A male infant showed a humeral diaphysis fracture at 5 months of age and a distal tibial physis fracture at 2 years of age. A specialized consultant ruled out child abuse. This child had the characteristic features of type I osteogenesis imperfecta: blue sclerae, osseous fragility, and presumably autosomal dominant inheritance, as his father suffered from similar disorders. Later on, multiple painful osteochondromas were also found and some of these were surgically treated. The child's mother showed several peripheral osteochondromas. We describe the follow-up of this patient up to the age of 18 years. To our knowledge, the fortuitous association of these two inherited conditions has not been reported in medical literature.

Barrett's esophagus consists of the replacement of normal squamous epithelium by a specialised columnar lined epithelium referred to as intestinal metaplasia in the esophagus. It represents a premalignant lesion. The prevalence of Barrett's esophagus is around 1.6%. Esophageal adenocarcinoma results from the development of dysplasia progressing from low to high grade dysplasia and finally adenocarcinoma. Radiofrequency ablation currently represents the treatment of choice in eradicating Barrett's esophagus with associated dysplasia. The technique is based on the application of a radiofrequency current that enables the destruction of the superficial modified epithelium. This new approach presents a good security profile and, compared to other ablative techniques, shows superior results regarding Barrett's eradication.

SUMMARY Barrett's esophagus (BE) is an acquired condition in which the normal squamous epithelium in the distal esophagus is replaced by a metaplastic columnar epithelium, as a complication of chronic gastroesophageal reflux. The clinical significance of this disease is its associated predisposition to esophageal adenocarcinoma (EAC). EAC is a highly lethal disease. Better understanding of the pathogenesis of columnar metaplasia and its progression to cancer might allow the identification of biomarkers that can be used for early diagnosis, which will improve the patient survival. In this study, an improved protocol for methylation-sensitive single-strand conformation analysis, which is used to analyze promoter methylation, is proposed and a methylation-sensitive dot blot assay is described, which allows a rapid, easy, and sensitive detection of promoter methylation. Both methods were applied to study the methylation pattern of the APC promoter in histologically normal appearing gastric mucosa. The APC promoter showed monoallelic methylation, and because the methylated allele differed between the different gastric cell types, this corresponded to allelic exclusion. The APC methylation pattern was frequently altered in noimal gastric mucosa associated with neoplastic lesions, indicating that changes in the pattern of promoter methylation might precede the development of neoplasia, without accompanying histological manifestations. An epigenetic profile of 10 genes important in EAC was obtained in this study; 5 promoter genes (APC, TIMP3, TERT, CDKN2A and SFRP1) were found to be hypermethylated in the tumors. Furthermore, the promoter of APC, TIMP3 and TERT was frequently methylated in BE samples from EAC patients, but rarely in BE samples that did not progress to EAC. These three biomarkers might therefore be considered as potential predictive markers for increased EAC risk. Analysis of Wnt pathway alterations indicated that WNT2 ligand is overexpressed as early as the low-grade dysplastic stage and downregulation by promoter methylation of the SFRP1 gene occurrs already in the metaplastic lesions. Moreover, loss of APC expression is not the only factor involved in the activation of the Wnt pathway. These results indicate that a variety of biologic, mostly epigenetic events occurs very early in the carcinogenesis of BE. This new information might lead to improved early diagnosis of EAC and thus open the way to a possible application of these biomarkers in the prediction of increased EAC risk progression. RESUME L'oesophage de Barrett est une lésion métaplasique définie par le remplacement de la muqueuse malpighienne du bas oesophage par une muqueuse cylindrique glandulaire, suite à une agression chronique par du reflux gastro-esophagien. La plus importante signification clinique de cette maladie est sa prédisposition au développement d'un adénocarcinome. Le pronostic de l'adénocarcinome sur oesophage de Barrett est sombre. Seule une meilleure compréhension de la pathogenèse de l'épithélium métaplasique et de sa progression néoplasique permettrait l'identification de biomarqueurs pouvant être utilisés pour un diagnostic précoce ; la survie du patient serait ainsi augmentée. Dans cette étude, un protocole amélioré pour l'analyse de la méthylation par conformation simple brin est proposé. De plus, une technique d'analyse par dot blot permettant une détection rapide, facile et sensible de la méthylation d'un promoteur est décrite. Les deux méthodes ont été appliquées à l'étude de la méthylation du promoteur du gène APC dans des muqueuses gastriques histologiquement normales. Le promoteur APC a montré une méthylation monoallélique et, parce que les allèles méthylés différaient entre les différents types de cellules gastriques, celle-ci correspondait à une méthylation allélique exclusive. La méthylation d'APC a été trouvée fréquemment altérée dans la muqueuse gastrique normale associée à des lésions néoplasiques. Ceci indique que des changements dans la méthylation d'un promoteur peuvent précéder le développement d'une tumeur, et cela sans modification histologique. Un profil épigénétique des adénocarcinomes sur oesophage de Barrett a été obtenu dans cette étude. Cinq promoteurs (APC, TIMP3, TERT, CDKN2A et SFRP1) ont été trouvés hyperméthylés dans les tumeurs. Les promoteurs d'APC, TIMP3 et TERT étaient fréquemment méthylés dans l'épithélium métaplasique proche d'un adénocarcinome et rarement dans l'épithélium sans évolution néoplasique. Ces trois biomarkers pourraient par conséquent être considérés comme marqueur prédicatif d'un risque accru de développer une tumeur. L'analyse des altérations de la voie Wnt a montré que WNT2 est surexprimé déjà dans des dysplasies de bas-grade et que la dérégulation de SFRP1 par méthylation de son promoteur intervenait dans les lésions métaplasiques. Une perte d'expression d'APC n'est pas le seul facteur impliqué dans l'activation de cette voie. Ces résultats montrent qu'une grande diversité d'événements biologiques, principalement épigénétiques, surviennent très tôt lors de la carcinogenèse de l'oesophage de Barrett. Ces nouveaux éléments pourraient améliorer le diagnostic précoce et rendre possible l'application de ces biomarqueurs dans la prédiction d'un risque accru de développer un adénocarcinome sur un oesophage de Barrett.

