Objective: First, to assess the clinical effectiveness of hylan G-F 20 in an appropriate care treatment regimen (as defined by the American College of Rheumatology (ACR) 1995 guidelines) as measured by validated disease-specific outcomes and health-related quality of life endpoints for patients with osteoarthritis (OA) of the knee. Second, to utilize the measures of effectiveness and costs in an economic evaluation (see accompanying manuscript). Design: A total of 255 patients with OA of the knee were enrolled by rheumatologists or orthopedic surgeons into a prospective, randomized, open-label, 1-year, multi-centred trial, conducted in Canada. Patients were randomized to 'Appropriate care with hylan G-F 20' (AC+H) or 'Appropriate care without hylan G-F 20' (AC). Data were collected at clinic visits (baseline, 12 months) and by telephone (1, 2, 4, 6, 8, 10, and 12 months). Results: The AC+H group was superior to the AC group for all primary (% reduction in mean Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain scale: 38% vs 13%, P=0.0001) and secondary effectiveness outcome measures. These differences were all statistically significant and exceeded the 20% difference between groups seta priori by the investigators as the minimum clinically important difference. Objective: To explore circadian variation in pain, stiffness, and manual dexterity inpatients with hand osteoarthritis (OA). Methods: Twenty one patients with hand OA, as defined by ACR criteria (17 women, four men, mean age 62.2 years, range 52-74 years) self rated pain and stiffness on separate 10 cm horizontal visual analogue scales and performed bead intubation coordinometry (BIC) six times each day (on waking up, at bedtime, and every four hours in between) for 10 consecutive days. Each series (using data with the trend removed if there was a significant trend) was analysed for circadian rhythmicity by a cosine. vector technique (single cosinor). With individual data expressed as the percentage of the mean, group rhythm characteristics at period 24 hours were summarised for each variable by population mean cosinor analysis. Results: Individual analyses identified significant circadian rhythms at pless than or equal to0.05 for pain (n=15/21), stiffness (n=16/20), and dexterity (n=18/21), and a significant circadian rhythm on a group basis was identified for pain (p=0.013), stiffness (p

Cases of high-sided vehicles striking low bridges is a large problem in many countries, especially the UK. This paper describes an experiment to evaluate a new design of markings for low bridges. A full size bridge was constructed which was capable of having its overhead clearance adjusted. Subjects sat in a truck cab as. it drove towards the bridge and were asked to judge whether the vehicle could pass safely under the bridge. The main objective of the research, was to determine whether marking the bridge with a newly devised experimental marking would result in more cautious decisions from subjects regarding whether or not the experimental bridge structure could be passed under safely compared with the currently used UK bridge marking standard. The results show that the type of bridge marking influenced the level of caution associated with decisions regarding bridge navigation, with the new marking design producing the most cautious decisions for the two different bridge heights used, at all distances away from the bridge structure. Additionally, the distance before the bridge at which decisions were given had an effect on the level of caution associated with decisions regarding bridge navigation (the closer to the bridge, the more cautious the decisions became, irrespective of the marking design). The dimethylsulfoxide (DMSO) reductase family of molybdenum enzymes is a large and diverse group that is found in bacteria and archaea. These enzymes are characterised by a bis(molybdopterin guanine dinucleotide)Mo form of the molybdenum cofactor, and they are particularly important in anaerobic respiration including the dissimilatory reduction of certain toxic oxoanions. The structural and phylogenetic relationship between the proteins of this family is discussed. High-resolution crystal structures of enzymes of the DMSO reductase family have revealed a high degree of similarity in tertiary structure. However, there is considerable variation in the structure of the molybdenum active site and it seems likely that these subtle but important differences lead to the great diversity of function seen in this family of enzymes. This diversity of catalytic capability is associated with several distinct pathways of electron transport.

