Aquest projecte es centra a donar una caracterització estadística del valor que tenen els pics de correlació sota diferents escenaris d’adquisició a un receptor GPS. En primer lloc, s’ha volgut donar una visió general de tots els fonaments del sistema GPS per tal de poder entendre el seu funcionament. A continuació, s’ha passat a analitzar el bloc d’adquisició d’un receptor GPS. Primer, hem estudiat quines operacions es realitzen en aquest bloc i quines són les diferents formes d’implementar-lo. Seguidament, sota un escenari d’adquisició per cerca de fase de codi en paral·lel i utilitzant integracions coherents, s’han estudiat les distribucions estadístiques de les pdf’s obtingudes pels pics de correlació de senyal+soroll i pels pics de correlació de només soroll, i s’ha vist com aquestes fan modificar la corba ROC del receptor . Les simulacions s’han realitzat amb MATLAB i en diferents escenaris d’adquisició per tal de poder comparar com varien les estadístiques obtingudes en casos diferents.

BACKGROUND AND OBJECTIVE Patients from a previous study of neuropathic pain (NP) in the Spanish primary care setting still had symptoms despite treatment. Subsequently, patients were treated as prescribed by their physician and followed up for 3 months. Since pregabalin has been shown to be effective in NP, including refractory cases, the objective of this study was to assess the effectiveness of pregabalin therapy in patients with NP refractory to previous treatments. METHODS This was a post hoc analysis of pregabalin-naïve NP patients treated with pregabalin in a 3-month follow-up observational multicenter study to assess symptoms and satisfaction with treatment. Patients were evaluated with the Douleur Neuropathique en 4 questions (DN4), the Brief Pain Inventory (BPI) and the Treatment Satisfaction for Medication Questionnaire (SATMED-Q) overall satisfaction domain. RESULTS 1,670 patients (mean age 58 years, 59 % women), previously untreated or treated with ≥1 drug other than pregabalin, were treated with pregabalin (37 % on monotherapy). At 3 months, pain intensity and its interference with activities decreased by half (p < 0.0001), while the number of days with no or mild pain increased by a mean of 4.5 days (p < 0.0001). Treatment satisfaction increased twofold (p < 0.0001). Patients with a shorter history of pain and those with neuralgia and peripheral nerve compression syndrome (PCS) as etiologies had the highest proportion on monotherapy and showed the greatest improvements in pain-related parameters in their respective group categories. CONCLUSION Treatment with pregabalin (as monotherapy or combination therapy) provides benefits in pain and treatment satisfaction in patients with NP, including refractory cases. Shorter disease progression and neuralgia and PCS etiologies are favorable factors for pregabalin treatment response.

Aquest és un estudi retrospectiu que compara la mobilitat i el conflicto escàpulo-humeral entre 2 models diferents de pròtesi invertida d’espatlla. Aquestes pròtesis s’han implantat en pacients amb ruptures del manegot dels rotadors irreparables. Aquesta cirugía no està exenta de complicacions, i una de les més habituals és el conflicto escàpulo-humeral o notch.

Les fractures de clavícula perinatals són una complicació impredictible i inevitable dels nounats i un percentatge presentarà paràlisi braquial (PBO) que freqüentment s'autoresolt. S'estudien els factors de risc de la fractura i és el primer treball que estudia la relació entre el tipus de fractura clavicular i la presència de PBO al naixement i la permanència d'aquesta complicació. L'afectació del terç proximal de clavícula i la distòcia d'espatlles són factors de risc per al desenvolupament de la PBO inicial, però no el desplaçament de la fractura. La permanència de la complicació no s'ha relacionat amb cap factor de risc.

The low quality of education is a persistent problem in many developed countries. Parallel to in the last decades exists a tendency towards decentralization in many developed and developing countries. Using micro data from the Programme for International Student Assessment (PISA) referred to 22 countries, we test whether there exists an impact of fiscal and political decentralization on student performance in the areas of mathematics, reading skills and science. We observe that fiscal decentralization exerts an unequivocal positive effect on students’ outcomes in all areas, while the effect of political decentralization is more ambiguous. On the one hand, the capacity of the subnational governments to rule on its region has a positive effect on students’ performance in mathematics. On the other hand, the capacity to influence the country as a whole has a negative impact on mathematics achievement. As a general result, we observe that students’ performance in Mathematics is more sensible to these exogenous variations than in Sciences and reading skills. La cesària és un procediment de cirurgia major que genera un dolor postoperatori agut, moderat o sever segons els casos. El dolor agut postoperatori es correlaciona amb un augment de la morbilitat postoperatòria. L’objectiu principal de l’estudi fou avaluar la incidència i la quantificació del dolor postoperatori en la cirurgia de cesària a l’Hospital General de Granollers. Durant 2 mesos es van incloure 66 pacients que van ser sotmeses a una cesària. El dolor postoperatori fou alt. Es recomanable realitzar un protocol de tractament del dolor postoperatori en cesàries que sigui eficaç, segur i amb mínims efectes secundaris per a la mare i el nadó.

