory (Nairne, 1990) is applied to an experiment testing individual differences in phonological confusions amongst a group (N=100) of participants performing a verbal memory test. By simulating the performance of an equivalent number of “pseudo-participants” the model fits both the mean performance and the variability within the group. Experimental data show that high-performing individuals are significantly more likely to demonstrate phonological confusions than low performance individuals and this is also true of the model, despite the model’s lack of either an explicit phonological store or a performance-linked strategy shift away from phonological storage. It is concluded that a dedicated phonological store is not necessary to explain the basic phonological confusion effect, and the reduction in such an effect can also be explained without requiring a change in encoding or rehearsal strategy or the deployment of a different storage buffer.

Although the co-occurrence of negative affect and pain is well recognized, the mechanism underlying their association is unclear. To examine whether a common self-regulatory ability impacts the experience of both emotion and pain, we integrated neuroimaging, behavioral, and physiological measures obtained from three assessments separated by substantial temporal intervals. Out results demonstrated that individual differences in emotion regulation ability, as indexed by an objective measure of emotional state, corrugator electromyography, predicted self-reported success while regulating pain. In both emotion and pain paradigms, the amygdala reflected regulatory success. Notably, we found that greater emotion regulation success was associated with greater change of amygdalar activity following pain regulation. Furthermore, individual differences in degree of amygdalar change following emotion regulation were a strong predictor of pain regulation success, as well as of the degree of amygdalar engagement following pain regulation. These findings suggest that common individual differences in emotion and pain regulatory success are reflected in a neural structure known to contribute to appraisal processes.