Several classes of recreational and prescription drugs have additional effects on the heart and vasculature, which may significantly contribute to morbidity and mortality in chronic users. The study presented herein focuses on pathological changes involving the heart possibly due to anabolic androgenic steroid use. The role these hormones may play in their occurrence of sudden cardiac death is also investigated. 98 medico-legal cases including 6 anabolic androgenic steroid users were retrospectively reviewed. Autopsies, histology, immunohistochemistry, biochemistry and toxicology were performed in all cases. Pathological changes consisted of various degrees of interstitial and perivascular fibrosis as well as fibroadipous metaplasia and perineural fibrosis within the myocardium of the left ventricle. Within the limits of the small number of investigated cases, our results appear to confirm former observations on this topic and suggest anabolic androgenic steroid's potential causative role in the pathogenesis of sudden cardiac deaths in chronic users.

The improvement of the reliability of the contact between the osseous tissues and the implant materials has been tested by recovering the metallic implants with ceramic materials, usually calcium phosphates. In our study, the calcium phosphate recovering layers were deposited by means of a pulsed-laser deposition technique. Our aim was to to evaluate the tissue interactions established between cortical bone and titanium implants covered by five different layers, ranging from amorphous calcium phosphate to crystalline hydroxyapatite, obtained by altering the parameters of the laser ablation process. The surgical protocol of the study consisted in the simultaneous implantation of the five types of implants in both the tibial dyaphisis of three Beagle dogs, sacrificed respectively one, two and three months after the last surgical procedures. After the sacrifice, the samples were submitted to a scheduled procedure of embedding in plastic polymers without prior decalcification, in order to perform the ultrastructural studies: scanning microscopy with secondary and backscattered electrons (BS-SEM). Our observations show that both in terms of the calcified tissues appearing as a response to the presence of the different coatings and of time of recovering, the implants coated with crystalline calcium phosphate layers by laser ablation present a better result than the amorphous-calcium-phosphate-coated implants. Moreover, the constant presence of chondroid tissue, related with the mechanical induction by forces applied on the recovering area, strongly suggests that the mechanisms implied in osteointegration are related to endomembranous, rather than endochondral ossification processes

O carcinoma renal sarcomatóide é uma neoplasia agressiva cujas características clínicas e radiológicas são similares às do carcinoma de células renais convencionais (células claras). O tumor é composto por camadas de células fusiformes malignas com aspectos imuno-histoquímicos e ultra-estruturais de células epiteliais e estromais, também podendo conter áreas mixóides de células gigantes osteoclasto-símile, células pleomórficas rabdomioblasto-símile, bem como outros componentes sarcomatóides raros. Os autores relatam um caso de carcinoma renal sarcomatóide em paciente do sexo masculino, com 54 anos de idade, apresentando a clássica tríade clínica do carcinoma de células renais. Ressaltam, também, as características macroscópicas e microscópicas típicas da lesão, e discutem os achados dos métodos de imagem e seu diagnóstico diferencial com sarcomas renais verdadeiros.

