Introduction: Bruxism is characterized by repeated tooth grinding or clenching. The condition can occur in all age ranges and in both genders, being related or not to other oral habits. Objective: The objective of the present study was to investigate the occurrence of bruxism in children with nasal obstruction and to determine its association with other factors. Methods: Sixty children with nasal obstruction seen at the Otorhinolaryngology Outpatient Clinic of the University Hospital. of Ribeirao Preto participated in the study. The data were obtained using a pre-established questionnaire applied to the person responsible and by orofacial evaluation of the patient. The participants were divided into two groups: group with bruxism (GB) as reported by the relatives and with the presence of tooth wear detected by clinical evaluation, and group without bruxism (GWB), consisting of children with none of the two symptoms of bruxism mentioned above. Results: The presence of bruxism exceeded its absence in the sample studied (65.22%). There was no significant difference (P < 0.05) between groups regarding gender, phase of dentition, presence of hearing diseases, degree of malocclusion, or child behavior. Conclusion: Bruxism and deleterious oral habits such as biting behavior (objects, tips and nails) were significantly present, together with the absence of suction habits, in the children with nasal obstruction. Objective: Variations in genes that are critical for tooth formation may contribute to the tooth agenesis. MMPs are potential candidate genes for dental alterations based on the roles they play during embryogenesis. The aim of this study was to investigate the possible association between MMP1, MMP3, and MMP20 and tooth agenesis. Methods: One hundred sixty-seven nuclear families from two different populations were analysed, 116 from Brazil and 51 from Turkey. Probands had at least one congenitally missing tooth. DNA samples were obtained from blood or saliva samples and genotyping was performed using TagMan chemistry. In addition, Mmp20 was selected for quantitative real-time polymerase chain reaction analysis with SYBR Green I Dye in mouse tooth development. Results: Associations between tooth agenesis and MMP1 (p = 0.007), and MMP20 (p = 0.03) were found in Brazilian families. In the total dataset, MMP20 continued to be associated with tooth agenesis (p = 0.01). Mmp20 was not expressed during the initial stages of tooth development. Conclusion: Our findings provide evidence that MMP1 and MMP20 play a role in human tooth agenesis. (C) 2010 Elsevier Ltd. All rights reserved.

The aim of this study was to determine the applicability of multislice and cone-beam computerized tomography (CT) in the assessment of bone defects in patients with oral clefts. Bone defects were produced in 9 dry skulls to mimic oral clefts. All defects were modeled with wax. The skulls were submitted to multislice and cone-beam CT. Subsequently, physical measurements were obtained by the Archimedes principle of water displacement of wax models. The results demonstrated that multislice and cone-beam CT showed a high efficiency rate and were considered to be effective for volumetric assessment of bone defects. It was also observed that both CT modalities showed excellent results with high reliability in the study of the volume of bone defects, with no difference in performance between them. The clinical applicability of our research has shown these CT modalities to be immediate and direct, and they is important for the diagnosis and therapeutic process of patients with oral cleft. Objectives: To investigate the prevalence of upper cervical vertebrae anomalies in patients with isolated cleft lip, isolated cleft palate, and complete cleft lip and palate, as well as to compare the prevalence of these anomalies between groups, between genders, and with noncleft patients. Design: Retrospective cross-sectional study of randomly selected patients. Setting: Radiology Section, Hospital for Rehabilitation of Craniofacial Anomalies and Department of Orthodontics, Bauru School of Dentistry, University of Sao Paulo, Sao Paulo, Brazil. Participants: The sample comprised 300 lateral cephalograms of cleft patients, aged 12 to 13 years, of both genders, from the files of the Hospital for Rehabilitation of Craniofacial Anomalies-University of Sao Paulo and 300 lateral cephalograms from noncleft patients of the Department of Orthodontics, Bauru School of Dentistry. Cephalograms of patients with syndromes were not included. Method: Radiographs were interpreted on a film viewer by a single examiner; the profiles of vertebrae were traced on acetate paper, and cervical vertebrae anomalies were registered and categorized into posterior arch deficiencies, fusion, and association of both. Main Outcome Measures: Statistical comparison of groups using the chi-square test. Results: In the cleft group, 38.67% of the patients had cervical vertebrae anomalies. Of those in the noncleft sample, 31% showed anomalies of the cervical spine. This difference was statistically significant. There was no statistically significant difference when the types of clefts were compared with each other or when both genders were compared in both samples. Conclusions: This study confirms the association between clefts and cervical anomalies. Additional research on this topic is necessary.

The observation of mirror-image clefts in conjoined twins may suggest an influence from environmental factors (e.g., poor blood supply) on the appearance of clefts. The present paper reports on a pair of male thoracopagus twins born to a 20-year-old woman. The twins were stillborn. Both twins exhibited complete unilateral cleft lip and palate with mirror-image configuration, affecting the left side for twin A and the right side for twin B. The twins also shared some organs. The case is discussed with similar information in the literature, with reference to possible related etiologic factors. Reporting on such occurrences throughout the world is important to shed light on important aspects underlying the formation of clefts.

