Há algum tempo o condicionamento físico vem sendo parte obrigatória no tratamento de portadores de DPOC. Estes pacientes apresentam comumente intolerância ao exercício de intensidade variável e relacionada à disfunção muscular esquelética. Neste sentido, o exercício físico apresenta-se como ramo mais importante no processo de reabilitação pulmonar. O exercício aeróbio e o treino de força com pesos são fundamentais no incremento de capacidade física e qualidade de vida, principalmente naqueles indivíduos que apresentam as formas moderada ou grave da DPOC. Além disso, espera-se atualmente maior desenvolvimento nas pesquisas em relação à aplicação de estimulação elétrica neuromuscular (EENM) e ao uso criterioso de substâncias ergogênicas tais como esteróides anabolizantes e creatina oral. Tendo em vista as repercussões negativas da disfunção muscular e a importância da reabilitação pulmonar no tratamento da DPOC, esta revisão tem como objetivo reunir informações de estudos relevantes acerca das principais estratégias para o recondicionamento muscular esquelético nestes pacientes nos últimos 15 anos.

Pacientes com doença pulmonar obstrutiva crônica apresentam comumente fraqueza muscular periférica associada à intolerância ao exercício. Embora efetivo, o exercício aeróbio apresenta pouco ou nenhum efeito na fraqueza e atrofia muscular, além de não ser tolerado pela maioria dos pacientes com DPOC. Nesse sentido, o treinamento de força é opção racional para aumentar a força muscular, além de já ter se mostrado mais tolerável que o exercício aeróbio. O aumento de força muscular periférica é o benefício mais consistente do treinamento de força e, quando este é associado ao exercício aeróbio, não resulta em melhora adicional da capacidade de exercício, da dispnéia e da qualidade de vida. Contudo, observa-se que o treinamento combinado é fisiologicamente mais completo e pode ser uma opção de condicionamento físico mais diversificado. O treinamento de moderada a alta intensidade resulta em maiores adaptações fisiológicas, entretanto o exercício de baixa intensidade é tolerável, simples, de fácil execução domiciliar, não requer equipamentos sofisticados e resulta em benefícios significativos. Este exercício é indicado, sobretudo, para os pacientes com DPOC mais avançada. Finalmente, há evidências recentes de que o treinamento de força para os músculos do tronco é alternativa válida para melhorar a capacidade funcional de exercício e a função pulmonar em pacientes com DPOC. A presente revisão de literatura sugere a incorporação do treinamento de força como estratégia de rotina nos programas de reabilitação pulmonar. Pesquisas futuras são necessárias para avaliar os efeitos do treinamento de força na saúde mental, no desempenho em atividades de vida diária, na saúde osteoarticular, no risco de quedas e na função pulmonar, entre outros.

A neuropsicologia investiga a expressão comportamental das disfunções cerebrais, isto é, a interação entre estruturas cerebrais preservadas e prejudicadas e as funções cognitivas, tais como linguagem, memória, atenção e outras. A neuropsicologia pediátrica possui especificidades: maturação, estratégias cognitivas, ensino formal e cultura, e características intrínsecas à reorganização cerebral. O objetivo da reabilitação neuropsicológica é estabelecer estratégias para adaptação de funções cognitivas afetadas em relação às demandas do ambiente da criança. A reabilitação cognitiva pediátrica auxilia crianças com deficiência mental, epilepsia, traumatismo craniencefálico, síndromes autísticas, tumores cerebrais, paralisia cerebral, etc. Programas de reabilitação neuropsicológica podem ser voltados para dificuldades acadêmicas ou para funções cognitivas. A investigação da efetividade de programas de reabilitação depende de diversos fatores. O neuropsicólogo deve contribuir para o desenvolvimento de novas estratégias de reabilitação cognitiva, ser qualificado para o uso das mesmas e partilhar, com a equipe interdisciplinar, as técnicas e experiências efetivas.