Whole-body coverage using MRI was developed almost 2 decades ago. The first applications focused on the investigation of the skeleton to detect neoplastic disease, mainly metastases from solid cancers, and involvement by multiple myeloma and lymphoma. But the extensive coverage of the whole musculoskeletal system, combined with the exquisite sensitivity of MRI to tissue alteration in relation to different pathologic conditions, mainly inflammation, has led to the identification of a growing number of indications outside oncology. Seronegative rheumatisms, systemic sclerosis, inflammatory diseases involving muscles or fascias, and multifocal osseous, vascular, or neurologic diseases represent currently validated or emerging indications of whole-body MRI (WB-MRI). We first illustrate the most valuable indications of WB-MRI in seronegative rheumatisms that include providing significant diagnostic information in patients with negative or ambiguous MRI of the sacroiliac joints and the lumbar spine, assessing disease activity in advanced (ankylosed) central disease, and evaluating the peripherally dominant forms of spondyloarthropathy. Then we review the increasing indications of WB-MRI in other rheumatologic and nonneoplastic disorders, underline the clinical needs, and illustrate the role of WB-MRI in the positive diagnosis and evaluation of disease burden, therapeutic decisions, and treatment monitoring.

Synovial chondromatosis is a benign condition characterized by synovial proliferation and metaplasia, with development of cartilaginous or osteocartilaginous nodules within a joint, bursa or tendon sheath. In the shoulder, synovial osteochondromatosis may occur within the glenohumeral joint and its recesses (including the tendon sheath of the biceps long head), and in the subacromial-deltoid bursa. Such condition can be identified either by radiography, ultrasonography or magnetic resonance imaging, showing typical features according to each method. Radiography commonly shows ring-shaped calcified cartilages and periarticular soft tissues swelling with erosion of joint margins. Ultrasonography demonstrates hypoechogenic cartilaginous nodules with progressive increase in echogenicity as they become calcified, with development of posterior acoustic shadow in case of ossification. Besides identifying cartilaginous nodules, magnetic resonance imaging can also demonstrate the degree of synovial proliferation. The present study is aimed at describing the imaging findings of this entity in the shoulder.

The development of load-bearing osseous implant with desired mechanical and surface properties in order to promote incorporation with bone and to eliminate risk of bone resorption and implant failure is a very challenging task. Bone formation and resoption processes depend on the mechanical environment. Certain stress/strain conditions are required to promote new bone growth and to prevent bone mass loss. Conventional metallic implants with high stiffness carry most of the load and the surrounding bone becomes virtually unloaded and inactive. Fibre-reinforced composites offer an interesting alternative to metallic implants, because their mechanical properties can be tailored to be equal to those of bone, by the careful selection of matrix polymer, type of fibres, fibre volume fraction, orientation and length. Successful load transfer at bone-implant interface requires proper fixation between the bone and implant. One promising method to promote fixation is to prepare implants with porous surface. Bone ingrowth into porous surface structure stabilises the system and improves clinical success of the implant. The experimental part of this work was focused on polymethyl methacrylate (PMMA) -based composites with dense load-bearing core and porous surface. Three-dimensionally randomly orientated chopped glass fibres were used to reinforce the composite. A method to fabricate those composites was developed by a solvent treatment technique and some characterisations concerning the functionality of the surface structure were made in vitro and in vivo. Scanning electron microscope observations revealed that the pore size and interconnective porous architecture of the surface layer of the fibre-reinforced composite (FRC) could be optimal for bone ingrowth. Microhardness measurements showed that the solvent treatment did not have an effect on the mechanical properties of the load-bearing core. A push-out test, using dental stone as a bone model material, revealed that short glass fibre-reinforced porous surface layer is strong enough to carry load. Unreacted monomers can cause the chemical necrosis of the tissue, but the levels of leachable resisidual monomers were considerably lower than those found in chemically cured fibre-reinforced dentures and in modified acrylic bone cements. Animal experiments proved that surface porous FRC implant can enhance fixation between bone and FRC. New bone ingrowth into the pores was detected and strong interlocking between bone and the implant was achieved.

Traunatic perineal hernia remains a rare clinical entity despite an overall increase in blunt trauma. Because of the incidence of other associated injuries, the mortality is high. Most of the perineal defects are repaired during the orthopaedics surgery to reconstitute the pelvis and few patients develop a true perineal hernia without pelvic instability. A 80-year-old woman was involved in a running over accident with disjunction of pubic symphysis, dislocation of sacrum-iliac junction and fracture of pubis and ischium. The patient was submitted to an orthopaedic surgery and latter development an perineal hernia through the genitalia. The diagnosis could be established with physical examination alone. Conventional radiology, computadorized tomography, and ultrasound should also be done to progran the surgery. The repair approach was performed using a marlex mesh, fixed in the pelvic bones, Cooper ligament, and the abdominal wall. The mesh was stood in a retro- peritoneal position, rebuilding the pelvic floor without reconstruction the pelvic bones. We conclude that this is an efficient approach to repair of traumatic postoperative perineal hernia, mainly in patients with high operative risk, when the osseous repair is not necessary.