Considering that orthognathic surgery promotes changes in orofacial structures constituting the resonating system, functional changes secondary to surgery are expected to affect speech, leading to the need for further speech and voice adjustments. Thus, understanding the possible relationships of these structures with voice production is important. Therefore, this Study aimed to describe the changes in voice fundamental frequency of a patient submitted to orthognathic surgery and observe if there is a relationship with hyoid bone positioning at the different treatment periods. The results revealed that voice fundamental frequency increased after surgery, returning to values close to the preoperative condition, which corresponded to vertical movement of the hyoid bone.

Objective: To evaluate whether teeth close to the cleft area present higher prevalence and severity of periodontal disease than teeth in other regions. Design: Cross-sectional. Setting: Hospital for Rehabilitation of Craniofacial Anomalies, University of Sao Paulo. Patients: There were 400 Individuals with complete unilateral or bilateral cleft lip and palate, aged 15 to 49 years, without any previous periodontal treatment. Main Outcome Measures: All clinical parameters were evaluated in six sites for each tooth. The arithmetic means were calculated for each sextant. Results: Of the sextants, 86.75% presented means of probing depth smaller than or equal to 3 mm. No sextant exhibited means of probing depth greater than or equal to 6 mm. There was a statistically significant difference (p < .001) in probing depth according to age, types of cleft, and sextant; 95.87% of sextants presented mean attachment levels smaller than or equal to 3 mm, The sextant with cleft did not present higher means of probing depth, clinical attachment level, plaque index, and gingival index. There was gingival bleeding in 99.08% of the sample and plaque In 97.40%. The type of cleft was not an Important factor Influencing the prevalence of periodontal disease. Age seems to be an Important factor influencing the prevalence and severity of periodontal disease for all aspects Investigated. Conclusions: Periodontal disease In individuals with clefts occurred in a similar manner as observed in other populations. The presence of the cleft does not seem to Increase the prevalence of the disease.

Chronic L-DOPA pharmacotherapy in Parkinson`s disease is often, accompanied by the development of abnormal and excessive movements known as L-DOPA-induced dyskinesia. Rats with 6-hydroxydopamine lesion of dopaminergic neurons chronically treated with L-DOPA develop a rodent analog of this dyskinesia characterized by severe axial, limb, locomotor and orofacial abnormal involuntary movements. While the mechanisms by which these effects occur are not clear, they may involve the nitric oxide system. In the present study we investigate if nitric oxide synthase inhibitors can prevent dyskinesias induced by repeated administration Of L-DOPA in rats with unilateral 6-hydroxydopamine lesion. Chronic L-DOPA (high fixed dose, 100 mg/kg; low escalating dose, 10-30 mg/kg) treatment induced progressive dyskinesia changes. Two nitric oxide synthase inhibitors, 7-nitroindazole (1-30 mg/kg) and NG-nitro-L-arginine (50 mg/kg), given 30 min before L-DOPA, attenuate dyskinesia. 7-Nitroindazolee also improved motor performance of these animals in the rota-rod test. These results suggest the possibility that nitric oxide synthase inhibitors may be useful to treat L-DOPA.-Induced dyskinesia. (C) 2009 IBRO. Published by Elsevier Ltd. All rights reserved.

Hookworms routinely reach the gut of nonpermissive hosts but fail to successfully feed, develop, and reproduce. To investigate the effects of host-parasite coevolution on the ability of hookworms to feed in nonpermissive hosts, we cloned and expressed aspartic proteases from canine and human hookworms. We show here that a cathepsin D-like protease from the canine hookworm Ancylosotoma caninum (Ac-APR-1) and the orthologous protease from the human hookworm Necator americanus (Na-APR-1) are expressed in the gut and probably exert their proteolytic activity extracellularly. Both proteases were detected immunologically and enzymatically in somatic extracts of adult worms. The two proteases were expressed in baculovirus, and both cleaved human and dog hemoglobin (Hb) in vitro. Each protease digested Hb from its permissive host between twofold (whole molecule) and sixfold (synthetic peptides) more efficiently than Hb from the nonpermissive host, despite the two proteases' having identical residues lining their active site clefts. Furthermore, both proteases cleaved Hb at numerous distinct sites and showed different substrate preferences. The findings suggest that the paradigm of matching the molecular structure of the food source within a host to the molecular structure of the catabolic proteases of the parasite is an important contributing factor for host-parasite compatibility and host species range.