Recent research advances in understanding the cellular and molecular mechanisms that underlie the processes of hypertrophy and atrophy. This may contribute to development of effective therapeutic strategies to attenuate or block the loss of muscle tissue associated with aging and pathological conditions. In this context, myogenic factors that control the activity of satellite cells have been studied to better understand the events involved in the recovery of muscle mass. Among them, we highlight the Myogenic Regulatory Factors (MRFs), which have been described as potential mediators of muscle growth. The objectives of this study evaluated the morphofunctional adaptations and gene expression of MRFs (MyoD and myogenin) in skeletal muscle (soleus) subjected to an atrophic stimulus followed by physical training. It was used 64 male Wistar rats (80 days, 250 to 300 g), divided into 8 groups (n = 8): C: control animals a week, I: Animals immobilized a week, C3: control animals 3 days; R3: Animals immobilized and recovered for 3 days, T3: Animals immobilized and submitted to exercise for 3 days; C7: Animals controls 7 days; R7: Animals immobilized and subsequently recovered by 7 days, T7: Animals immobilized and subsequently subjected to exercise for 7 days. Initially, the animals in groups I, R3, R7, T3 and T7, were submitted to 7 days of immobilization of the hind limb. Muscle atrophy was confirmed after a direct statistical comparison of the values of cross-sectional area (CSA) of muscle fibers studied in animals in groups I and C, sacrificed immediately after the immobilization period. Then, the groups T3 and T7 were submitted a rehabilitation program with muscle aerobic exercise (swimming) for 3 and 7 days respectively. The groups C, C3 and C7 were kept without stimulus atrophic and were not subjected to exercise. At the end of the experiment, the animals were sacrified and the soleus muscle removed. Background: Conventional cardiac rehabilitation program consist of 15 min of warm-up, 30 min of aerobic exercise and followed by 15 min calisthenics exercise. The Pilates method has been increasingly applied for its therapeutic benefits, however little scientific evidence supports or rebukes its use as a treatment in patients with heart failure (HF). Purpose: Investigate the effects of Pilates on exercise capacity variables in HF. Methods: Sixteen pts with HF, left ventricular ejection fraction 27 +/- 14%, NYHA class III were randomly assigned to conventional cardiac rehabilitation program (n = 8) or mat Pilates training (n = 8) for 16 weeks of 30 min of aerobic exercise followed by 20 min of the specific program. Results: At 16 weeks, pts in the mat Pilates group and conventional group showed significantly increase on exercise time 11.9 +/- 2.5 to 17.8 +/- 4 and 11.7 +/- 3.9 to 14.2 +/- 4 min, respectively. However, only the Pilates group increased significantly the ventilation (from 56 +/- 20 to 69 +/- 17 L/min, P= 0.02), peak VO2 (from 20.9 +/- 6 to 24.8 +/- 6 mL/kg/min, P= 0.01), and O-2 pulse (from 11.9 +/- 2 to 13.8 +/- 3 mL/bpm, P= 0.003). The Pilates group showed significantly increase in peak VO2 when compared with conventional group (24.8 +/- 6 vs. 18.3 +/- 4, P= 0.02). Conclusions: The result suggests that the Pilates method may be a beneficial adjunctive treatment that enhances functional capacity in patients with HF who are already receiving standard medical therapy.

Fragestellung: In der vorliegenden Arbeit wird erstmals das Sahaja Yoga (SY) bezüglich seiner Effektivität in der komplementären Behandlung von Patienten mit Mamma- oder Prostatakarzinom untersucht und mit der Progressiven Muskelrelaxation nach Jacobson (PMR) verglichen. Methoden: Zu diesem Zweck wurden 16 Patienten mit Prostatakarzinom und 21 Patientinnen mit Mammakarzinom im Rahmen einer stationären Anschlußheilbehandlung (AHB) rekrutiert und jeweils mittels Zufallstabelle entweder einer SY-Gruppe oder einer PMR-Gruppe zugeteilt. Die Patienten übten während des dreiwöchigen Aufenthalts zusätzlich zu den jeweiligen physikalischen und physiotherapeutischen Therapien die entsprechenden Entspannungsverfahren (SY oder PMR) insgesamt acht mal und wurden dazu angehalten, die Verfahren auch in Eigenregie fortzuführen. Die Patienten füllten zu Beginn ihres Aufenthaltes (t1), nach 3 Wochen (Ende der AHB, t2) sowie 2 Monate nach dem Beenden der AHB (t3) folgende Fragebögen aus: den SF-36 von Bullinger et al. (1995) zur Erfassung von gesundheitsbezogener Lebensqualität, die HADS von Hermann et al. (1995) zur Erfassung von Angst und Depressivität und zwei vom Autor selbstkonzipierte Fragebögen, mit denen die Anzahl und Dauer der durchgeführten Übungen registriert wurden. Den MDBF von Steyer et al. (1997) (für aktuelle Befindlichkeit) füllten die Patienten jeweils direkt vor und nach der ersten (t1v und t1n) und letzten (t2v und t2n) Entspannungsübung und zum Zeitpunkt t3 aus. Ergebnisse: Fast alle Testwerte des Gesundheitsfragebogens SF-36 steigen (teils statistisch signifikant) bei beiden Gruppen von dem Zeitpunkt t1 über t2 zu t3 an. Es scheinen zwar die Verbesserungen der SY-Gruppe gegenüber der PMR-Gruppe etwas zu überwiegen, statistisch konnte dies jedoch nicht belegt werden. Bei den Subskalen der HADS (Angst und Depression) fallen die Werte bei beiden Gruppen von Zeitpunkt t1 über t2 zu t3 ab, statistisch signifikant ist der Abfall jedoch nur von t1 zu t2 bei den Werten der HADS-D (Depression) bei der SY-Gruppe. Die Testwerte sinken bei der SY-Gruppe im Verlauf etwas steiler ab als die Testwerte der PMR-Gruppe, signifikanten Unterschiede zwischen den Gruppen lassen sich jedoch nicht finden. Die Testwerte des MDBF (Kurzform A) steigen von dem Zeitpunkt t1v über t2v zu t3 bei beiden Gruppen (bei der SY-Gruppe etwas steiler, jedoch ohne signifikanten Unterschied zur PMR-Gruppe) nur zum Teil signifikant an. Die Werte vor und nach den jeweiligen Anwendungen (Zeitpunkt t1v zu t1n und t2v zu t2n) unterscheiden sich signifikant bei beiden Gruppen. Bezüglich der Dauer und Anzahl der durchgeführten Anwendungen zeigen sich keine statistisch signifikanten Unterschiede zwischen den Gruppen. Schlußfolgerungen: Die Patienten konnten beide Verfahren gleich gut annehmen. Bezüglich der gesundheitsbezogenen Lebensqualität, Angst und Depressivität sowie der aktuellen Befindlichkeit scheint sowohl das SY als auch die PMR (in dieser Studie) relevante Effekte zu haben. Diese Effekte scheinen sich auch nach 2 Monaten weiter positiv auszuwirken. Statistisch ließen sich zwischen beiden Verfahren keine Unterschiede hinsichtlich ihrer Wirkung belegen. Die Ergebnisse müssen allerdings durch weitere Untersuchungen mit größerer Probandenzahl genauer geprüft werden.