OBJETIVO: Definir qual a melhor potência a ser empregada de forma a atingir a profundidade necessária para ablação com o menor número de sintomas pós-procedimento. MÉTODO: Foram estudados 28 pacientes com esôfago de Barrett, após tratamento cirúrgico do refluxo ou em uso de bloqueadores de bomba de prótons, submetidos à ablação endoscópica, randomizados em dois grupos com potências diferentes - 50 ou 70W. Imediatamente após, foram realizadas biópsias endoscópicas das áreas fulguradas. A sintomatologia foi avaliada por questionário telefônico. RESULTADOS: Não houve diferença entre os grupos quanto a idade, a extensão do esôfago de Barrett, a porcentagem da circunferência esofagiana coagulada e a duração dos sintomas. A dor foi o sintoma predominante e a disfagia ocorreu de forma transitória. Houve correlação negativa moderada entre número de sintomas e potência (potência mais baixa com maior número de sintomas), porém sem diferença significativa. Em 40% dos casos em que se utilizou menor potência obteve-se fragmentos que atingiram apenas a porção superficial da mucosa, o que só ocorreu em 10% dos casos no grupo de 70W. Não foi observada diferença significativa entre a potência utilizada ou o acometimento da muscular da mucosa e o número de sintomas. CONCLUSÕES: A utilização de potência de 70W durante a coagulação do esôfago de Barrett com plasma de argônio sugere associação com menor incidência de metaplasia colunar especializada residual abaixo do epitélio escamoso neoformado.

Objetivo: verificar pela microscopia eletrônica as alterações ocorridas no âmnio empregado para a construção de neovaginas e a presença de receptores para estrogênio (REc) nesse epitélio. Métodos: um grupo de 33 pacientes jovens portadoras de agenesia vaginal, em sua maioria amenorréicas, com dificuldade ou impossibilidade de manter atividade sexual, submeteu-se à correção cirúrgica da anomalia pela técnica de McIndoeBanister. Em cada uma das mulheres, a vaginoplastia criou uma cavidade ampla pela dissecção romba do espaço vésicoretal. Um molde de esponja sintética revestido pelo condom e pela membrana amniótica era introduzido e fixado na cavidade criada. No 8º dia do pós-operatório, o molde era retirado, permanecendo um neoepitélio em formação, oriundo da metaplasia da membrana amniótica. Resultados: a microscopia eletrônica de transmissão (MET) realizada em tecidos obtidos de biópsias das neovaginas mostrou características semelhantes nas três camadas destes epitélios, quer no citoplasma, quer nas organelas celulares em comparação com mulheres normais. Comparou-se a intensidade de coloração dos REc em cortes histológicos do neoepitélio vaginal do grupo de mulheres submetidas à neovaginoplastia pela técnica de McIndoe modificada com mulheres normais no menacme. Os REc apresentaram intensidade distinta nas três camadas da mucosa no grupo de mulheres em que foi elaborada a neovagina com revestimento de membrana amniótica. A análise do neoepitélio vaginal obtido com enxerto de membrana amniótica para correção cirúrgica de agenesia vaginal evidenciou pela MET características de um epitélio vaginal trófico. Conclusões: o tecido amniótico enxertado sofre um processo de transformação completa e esta mudança foi evidenciada à MET; os graus de intensidade dos REc das diferentes camadas do epitélio vaginal estudado, em cortes histológicos oriundos de mulheres no menacme, são semelhantes aos cortes histológicos obtidos em biópsias de neovaginas revestidas com membrana amniótica.

OBJETIVO: apresentar série de casos de endometriose na cicatriz umbilical, em pacientes no menacme, sem cirurgias pélvicas prévias. MÉTODOS: foram incluídas no estudo quatro pacientes com idade entre 33 e 43 anos, com queixa de sangramento umbilical associado ou não a dor pélvica, com evolução de dois meses a quatro anos. A ultra-sonografia da parede abdominal foi utilizada para o diagnóstico, procedendo-se à exérese cirúrgica das lesões sugestivas de endometriose umbilical e confirmação anatomopatológica. RESULTADOS: as avaliações ultra-sonográficas das quatro pacientes evidenciaram imagens umbilicais hipoecogênicas compatíveis com a hipótese diagnóstica de endometriose e, assim, todas elas foram encaminhadas para exérese cirúrgica da lesão. A dosagem do marcador sérico CA-125 foi realizada em três das quatro pacientes, com níveis dentro da normalidade, variando de 6,8 a 10,1 U/mL. A concomitância de endometriose pélvica apenas foi confirmada em uma paciente. Durante o seguimento de um ano, as pacientes não apresentaram recidiva dos sintomas nem das lesões. CONCLUSÕES: a endometriose umbilical é uma entidade nosológica rara, mas que deve ser sempre lembrada quando da presença de nodulações ou sangramento umbilicais, ainda que não exista relato de cirurgia pélvica prévia com manipulação endometrial. Seu tratamento cirúrgico é, em geral, suficiente para a remissão total da lesão e dos sintomas.