This paper addresses robust model-order reduction of a high dimensional nonlinear partial differential equation (PDE) model of a complex biological process. Based on a nonlinear, distributed parameter model of the same process which was validated against experimental data of an existing, pilot-scale BNR activated sludge plant, we developed a state-space model with 154 state variables in this work. A general algorithm for robustly reducing the nonlinear PDE model is presented and based on an investigation of five state-of-the-art model-order reduction techniques, we are able to reduce the original model to a model with only 30 states without incurring pronounced modelling errors. The Singular perturbation approximation balanced truncating technique is found to give the lowest modelling errors in low frequency ranges and hence is deemed most suitable for controller design and other real-time applications. Background: Steatosis occurs in more than 50% of patients with chronic hepatitis C and is associated with increased hepatic fibrosis. In many of these patients the pathogenesis of steatosis appears to be the some as for patients with non-alcoholic fatty liver disease-that is, related to visceral adiposity and obesity. Methods: The effect of a three month weight reduction programme on liver biochemistry and metabolic parameters was examined in 19 subjects with steatosis and chronic hepatitis C. Paired liver biopsies were performed in 10 subjects, prior to and 3-6 months following the intervention, to determine the effect of weight loss on liver histology. Results: There was a mean weight loss of 5.9 (3.2) kg and a mean reduction in waist circumference of 9.0 (5.0) cm. PURPOSE: To determine the effects of aggressive lipid lowering on markers of ischemia, resistance vessel function, atherosclerotic burden, and Symptom status in patients with symptomatic coronary artery disease. METHODS: Sixty consecutive patients with coronary artery disease that was unsuitable for revascularization were assigned randomly to either usual therapy of lipids for patients with a low-density lipoprotein (LDL) cholesterol target level <116 mg/dL, or to a, more aggressive lipid-lowering strategy involving up to 80 mg/d of atorvastatin, with a target LDL cholesterol level <77 mg/dL. The extent and severity of inducible ischemia (by dobutamine echocardiography), vascular function.(brachial artery reactivity), atheroma burden (carotid intima-media thickness), and symptom status were evaluated blindly at baseline and after 12 weeks of treatment. RESULTS: After 12 weeks of treatment, patients in the aggressive therapy group had a significantly greater decrease in mean (+/- SD) LDL cholesterol level than those in the usual care group (29 +/- 38 mg/dL vs. 7 +/- 24 mg/dL, P = 0.03). Patients in the aggressive therapy group had a reduction in the number of ischemic wall segments (mean between-group difference of 1.3; 95% confidence interval: 0.1 to 2.0; P = 0.04), flow-mediated dilatation (mean between-group difference of 5.9%; 95% confidence interval: 2.5% to 9.4%; P = 0.001), and angina score after 12 weeks. There were no significant changes in atherosclerotic burden in either group. CONCLUSION: Patients with symptomatic coronary artery disease who are treated with aggressive lipid lowering have improvement of symptom status and ischemia that appears to reflect improved vascular function but not atheroma burden. Posteroanterior stiffness of the lumbar spine is influenced by factors, including trunk muscle activity and intra-abdominal pressure (IAP). Because these factors vary with breathing, this study investigated whether stiffness is modulated in a cyclical manner with respiration. A further aim was to investigate the relationship between stiffness and IAP or abdominal and paraspinal muscle activity. Stiffness was measured from force-displacement responses of a posteroanterior force applied over the spinous process of L-2 and L-4. Recordings were made of IAP and electromyographic activity from L-4/L-2 erector spinae, abdominal muscles, and chest wall. Stiffness was measured with the lung volume held at the extremes of tidal volume and at greater and lesser volumes. Stiffness at L-4 and L-2 increased above base-level values at functional residual capacity (L-2 14.9 N/mm and L-4 15.3 N/mm) with both inspiratory and expiratory efforts. The increase was related to the respiratory effort and was greatest during maximum expiration (L-2 24.9 N/mm and L-4 23.9 N/mm). The results indicate that changes in trunk muscle activity and IAP with respiratory efforts modulate spinal stiffness. In addition, the diaphragm may augment spinal stiffness via attachment of its crural fibers to the lumbar vertebrae.

This paper describes the buckling phenomenon of a tubular truss with unsupported length through a full-scale test and presents a practical computational method for the design of the trusses allowing for the contribution of torsional stiffness against buckling, of which the effect has never been considered previously by others. The current practice for the design of a planar truss has largely been based on the linear elastic approach which cannot allow for the contribution of torsional stiffness and tension members in a structural system against buckling. The over-simplified analytical technique is unable to provide a realistic and an economical design to a structure. In this paper the stability theory is applied to the second-order analysis and design of the structural form, with detailed allowance for the instability and second-order effects in compliance with design code requirements. Finally, the paper demonstrates the application of the proposed method to the stability design of a commonly adopted truss system used in support of glass panels in which lateral bracing members are highly undesirable for economical and aesthetic reasons.