Psychological distress is common in patients with chronic heart failure. The impact of different psychological variables on prognosis has been shown but the comparative effects of these variables remain unclear. This study examines the impact of depression, anxiety, vital exhaustion, Type D personality, and social support on prognosis in chronic heart failure patients. One hundred eleven patients (mean age 57 ± 14 years) having participated in an exercise based ambulatory cardiac rehabilitation program were enrolled in a prospective cohort study. Psychological baseline data were assessed at program entry. Mortality, readmission, and health-related quality of life were assessed at follow up (mean 2.8 ± 1.1 years). After controlling for disease severity none of the psychological variables were associated with mortality, though severe anxiety predicted readmission [HR = 3.21 (95% CI, 1.04-9.93; P = .042)]. Health-related quality of life was independently explained by vital exhaustion, anxiety and either body mass index (physical dimension) or sex (emotional dimension). As psychological variables have a strong impact on health-related quality of life they should be routinely assessed in chronic heart failure patients` treatment.

A word-length effect is often described in pure alexia, with reading time proportional to the number of letters in a word. Given the frequent association of right hemianopia with pure alexia, it is uncertain whether and how much of the word-length effect may be attributable to the hemifield loss. To isolate the contribution of the visual field defect, we simulated hemianopia in healthy subjects with a gaze-contingent paradigm during an eye-tracking experiment. We found a minimal word-length effect of 14 ms/letter for full-field viewing, which increased to 38 ms/letter in right hemianopia and to 31 ms/letter in left hemianopia. We found a correlation between mean reading time and the slope of the word-length effect in hemianopic conditions. The 95% upper prediction limits for the word-length effect were 51 ms/letter in subjects with full visual fields and 161 ms/letter with simulated right hemianopia. These limits, which can be considered diagnostic criteria for an alexic word-length effect, were consistent with the reading performance of six patients with diagnoses based independently on perimetric and imaging data: two patients with probable hemianopic dyslexia, and four with alexia and lesions of the left fusiform gyrus, two with and two without hemianopia. Two of these patients also showed reduction of the word-length effect over months, one with and one without a reading rehabilitation program. Our findings clarify the magnitude of the word-length effect that originates from hemianopia alone, and show that the criteria for a word-length effect indicative of alexia differ according to the degree of associated hemifield loss.