Os sintomas otológicos são freqüentes em pacientes com desordem temporomandibular, e estudos são necessários para elucidar os mecanismos envolvidos. OBJETIVO: O objetivo desse estudo clínico foi investigar a associação de sintomas otológicos (otalgia, zumbido e plenitude auricular) com os achados audiológicos, os outros sinais/sintomas relacionados à desordem temporomandibular, e os hábitos parafuncionais orais. FORMA DE ESTUDO: Prospectivo clínico. MATERIAL E MÉTODO: 27 pacientes com desordem temporomandibular, da Clínica de Oclusão da Faculdade de Odontologia de Ribeirão Preto da Universidade de São Paulo, responderam um questionário sobre sinais, sintomas e hábitos orais, e passaram por avaliações otorrinolaringológica e audiológica. Os dados foram analisados pelos testes Binomial, Exato de Fisher e correlação produto-momento de Pearson. O índice de significância adotado foi p<0,05. RESULTADOS: Os sintomas otológicos foram presentes em 88,88% dos pacientes (59,26% apresentavam otalgia, 74,07% zumbido e 74,07% plenitude auricular). Não houve associação significante entre os sintomas otológicos e os achados audiológicos. Houve associação significante entre os sintomas otológicos e os movimentos mandibulares e funções (falar, abrir e fechar a boca). Houve também correlações significantes entre o grau de severidade dos sintomas otológicos e o grau de outros sinais/sintomas de desordem temporomandibular; e entre o sintoma plenitude auricular e número de hábitos parafuncionais. CONCLUSÃO: Este estudo fornece sustentação adicional à noção de que há relação entre desordem temporomandibular e sintomas otológicos. Nos pacientes com desordem temporomandibular as alterações do sistema estomatognático, como a dor orofacial e a dificuldade nas atividades funcionais, foi associada de modo significante aos sintomas otológicos.

Dentre as alterações sensoriais que acompanham o processo de envelhecimento, a deficiência auditiva, conhecida como presbiacusia, é uma das mais incapacitantes. Esta alteração diminui o contato social, gerando alterações emocionais muitas vezes devastadoras. Além da limitação auditiva decorrente da deficiência auditiva adquirida, verifica-se o aparecimento do handicap auditivo, relacionado a aspectos não auditivos e às alterações emocionais e sociais desencadeados pela deficiência auditiva. FORMA DE ESTUDO: Clínico prospectivo. MATERIAL E MÉTODO:Fizeram parte deste estudo 7 Sujeitos, 3 do sexo masculino e 4 do sexo feminino, com idades que variaram de 60 a 77 anos e portadores de perda auditiva sensorioneural bilateral usuários de aparelho de amplificação sonora unilateral. Foram realizadas 7 sessões de reabilitação auditiva com duração de 1 hora cada, semanal e em grupo, nas quais foram treinados Leitura Orofacial de todos os fonemas da língua portuguesa e trabalhadas estratégias situacionais e comportamentais. RESULTADOS: Foi verificada uma redução significativa da percepção do handicap auditivo em todos os sujeitos após o término das 7 sessões, observados a partir da aplicação do questionário HHIE-S pré- e pós-reabilitação auditiva. CONCLUSÃO: Para minimizar as reações psicossociais do idoso decorrentes da deficiência auditiva, faz-se necessário a seleção, indicação e adaptação de AASI conjuntamente a programas de reabilitação auditiva, auxiliando o idoso portador de deficiência auditiva e seus familiares a lidarem de forma positiva frente às dificuldades de comunicação. Através de programas de reabilitação auditiva é possível reduzir a percepção do handicap auditivo da população idosa que irá refletir na melhora de qualidade de vida, promoção de contatos sociais e diminuição do isolamento.

A síndrome de Melkerson-Rosenthal (SMR) caracteriza-se por edema orofacial, paralisia facial recorrente e língua plicada. A tríade completa é incomum, com freqüência variando de 8 a 25%, sendo que a apresentação mais comum é a presença de somente um sintoma. A queixa mais freqüente é o edema facial e/ou no lábio. No presente relato, descreve-se o caso de uma jovem, 17 anos, com edema no lábio persistente e língua plicada devido à SMR. A paciente informou que o edema e as alterações na língua haviam se iniciado há 2 anos. Tratamentos prévios haviam sido realizados, porém sem sucesso. Propôs-se a injeção intralesional de 20mg de triancinolona a cada 15 dias, associada a 5mg ao dia de clofazimine por três meses. O lábio voltou ao seu aspecto normal após quatro infiltrações da medicação. Estudos recentes têm considerado a SMR como uma doença granulomatosa, sendo a fase inicial da apresentação orofacial da Doença de Crohn em alguns pacientes. Assim, pacientes com SMR deveriam ser avaliados e seguidos quanto à presença de sintomas gastroenterológicos. O tratamento com corticosteróides tem se mostrado efetivo em reduzir a tumefação do lábio associada a essa doença. Discute-se características clínicas, tratamento e importância da terapia com corticosteróides na paralisia facial associada à SMR.