Os métodos de análise de estruturas de contenção de solo reforçado sob condições de trabalho, em geral, desconsideram a contribuição da face para o equilíbrio da estrutura. Visando estudar a influência do peso específico da face e das propriedades relacionadas à rigidez da mesma sobre o desempenho das estruturas de solo reforçado, são realizadas simulações numéricas de diversas estruturas, utilizando a versão de dupla precisão do programa CRISP92-SC. Avalia-se, também, o emprego de diferentes tipos de elementos para a representação da face. Verifica-se que a face rígida impõe redução significativa das solicitações máximas de tração nos reforços e dos deslocamentos das estruturas de solo reforçado. A influência do peso específico da face sobre a estabilidade interna dos maciços reforçados mostrase desprezível e constata-se que a rigidez à flexão e a rigidez axial da face, função da sua geometria e do seu módulo de Young, são parâmetros influentes no comportamento das estruturas de contenção de solo reforçado. As variações da tração no reforço e da resultante de força cortante na face, em decorrência do enrijecimento da face, são analisadas e propõe-se uma relação entre elas. Quanto à forma de representação de uma face com rigidez expressiva, na simulação de uma estrutura de solo reforçado com o CRISP92-SC, é observado que a representação da face, seja por elementos de viga, seja por elementos quadriláteros, não altera os resultados da análise.

Reduction in leaf area in corn plants during reproduction changes physiological metabolism and consequently the accumulation of dry matter in grains. The aim of this work was to study changes in agronomic characteristics caused by defoliation in corn during the reproduction phase. The experiment was carried out in Uberlândia, Minas Gerais state, in the agricultural year 2007/2008. The experiment was arranged in a randomized block design, consisting of seven treatments: control without defoliation, removal of two apical leaves, removal of four apical leaves, removal of all leaves above spike, removal of four intermediate leaves, removal of all leaves below spike, and removal of all plant leaves, with five repetitions. The genotype used for the evaluations was hybrid NB 7376. Defoliation was carried out when plants were at the growth stage R2. The variables assessed were: yield, density of spikes and corncobs, root resistance and stem integrity. When all leaves above the spike were removed, grain yield was reduced by 20%. Corncob density, stem integrity and root resistance to uprooting were also affected. Spike density was only affected when all plant leaves were removed. The leaf area remaining physiologically active above the spike was found to be most efficient in terms of grain yield.

A maioria das infra-estruturas de transportes, nomeadamente os pavimentos rodoviários e aeroportuários, são constituídas por misturas betuminosas, o que permite um bom desempenho e uma adequada durabilidade, nas condições usuais de serviço. As misturas betuminosas são ainda amplamente utilizadas na construção de zonas de estacionamento de veículos, tendo-se verificado recentemente a sua aplicação também em infra-estruturas ferroviárias. Face à necessidade de melhorar o desempenho das vias-férreas, permitindo uma concepção mais durável de linhas de alta velocidade e uma redução dos custos da sua manutenção, tem-se vindo a desenvolver diversos estudos para promover a utilização de novos materiais, principalmente através da incorporação de misturas betuminosas. O presente trabalho tem como objectivo a caracterização do comportamento mecânico de misturas betuminosas a aplicar em infra-estruturas de transportes. Como metodologia para o estudo do comportamento mecânico das misturas betuminosas foram realizados em laboratório ensaios de cargas repetidas, nomeadamente, ensaios de flexão em quatro pontos para determinação da rigidez e da resistência à fadiga e ensaios de compressão triaxiais cíclicos para avaliação do comportamento à deformação permanente. A resistência à fadiga das misturas betuminosas em estudo foi avaliada através do ensaio de flexão em quatro pontos, com extensão controlada, e aplicação de um carregamento sinusoidal com diferentes frequências, de acordo com o procedimento de ensaio da norma europeia EN 12697-24 (2004 + A1: 2007). A resistência à deformação permanente das misturas betuminosas foi analisada através de ensaios de compressão triaxiais cíclicos, submetendo-as a uma tensão de confinamento estática pela aplicação parcial de vácuo e a uma pressão axial cíclica sob a forma rectangular, de acordo com a norma europeia EN 12697-25 (2004). O conhecimento destas propriedades mecânicas assume particular importância ao nível da formulação das misturas betuminosas, do dimensionamento de uma estrutura ou do estabelecimento de uma adequada solução para uma obra de reabilitação duma infra-estrutura de transportes. Para este estudo foi utilizado um modelo físico construído numa fossa no LNEC, com o propósito de serem testadas três substruturas ferroviárias não convencionais, utilizando sub-balastro betuminoso. A selecção das substruturas foi efectuada após uma análise de várias secções de estruturas já testadas e aplicadas noutros países, de forma a proporcionar comparações fiáveis entre elas. Os resultados obtidos mostraram que a mistura betuminosa AC20 base 50/70 (MB) aplicada na camada de sub-balastro é adequada para ser aplicada nas infra-estruturas de transportes pois apresenta um bom desempenho à fadiga e à deformação permanente. Através dos ensaios efectuados foi ainda possível entender a importante influência das características volumétricas, principalmente da porosidade para o bom comportamento da mistura betuminosa.