BACKGROUND: Though guidelines emphasize low-density lipoprotein cholesterol (LDL-C) lowering as an essential strategy for cardiovascular risk reduction, achieving target levels may be difficult. PATIENTS AND METHODS: The authors conducted a prospective, controlled, open-label trial examining the effectiveness and safety of high-dose fluvastatin or a standard dosage of simvastatin plus ezetimibe, both with an intensive guideline-oriented cardiac rehabilitation program, in achieving the new ATP III LDL-C targets in patients with proven coronary artery disease. 305 consecutive patients were enrolled in the study. Patients were divided into two groups: the simvastatin (40 mg/d) plus ezetimibe (10 mg/d) and the fluvastatin-only group (80 mg/d). Patients in both study groups received the treatment for 21 days in addition to nonpharmacological measures, including advanced physical, dietary, psychosocial, and educational activities. RESULTS: After 21 days of treatment, a significant reduction in LDL-C was found in both study groups as compared to the initial values, however, the reduction in LDL-C was significantly stronger in the simvastatin plus ezetimibe group: simvastatin plus ezetimibe treatment decreased LDL-C to a mean level of 57.7 +/- 1.7 mg/ml, while fluvastatin achieved a reduction to 84.1 +/- 2.4 mg/ml (p < 0.001). In the simvastatin plus ezetimibe group, 95% of the patients reached the target level of LDL-C < 100 mg/dl. This percentage was significantly higher than in patients treated with fluvastatin alone (75%; p < 0.001). The greater effectiveness of simvastatin plus ezetimibe was more impressive when considering the optional goal of LDL-C < 70 mg/dl (75% vs. 32%, respectively; p < 0.001). There was no difference in occurrence of adverse events between both groups. CONCLUSION: Simvastatin 40 mg/d plus ezetimibe 10 mg/d, on the background of a guideline-oriented standardized intensive cardiac rehabilitation program, can reach 95% effectiveness in achieving challenging goals (LDL < 100 mg/dl) using lipid-lowering medication in patients at high cardiovascular risk.

BACKGROUND: Lung volume reduction (LVR) surgery is an effective and organ-preserving treatment option for patients suffering from severe dyspnea due to endstage emphysema. METHOD: Resection of functionally inactive lung parenchyma reduces over-inflation and restores the elastic recoil of the lungs. Thus it results in improvement of dyspnea, mobility and pulmonary function. Patient selection is crucial. Of simliar importance is pulmonary rehabilitation, as well as sufficient expertise in the treatment of endstage chronic respiratory failure. RESULTS AND CONCLUSION: The in-hospital morbidity and mortality after LVR are acceptable (0 to 5%) and the good results seem to last at least 18 to 24 months. LVR can be offered to selected patients either as an alternative or as bridge to lung transplantation.

Refixation of a trochanteric osteotomy carries a high complication rate. To enhance stability and facilitate anatomic reduction of the trochanteric fragment, we have introduced a stepped osteotomy. Between April 2006 and June 2007, we performed surgical hip dislocations using the modified trochanteric osteotomy combined with a relatively aggressive rehabilitation program. Full weightbearing was allowed at a mean of 42 days (range, 33-54 days). The minimum followup was 8 months (median, 13 months; range, 8-24 months). Postoperative radiographs were assessed prospectively for consolidation or the appearance of malreduction/nonunion/malunion of the osteotomy and heterotopic ossification. In 110 of 113 hips, the trochanteric osteotomy healed in the anatomic position. Two patients had a trochanteric delayed union with loss of anatomic position, and one additional patient underwent revision surgery for a pseudarthrosis and cranial migration of the trochanteric fragment. All three complications related to healing occurred in the first 60 patients when the step height was 3 to 4 mm. After increasing the step heights to 6 mm, we observed no healing complications. Despite more aggressive postoperative mobilization, the incidence of malunion or nonunion related to the new stepped osteotomy is low and approaches zero for steps of 6 mm. It is now our technique of choice.

Aggressive behavior can be classified in hostile and instrumental aggressions (anderson & bushman, 2002). this classification is mostly synonymously used with reactive and proactive aggression, whereas the differences between hostile and instrumental aggression lie on three dimensions, the primary goal, amount of anger and planning and calculation(bushman & anderson, 2001). although there are rating instruments and experimental paradigms to measure hostile aggression, there is no instrument to measure instrumental aggression. the following study will present an account to measure instrumental aggression with an experimental laboratory paradigm. the instrument was firstly tested on two samples of normal young adolescents (n1 = 100; amage. = 19.14; n2 = 60; amage. = 21.46). the first study revealed a strong correlation with a laboratory aggression paradigm measuring hostile aggression, but no correlations with self-reported aggression in the buss and perry questionnaire. these results were replicated in a second study, revealing an additional correlation with aggressive but not adaptive assertiveness. secondly the instrument was part of the evaluation of the reasoning and rehabilitation program r&r2 (ross, hilborn & lidell, 1984) in an institution for male adolescents with adjustment problems in switzerland. the r&r2 is a cognitive behavioral group therapy to reduce antisocial and promote prosocial cognitions and behavior. the treatment group (n= 16; rangeage = 15-17) is compared to a no treatment control group (n=24; rangeage = 17-19) preand post- treatment. further aggressive behavior was surveyed and experimentally measured. hostile rumination, aggressive and adaptive assertiveness, emotional and social competence were included in the measurement to estimate construct validity.