La nefrectomía parcial se ha constituido en el tratamiento de referencia de los tumores renales pequeños. En pacientes de alto riesgo quirúrgico se han planteado técnicas mínimamente invasivas como la ablación por radiofrecuencia como una alternativa terapéutica. En este estudio evaluamos nuestra experiencia en la aplicación de esta técnica en cuanto sus resultados oncológicos y en cuanto a su efecto sobre la función renal a los 3 y 12 meses de seguimiento. Se concluye que se trata de una técnica que no altera la función renal con resultados oncológicos prometedores que deben ser validados por estudios a largo plazo.

La fractura de cadera es una patología prevalente en la población anciana, con un crecimiento exponencial, altas tasas de morbimortalidad asociada, implicaciones en la calidad de vida y consumo de recursos sanitarios. Objetivos: Conocer las variables epidemiológicas de esta patología, el impacto en la atención médica y el proceso que implica para la adecuada previsión de recursos. Describir las causas de demora quirúrgica y sus posibles consecuencias, e implicaciones de todo lo anterior en la práctica anestésica. Material y métodos: Se realizó un estudio observacional retrospectivo, se revisan pacientes mayores de 65 años con diagnostico de fractura de cadera intervenidos en nuestro hospital entre enero de 2007 y diciembre de 2008, analizando datos epidemiológicos generales, morbimortalidad, antiagregación/anticoagulación, tipo de anestesia y complicaciones, tasa transfusional, estancia hospitalaria, demora quirúrgica y mortalidad, entre otros. Conclusiones: La fractura de cadera es un problema de salud serio y global, con implicaciones importantes en nuestra práctica diaria, en los costos inmediatos y a largo plazo. Siendo responsable de una alta morbimortalidad precoz y tardía. La demora quirúrgica se asocia a un aumento de complicaciones postoperatorias y mortalidad.

BACKGROUND The purpose of the present study is to translate and validate the "Hip and Knee Outcomes Questionnaire", developed in English, into Spanish. The 'Hip and Knee Outcomes Questionnaire is a questionnaire planned to evaluate the impact in quality of life of any problem related to the human musculoskeletal system. 10 scientific associations developed it. METHODS The questionnaire underwent a validated translation/retro-translation process. Patients undergoing primary knee arthroplasty, before and six months postoperative, tested the final version in Spanish. Psychometric properties of feasibility, reliability, validity and sensitivity to change were assessed. Convergent validity with SF-36 and WOMAC questionnaires was evaluated. RESULTS 316 patients were included. Feasibility: a high number of missing items in questions 3, 4 and 5 were observed. The number of patients with a missing item was 171 (51.35%) in the preoperative visit and 139 (44.0%) at the postoperative. Internal validity: revision of coefficients in the item-rest correlation recommended removing question 6 during the preoperative visit (coefficient <0.20). Convergent validity: coefficients of correlation with WOMAC and SF-36 scales confirm the questionnaire's validity. Sensitivity to change: statistically significant differences were found between the mean scores of the first visit compared to the postoperative. CONCLUSION The proposed translation to Spanish of the 'Hip and Knee Questionnaire' is found to be reliable, valid and sensible to changes produced at the clinical practice of patients undergoing primary knee arthroplasty. However, some changes at the completion instructions are recommended. LEVEL OF EVIDENCE Level I. Prognostic study.

OBJECTIVE To assess Spanish and Portuguese patients' and physicians' preferences regarding type 2 diabetes mellitus (T2DM) treatments and the monthly willingness to pay (WTP) to gain benefits or avoid side effects. METHODS An observational, multicenter, exploratory study focused on routine clinical practice in Spain and Portugal. Physicians were recruited from multiple hospitals and outpatient clinics, while patients were recruited from eleven centers operating in the public health care system in different autonomous communities in Spain and Portugal. Preferences were measured via a discrete choice experiment by rating multiple T2DM medication attributes. Data were analyzed using the conditional logit model. RESULTS Three-hundred and thirty (n=330) patients (49.7% female; mean age 62.4 [SD: 10.3] years, mean T2DM duration 13.9 [8.2] years, mean body mass index 32.5 [6.8] kg/m(2), 41.8% received oral + injected medication, 40.3% received oral, and 17.6% injected treatments) and 221 physicians from Spain and Portugal (62% female; mean age 41.9 [SD: 10.5] years, 33.5% endocrinologists, 66.5% primary-care doctors) participated. Patients valued avoiding a gain in bodyweight of 3 kg/6 months (WTP: €68.14 [95% confidence interval: 54.55-85.08]) the most, followed by avoiding one hypoglycemic event/month (WTP: €54.80 [23.29-82.26]). Physicians valued avoiding one hypoglycemia/week (WTP: €287.18 [95% confidence interval: 160.31-1,387.21]) the most, followed by avoiding a 3 kg/6 months gain in bodyweight and decreasing cardiovascular risk (WTP: €166.87 [88.63-843.09] and €154.30 [98.13-434.19], respectively). Physicians and patients were willing to pay €125.92 (73.30-622.75) and €24.28 (18.41-30.31), respectively, to avoid a 1% increase in glycated hemoglobin, and €143.30 (73.39-543.62) and €42.74 (23.89-61.77) to avoid nausea. CONCLUSION Both patients and physicians in Spain and Portugal are willing to pay for the health benefits associated with improved diabetes treatment, the most important being to avoid hypoglycemia and gaining weight. Decreased cardiovascular risk and weight reduction became the third most valued attributes for physicians and patients, respectively.