A interação entre disfunção temporomandibular e otalgia é, mesmo nos dias atuais, motivo para especulações e hipóteses. Vários pesquisadores sugerem causas, conseqüências e supostos tratamentos. OBJETIVO: Verificar a prevalência de pacientes portadores de DTM em um serviço de otorrinolaringologia. TIPO DE ESTUDO: Este é um estudo epidemiológico do tipo descritivo com amostra transversal. MATERIAL E MÉTODO: Foram avaliados 221 pacientes do Serviço de Otorrinolaringologia do Hospital da Cidade, em Passo Fundo, Rio Grande do Sul, durante um período de dois meses. Para captação e interpretação dos dados, bem como verificação da disfunção temporomandibular, foi utilizado um questionário auto-aplicado previamente validado. RESULTADO: Após coleta e interpretação dos dados de 221 pacientes, os resultados obtidos foram: 48 pacientes (21.72%) considerados como necessitando de tratamento para DTM (índice de DTM moderada e severa), dos quais 35 pertenciam ao gênero feminino (72.9%) e 13 ao masculino (21.1%). Apenas 15 indivíduos do total (7.24%) estavam totalmente livres de sintomas de DTM. Quanto aos demais, apresentaram: dor de cabeça (33,5%), dor no pescoço e ombro (28,5%), dor na região do ouvido (29%) e ruídos articulares (25%). CONCLUSÃO: A prevalência de DTM foi de 21.72% sendo significantemente maior no gênero feminino (p: 0.0001); e as prevalências, em relação aos índices, foram: DTM ausente 37.56%; DTM leve 40.72%; DTM moderada 19%, e DTM severa 2.72%.

Malaria, caused by Plasmodium falciparum (P. falciparum), ranks as one of the most baleful infectious diseases worldwide. New antimalarial treatments are needed to face existing or emerging drug resistant strains. Protein degradation appears to play a significant role during the asexual intraerythrocytic developmental cycle (IDC) of P. falciparum. Inhibition of the ubiquitin proteasome system (UPS), a major intracellular proteolytic pathway, effectively reduces infection and parasite replication. P. falciparum and erythrocyte UPS coexist during IDC but the nature of their relationship is largely unknown. We used an approach based on Tandem Ubiquitin-Binding Entities (TUBEs) and 1D gel electrophoresis followed by mass spectrometry to identify major components of the TUBEs-associated ubiquitin proteome of both host and parasite during ring, trophozoite and schizont stages. Ring-exported protein (REX1), a P. falciparum protein located in Maurer's clefts and important for parasite nutrient import, was found to reach a maximum level of ubiquitylation in trophozoites stage. The Homo sapiens (H. sapiens) TUBEs associated ubiquitin proteome decreased during the infection, whereas the equivalent P. falciparum TUBEs-associated ubiquitin proteome counterpart increased. Major cellular processes such as DNA repair, replication, stress response, vesicular transport and catabolic events appear to be regulated by ubiquitylation along the IDC P. falciparum infection.

OBJETIVO: A ocorrência de dor orofacial e dor crônica é tema freqüente nos estudos da atualidade; porém, a dor de origem dental é pouco estudada no Brasil. O estudo tem como objetivo conhecer a prevalência de dor de dente como motivo de consulta odontológica e os fatores associados em indivíduos adultos. MÉTODOS: Realizou-se um estudo transversal com 860 funcionários de uma cooperativa localizada no Estado de Santa Catarina, com idade entre 18 e 58 anos, em 1999. Exames clínicos e entrevistas foram realizados por uma cirurgiã-dentista previamente treinada. Analisou-se a queixa de dor de origem dental como motivo da última consulta odontológica como variável dependente em relação às condições socioeconômicas, demográficas, acesso ao serviço odontológico, turno de trabalho e ataque de cárie por meio do índice CPO-D como variáveis independentes. Foi utilizada a análise de regressão logística múltipla não-condicional. RESULTADOS: A prevalência de dor de origem dental foi de 18,7% (IC 95%[15,9-20,1]) e o CPO-D médio 20,2 dentes (IC 95%[19,7-20,7]), com 54% representados pelo componente perdido. Foram associados independentes para a presença de dor de origem dental a escolaridade menor ou igual a oito anos de estudo (OR=1,9[1,1-3,1], a perda por cárie de quatro a 15 dentes (OR=2,6[1,4-4,9]) e de 16 a 32 dentes (OR=2,5[1,1-5,8]) e não ter freqüentado o serviço odontológico da empresa (OR=2,8[1,6-5,1]). CONCLUSÕES: A dor de origem dental reflete a gravidade da cárie dentária, expressa pelo componente perdido do CPO-D e o não uso de serviços odontológicos da empresa. Esses fatores são determinados pelas condições sociais, representadas pela escolaridade.
